WE DON’T WANT TO SAVE CHILDREN’S LIVES Children’s lives shouldn’t need saving from entirely preventable causes. Every day tens of thousands of children worldwide die needlessly from illnesses such as measles, tetanus and diarrhoea. UNICEF wants you to help prevent these deaths. We believe that one child dying is one too many. We believe in zero and we desperately need your help. Call 01 878 3000 or visit unicef.ie today to give your support.

built on a philosophy of patient care Mr Mark Towey, CEO, St Joseph’s Hospital.

S

t Joseph’s Hospital has been an iconic part of Sligo Town’s landscape for over 60 years and today continues the tradition of excellence in healthcare provision as one of the leading independent medical providers in the north west. Here at St Joseph’s we have worked exceptionally hard, since the change of ownership and reopening as an independent facility in 2004, in terms of healthcare provision and planned, elective surgical procedures. Our hospital is a 19 bed, nonemergency facility which aims to be the hospital of choice for those seeking elective procedures following referral from their General Practitioner. We have assembled and retained a strong team of full-time private and public consultants, over 35 in various specialities, and all treatments here at St Joseph’s are consultant-led. I think that the work that we have done over the past eight years is reflected in the fact that we secured internationally recognised JCI accreditation in both 2009 and again,

recently, in 2012. Receiving this accreditation is a proud moment for us because it helps validate the efforts which we make to ensure our patients receive the best possible care in the friendliest and safest possible environment. St Joseph’s Hospital has worked hard to complement the efforts of its staff with investment in technology and services where necessary and we remain committed to further developing the hospital to keep apace of developments in patient care. Our greatest strength is the staff here, many of whom have been with the hospital for many years. We have excellent crossdepartmental relations and extremely competent staff working at all levels within the hospital. Like all healthcare providers we strive to continuously improve our services to patients. Central to this is our ability to retain key staff and to manage costs effectively in order to ensure St Joseph’s can continue to develop its services in a structured and costeffective manner. We greatly appreciate the loyalty also of General Practitioners and other healthcare professionals

Excellence in healthcare,

throughout the North West region, which provides us with confidence going forward in continuing to improve patient services here at St Joseph’s. We have established a GP liaison service to network with GPs throughout the region and we look forward to working with them in improving and promoting the services we provide. My challenge as CEO is to maintain and increase the strength of the St Joseph’s brand in the North West region and to work with staff here at the hospital to continuously maintain the high standards we have set and which have been reflected in our international standard of accreditation. We are recognised by all the major insurance companies, both new and established and very much aim to remain an accessible and affordable option to patients. The philosophy behind St Joseph’s Hospital, throughout our history, has been to “create an excellent patient experience, with the highest standard of medical and nursing care.” With the assistance of our staff, the support of the local medical community and the trust of our patients I am confident we can continue to provide this at St Joseph’s Hospital Sligo. RECOGNISED

Maria Bailey, Accreditation and Quality Manager at St Joseph’s Hospital, explains why adherence to exceptionally high standards is a continuous process.

A

lthough modest in size, St. Joseph’s Hospital has the same standards required as any other hospital. This is consistent at all levels here, from the managerial to the clinical, to the administrative and any other departments. Standards have been set by the Joint Commission International (JCI) and we have to maintain these standards to maintain our recognition. The hospital was recently reaccredited up to 2015 as being in accordance with JCI international standards. Accreditation promotes patient confidence by assisting an organisation to ensure a safe and effective environment for the delivery of healthcare. The needs of the patient, the patient’s family and employee needs are at its core. JCI accreditation is acknowledged worldwide as a symbol of achievement and gives us the opportunity to compare our services with other hospitals. It is a voluntary process whereby an accrediting body assesses a hospital to determine if the services that are being provided meet the standards set out by the accreditation body. Once those standards have been met, the challenge is to maintain them.

Maria Bailey, Accreditation & Quality Manager.

ACCREDITATION AND QUALITY

INTERNATIONALLY

Maintaining accreditation status means ensuring that adherence to quality is part of the work of everyone who works at St. Joseph’s. We have an excellent team spirit at the hospital and inter-departmental communication is very strong so there is a real sense that we are all pulling in the same direction for the ultimate benefit of our patients. St. Joseph’s occupies an iconic building in Sligo town and it really is woven into the fibre of the area. The hospital has changed in how it is run over the years but it still retains its reputation as a facility where patients can receive the highest quality care in a warm, friendly and professional environment. I also have a role in reviewing a patient’s prescriptions and monitoring their prescriptions throughout the course of their treatment and in terms of their post operative care or any further treatment that may be required.

Mary Quill, Pharmacy Manager at St Joseph’s Hospital.

I

have been with St Joseph’s Hospital for just over five years, having studied in Dublin and graduated with qualifications in pharmacy before working in St James’s Hospital and also in the UK where I also studied for a Master’s in Clinical Pharmacy and worked with the National Health Service (NHS) there. I returned to Sligo in 2006 and took up my role here in the hospital in 2007. My role in the hospital is the management of medicine here, overseeing all roles which pharmaceutical medications may have here in the hospital. The vast majority of our cases here are day-cases or are done on an outpatient basis, so I interact with the patient by explaining what medication may be prescribed. The pharmacy purchases, stores

The priority for any pharmacist is patient safety so to that extent my role is to monitor the medication the patient receives during the course of their treatment at St Joseph’s. When a patient begins treatment here, it is my job to analyse a patient’s medical history with any medications that they have been prescribed. Each medication is reviewed for its appropriateness to the patient’s condition, taking into account the dosage, frequency and method of administration. It is also part of the pharmacist’s role to maximise the benefits of any prescribed medication to patients. One of the biggest strength of St Joseph’s is its relatively small size and the ease of communication between the various departments. Decisions here can be made quickly and always with the patients’ benefit in mind. Over the last six years the hospital has improved and evolved a lot. The recent reaccreditation

of JCI standards reflects the commitments to excellence and quality in patient care that are in place throughout the hospital. We very much believe that St Joseph’s provides an excellent service and standard of care to the public. In the future we would hope to develop our day medical services and from a pharmaceutical perspective expand the range of drug infusions that can be provided to patients. St. Joseph’s Hospital

INFECTION CONTROL and adapted to adhere to national and international best practice guidelines.

Siobhán Meehan, Infection Control Manager

T

he infection control process is hugely important at every level here at St Joseph’s. It’s a vital component of the accreditation process for JCI International and its importance is reinforced here on a continual basis. A comprehensive infection control programme is in place here at the hospital, with protocols constantly reviewed

All new staff at the hospital, full time, part-time or contract are inducted into our infection control protocols and procedures and regular audits and spot-checks are carried out in all departments to ensure conformance and that any issues are swiftly rectified. We are extremely effective at the hospital in regards to our processes. For example, we have 0 per cent MRSA rates and 0 per cent infection rates. All patients are screened at the point of entry into the hospital and throughout the hospital our commitment to hygiene is evident with hand-hygiene dispensers frequently available throughout the facility. Other aspects of infection control here at St Joseph’s include standard and

transmission-based precautions for staff, prevention of needle stick injury and safe and sustainable decontamination of reusable medical equipment and effective waste management. Senior management governs infection control procedures at the hospital and oversees the development and implementation of any infection control procedures under the guidance of the infection control manager. Robust, strictly observed infection control procedures are vital in order to provide peace of mind to our patient community and to safeguard both the staff who work at the hospital and also to safeguard the high reputation which the hospital holds amongst the medical community in the North-West region. Together with our excellent housekeeping staff we all work together to maintain the required standard. St. Joseph’s Hospital

MEDICAL DIRECTOR

GUIDING HAND Working as an interface between the clinical side and other departments within the hospital, Medical Director Mr Peter Morrison is ideally placed to observe the progress which St Joseph’s Hospital has made.

Mr Peter Morrison, Medical Director.

O

riginally hailing from Dublin, Mr Morrison has lived in the Sligo area for the past 24 years and worked as a General Surgeon, with a special interest in urology before taking up his current post as Medical Director at St Joseph’s hospital. “I have a number of roles here in the hospital in my position as Medical Director. One of my primary tasks is to act as a guardian of the patient’s advocacy of this hospital,” he explains. “At St Joseph’s we have a safe, friendly and caring environment which is reflected throughout all departments in the hospital, people here are on a first name basis and it’s all part of making the patient feel valued and cared for,” explains Peter. The Medical Director at St

10

Joseph’s is the hospital’s clinical leader. Peter oversees clinical care in the facility, works with the interdisciplinary team to ensure quality care and implement appropriate clinical policies, procedures and programmes, and works with the facility administration regarding staff management and survey issues, quality assurance activities, and education and training for physicians, nurses, and caregivers. “I also advise management in relation to the business and other non-medical aspects of the hospital here. It’s a very good place to be involved in and it’s good to feel valued and to have a real influence on the future direction of the hospital,” he adds.

Having come from a clinical background, Peter is ideally placed to act as a conduit between the medical and non-medical departments within the hospital. “What’s great about St Joseph’s is that there is mutual respect between the clinical and non-clinical roles. I have experience as a clinician and can see their point of view, whilst also being able to help the CEO in his position. There is also a very strong system of governance and there is also a medical advisory committee which provides another tier of support and guidance for the management of the hospital. The hospital has also recently been reaccredited by JCI International and I think that’s another hugely positive step.”

as endoscopy or surgery. “We would be the first point of contact for patients, on the phone and face to face.” This is an important role for patients and hospital alike. “We have the knowledge of the hospital and how it runs, and if we don’t have what’s needed we know who we can talk

o hospital can deliver excellent care without a solid team of support staff. That’s what Administration Supervisor Kathleen Gardiner and the Administration Section provide at St Joseph’s: they’re an efficient and knowledgeable team dedicated to helping patients throughout their treatment.

“The Administration Department is an integral part of the hospital,” explains Kathleen: they assist all patients visiting St Joseph’s,

Kathleen and her team are there to interact with the consultants as well. In a busy environment they schedule appointments and dispense information such as details of the services offered by consultants at the hospital.

INSURANCE MATTERS They also work with insurance companies, with whom a good relationship has been built. “We do like to be sure that we can talk to the patients before they come in, so that they understand that they need to contact their insurance company and be aware of what their insurance plan is.

inpatients and outpatients alike. “We have an excellent team. Some of them have been there for 15 years, some are newer – but they all work well; they’re trained to work with every department and they can multitask. “Customer service is our priority: to make sure that every patient who is brought in is treated as an individual – and we have great empathy with patients who are coming in. A lot of patients are nervous; they mightn’t have

In fact, being aware of one’s insurance arrangements is more important than ever. “There have been so many changes in the insurance market with people upgrading, downgrading and moving to other insurance companies that there’s often a difficulty with keeping up to date with what’s covered and at what hospital. That’s why we do try to contact each patient on an individual basis to make sure

Good facilities, such as a new computer system, are a further boost. “We link in with the theatre staff, endoscopy staff and so on. A lot of the lists of patients will come through us. We admit patients into the hospital, and bring them on up to whatever department they’re coming in for, so we have to be sure that everybody has the most up-to-date list and that all the information on it is accurate, because it is a legal document. Our interaction between the departments is vital,” she explains. “I think patients are happy with the service we provide – patients are asked to complete feedback questionnaires after their visit is over, and there’s been good feedback. “Teamwork within the hospital is important and the Administration team works well together. We have a range of duties to perform and we do take our jobs very seriously,” she says. Friendly and knowledgeable, Kathleen and the St Joseph’s Administration team will do everything they can to help. St. Joseph’s Hospital

NURSING

PROVIDING LEADERSHIP

Siobhán McMurray (right) with Theresa Lynch RGN and Louise Walsh RGN.

Siobhan McMurray, Senior Nurse Manager at St Joseph’s, explains what her role is at the hospital and how St Joseph’s maintains an excellent level of nursing care.

I

have worked here at St Joseph’s since 2004, and I come from a clinical background in nursing. I think that’s particularly beneficial in a compact facility like St Joseph’s and assists in working with the consultant staff. I was appointed Senior Nurse Manager in 2008 and my key role here is to provide leadership to the nursing staff in the hospital and to work with the CEO and the Medical Director to continue to improve the

12

facilities and services which we have here at St Joseph’s. One of the great strengths of St Joseph’s is the warmth and personal touch which patients receive during the course of their care and which also exists in the working relationships between departments. Myself and the other nursing staff work extremely hard to maintain the high standards of care for which the hospital has become renowned and

we realise that the whole issue of quality and standards is an ongoing process, and that is reflected in the recent reaccreditation of the hospital to JCI standards. I work with the nursing team here, made up of both full time and part time staff. All our staff are working at the same level in terms of providing excellent quality nursing care to our patients. Induction into our way of doing things

NURSING “One of the great strengths of St Joseph’s is the warmth and personal touch which patients receive during the course of their care and which also exists in the working relationships between departments” is very important and our systems are very thorough in that regard. Patients here are kept very well informed in terms of the progress of their treatment as we have a very good staff to patient ratio. We strive to learn from each patient and to continuously improve our service. Staff receive regular training in relation to any new protocols and are strictly observant of infection control and hygiene procedures in the hospital.

As the CEO has said, we are looking to broaden our services and increase the number of procedures we can offer. Management has been ambitious and successful in recruiting consultant staff of an excellent calibre and we’re confident that we are matching that ambition in terms of our nursing care. At St Joseph’s we realise that standards are very important and that patients have a right

to feel safe, secure and cared for in a hospital environment. The nursing staff at St Joseph’s, conscious of the traditions and reputation of the hospital, are proud to play their part in providing an excellent level of patient nursing care. An important part of our team is the support delivered by the maintenance, housekeeping and administrative/finance staff in the hospital. St. Joseph’s Hospital

CLINICAL PROJECT MANAGER Clinical Project Manager Michelle McDonnell discusses how advances and improvements in her liaisons with other departments in the hospital has improved the quality of care given to patients.

C

linical Project Manager Michelle McDonnell discusses how advances and improvements in her liaisons with other departments in the hospital has improved the quality of care given to patients. Michelle’s main role in the hospital is to manage any HSE referral patients, and to oversee their smooth transition through St Joseph’s Hospital. “The different Project Managers in the HSE hospitals refer their patients out to us here at St Joseph’s, and it is my job to liaise with those people regarding the number of patients and the types of procedure they may need”. Once Michelle is aware of the patients that will be referred to the hospital, she liaises with the anaesthetist and surgeons regarding patient suitability for certain procedures, ensuring that the highest patient safety standards are met. Michelle’s liaison with anaesthetists and surgeons ensures the highest level of patient care is adhered to. To ensure that all patients are suited to their procedure some may be referred to a pre-admission clinic to be assessed further. “We would

14

liaise with a patient regarding the pre-admission clinic, and they would be seen by our resident doctor and anaesthetist. This is especially important when dealing with children, as it reassures them, allows them to become familiar with the hospital and reduces anxiety for their day of admission”. One of Michelle’s roles is to devise suitable theatre lists. This is of huge benefit to all patients as it provides them with the best possible care. She liaises with the Theatre Manager to devise these lists, accomodating needs so that “younger patients would be fasting for the least amount of time, and anyone with a diabetic history or any physical conditions that would make it unsuitable for them to fast for long periods, would be considered first for the surgery and some of our clinical procedures may be more suitable for the end of a theatre list, for sterility and safety purposes”. A patient’s smooth journey, from admission to discharge, is of utmost importance to St Joseph’s Hospital. The hospital strives to obtain all relevant medical history for a patient from the referring hospital, to

A patient’s smooth journey, from admission to discharge, is of utmost importance make sure they provide the best care. St Joseph’s keeps detailed medical notes about a patient procedures in the hospital, so that when they are referred back to their own hospital, there can be continuity in the level of their care. “A patient might need ongoing review in a public hospital, so it would be very important that they are aware of the procedures they had here, how we carried it out, and how they recovered”. Looking forward, Michelle explains how closely she works with HSE hospitals, and GPs in the region, to develop services in a more focused way, establishing patient comfort and safety at the forefront. The ambition is to continue to build on the existing excellence of service, and provide a medical facility that serves patients well in the future.

Dr Orla Conlon

Dr Orla Conlon, Consultant Obstetrics and Gynaecology has been granted admitting privileges at St Joseph’s Hospital in Sligo. Dr Conlon is also a Consultant in this field at the Erne Hospital in Enniskillen, Co. Fermanagh.

D

r Conlon has a keen interest in audit and clinical risk management and patient safety is of paramount importance. Her special interests include urogynaecology, advanced hysteroscopic surgery, ambulatory gynaecological surgery, menopause management, psychosexual medicine, paediatric and adolescent gynaecology,

forensic gynaecology and colposcopy. Dr Conlon graduated in medicine from Queen’s University Belfast in 1995 and has been working in the field of obstetrics and gynaecology since 1996. She became a member of the Royal College of Obstetricians and Gynaecologists in 2000 and was appointed to the Erne Hospital, Enniskillen as a Consultant in March 2007. Some of the procedures undertaken at St Joseph’s Hospital include: • Laparoscopy • Hysteroscopy • Removal or insertion of Mirena Coil • Excision of lesions (cervical, vulval, vaginal) • Cervical Smear • TVTO-Transvaginal tape • Colposcopy-using general or local anaesthetic • Cauterisation of cervix • Revision Vaginal suturing • Endometrial ablation • Cystoscopy One of the leading gynaecologists and obstetricians in her field, St Joseph’s Hospital are very proud that she is able to offer her services to patients

of the hospital. The field of gynaecology procedures has evolved greatly over recent years with huge advances in both procedures and available technology. The majority of gynaecological procedures have changed from major surgical procedures to minimally invasive laparoscopic procedures, which have become very common in this field. This surgeons to access the abdomen and pelvis through a key-hole incision. This minimally invasive procedure has greatly reduced post-operative pain, the duration of a patient’s stay at a hospital; and it also greatly reduces the necessary recovery time for the patient. The levels of nursing care at St Joseph’s and the holistic approach to patient wellbeing at all levels is one of the great strengths of the hospital, which it has rightly gained a reputation for. The provision of the finest gynaecological and obstetrics services by consultants such as Dr Conlon is a testament to the ambition and aspirations of the hospital to continue to provide the finest in diagnostics and procedures to patients in the North West region.

Theatre Manager Una Browne explains the ethos of excellent surgical care at St Joseph’s Hospital.

T

he operating theatre here at St Joseph’s Hospital was completely refurbished in 2009, incorporating an air handling unit in the theatre which enables a complete airchange every three minutes. All the surgical and operating technology within the theatre was procured within the last decade. My role as Theatre Manager is to co-ordinate operating lists for the various consultants we have here at the hospital across their respective disciplines. Essentially my job involves maximising the operational efficiency at the facility with regard also to resources and costs. I also need to deal with the various departments and personnel who would work in the operating facility here.

We operate to the very highest standard within the scope of the surgical procedures that we can provide here at the hospital. Everything here is done to a consistently high quality. While we want to expand the surgical procedures we offer, this is all done through strict safety and quality control mechanisms. Primary in any pre-operative and post-operative environment is patient safety and the need for us, as medical professionals, to reassure patients and to encourage them to give us feedback as to their experiences here at the hospital. We are dependent upon the trust and patronage of our patient population so it’s

very important that we make them feel cared for and part of the hospital community; the personal touch is hugely important. In addition to our established surgical procedures we have begun to offer new services now such as urodynamics, which signifies progress, and we have also broadened the range of plastic and aesthetic surgery which we offer. With consideration to the size of the hospital here and our role as a centre for diagnostics and elective procedures, we hope to be able to expand our range of surgical services while maintaining the connection to our patient population, and their trust, which have earned St Joseph’s Hospital its strong reputation.

The primary surgical disciplines at St Joseph’s Hospital include: • • • • • • • •

RADIOLOGY

Radiology

at St Joseph’s Hospital St Joseph’s Radiology Department provides a first-class dedicated ultrasound service.

PATIENT CARE As with all other departments at the hospital, the Radiology department focuses on patient centred care, and maintains an excellent working relationship with referring GPs, with whom they discuss patients and results. Staff at the hospital take a proactive approach in this case, and believe that ensuring they have a good relationship with GPs is the best way of taking care of their patients and ensuring that their needs are met. The Consultant Radiologists at St Joseph’s Hospital are Dr Brendan Morrissey, Dr Ann McNamara, Dr Stephen Davitt and Dr Andreas Mohr.

FACILITIES The Radiology department at St Joseph’s provides 18

ultrasound for patients under their care. The comfort of the patient undergoing the procedure is of the highest concern to staff at the department.

SERVICE Efficiency in patient care is of paramount concern, many of whom are GP referrals, and once a referral letter is received by staff, contact is made with the patient regarding making an appointment. All referrals are assessed by the Consultant Radiologist so they are prioritised correctly.

ON THE DAY Following their arrival, staff will take a patient’s details and usually the scan will have been completed within an hour. If they are advised

to see a specialist further down the line then they can simply give them their scans in person. Reports are completed on the day of the scan which the GP should receive within a day or two.

INSURANCE & BILLING As a matter of course, St Joseph’s Hospital deals directly with insurers such as VHI, Laya Health Care and VIVAS and the hospital can directly liaise with the patient’s insurance provider. We also welcome patients who do not have insurance and can provide a cost on enquiry. On arrival, patients can either inform reception of the healthcare details, or alternatively the hospital can arrange for direct billing.

ORTHOPAEDICS

EXCELLENCE IN ORTHOPAEDICS Amongst the repertoire of patient services provided by St. Joseph’s Hospital is Orthopaedics delivered by a team of experienced Consultants.

A

s part of its wide range of patient activities, St Joseph’s Hospital also deals considerably with orthopaedic surgery, concerned with the musculoskeletal system. Mr Paul O’Grady, Consultant in Trauma and Orthopaedic Surgery, is a graduate of both Clongowes Wood College, Naas, Co. Kildare and the Royal College of Surgeons. Holding a Master’s in Medical Science, Mr O’Grady went on to obtain a Diploma in Sports Medicine. He also was the team doctor for Shamrock Rovers Football Club and was later appointed to the panel of Medical Doctors for the Irish soccer teams with the Football Association of Ireland. As part of the team of experienced and competent consultants operating at St Joseph’s, Mr O’Grady provides hospital patients with a wide range of musculoskeletal care surrounding the major joints – hands, knees, shoulders and ankles. Patient services include cruciate ligament reconstructions, shoulder stabi20

lisation procedures in order to prevent recurrent dislocation, carpal tunnel release for patients experiencing the uncomfortable symptoms surrounding inflammation in the wrist’s carpal tunnel, epidural injections which deliver medication to the spinal canal and which temporarily relieves lower back and leg pain, the removal of

plates, pins and screws used in internal fixation (the use of implants to help guide the healing process of a bone). Mr O’Grady was also elected president of the Association of Graduates of the Royal College of Surgeons in Ireland while his specialist orthopaedic training was undertaken in con-

junction with the Irish Higher Surgical Training Scheme. Mr O’Grady’s experience in the field of sports medicine is quite substantial. He was part of the central medical organising committee for the Special Olympics Ireland (2002) and the World Summer Games (2003). He was responsible for the recruitment and training

of medical staff for the games, while also Chief Medical Doctor for the closing ceremony held in Croke Park. Mr O’Grady also provided local orthopaedic cover to visiting teams during the Tri Nations and the Irish and British Lions’ first test in 2005. He has also been appointed as Consultant in Trauma and Orthopaedic Sur-

gery at Mayo General Hospital in Castlebar, as well as St. Joseph’s. Working alongside fellow highly trained and skilled consultants, Mr O’Grady and his colleagues constantly strive to emulate St. Joseph’s ethos in providing the highest possible level of patient care and experience that the hospital can provide.

ENDOSCOPY

A STREAMLINED & SAFE PROCESS

Clinical Nurse Manager at the Endoscopy Unit at St Joseph’s Hospital, Ms Geraldine Maxwell, explains why patients in the North West are availing of endoscopy at the hospital, making it the most common procedure undertaken at the facility.

Geraldine Maxwell

H

aving worked at St Joseph’s Hospital for over eight years, first as a Staff Nurse and now in the Endoscopy Unit, Geraldine is proud of how the service has developed at the hospital. “As Clinical Nurse Manager at the hospital, it is my role to effectively run the unit alongside the consultant and nursing colleagues. I liaise with all clinical staff to ensure we can continue to provide our endoscopy services at current levels and I also liaise with the patient population to ensure we continue to retain a high level of patient satisfaction in our services,” she explains.

There are three dedicated consultants who work in the Endoscopy Unit, providing clinics for patient lists on Monday, Tuesday and Thursday afternoons: Mr Tim O’Hanrahan: Monday afternoons Mr Kevin Walsh: Tuesday afternoons Mr Martin Caldwell: Thursday afternoons. “On average, we would see between 10 and 16 patients per list, and the waiting lists are very low. Our consultants are very well known amongst the GP community in the North West and many doctors would refer their patients directly to a particular consultant, which helps streamline the process and further reduce the waiting time,” says Geraldine. Although St Joseph’s is not a new medical facility, the hospital has invested heavily in both staff and technology in order to maintain its excel-

lent reputation as a diagnostic centre. “All the technology in our Endoscopy Unit has been purchased very recently and is the most modern available on the market. This, coupled with the staff we have makes for a very strong service. On the day of the procedure, a patient will be admitted to the hospital, then we will do a pre-assessment. They will then meet their consultant for the relevant permission forms to be processed before undergoing their procedure. They are then taken to a recovery area, before being taken to a waiting room for some light refreshments before receiving their results and then leaving the hospital. On average, the process will take in the region of three to four hours from admission to discharge. The entire procedure is consultant led and this gives a great deal of reassurance to the patient, in addition to the warm, personal atmosphere at the hospital,” says Geraldine. St. Joseph’s Hospital

ANAESTHESIOLOGY

ANAESTHESIOLOGY AT ST JOSEPH’S Consultant Anaesthestist Dr Bernadette Egan discusses how advances in anaesthetic medicine have led to reductions in length of hospital stay and improved patient outcomes. Children’s Univeristy Hospital Temple Street and Cappagh National Orthopaedic Hospital.

Dr Bernie Egan

D

r Egan came to medicine following a serious car accident, which required her to undergo several operations. This, as she explains, gives her a unique insight into some of the anxieties that patients can experience prior to surgery and an understanding of how to allay them. “I think the key is helping people overcome their natural fear and anxiety around surgery. Because of my personal experience I can establish a rapport with patients and get to the heart of their concerns,” says Dr Egan. After graduating in 1990 from the Royal College of Surgeons of Ireland she undertook specialist training in Anaesthesia, Intensive Care Medicine and Pain Medicine, in the major Dublin hospitals including Beaumont, St James’s, the Mater, the Coombe Women’s Hospital and Our Lady’s Hospital for Sick Children in Crumlin. She also worked as a Consultant at the 26

Having gained extensive experience as a consultant in these hospitals, Dr Egan decided to relocate to Sligo and St Joseph’s Hospital where the focus on day case surgery and paediatric admissions for ENT and dental surgery proved a perfect fit for her expertise. The role of a consultant anasthestist is a complex one and involves much groundwork in the pre-admission stage working with the patient to ensure optimal surgical results and a positive patient experience. According to Dr Egan, “Good patient work-up is essential and in a smaller hospital, like St Joseph’s, there is the opportunity to spend more time with individual patients pre-admission. It’s a chance to get as much medical background as possible and to give patients information and address any questions or concerns they might have.” Throughout her career, Dr Egan has noted the improvement in anaesthetics, which benefits patients. Chief among these are the advances in rapid onset and offset drugs, which means that patients have shorter

recovery and are able to leave hospital much sooner. More procedures can be carried out on a day admissions basis and patients are mobile sooner, reducing the risk of secondary complications such as infection or thrombosis. The drugs themselves are also more quickly metabolised and eliminated from the body and produce fewer side effects. As a private hospital, St Joseph’s is in a position to schedule procedures more rapidly and this, Dr Egan believes, is crucial in providing the best patient outcome possible. “People often focus on the convenience of smaller private hospitals but one of the most important factors is timely intervention. The sooner a health issue is dealt with, the less chance it has of deteriorating or causing other complications,” as Dr Egan explains. Looking at how the future of the hospital and its services are being shaped, Dr Egan explains that St Joseph’s works very closely with GPs in the region to develop services in a more focused way, based on patient requirements. The ambition to build on existing excellence and provide a finely-tuned medical facility will serve patients in the North West region well into the future.

Dr Andreas Jahnke is a German neurologist who has recently fulfilled his lifelong dream to set up a practice in the North West of Ireland. Here he discusses his Neurological Clinic in St. Joseph’s Hospital and his vision for its development.

O

ver 700,000 people in Ireland suffer from ailments that affect the brain or the spinal cord, according to the HSE Strategic Review of Neurology and Neurophysiology Services. Many of these conditions are treatable, but early intervention is the key.

adults and children from later school-age suffering from any kind of neurological disorder. This includes headaches, strokes, post-traumatic brain damage, people with disabilities, people with autism who have neurological and neuropsychiatric disorders and epileptic seizures, and people with peripheral nerve damages.

“I have found that GPs take a lot of responsibility themselves and identify a lot of neurological problems that require treatment. Obviously, there are some conditions that require the opinion of a neurologist. The referrals I see from GPs are very good, a lot of important information is provided and a lot of preparatory work is done before I see the patient.

Dr Jahnke also refers patients he treats to rheumatologists; ear, nose and throat specialists, orthopaedics, surgeons and other consultants if he feels any ailment needs further investigation.

“This isn’t something you can definitely expect as a consultant and I really appreciate this. The fact that I don’t have a waiting list is hopefully of benefit to patients,” he says.

Services available at St Joseph’s Hospital include outpatient and inpatient consultations, nerve conduction studies, electromyography (EMG) and nerve conduction velocities (NCV). Patients have to pay for outpatient consultations and those with voluntary health insurance are covered for NCV and EMG and inpatient consultations.

He provides a wide range of neurology specialities every week at St Joseph’s Hospital Sligo. The provision of this new clinical speciality is regarded as a major boost for St Joseph’s Hospital. Dr Jahnke receives referrals from general practitioners for

NCV with EMG could be used, for example, in the diagnostics of carpal-tunnel syndrome and other peripheral nerve and neuromuscular disorders by testing the function of the peripheral motor and sensory nerves. EMG is considered effective for the

NEUROLOGY

NEUROLOGY AT ST JOSEPH’S diagnosis of myositis, motor neurone disease, root damage and post-traumatic nerve damage. This can be achieved by measuring the muscles’ electrical activity. Plans have also been drawn up for the provision of evoked potentials (EP). They are used, for example, to diagnose multiple sclerosis and other brain and spinal cord damages by using nerve stimuli to measure the electrical signals generated by the central nervous system. Born in Vaihingen/Enz near Stuttgart in Germany, Dr Jahnke, who also has Irish citizenship, spent his childhood partly close to Bern in Switzerland and close to Lake Constance in SouthBavaria, Germany. He studied Medicine in Budapest, Hungary. His mother, Elizabeth (née Cummins) was born in Dublin in 1945 and his father Hubert is a retired bookbinder who hailed from Stolp in Germany. Having lived in a number of countries including Germany, Switzerland and Hungary, he always held a desire to live and work in Ireland. “It’s a dream come true to be able to work in the North West of Ireland,” concludes Dr Jahnke.

PAIN MANAGEMENT

Pain MANAGEMENT St Joseph’s can match excellence in clinical staff with modern technology in an environment that reflects the ethos of the hospital, which is the provision of “timeless access to quality healthcare, through our commitment to clinical excellence.”

B

ecause he also work’s in the Sligo Regional Hospital, Dr Lane is well placed to assess the contribution which St Joseph’s can make in helping patients manage pain. Dr Lane can provide diagnostic and therapeutic facilities for patients suffering from pain. The most common conditions presenting are chronic, nonmalignant conditions such as severe back and neck pain and muscular pain. Treatments at St Joseph’s Hospital range from medication and therapy to interventional procedures. Clinical assessment is the first step in pain management. Dr Lane is a fully accredited Consultant Anaesthetist, registered on the Medical Specialist section and his particular areas of speciality and interest are: • Lower back pain • Neck pain • Upper and lower limb pain

28

• Headaches • Muscular pain • Botox use for chronic pain and dystonia • Interventional pain management techniques • Chronic regional pain syndrome • Post Herpetic Neuralgia • Trigeminal Neuralgia Dr Lane entered the specialist register having completed higher specialist training in Anaesthesia, Pain Management and Intensive Care. Following on from this, he completed a Fellowship in Chronic Pain in McMaster University, Hamilton, Ontario. While there he gained further training and experience in the management of chronic pain conditions using interventional as well as medical techniques. Dr Lane also worked in Buffalo, New York with Professor Roman Waghmarae, who specialises in advanced interventional pain management and neuromodulation techniques.

St Joseph’s Hospital has become a lot more established within the North-West region and is recognised as a high quality independent medical and diagnostic facility with a growing range of services for its patients. St Joseph’s can match excellence in clinical staff with modern technology in an environment that reflects the ethos of the hospital, which is the provision of “timeless access to quality healthcare, through our commitment to clinical excellence. We do so to meet the needs and expectations of all who use our services.”

At the hospital, a dedicated Occupational Health clinic is providing real results for businesses of all sizes in the North West.

D

r Tony McGread is an Occupational Health (OH) Physician who has worked at St Joseph’s for over five years, providing a clinic once per month. “What we do at St Joseph’s is provide medical advice for a very wide range of public and private sector clients on issues of workplace safety and fitness and disability matters. We also provide expert advice to occupational pension funds and the legal profession,” he explains. Tony provides a full range of OH related services including risk analysis and the assessment of any disability or injury related to employment. The cost of work-related illness and accidents in Ireland cost €3.6 billion (2.5% GNP) annually, according to recent statistics. Fundamentally the key to managing absence from work, whatever the cause, lies with the employer or manager. When it comes to

absence related to health, disability or injury, professional independent occupational health services can provide the employer with the type of help and advice they need. Also, while much of the legal framework related to the workplace and employment may be broadly welcomed by society, there is no doubt the impression is one of an ever increasing ‘mine field’ for the employer and manager. An Occupational Health Physician will provide you with independent medical advice, helping assess your needs and deliver solutions and services especially tailored for your own needs. At St Joseph’s Hospital, the occupational health clinic can help your business, assisting in providing reports to: • Reduce absence from work • Control costs • Meet legislative responsibilities • Improve corporate image • Improve business performance

“Managing the health of your staff enables your business to perform and your workers to perform and be happy in their work,” adds Tony.

OCCUPATIONAL HEALTH

Occupational Health at St Joseph’s Hospital

“People have become much more aware of occupational health concerns and equality legislation and we provide on-site clinics at workplaces also so we can identify the root causes of any problems and of course a solution. Equality and Disability legislation is something which businesses need to be very aware of as it is very much an issue in today’s workplace,” adds Tony. Our Head Office is in Belfast and we were established in 2007 – it is led by our Medical Director and Consultant Occupational Health Physician Dr Tony McGread. Our company is staffed by experienced professionals delivering from different locations within Northern Ireland and Eire. Our main office is a nine-room fully equipped clinic in Belfast city centre. aving been granted admitting and operating privileges at St. Joseph’s, Mr Hegarty provides a high quality patient service in this field. Mr Hegarty is an internationally trained urologist; having graduated from University College Cork he completed the Western Pre-fellowship Training Scheme in Surgery, working in both Sligo General Hospital and University College Hospital Galway. Mr Hegarty then went on to complete a PhD in University College Dublin and, following his training, spent two fellowship years in the world-renowned Cleveland Clinic in Cleveland, Ohio, the multispecialty academic medical centre regarded as one of the top four hospitals in the United States and the site of many medical firsts. He was appointed Consultant Urologist in Guy’s and St. Thomas’ Hospitals in London in 2006 and returned to Ireland in 2011 to take up a Consultant Urologist post in the Mater Private Hospital in Dublin. Amongst Mr Hegarty’s areas 32

of interest are prostate cancer, benign prostatic hyperplasia (BPH), renal stone, urological oncology and general urology. One of the things on which St Joseph’s prides itself is the wide range of patient services it has to offer. The Urology department is no different and amongst the services that will be provided under the direction of Mr Hegarty will be comprehensive outpatient evaluation of key urological symptoms and conditions including haematuria, renal colic and stone disease, erectile dysfunction, UTI, urinary incontinence and suspected prostate cancer to name but a few. The treatments that will be available at the Urology Department of St Joseph’s will include flexible and rigid cystoscopy, urethral stricture dilatation, circumcision, intracavernosal injections, inguinal and scrotal surgery amongst others. More complex treatments and in-patient surgeries will be performed in Dublin, while all cancers which have been diagnosed will be referred for discussion at multidisciplinary team (MDT) meetings.

One of the things on which St Joseph’s prides itself is the wide range of patient services it has to offer. The Urology department is no different.

URODYNAMICS

Urodynamics at St Joseph’s Hospital

Lorraine Cox, Clinical Lead Nurse

Lorraine Cox and Emma Davitt, Clinical Lead Nurses at the Urodynamics Clinic at St Joseph’s Hospital explain the establishment of the clinic and the demand for its services in the North West.

rodynamics is essentially a diagnostic study of bladder pressure used for the treatment of incontinence. It’s a scientific process involving precise measurements which can then possibly identify the root causes for the symptoms and then we can look at a suitable procedural remedy.

symptoms. We have four consultants here who will be linking in with the clinic which we will be running here at the hospital and they were the key drivers behind the establishment of the clinic. We already have a complement of patients booked into the first clinic and we envisage significant growth within the patient base thereafter.

From April 2013, St Joseph’s Hospital will be holding a once weekly urodynamics clinic which it is planned to expand to twice weekly in the foreseeable future. We believe that there is a definite demand for this service in the North West region, with studies showing that symptoms requiring urodynamics assessment are more prevalent and also that patients are far more aware of these problems and their

St Joseph’s Hospital has invested significantly in technology in this sector, with all urodynamics equipment also calibrated to deliver electronic reports via email to consultants. There is also a designated clinical treatment area within the hospital which has been specially identified as suitable for the personal requirements of the urodynamics investigation process. The process involves

a nurse conducting a patient assessment and ascertaining patient history for the purposes of the clinical assessment. Then a urine flow test and bladder scan are performed, both non-invasive processes. Then, the only invasive part of the procedure is carried out when the patient undergoes a cystometrogram, a well established and straightforward, catheter- based procedure used to assess the patient for an overactive bladder, urinary retention or incontinence. All our procedures and all advice given is consultant led with the added expertise of our experienced nursing staff adding to the best clinical outcomes for our patients. This referral-based outpatient clinic is another positive addition to the ever expanding range of services available at St Joseph’s Hospital, with ease of access and minimal waiting times guaranteed.
