CONCORD – MOUNT CARMEL COLLABORATION Integrated Care Navigator
NEED Mount Carmel Health Stations offer primary care, preventative and OB services to
members of the community who are uninsured or underinsured.
Three locations – Parsons & Whittier, Refugee Road, and Reynoldsburg – They see
200-300 patients per month
Many patients have mental health needs that are not being addressed or only
addressed in a primary care setting
Many patients do not follow through with prescribed medical testing Many patients do not have a home environment that is conducive to medical
adherence for chronic illnesses
SUPPORT Providing assistance with applying for and managing benefits Providing assistance with finding housing, or securing resources to manage current
residence
Linking with a psychiatrist and/or counselor in a Community Mental Health Center Assists in finding additional medical/needed resources in the community
(Charitable Pharmacy, food pantries, etc.)
ADVANTAGES Increased communication between primary health care provider and mental
health providers
“one stop shop” mentality Increased likelihood of follow through with onsite assistance and coordination Reduced stigma for those seeking mental health services
NUMBERS  Have seen around 120 individuals over the past year. 2 days/week at Parsons
location, 1 day at Refugee location (in last 3 months)
 Provided close to 200 hours of service
LESSONS LEARNED The need for consultations between PCP’s and Psychiatrists to free up psychiatric
time and to promote ongoing medical care for common psychiatric conditions by the PCP
Many individuals with a low socio-economic status do not have access to the
resources needed to follow through with medical requirements
Some of these individuals do not have a Behavioral Health diagnosis that could
include case management services that would assist in accessing needed resources
