he holidays are a wonderful, but stressful time of year. In fact, some say that the start of school in September is what triggers the annual stress-fest all the way through January. But don’t let that exhaustion and worry show up on your face. Botox and injectables have come a long way. The Future Of Treating Liver Disease

020 brings with it significant hope for advancements in liver disease across many different conditions. We will see the first medication approved for the treatment of non-alcoholic fatty liver disease. Fatty liver disease is the most common liver disease in America. Currently, diet and exercise are the only treatments. Drug development for its treatment has been slow with most drugs failing to show efficacy or being associated with significant side effects. Obeticholic acid, however, a drug already approved for the treatment of primary biliary cholangitis, has shown promise in treating fatty liver, and will likely be approved by the FDA in late March to

Sadly, there is a rise in the number of new hepatitis C cases due to the opioid epidemic, especially in our youth. The challenge in 2020 is to find and treat these patients while simultaneously treating their opioid addictions. If this can be done, we can prevent the spread of hepatitis C and hopefully get back on track to eliminate this disease in the U.S. by the stated goal of 2030. Perhaps the real need and excitement revolves around hepatitis B. More potent vaccines were recently introduced to better prevent this infection. This common condition can

be treated with essentially lifelong suppressive medications, which are not curative therapy. These treatments have been shown to reverse fibrosis and to reduce but not eliminate the risk of developing liver cancer. There is now real interest in developing new therapies to not only treat but cure hepatitis B. Several new diagnostic tests are being introduced,which will allow physicians to better monitor patients for response. 2020 looks to be an exciting year in liver medicine. I hope that by this time next year, there will be many new and exciting developments to discuss that will improve the lives of people suffering from liver disease as well as their families.

hen Hicksville resident Arjun Mainali was attending college in his home country of Nepal, he read a story that would change his life and hundreds of others. The piece, entitled “The Blood Donor,” inspired Mainali to give blood for the first time at age 19 in 1987. “I have seen, so many times, [people] were afraid to donate blood as they used to think donating blood makes their body weaker,” he explained. “Seeing this situation, I decided to donate blood. I thought maybe I could save people.” Mainali has since gone on to donate blood 172 times across 17 countries, helping to save the lives of 516 people. January is National Blood Donor Month, the perfect time to resolve to be a regular blood donor and help save lives. Oyster Bay Town Supervisor Joseph Saladino proclaimed Dec. 22, 2019 to be “World Blood Donor Arjun Mainali Day” in the Town of Oyster Bay in recognition of his astounding accomplishments. a heart transplant and still needs blood transfusions. “Blood donors give people a second chance to live everyday lives,” Lentini said. “If it wasn’t for donations, I wouldn’t be here today. Donors are angels.” The 36-year-old Lentini frequently shares her personal story during New York Blood Center events to encourage others to donate. “The greatest thing is when someone comes up to me and says they want to donate after they hear my story,” she said. “They say they hope to help save lives. They are giving the greatest gift.” For 32 years, Mainali has dedicated his life to educating, advocating, encouraging, promoting and creating awareness about the importance of blood donations. He has traveled to six continents and 14 states for the sole purpose of donating blood. Since moving to Hicksville in 2000, Mainali has also organized dozens of local donation drives. Everything You Want To Know About Bunions But Never Asked

bunion deformity is one of the most common foot conditions. The big toe shifts toward the other toes and the inside of the joint bulges inward. The widening of the foot causes pain from tightness in shoegear something I call “shoe conflict.” As the joint becomes misaligned more pain occurs. This deformity can be corrected with foot surgery when indicated.

Definition and Symptoms

The medical term for bunion is Hallux Valgus which in Latin means big toe bending outward. Bunio means enlargement in Latin. The base of the big toe joint pushes inward as the big toe itself pushes over toward the other toes. The joint bulges inward causing a medial eminence (bump), which rubs in shoes and becomes red and inflamed over time. This pain and rubbing left unchecked often leads to greater complications.

One study of aboriginal tribes showed that barefoot walkers developed bunions at a similar frequency as more traditional shoe wearing populations. Shoe gear may exacerbate symptoms of bunions but aren’t necessarily causative.

Treatment

puts extra pressure on the big toe joint. This extra stress makes the big toe joint (first metatarsophalangeal joint) unstable which causes the attaching tendons and ligaments to pull the bone out of place. Over time the joint moves over and a bunion forms. Overly elastic ligaments can also contribute to a bunion’s growth.

Prevalence

About 30 percent of the population develops bunions, which indicates how common they are. Genetics play a role as do foot type and structure. Women suffer with bunions more than men, but men are not immune.

Conservative measures are tried first. These include: paring of calluses, changing shoegear, altering activity level, paddings and toe spacers, custom prescribed orthotics, anti-inflammatory injections, PRP (platelet rich plasma) therapy and physical therapy. When conservative measures do not work or the deformity is too large surgery may be recommended. Modern bunion surgery offers sophisticated and gentle options. Depending on the patient and type of bunion the right procedure is determined. I often employ a near pain-free procedure with a smaller, mini-invasive approach. The biomechanics of the foot and the patient’s lifestyle are taken into account. The days of crutches and

non weight-bearing are behind us. A cast post procedure is not necessary. With proper bandaging and use of a surgical shoe one can actually walk and bear weight right away, albeit carefully and only as needed during the first week. Stitches are removed around two weeks after surgery. Bandages are changed weekly. Each patient varies; some return to work in a couple weeks. Others may return after 4-5 weeks. Today’s diagnostics and procedures available for bunion correction assure an optimal result. Less pain, less swelling and smaller and barely visible incisions have become commonplace when a thoughtful plan is used.
