Chiricahua Community Health Centers, Inc. (Chiricahua) is a mission driven Federally Qualified Health Center and a tax exempt not-for-profit 501(c)(3) organization. Founded in 1996 as a small, rural health clinic operating in a community center, Chiricahua has since grown to become the largest primary care organization in southeastern Arizona, serving more than 30,000 patients annually. Chiricahua has developed a multi-dimensional model that includes medical, dental, and behavioral health carefully integrated with an approach that address every aspect of life (physical, emotional, social, occupational, intellectual, and environmental) for the uninsured, underinsured, veterans, migrant and seasonal farmworkers. Chiricahua operates fourteen fixed-site medical, pharmacy and behavioral health facilities, as well as seven mobilemedical and mobile-dental units that serve patients throughout the more than 6200 square mile borderlands of Cochise County. Chiricahua is not only the largest primary care provider in Cochise County, but also offers the only true Sliding Fee Discount Program in the region.

Circle the City

Circle the City (CTC) is a dynamic, Arizona-based, 501(c)(3) nonprofit organization that provides integrated, holistic healthcare to men, women, and children facing homelessness in Maricopa County. CTC has two outpatient clinics, where patients can receive primary, preventive healthcare, behavioral healthcare, and assistance with connecting to community resources and services. CTC also operates two recuperative care, respite centers for people healing from an acute illness or injury. CTC’s respite care program began in 2012 to meet the needs of people who were not ill enough to be in the hospital, but too ill to be on the street. Each 50-bed respite center provides patients facing homelessness, a safe place to rest, heal, and recover. Respite patients receive a bed, meals, daily medical evaluation, 24/7 nursing support, behavioral healthcare, and case management to help connect patients to resources, including stable housing. Approximately 70% of respite patients are connected to affordable, long-term housing upon discharge. CTC also operates four mobile medical units that provide healthcare to people in all parts of Maricopa County. Additionally, our street medicine teams provide backpack medicine and mental health services to people facing homelessness in encampments throughout the county. Most recently, CTC expanded its patient outreach to hospitals with its innovative Health Navigator Program, where a CTC employee is embedded in 7 hospital systems within Maricopa. CTC has received national recognition for its seamless continuum of healthcare for people facing homelessness in Maricopa County.

In 2011, Valle del Sol introduced primary care to its portfolio of medical services. VH provides primary and specialty care services predominantly to underserved, low-income and ethnically diverse populations. Over a nearly 150-year history, VH has built a reputation of quality health care by placing compassion at the forefront of care. The mission of VH is “To provide exceptional care, without exception, every patient, every time.” Accordingly, the System employs a comprehensive approach to care, clinical training, and population health management. Our facilities include the Valleywise Medical Center, three inpatient and three outpatient behavioral health clinics, an Assertive Community Treatment Team, the First Episode Center, the largest Ryan White HIV/AIDs program in the state, and 11 Federally Qualified Health Centers.

Valleywise Health has 11 FQHC sites in Avondale, Chandler, Guadalupe, West Maryvale, Mesa, Peoria and various underserved communities throughout Phoenix, Arizona in Maricopa County, and a mobile unit.

Since 1950, Wesley Community and Health Centers (Wesley) has been serving the Phoenix area by providing key community services. The mission of Wesley “Together We Empower Positive Change,” extended to West Phoenix in 2014 with the acquisition of Golden Gate Community Center. Wesley now has four Federally Qualified Health Centers (FQHC) and community centers in several Federally Identified Poverty Areas in the Phoenix Metro area. Wesley is unique in that it is the only FQHC which provides key primary healthcare services alongside community programs and activities such as, a DES Certified AfterSchool Program, daily fitness programs like Zumba, no-cost diabetes selfmanagement education and nutrition education classes, and Prenatal classes are among the many offerings available to the communities they serve.

Wesley Health Center has 3 sites in Phoenix and 1 in Mesa, and a mobile medical unit.
