N

ew Year's resolution time is nearing. Exercise more, get fit, lose weight, sign up for classes - go for it! Then ouch - old knee pain come's back, nagging bad back returns, achilles tendon is sore again, does this sound familiar? Exercise increases stress on our joints, muscles, tendons, ligaments and tissues which is good if done progressively. All too often we see patients who are relativity sedative to suddenly doing 50-star jumps, 100 burpees, countless lunges and 20 shuttle runs 5 nights a week. Then they get injured unable to exercises for weeks, motivation drops off and the momentum to exercise is lost. Between the ages of 30-50 years old muscle strength and muscle mass are constant. As we enter the fifth decade this begins to change with muscle mass reducing (myopenia) along with strength loss (dynapenia). With muscle strength decreasing by 12%-15% each decade after our fifth, with muscle mass reduction by 30%40% in these years, tissues are made up of type I & type II fibres. With aging we lose type II fibres in our tissues. Type II (fast twitch) fibres have a greater force generating capacity than type I. So, with the loss of type II fibres it's understandable that there will be a reduction in strength of the muscles. It has also been found that with age, type II fibre peak-force ability greatly reduces. These type II fibres have the greatest capacity to absorb load that exercise puts through our tissue so it's

16

understandable that if we lose these loading absorbing fibres, then injuries will occur if we load tissues too high, too much and too sudden. However, if we slowly and progressively increase the loads over time the body adapts and handles exercise loads far better, significantly reducing risk of injury. It's far better to build up your exercise duration and intensity gradually over weeks and months especially if you're just getting back into exercising. Complications associated with sarcopenia, the term given to muscular power and mass loss in ageing will be more prevalent in our community in the future. Relating to reduced mobility and the reduction in preforming daily tasks. Much is known of the benefits of resistance training for greater strength and mass amongst the younger population. However, strength training amongst 60 yrs old has the disadvantage of working to combat, reduce or reverse sarcopenia. Regular resistance training in the older population greatly increased type II fibre, thus increasing the force generating capacity of the muscle. The take home message is, if you do start a new fitness programme for the New Year, start slowly and build up gradually to avoid injuries that might stop you exercising all together. The main part of the outfit is either left to the last, trying to match the shoes to the rest of the outfit, or, for those of us with foot problems, the outfit is built up around the footwear. It makes sense to choose comfortable footwear first when you think that sore feet can cut a good party night short, or, at the very least, make you feel miserable because of the pain or discomfort. For the die-hard fashionistas out there, who insist that the shoes make the outfit and won’t wear anything less than the gorgeous, sparkling beauties that grace the pages of the fashion magazines, we offer you the following advice: 1. WHAT IS GUM DISEASE?

When your gums are sore, inflamed, swollen, bleed or infected this is gum disease. There are two main forms of gum disease: gingivitis and periodontal disease.

WHAT IS GINGIVITIS?

Gingivitis means ‘inflammation of the gums'. This is when the gums around the teeth become very red and swollen. Often the inflamed gums bleed when you clean your teeth. Some people are scared to keep brushing if their gums bleed, but it is really important to continue to do so. Spitting blood out is certainly not pleasant but if you stop cleaning you will not remove the harmful plaque and bacteria that cause the gum inflammation. So persevere, keep brushing, and with good oral hygiene at home the gum inflammation should improve. It is however important to visit your dental team to get the correct advice about

your gums and how best to clean your teeth at home and monitor your gum health.

WHAT IS PERIODONTAL DISEASE?

Teeth are held into place by the jaw bone which is under the gum. When healthy there is usually a very small space between the tooth and the gum where the tooth meets the gum. If there is gum disease, this space between the tooth and gum gets bigger forming a deeper space called a pocket. The deeper the pocket or space surrounding the tooth, the less the tooth is attached to the gum and the more difficult it is to clean; harmful plaque and bacteria can get under the gum and destroy the gum attachment further but also destroy the bone anchoring the tooth. This bone loss leads to the tooth becoming loose and if periodontal disease is not treated can ultimately lead to the loss of a tooth. 23

Gum disease has been linked with other medical conditions such as diabetes, stroke, heart disease, poor pregnancy outcomes and dementia. Although more research is needed to understand the exact link, there is evidence that having a healthy mouth and gums can help improve your general health.

WHAT CAUSES GUM DISEASE AND HOW DO I TRY TO PREVENT IT?

All gum disease is caused by plaque due to the harmful bacteria it contains. There is also a link to periodontal disease with a family history (hereditary), stress, hormonal and diabetes. Smoking too has a strong link with periodontal disease and the advice to all smokers is to stop smoking. To prevent and treat gum disease, you need to make sure you attend the dentist for regular examinations and also for any necessary treatment. However treatment at the dentist or hygienist will not be successful unless you remove all the plaque from your teeth every day. I'm afraid there is a lot of homework for you to do in between dental appointments; you cannot rely on your 3 monthly hygienist visits to do all the work for you. Treatment of periodontal disease will not be successful if you don't clean the plaque off your teeth; your plaque removal at home must be impeccable to remove the harmful bacteria to prevent the disease process progressing. I cannot stress the importance of this enough! Without good plaque removal at home then the disease process will continue. So you must: 1. Brush your teeth twice a day for 2 minutes, especially before bed 2. Clean interdentally every day. Your dental team will advise you of the best products for you to use for every area of your mouth. Examples of some of the products

are floss, interdental brushes and single tufted brushes.

SMOKING AND GUM DISEASE

There is a definite link between smoking and gum disease, so if you are a smoker, stopping is one of the best ways to improve your gum health. Smokers tend to have more hard calculus and plaque, plus smoking reduces the blood supply to the gums. This means infected gums do not heal as well and the reduced blood supply could mask the symptoms of gum disease (bleeding gums in particular). So, if you are a smoker it really is worth the health benefit for your gums if you quit.

WHY VISITING YOUR DENTIST REGULARLY IS SO IMPORTANT FOR YOUR GUM HEALTH

When you visit the dentist, they will screen you for any gum disease by performing a basic periodontal examination (BPE). The dentist will gently feel around your teeth and gums and assess the pocket depths (looseness of the gums), and if there is any bleeding, plaque and calculus. The dentist will then have a score for your gums that will indicate both the condition of your gums and also the treatment that is needed. X-rays may also be needed to see the amount of bone that has been lost. This assessment is very important, so the correct treatment can be prescribed for you.

If you have gum disease you may need multiple

bacteria deep in the pockets. Your gums will then be reassessed to see if there has been improvement. If there is improvement then you will need to have a maintenance treatment plan, but if there is no improvement or worsening you may be offered more specialist treatment or even a referral to see a dentist whom specialises in periodontal disease for further help.

WHAT HAPPENS IF GUM DISEASE IS NOT TREATED?

Unfortunately, if you do not go regularly to the dentist you may not notice the destruction caused by gum disease until you have an abscess or loose tooth or bleeding/sore gums. Over a number of years, disease progression can lead to loss of the bone supporting the teeth, leading to tooth mobility, infections and tooth loss. If the disease is left untreated for a long time, treatment can also be more difficult. It is vital you visit your dentist/hygienist regularly whom will monitor your gum health and advise you of the best treatment needed for you. Periodontal disease treatment is not a quick fix; it needs a lot of motivation and commitment from you the patient. Without good plaque removal at home, gum disease will not improve. Even after gum disease treatment, your gum health will need to be monitored regularly and maintained.

IF I HAVE IMPLANTS, CAN I STILL GET PERIODONTAL DISEASE?

You can still get gum problems around implants that can lead to loss of the bone surrounding and supporting the dental implant. This bone loss can lead to the failure of an implant and it may need to be removed. This condition is called peri-implantitis. It is really important with implants that the surrounding gum health is good with excellent removal of plaque at home every day. It is important to visit your dentist/hygienist to get the appropriate advice on how best to clean your implants and to receive the correct maintenance.

IN CONCLUSION appointments with your dental team. They will remove all plaque and calculus, (hard deposits) from your teeth and show you how to remove plaque successfully yourself, so you can thoroughly and effectively clean all the surfaces of your teeth. A good oral care routine at home with brushing and interdental cleaning is the most important thing you can do to help prevent gum disease getting worse. There is no cure for periodontal disease, but it can be controlled as long as you keep up the home care you have been taught by your dental team. The success of gum disease treatment and maintaining stable gum health relies very heavily on good plaque removal and oral hygiene at home to remove the harmful bacteria that drives this disease process. This message is so important; I'm sure you are sick of reading it but I don't think I've repeated myself enough!!!! Yes I do encourage (nag) my patients just as much to brush their teeth twice a day and to use interdental cleaning products every day. (If they do it, they get the benefit of good gum health plus I stop nagging!) Durham's award-winning Traditional Christmas Festival will return from Friday 29th November 2019 for three days, with a wide range of festive entertainment for all the family in and around the World Heritage Site of the Cathedral and the Castle and throughout the City Centre. The huge heated Craft and Gift Marquee on Palace Green will feature over 180 stalls across all three days of the event. It's open Friday 29th November from 10am to 6pm, Saturday 30th November from 9am to 6pm and Sunday 1st December from 10am to 4:30pm. At the entrance, tickets for the Craft and Gift Marquee will cost £4. In the Cloisters of Durham Cathedral, discover a Regional Food and Drink Producers' Market featuring more than 30 traders. GIVE SOMEONE POWER OF ATTORNEY… BEFORE IT'S TOO LATE Lasting Powers of Attorney are invaluable documents as part of the overall planning of your estate as managing your financial affairs can become more difficult as you get older. If a person is unable to manage their affairs due to mental incapacity and they have not made a valid Enduring or Lasting Power of Attorney, their assets will be frozen as no one (including their spouse and children) has the authority to deal with them. There are two types of Lasting Power of Attorney that can be made, one dealing with your property and financial affairs and the other dealing with your personal welfare.

This power gives your attorney the authority to make decisions regarding your healthcare and medical treatment.

Property and Affairs

This power enables your attorney to deal with your bank accounts and your property. This includes decisions in relation to selling or renting your property, paying your bills, applying for benefits you are entitled to and all aspects of dealing with your bank accounts, savings and investments. 38

Winter Wellbeing

As the temperature drops and the days get shorter, here's some tips to winter-proof your wellbeing

he colder seasons can wreak havoc on our wellbeing. In cold weather, some people are at increased risk of stroke, heart attack and pneumonia, flu, falls and hypothermia. It can also affect people with mental health conditions, such as depression.

Protect yourself from flu

For most healthy people, flu is something unpleasant that they generally get over in a week or two. However, older people, the very young and those with an underlying disease are at particular risk of serious illness. To reduce the likelihood of spreading flu, use tissues to trap the germs when you cough or sneeze, wash your hands often with soap and water and bin used tissues as soon as possible. Contact your GP for details of their flu vaccination programme. And if you don’t qualify for a free NHS flu jab, many local pharmacies offer the vaccine privately for between and £10 and £15.

Eat & Drink Well

It may be tempting during the colder months to reach for unhealthy snacks such as biscuits, crisps and chocolate, especially over Christmas when we find ourselves surrounded by tine of sweets. But being overweight or indeed underweight can affect your

energy levels and your health, so it’s important to eat a balanced diet. Porridge and soup are great winter warmers.; and remember to have plenty of fruit and vegetables and to avoid eating too much processed food that is high in fat and sugar.

GIve yourself a boost

The lack of sunlight nay leave you feeling tired and sluggish. To counteract this get outdoors in natural daylight as much as you can and make time for regular exercise such as a brisk winter walk. Physical activity will lift your mood, leave you feeling energised, and help to keep you fit and healthy. Wrap up in layers and don’t forget a hat, gloves, thick socks and a scarf.

Keep warm

Heat your home to at least 18°C (you may prefer your living room closer to 21°C) and wear several layers of light clothes rather than one chunky layer. On cold nights, keep your bedroom window closed because breathing in cold air can increase the risk of a chest infection. And use a hot water bottle or electric blanket to keep warm in bed - but don’t use both at the same time.
