Introduction  HAPU are major complications for hospitalized patients.  They results in non-healing wounds, sepsis, and even death  These injuries amplify the burden of care  A reduction in HAPU would certainly reduce the cost of care  This presentation focuses on the implementation of a Pink Paper Reminder System to resolve HAPUs


Targeted Goals

 To enhance the quality of care  To reduce the rate of HAPU in the hospital.  To increases nurses' attention in providing care  To offer helpful reminders for the prevention of pressure ulcers


Measurement of change

Comparison of pre-and post-implementation results will help to determine if the anticipated change was achieved. Data for comparison will include:Incidences of pressure injuries, The burden and cost of care The length of hospitalization Number of deaths. Level of satisfaction


Projected Outcomes

 The new System is expected: To assist nurses to focus attention on patient care  To improve nurse’s compliance with care delivery protocols.  To reduce the incidences of pressure injuries.  To reduce the burden and cost of care.  To protect patients from unnecessary suffering


Actions to achieve the outcomes

 The project activities include:  A brief educational session.  The categorization of at-risk patients.  Identification of suitable preventative measures  Notifying the hospital administration of the proposed intervention


Stakeholders  Stakeholders will be: the hospital administration,  nurses,  nurse informaticists,  physicians, and patients.

 The nurse informaticist will design the pink paper.  The nurses will assess the patient’s level of risk.  Nurses will all provide injury prevention care  The hospital management will allocate the required resources.


Budget

 The projected budget for the project is approximately $10,000.  Finances will cater to the printing costs and wages for facilitators  The hospital accountant will be the project finance manager  The accountant will approve the release of funds  The accountant will follow-up with the team to ensure money was utilized appropriately.


Timeframe

 The project will be implemented over one month.  Project milestones will include: appropriate identification of the at-risk patients  The use of the pink paper reminder system to flag high-risk patients.  The identification of the best pressure ulcer prevention tactics


Potential Setbacks

 Resistance among nurses  A negative attitude towards the new system.  Insufficient finances


Risk Management Plan

 Privacy and confidentiality of patients will be safeguarded  Pressure ulcers prevention tasks will be performed for all patients  No patient will be denied treatment based on lowpressure injury risk.


Evidence-Based Model

 The best evidence base model is the John Hopkins, Evidence-based Practice Model.  This model is clinician focused  It allows for appropriate application of interventions  It facilitates the cultivation of a culture guided by evidence.


Evaluation Plan

 Evaluation will focus on major project activities  The project budget and resources utilization will be evaluated.  The time used to implement the project will also be assessed.  The ability of the new system to reduce the incidence of pressure ulcers will be assessed.  Data will be collected through observation of hospital records
