Community resources are useful in providing information on the diagnosis, prevention, management and risk factors associated with various chronic illnesses. Health screenings and outreach enable early diagnosis for better treatment outcomes and sensitize healthy lifestyle practices to prevent chronic conditions. Lastly, referrals and follow-ups are important to ensure the continuity of care during transitions from different settings (Clarke, Bourn, Skoufalos, Beck & Castillo, 2017). Evidence-based approaches can be used to guide all these measures to achieve better health among the chronically ill.
