THE ELDERCARE CONTINUUM HOW TO KNOW WHEN TO PLAN

Few adults are prepared to deal with aging parents. As author Stella Henry, RN, author of The Eldercare Handbook (available on Amazon) puts it, "We are all pioneers in this strange frontier of caregiver and decision maker for parents. Just as children do not come with instructions, neither do parents."

In our practice, we witness firsthand the difficult choices and dynamics families face. But I have personal experience, as well, from when my parents began their journey through the elder care continuum. In our experience, there are two primary ways one finds them self on the eldercare continuum. First, a medical crisis. Second, after a long and protracted decline. While I haven’t crunched the numbers, I can comfortably say that a very high (85% or more) portion of the families seeking help defraying long-term care costs are in crisis, meaning that they are imminently facing decisions about where to place a loved one and how to pay for long-term

care. And that, my reader, is not good for the parent, the family, or the parent’s nest egg. So, how can one recognize that a parent may require long-term care and/or may not be safely cared for at home? An excellent article about 12 warning signs can be found at SeniorsSafetyAdvice.com. Most of us that deal with elders going through the elder care continuum know that a quick, peaceful death is rare. As people live longer, it is more likely that death will follow a period of protracted illness or disability. I strongly recommend meeting with an Elder Law Attorney or an Aging Life Care Expert sooner rather than later. VA WILL CHANGE RATINGS FOR SOME MEDICAL CONDITIONS

The Department of Veterans Affairs has proposed changes to the way it rates respiratory, auditory and mental disorders body systems. VA officials have explained that these proposed updates will enable them to incorporate modern medical data and terminology to provide veterans with more accurate and consistent decisions. According to a recent VA news release, proposed updates include:

Modernizing the evaluative rating criteria for sleep apnea, using developments in medical knowledge to evaluate it based on its responsiveness to treatment, bringing the rating criteria for sleep apnea more closely in line with the stated purpose of the rating schedule.

Evaluating mental health conditions based on a more robust and holistic approach that assesses how impactful the disability is to cognition, interpersonal relationships, task completion, life activities and selfcare. Additionally, the proposed evaluation criteria include a 10% minimum evaluation for having one or more service-connected mental health conditions and will no longer require “total occupational and social impairment” to attain a 100% evaluation.

Evaluating tinnitus (ringing in the ears) as a symptom of the underlying disease that causes it, rather than as a stand-alone disability.

If the proposed changes are finalized, veterans who currently receive compensation for a service-connected condition can apply for increased compensation, but no reductions shall be made unless an improvement in the veteran’s disability is shown to have occurred. An extremely important set of bills recently came to my attention, and I thought I would share them with you, in hopes that you will join me in the fight to make these bills law.

These should be a no-brainer bills to pass. The disparity of the civilian vs. military compensation makes this a very easy issue to argue with a legislator. As of this moment, there are only 45 co-sponsors to this bill in the House (out of 445) and 10 co-sponsors in the Senate (out of 100.) The last similar bill to this had about 90% co-sponsorship between both parties, where this is co-sponsored by less than 10%. An important note about these bills - the survivor benefits are paid out as dependency and indemnity compensation (DIC) meaning the monies paid to the survivor is done so tax-free through the VA. This bill does not change the nature of this income - those already receiving benefits as a surviving spouse will simply get a raise - this will not make your benefits taxable. Everyone has one Representative and two Senators. Contact them today and either kindly ask them to become a co-sponsor on these bills, or if they're in the minority who have already done so, please contact them and thank them for their support. WHAT IF I NEED HELP PAYING FOR MY MEDICARE HEALTH CARE COSTS? MEDICARE SAVINGS PROGRAMS

1. Qualified Medicare Beneficiary (QMB) Program: If you’re eligible, the QMB Program helps pay for Part A and/or Part B premiums. In addition, Medicare providers aren’t allowed to bill you for services and items Medicare covers, including deductibles, coinsurance, and copayments. If you get a bill for these charges, tell your provider or the debt collector that you’re in the QMB Program and can’t be charged for Medicare deductibles, coinsurance, and copayments. If you’ve already made payments on a bill for services and items Medicare covers, you have the right to a refund. If you’re enrolled in a Medicare Advantage Plan, you should also contact the plan to ask them to stop the charges. In some cases, you may be billed a small copayment through Medicaid, if one applies. Note: To make sure your provider knows you’re in the QMB Program, show both your Medicare and Medicaid or QMB card each time you get care. If you have Original Medicare, you can also give your provider a copy of your “Medicare Summary Notice” (MSN). Your MSN will show you’re in the QMB Program and shouldn’t be billed. Log into (or create) your secure Medicare account at Medicare.gov at any time to sign up to get your MSNs electronically. 2. Specified Low-Income Medicare Beneficiary (SLMB) Program: Helps pay Part B premiums only. 3. Qualifying Individual (QI) Program: Helps pay Part B premiums only. Applications are granted on a first come, first-served basis. 4. Qualified Disabled and Working Individuals (QDWI) Program: Helps pay Part A premiums only. You may qualify for this program if you have a disability, you’re working, and you lost your Social Security disability benefits and premium-free Part A because you returned to work. If you qualify for a QMB, SLMB, or QI Program, you automatically qualify to get Extra Help paying for Medicare drug coverage (Part D). Important: The names of these programs and how they work may vary by state. Medicare Savings Programs aren’t available in Puerto Rico or the U.S. Virgin Islands. How do I qualify? In most cases, to qualify for a Medicare Savings Program, you must have income and resources below a certain limit. States have different limits and ways of counting your income and resources, so you should check with your state Medicaid office to see if you qualify. 12

MEDICAID

Medicaid is a joint federal and state program that helps pay health care costs if you have limited income and (in some cases) resources and meet other requirements. Some people qualify for both Medicare and Medicaid. What does Medicaid cover? • If you have Medicare and full Medicaid coverage, most of your health care costs are covered. You can get your Medicare coverage through Original Medicare or a Medicare Advantage Plan. • If you have Medicare and full Medicaid coverage, Medicare covers your prescription drugs. You automatically qualify for Extra Help paying your Medicare drug costs. Medicaid may still cover some drugs that Medicare doesn’t cover. • People with full Medicaid coverage may get coverage for services that Medicare doesn’t cover or only partially covers, like nursing home care, personal care, transportation to medical services, home- and community-based services, and dental, vision, and hearing services. How do I qualify? Medicaid programs vary from state to state. They may also have different names, like “Medical Assistance” or “Medi-Cal.” Each state has different income and resource requirements. Call your Medicaid office for more information and to see if you qualify.

MEDICARE-MEDICAID PLANS

Medicare is working with some states and health plans to offer demonstration plans for certain people who have both Medicare and Medicaid and make it easier for them to get the services they need. They’re called MedicareMedicaid Plans. These plans include drug coverage and are only in certain states. If you’re interested in joining a Medicare-Medicaid Plan, visit Medicare.gov/plancompare to see if one is available in your area.

State Pharmacy Assistance Programs Many states have State Pharmacy Assistance Programs that help certain people pay for prescription drugs based on financial need, age, or medical condition. To find out if there’s a State Pharmacy Assistance Programs in your state and how it works, call your State Health Insurance Assistance Program (SHIP). Pharmaceutical Assistance Programs (also called Patient Assistance Programs) Many major drug manufacturers offer assistance programs for people with Medicare drug coverage (Part

Program of All-inclusive Care for the Elderly (PACE) PACE is a Medicare and Medicaid program offered in many states that allows people who need a nursing home-level of care to remain in the community. Supplemental Security Income (SSI) benefits SSI is a cash benefit Social Security pays to people with limited income and resources who are blind, 65 or older, or have a disability. These benefits aren’t the same as Social Security retirement benefits. You may be able to get both SSI and Social Security benefits at the same time if your Social Security benefit is less than the SSI benefit amount, due to a limited work history, a history of low-wage work, or both. If you’re eligible for SSI, you automatically qualify for Extra Help, and are usually eligible for Medicaid. SPRING FEVER Thinking about gardening or desiring replacement plants after being hit by the winter freeze? Visit DeLand’s Wildflower and Garden festival on March 26th. Check out the vendors and educational programs. As a child, I loved spending time in the gardens tended by my mother, grandmother and aunts. They seemed the happiest among their vegetables and flowers. We harvested berries, figs, beans, corn, cucumbers and tomatoes for canning and pickling. I didn’t know how special that time was until it was too late. My family gardeners are gone. Mama used to say, “It’s not just the flowers, it’s the hours spent working in the soil that helps you.” In the garden we can experience healing from sorrow and concerns about tomorrow. Many of us become gardeners after suffering a loss. Perhaps we have learned the solace, comfort, and faith we feel in knowing that we can help nurture life. Each month I have the opportunity to interview gardeners for DeLand Garden of the Month; two recent recipients were both over eighty years old. They expressed their joy of being outdoors and benefiting from gardening. One mentioned that he had no idea how difficult it was for a butterfly to go through its life cycle. Perhaps that is one of the benefits; we observe struggle as part of nature. Caterpillars and butterflies have predators, difficult food conditions, as well as the threat of pesticides and development destroying their habitat. If a plant doesn’t have the right soil combination and moisture it withers and dies. We too need to nourish ourselves. We become closer to the symbiotic relationship of all life when we are outdoors. Urban sprawl encroaches not only on our way of life but also the source of our breath, as trees provide us with oxygen. And of course, we need pollinators; they are necessary in contributing to our food supply. Consider gardening or being out in nature as your green therapy. Spending time outdoors has many benefits. We can experience the effects of color as we observe harmonious greens. The color yellow in the flowers and butterflies offers an emotional stimulator; blue is calming, and red contributes enthusiasm, warmth, energy. Enjoy a color walk outdoors and focus on what you see and feel. Studies reveal that being in the sunshine improves sleep, reduces inflammation, and stimulates the functioning 14

of organs. Bathing in morning sunlight gives the benefit of vitamin D; spending time outdoors improves wellbeing and regulates heart health by synchronizing your heartbeat, reduces stress and may add to longevity. Our green spaces help us to connect with life and its vitality. Grounding is a way to connect energetically. Remove your shoes and feel the earth below. FILLING AN INCREASING NEED: CARING FOR THOSE WITH DEMENTIA Dementia usually starts with occasional instances of being forgetful, but over time, it progresses to cognitive decline… times when certain daily routines become more challenging. Dementia has many faces. The one common denominator is that is most always creates and increasing need for care from a family member or other caregiver. Spouses of someone with dementia often struggle with releasing control of daily care, and may feel guilt that they should be able to somehow do it all. The adult children of people living with dementia may step in and try to assist when needed, but work responsibilities make it difficult to balance family needs with work demands.

A GROWING NEED

Adult Day Service programs allow respite care and alleviate caregiver stress. In many instances, the great amount of stress that caregivers experience can cause their health to decline. Both physical health and emotional health are at stake. • In the past five years, more than 40 million family caregivers provided 37 billion hours of care for loved ones. The value of this care is estimated at $470 billion. • At least 20% of adult children are taking care of an older parent. • About 85% of family caregivers in the U.S. do not receive any respite care. Source: Aging in Place Activities that stimulate cognition, such as math and word games are valuable for keeping ‘mentally fit’ for those with early stage mild cognitive impairment. Having a place to go where you are welcomed and accepted creates not only a feeling of wellbeing but a sense of purpose and change of scenery from the home environment. “We improve quality of life and provide a sense of purpose and accomplishment through arts and activities, promote individual independence, eliminate social isolation, reduce loneliness and depression, promote a sense of bonding with others through activities, provide cognition stimulation, a chance to make friendships and have meaning social interaction,” says Meg Ann Clark, Adult Day Services Manager for Senior Friendship Centers in Venice, Florida. It is important to recognize the changes in cognitive capabilities in our loved ones and to realize that it is common in older ages. In fact, one in three people will have some form of dementia in their life. It is imperative to dispel the stigma that there is anything ‘wrong” with these changes. Compassion is key as a caregiver. By focusing on the gifts that remain, we honor and respect the individual. In doing this, we help support our loved ones throughout the journey of aging. STOPPING ALZHEIMER’S BEFORE IT BEGINS! It is safe to say that most, if not all, of us have in some way been affected by Alzheimer's. Whether we are currently learning to live with this terrible disease on a personal level or, tragically, have seen what it can do to a friend or loved one, it is very apparent that something needs to be done to eradicate Alzheimer's disease from our planet.

Alzheimer's is the most common form of dementia, the severe loss of memory and other cognitive abilities that interfere with daily life. It accounts for 60 to 80% of dementia cases. Alzheimer's is a progressive disease in which dementia symptoms gradually worsen over time. Mild memory loss in the early stages will increase in severity. As the disease progresses, most patients will lose the abilities that we take for granted daily. For example, they will experience severe memory loss, the inability to carry on conversations, and the mental capacity to respond to their environment. On average, a person with Alzheimer's will live for four to eight years but, in some cases, can live for as long as 20 years. Sadly, at this point in time, there is no cure. YET! The darkness and hopelessness associated with this terrible disease might finally be lifted. An end could very possibly be in sight! In conjunction with mega-pharmaceutical company Eli Lily & Co., Care Access is planning on doing just that. The team has launched a groundbreaking nationwide Phase 3 clinical study. They hope to discover a clear way to delay the onset of Alzheimer's and potentially stop the disease before it starts. Ironically, the first part of this study begins with a task that we take for granted – a simple blood draw. The study is seeking healthy adults between the ages of 55 and 80 who wish to discover if they are predisposed to Alzheimer's. After a brief screening process, volunteers will receive a free blood draw that could indicate if a patient is a highrisk candidate for cognitive and functional decline associated with the disease. Next, their blood will be analyzed for biomarkers linked to Alzheimer's. If positive, they will be invited to partake in the study's second phase. Volunteers will receive a newly developed medication that could potentially slow and end the cognitive and physical issues linked to the disease. This medication contains an antibody called Donanemab, with early symptomatic Alzheimer's. Research shows that adverse changes in the brain

can occur up to 20 years before the first symptom of Alzheimer's disease rears its ugly head. Therefore, the purpose of the clinical study is to discover if Donanemab, when taken early in life, can STOP Alzheimer's before it even begins. The end to Alzheimer's could very well be insight. This clinical study will hopefully prove that "a treatment that can reverse or prevent the onset of symptoms will ultimately lead to a future where Alzheimer's can be a disease of the past."
