Aim Decrease barriers to health care for the Mennonite/Amish Community in East Elgin by providing primary health care services via our Mobile Bus to: those without a Physician/Nurse Practitioner those who are not covered by OHIP those with transportation barriers those experiencing language/cultural barriers

Goal
Start offering primary health care services February 9th, 2017
Every 2nd and 4th Thursday of the month (9:30-2:20)
Direct billing for non-insured clients
Achievements
103 new clients (rostered) 480 total encounters by a provider (approx 10 visits per day) 30% of clients seen are without OHIP coverage drop in social group for mothers and their children educational services on health through FESPA Newcomer Champion Award for Excellence in Diversity and Inclusion
CCHC currently pays for DI/labs-looking for funding
Next Steps Goals:
Expand mobile locations to Port Burwell, Vienna, and/or Straffordville Explore funding opportunities Increase availability to weekly Expand health promotion programs Improve data collection efficiency
