Chief cells secrete digestive enzymes, such as pepsinogen.

The stomach empties in less than 4 hours following a liquid meal, or it may take as long as 6 hours to empty following a meal high in fat.

Swallowing signals the stomach to relax so as to allow food to enter. Once the food has entered, the stomach goes to work, mixing the food with gastric juices and propelling it toward the duodenum.

Peristaltic contraction

Pyloric sphincter

Chyme

1 Contractions originate in the fundusand move toward the pyloric region. Starting out gently and growing progressively stronger, wave-like peristaltic contractions break down food and mix it with gastric juices to form chyme.

2 Chyme moves toward the pyloric sphincter, propelled by the rhythm of the contractions. Under the pressure of a contraction, about 30 ml of chyme squirts into the duodenum.

Duodenum

3 The stomach continues to churn and mix its contents as it gradually releases chyme into the duodenum. This continues until the stomach empties, which takes about 4 hours following a typical meal.

The Body at Work—Brief summaries explain physiological processes.

Older persons, those with sedentary lifestyles, smokers, and women taking oral contraceptives also have a higher risk of hypertension.

CLIENT COMMUNICATION

Videos of interactions between health care providers and patients illustrate how to communicate effectively.

Persons with hypertension may remain asymptomatic for months or years or until vascular changes in the heart, brain, or kidneys occur. There may be vague symptoms of light-headedness, tinnitus (ringing in the ears), nocturia, a tendency to tire easily, and palpitations. Headaches in the occipital area can be a symptom as well as blurring of vision.

Diagnostic Procedures

Diagnosis is relatively easy and usually occurs during a medical examination. Blood pressure readings taken on at least two separate occasions after the individual has rested will show pressure greater than 130/80 mm Hg. It is important that a history of blood pressure readings be kept for comparison, because blood pressure can vary

Because the client’s lifestyle likely needs modification, stress reduction and weight loss should begin. A nutritionist can assist with diet modification. Encourage regular physical activity. It may be helpful for clients to purchase a home blood pressure kit in order to regularly monitor their blood pressure.

Author’s Note: Take a few moments to view the video on the importance of diet in treating hypertension on http://FADavis.com, keyword Tamparo.

Prognosis

Client Communication sections remind students to think about teaching opportunities for their clients and describe important topics to be discussed.

The prognosis is good if the disorder is detected early and if clients carefully follow the prescribed treatment regimen. Adequate control is key. The younger the age at which the person is diagnosed with hypertension, the greater is the reduction in life expectancy if untreated. Complications may include atherosclerosis, CVA, heart failure, kidney failure, and myocardial infarction.
