throughout the United States, it never ceases

to ama ze me that the system most frequently in need of help is the scheduling system. Broken

appointments (BA’s) and changed appointments (CA’s) are an expensive drain on the practice.

Understanding “why” they occur is an essential first step in reducing and possibly eliminating them. The second step is to learn how to

prevent them and the third is to reeducate your existing patients to exhibit greater respect and

responsibilit y toward their reser ved time. Finally, to ensure your progress, it is impor tant to track your practice results to see how well your new system is working. Overcoming this practice

handicap results in less stress and increased satisfaction for the entire team.

When staf f members list the handicaps BA’s and

CA’s create in the practice, almost ever yone lists

the loss of income to the practice, the time spent

changing appointments and the time spent “filling holes” in the schedule. Only a few realize the

drain on the team’s energy and enthusiasm when patients’ no-show or are late. A staf f member’s

good at titude is quickly replaced by frustration with last minute, unexpected changes and at

times by complacency: staf f may think, “Oh well, I only have 10 minutes now before the nex t patient is due and there’s really nothing I can do in such a shor t amount of time.”

What Patients Think:

Therefore, the statements: “I won’t be missed if I cancel”; “Why is there a problem if I arrive late?

They are always running late anyway,” are common

patient expressions. Most patients do not remember or were never told of the impor tance of their

treatment sequencing and the urgency for their

optimal health to complete their treatment within a

specified time frame. And finally, patients think the Doctor is too busy to notice whether they come in today, tomorrow, or nex t week. Patients express,

“The Appointment Coordinator is the only one who

actually knows if I show up and it’s her job to make

sure I come in at a time that is convenient for her.”;

“my dentist will be happy to see me whenever I can come.” Once you have an understanding of how

the patient thinks and feels, then prevention can be your nex t step.

Mutual understanding begins with the first

telephone call. To begin the prevention step the Appointment Coordinator should discuss with

the new patient the “reser ved time necessar y”

Studies have shown that patients place high value on the dentist’s opinion during the diagnosis and

prognosis. Discussion of scheduling for reser ved

time, which comes directly from the Doctor during the consultation, makes the greatest impact. Example Script: Doctor:

“Ms. Patient, to enhance your smile and

create a maintainable state of oral health, time is

also going to be an important factor. For optimum

results, your treatment plan (or Phase I) should be completed within the next (time) days. During this

time frame there are certain appointments that will need my close attention and certain times I need

to work closely with my laboratory for the precision required. For that reason, I have reserved special

blocks of time during particular days in the week for those appointments. This may mean taking some

time away from work. How will this work with your schedule?”

“Well, Ms. Patient, Amy, our Financial Coordinator,

is going to answer your questions about insurance, financial and scheduling arrangements. Before I

leave, do you have any other questions of me about the treatment we have planned, the importance of each appointment, or the time frame I have recommended?”

The Financial Coordinator discusses financial

arrangements then reinforces the impor tance of the reser ved appointment time and of refraining

from any unexpected changes unless there is an emergency.

The dentist lays the foundation for prevention with the patient. His or her conversation at the time of the consult is the key ingredient. The business of fice staf f can then build on that foundation

and reinforce the Doctor’s desires for special scheduling arrangements. When making the

confirmation phone call or when the patient calls

in to change an appointment, this foundation also provides the Appointment Coordinator with the

information necessar y to discourage any change in

However, if you use sizes 0, 1, 2, 3, 4, panoramic, cephalometric and/or TMJ, you will probably decide that PSPs are your only digital option. – Reason: ONLY Phosphor Storage Plates are available in every film size used in the dental profession.

FINANCIAL AID ELIGIBILITY

If you qualify for federal financial aid, there are a number of strategies you can try to implement to enhance the amount of aid your child should receive when you apply for financial aid. The idea is to lower your expected family contribution (EFC), which in turn should raise your child’s aid eligibility. Although some of these strategies can be employed as late as the base year—the year prior to the year you complete the Free Application for Federal Student Aid form (FAFSA)—others can be implemented years before your child should start college. It is important to note that these strategies are perfectly legal and are not in any way meant to undermine the federal financial aid process. These strategies examine the federal methodology and take advantage of its rules regarding which family assets and income are included in determining a student’s financial aid eligibility. Strategies to Reduce Available Income There are a number of steps you can take to reduce your adjusted gross income (AGI), under the federal methodology for

determining ﬁnancial aid. The lower your AGI, the less money you should be expected to contribute toward college costs and the higher your child’s aid eligibility. Remember, you apply for ﬁnancial aid each year. Thus, you should consider the following strategies for each of the years you apply for aid, not just for the initial application. Time the Receipt of Discretionary Income to Avoid the Base Year

Your income in the base year will directly affect your child’s ﬁnancial aid eligibility in the following year. Although it is highly unlikely you will be able to defer your weekly (or monthly) paycheck, it may be possible to defer other types of discretionary income beyond the base year. For example, if possible, you should try to: �Defer receiving employment bonuses until after December 31 of the base year. �Avoid selling investments that have taxable capital gains or interest, such ...continued on last page.

2013 will bring some big changes for investors, and none of them for the better. Taxpayers affected by these upcoming changes may wish to consider taking actions in 2012 to mitigate the impact of these changes. The following are the changes that will affect investors in 2013. Long-Term Capital Gains Rates Increase Taxpayers have enjoyed reduced long-term capital gains rates for several years, as a result of the Bush era tax cuts. However, without Congressional action, which is not expected, those reduced rates will return to the higher rates in effect prior to 2003. The table below compares the current long-term capital gains rates to the anticipated Taxpayer’s Regular Tax Bracket:

Taxpayers with unrealized long-term capital gains may wish to review their holdings and consider whether it is appropriate to sell during 2012 at the lower rates, or whether to continue to hold for additional increases in value. Where future increases in value are anticipated, a taxpayer could sell and realize existing gains in 2012, and then repurchase the investment for future anticipated increases. Investment strategies depend on a variety of issues, including existing capital loss carryovers, growth potential of individual investments, and other factors related to each individual. These should be carefully analyzed before taking action. Regular Tax Rates In addition to lower long-term capital gains rates, the regular marginal tax rates have been declining since 2001. TAX TIP: Charity Purchases & Auctions A regular form of fundraising by charitable organizations consists of sales or auctions of property or services at a price in excess of value. These are referred to as “quid pro quo” contributions, or dual payments made that consist partly of a charitable gift and partly of consideration for goods or services provided to the donor. Quid pro quo contributions, typically, include the purchase of tickets for sightseeing tours, all-expense-paid trips, theatrical or concert performances, books or subscriptions to magazines, stationery, candy, etc., and are sold with a generous mark-up that is designed to help the charity in performing its functions. In these cases, the charitable deduction is the excess of the payment over the value received by the purchaser-contributor. For instance, when tickets to a show are purchased from a charity at a price in excess of the normal admission charge, the excess over the latter (plus tax) is a charitable contribution. Determining and documenting the amount of the purchase that represents the charitable portion is the key to being able to take a charitable tax deduction for quid pro quo purchases. Tax law requires charitable organizations that receive a quid pro quo contribution in excess of $75 to provide a

written statement to the donor, in connection with soliciting or receiving the contribution, that informs the donor that the amount of the contribution that is deductible for federal income tax purposes is limited to the amount of the purchase that is in excess of the value of the property or service purchased and a good-faith estimate of the value of the good or services purchased. Example No. 1: A taxpayer purchases a cookbook from a charity for $100. The charity provides the taxpayer with a good faith estimate of $20 for the value of the book in a written disclosure statement. Thus, the taxpayer’s charitable deduction is $80 ($100 minus the $20 value of the book). Example No. 2: A taxpayer attends a charity auction. The charity provides a catalog of the items for auction and a good-faith estimate of the value of each item. The taxpayer is the successful bidder for a vase valued at $100 in the catalog, for which the taxpayer bid and paid $500. The taxpayer’s charitable deduction is $400 ($500 minus the goodfaith valuation of $100). Example No. 3: A taxpayer pays $40 to see a special showing of a movie for the beneﬁt of a qualiﬁed charity. The ticket read “Contribution

$40”. If the regular price for the movie is $10, the contribution would be $30 ($40 minus the regular $10 ticket price). As with assets eligible for the long-term capital gains rates, it may be appropriate for some taxpayers to accelerate ordinary income into 2012 to take advantage of the lower rates. Surtax on Investment Income Depending upon what the Supreme Court ultimately decides about the Health Care Law, effective 2013 a new surtax, called the Unearned Income Medicare Contribution Tax, will be imposed on individuals, estates, and trusts. For individuals, the surtax is 3.8%

of the lesser of: �The taxpayer’s net investment income, or �The excess of modified adjusted gross income over the threshold amount ($250,000 for a joint return or surviving spouse, $125,000 for a married individual filing a separate return, and $200,000 for all others). Thus, this surtax will only impact higher income individuals. “Net” investment income is investment income reduced by allowable investment expenses. Investment income includes: �Income from interest, dividends, annuities and royalties; �Rents (other than derived from a trade or business); �Capital gains (other than derived from a trade or business); �Trade or business income that is a passive activity, with respect to the taxpayer, and �Trade or business income, with respect to trading financial instruments or commodities.

For surtax purposes, the net investment income does not include excluded items, such as, interest on tax-exempt bonds, veterans’ beneﬁts, and excluded gain from the sale of a principal residence. For planning purposes, existing law favors tax-exempt bond interest, which avoids both the surtax and the regular income tax. However, you should be aware that President Obama’s tax plan would also tax the income from “tax-exempt” bonds for higher-income individuals at generally the same threshold, as this surtax kicks in. Henssler Financial believes you should Live Ready. It’s not too early to start planning for the 2013 tax increases. Prudent planning can signiﬁcantly reduce the tax bite. At the same time, keep a watchful eye on Congress. Since this is an election year, tax changes are most likely to come after the November elections.

Tax Credits and Deductions

For parents and students trying to manage college bills and student loan payments, the federal government offers education-related tax benefits. The requirements for each are different, so here’s what you need to know. American Opportunity Credit The American Opportunity credit is a tax credit available for the first four years of a student’s undergraduate education, provided the student is attending school at least halftime in a program leading to a degree or certificate. The credit is worth up to $2,500 in 2012 (it’s calculated as 100% of the first $2,000 of qualified expenses, plus 25% of the next $2,000 of expenses). The credit must be taken for the tax year that the expenses are paid, and parents must claim their child as a dependent on their tax return. To be eligible for the credit, your income must fall below certain limits. In 2012, a full credit is available to single filers with a modified adjusted gross income (MAGI) below $80,000 and joint filers with a MAGI below $160,000. A partial credit is available to single filers with a MAGI between $80,000 and $90,000 and joint filers with a MAGI between $160,000 and $180,000. One nice feature is that it’s calculated per student, not per tax return. So parents with two (or more) qualifying children in a given year can claim a separate credit for each child (assuming income limits are met). The mechanics of claiming the credit are relatively easy. If you paid tuition and related expenses to an eligible educational institution during the year, the college, generally, must send you a Form 1098-T by February 1 of the following year. You then file Form 8863 with your federal tax return to claim the credit. Lifetime Learning Credit Another education tax credit, the Lifetime Learning Credit has a broader reach than the American Opportunity credit. As the name implies, the Lifetime Learning credit is available for college or graduate courses taken throughout your lifetime (the student can be you, your spouse, or your dependents); even if those courses are taken on a less than half-time basis and don’t lead to a formal degree. However, this credit can’t be taken in the same year as the American Opportunity credit on behalf of the same student. The Lifetime Learning credit is worth up to $2,000 in 2012 (it’s calculated as 20% of the first $10,000 of qualified expenses). The Lifetime Learning credit must be taken for the same year that expenses are paid, and www.henssler.com

you must file Form 8863 with your federal tax return to claim the credit. In 2012, a full credit is available to single filers with a MAGI below $52,000 and joint filers with a MAGI below $104,000. A partial credit is available to single filers with a MAGI between $52,000 and $62,000 and joint filers with a MAGI between $104,000 and $124,000. Unlike the American Opportunity credit, the Lifetime Learning credit is limited to $2,000 per tax return per year, even if more than one person in your household qualifies independently in a given year. Decision Time If you have more than one family member attending college or taking courses at the same time, you’ll need to decide which credit to take. For example, Joe and Ann have a college freshman and sophomore, Mary and Ben, who are attending school full-time. In addition, Joe is enrolled at a local community college taking two graduate courses related to his job. Mary and Ben each qualify for the American Opportunity credit. Plus, Mary, Ben, and Joe each qualify for the Lifetime Learning credit. Because the American Opportunity credit isn’t limited to one per tax return, Joe and Ann should claim this credit for both Mary and Ben, then claim a Lifetime Learning credit for Joe. Joe and Ann can claim both the American Opportunity credit and the Lifetime Learning credit in the same year, because each credit is taken on behalf of a different qualified student. Student Loan Interest Deduction The student loan interest deduction allows borrowers to deduct up to $2,500 of interest paid in 2012 on qualified student loans.

Generally, federal student loans, private bank loans, college loans, and state loans are eligible. However, the debt must have been incurred while the student was attending school on at least a half-time basis in a program leading to a degree, certificate, or other recognized educational credential. So loans obtained to take courses that do not lead to a degree or other educational credential are not eligible for this deduction. Your ability to take the student loan interest deduction depends on your income. For 2012, to take the full $2,500 deduction (assuming that much interest is paid during the year) single filers must have a MAGI of $60,000 or less and joint filers $120,000 or less. A partial deduction is available for single filers with a MAGI between $60,000 and $75,000 and joint filers with a MAGI between $120,000 and $150,000. To be eligible, an individual must have the primary obligation to pay the loan, and must pay the interest during the tax year. The deduction may not be claimed by someone, who can be claimed as a dependent on another taxpayer’s return. Borrowers can take the student loan interest deduction in the same year as the American Opportunity credit or Lifetime Learning credit, provided they qualify for each independently.

as, mutual funds, stocks, or savings bonds, until after December 31 of the base year. To avoid taking an untimely distribution from an investment that is earning a favorable rate of return, use the investment as collateral for a low-interest loan instead. �Sell investments that can be taken at a loss during the base year, as long as the investments are not expected to recover. �Avoid pension and IRA distributions in the base year. �If you are on an expense account, ask your employer to reimburse you directly so that any reimbursement amounts do not artificially inflate your income. Pay All Federal and State Income Taxes Due During the Base Year This strategy is advantageous for two reasons: It reduces the amount of available cash on hand, and you can deduct the total amount of federal and state taxes you pay during the base year on the FAFSA. Leverage Student Income Limit For the academic year 2011/2012, the first $5,250 of income a student earns is not considered in determining a child’s total income. This is known as the student’s income protection allowance. However, everything a student earns beyond the allowance is first taxed and then assessed at 50% for financial aid purposes. In other words, the federal government expects your child to contribute 50% of all income earned over the allowance (after taxes). To avoid this result, parents may want to consider having their children perform volunteer work once their kids reach the allowance limit. Some children may balk at this suggestion, because they want a job to earn extra spending money. Strategies to Reduce Available Assets There are a number of steps you can take to reduce the amount of assets that will be included under the federal methodology. Under this formula, the federal government includes some assets and excludes others in arriving at your family’s total assets. The lower your assessable assets, the less money you will be expected to contribute toward college costs and the higher your child’s aid eligibility. It is important to remember that the relevant

date for determining whether you own a particular asset is the date that you submit the FAFSA. Consequently, the following strategies can be implemented up to the time you complete the FAFSA. Use Cash to Pay Down Consumer Debt The federal methodology does not care about the amount of consumer debt you may have. So if you have $10,000 in assets and $10,000 worth of consumer debts, the federal government still lists your total assets as $10,000. When you use available cash to pay down consumer debt, you reduce the amount of your cash on hand. It is usually a good idea to retain three to six months of liquid assets for emergencies. Use Cash to Make Large Purchases Another strategy to reduce cash on hand (an assessable asset) is to make large planned purchases the year before your child begins college. Some items may include a car, furniture, or the like for parents and a car (second-hand, of course), computer, or the like for students. Remember, the idea is not to go out and spend the money on anything; the purchase should have been previously planned. Increase Home Equity The federal methodology does not count home equity as an asset in determining your child’s financial aid eligibility. So using assessable assets to pay down the mortgage on your home is one way to reduce these assets, and benefit yourself at the same time. Although the federal government does not include home equity in determining a family’s total assets, most private colleges do include home equity in deciding which students are most deserving of campus-based aid. In addition, some colleges may expect parents to borrow against the equity in their homes to help finance their child’s college education. Leverage Allowance

For example, if parents are married and the older parent is 48, when the child applies for financial aid, the asset protection allowance is $46,200 in the 2011/2012 academic year. Once parents determine what their asset protection allowance will be, one strategy is to consider saving an equal amount of money in assets that are counted under the federal methodology. Then, any savings above this amount can be shifted to assets that are excluded by the federal methodology, such as, home equity, retirement plans, cash value life insurance, and annuities. Use Student’s Assets for the First Year Under the federal methodology for financial aid, the federal government expects a child to contribute 20% of his or her assets each year to college costs. Whereas, parents are expected to contribute a maximum of 5.6% of their assets. If assets have been accumulated in a child’s name, parents may want to consider using these assets to pay for the first year of college. By reducing the child’s assets in the first year, the family will likely increase its chances to qualify for more financial aid in subsequent years. Points to Keep in Mind �Colleges don’t use the same formula as the federal government in determining financial aid eligibility—their formula (the institutional methodology) is generally stricter. �Any increased financial aid your child receives may consist entirely of loans, not grants or scholarships. �Be careful about disrupting an otherwise sound investment program.
