Digital Complete Dentures Shweta Tawde BDS, Donald Belles DDS, MS Objective:
To introduce an effective method of fabricating complete dentures in 2-3 appointments.
Introduction:
Conventional dentures have been the back-bone of dentistry for the past 250 years. Their fabrication takes approximately 6-7 appointments and adequate chair-side/laboratory time is required. Digital complete dentures are fabricated in 2-3 appointments. They involve the same clinical procedures of impression making and recording OVD, eliminates some of the laboratory time.
Digital Complete Dentures:
A system by which impressions, interocclusal records and tooth selection is completed in one appointment and the dentures are fabricated using CAD/CAM technology
Impression Procedure: Thermoplastic trays are used to adapt to the patient’s ridge accurately. Tray must be immersed in water at 160o F for 60seconds.
Trays are molded to the patient’s ridges multiple times. Border molding is started by forming a rope on the inside of the tray edge. This is followed with more material on the top of the edge.
Anatomic Measuring Device (AMD)
Adhesive application AMD is stabilized by injecting bite registration material between both trays.

Immediate Custom Tray: Mandibular AMD tray placement Border molding is conducted by having the patient execute a series of movements to form accurate border and muscular molding.

Manipulating the mandible into CR to achieve correct VDO
Final Prosthesis Bite plane is placed on the lip support and adjusted parallel to the inter-pupillary line.

Light or Medium body PVS impression material is poured in the tray and Final impressions are made.

Patient’s VDO is estimated by marking dots on the patient’s maxillary and mandibular facial features to determine the accurate vertical height. This height I s measured with the caliper.
Vertical is adjusted to the desired height by turning the Hex Key.
Gothic arch tracings at the correct CR.
Gothic arch tracing point represents Centric Relation.
Appropriate tooth mold and shade is selected.
Shortcomings:
1. Light body impression material creates voids due to low viscosity. 2. Device for measuring the VDO has a plastic central pin which is not very stable when recording the Gothic Arch Tracings.
