March 30th 2022 – you went out on medical Leave of Absence (that was later approved), and elected to apply 51 hours of PTO from 3/29/22 – 4/14/22. April 29th 2022- you received a paycheck that was mistakenly zeroed out due to improper coding for 51 hours of regular time instead of requested PTO time from 3/29/22 to 4/14/22. In addition, medical deductions of $65.22 were applied to the STD pay of $65.22. May 3rd 2022 – correcting the error made on 4/30, you received a check of $1122.84 for 51 hours that was properly coded to PTO time and an STD payment of $25.36. May 5th 2022 – you received a payment of $767.87 for your accrued but unused vacation time. May 31st 2022 – you received 2 payments totaling $8,114 ($3801.86 + $4312.34) from our shortterm disability vendor. This was paid after Disability and Leave Services learned you were no longer working in Washington in April and the State of Washington had denied your application for paid state medical leave benefits.
