Ontario Public Health Standards • “Addressing the determinants of health and reducing health inequities are fundamental to the work of public health in Ontario” • “Effective PH programs and services consider the impact of the determinants of health on the achievement of intended health outcomes” • “As a sector, PH not only acknowledges the impact of the determinants of health but also strives to influence broader societal changes that reduce health disparities and inequities… PH has a leading role in fostering relationships to support broader health goals to achieve the best possible outcomes for Ontarians.”

• Shared goal of population health improvement; • community engagement; • aligned leadership; • infrastructure to ensure sustainability, and • sharing and collaborative use of data and analysis


Primary Care and Public Health (IOM 2012) 5 recommendations:

• identify coordinated funding streams; • create common research and learning networks; • develop workforce together; • support pilots that aim to collaboratively affect the SDOHs, and • develop a strategy for a strong PC-PH infrastructure to support agencies to play their roles in furthering population health goals
