N-CHIP Lead Health Informaticist NOTE: This job description does not form part of the employee’s contract of employment but is provided for guidance. The precise duties and responsibilities of any job may be expected to change over time. 1. JOB TITLE: Lead Health Informaticist 2. REPORTS TO: The Lead Health Informaticist will be a direct report to the HIT Program Manager with guidance from the Medical Director of the Fort Drum Regional Health Planning Organization. PURPOSE: Provide support for enabling effective and efficient access to clinical information as necessary to support the Patient Centered Medical Home (PCMH) target population and improve the quality of health care for North Country patients through the use of health information technology (HIT). 3. PRINCIPAL DUTIES
Act as a clinical liaison between the physician practices, the hospitals, the consultants, services EHR vendor(s) and the N-CHIP Provider Executive Committee
Determine the needs and requirements for clinical applications in support of the PCMH target population
Act as an informational resource for supported practices that are seeking Meaningful Use (MU) certification
Maintain current knowledge of CMS rulings on Meaningful Use certification
Support organizational objectives in the areas of process flow and reporting for quality, outcome and care coordination measures
Collection of research data from participating practices and hospitals as needed to meet objectives and care coordination measures
Support health research by linking activities of the grant to results or outcomes, work closely with HITEC on evaluation initiatives
Record research results/outcomes on N-CHIP SharePoint site
Seek to publish research results/outcomes to enhance public knowledge of the HEAL 10 project
Encourage learning by developing clinical decision support, prompting and providing feedback loops to project stakeholders
Provide monthly reports to the Provider Executive Committee and subcommittees
Create and report status of the program to the HIT Program Manager of FDRHPO (weekly)
5. CONTEXT: The Lead Health Informaticist (LHI) will provide support for enabling access to clinical information necessary to support improvements in care coordination and disease management. The LHI will assist the HIT Program Manager in the support of the PCMH target population and promote the implementation, adoption, training and effective use of interoperable EHR and other health IT tools by providers. The LHI is also responsible for coordination of the support necessary for practice transformation, reimbursement changes and patient engagement, to vastly improve the availability and use of health information and to help ensure that the expected quality and efficiency goals are realized from interoperable health IT. The LHI will be the liaison between the private practices, the hospitals’ clinical areas and the IT departments of the various clinics and providers affiliated with those hospitals.
6. DIMENSIONS: The Lead Health Informaticist (LHI) will be a direct report to the HIT Project Manager and the HEAL 10 Medical Director. The primary responsibility of the LHI is to determine the needs and requirements of the clinical applications required to support a Patient Centered Medical Home (PCMH) model and implement a chronic disease intervention model in Jefferson, Lewis and southern St. Lawrence Counties in Northern New York. 7. QUALIFICATIONS
Licensed RN (New York State Licensure Preferred)
Bachelor degree in Nursing or Informatics, Master’s preferred
3+ years acute care clinical experience that includes demonstration of clinical education
Ability to use the Patient Centered Medical Home Model as the standard for equitable, efficient, and effective healthcare services.
Experience with Meaningful Use registration and attestation
Ability to manage projects, prioritize requirements, and meet outcomes
2+ years experience with clinical and information technology, including clinical software applications with a working knowledge of multiple EHR software system(s); eCW, Medent and Vitera(Sage) preferred
Comprehensive understanding of clinical principles to evaluate information system needs
Technical teamwork and interpersonal skills
Flexibility, adaptability and a willingness to learn
Strong verbal and written communication skills
