Our values

Chair of Trustees Foreword My name is Jackie Churchward-Cardiff and I succeeded Peter Smallridge, as Chair of the Board of Trustees for Avante Care & Support, on 1 January 2018. I would like to record my thanks to Peter for his Chairmanship of the Charity and his continued service as Trustee.

The governance structure of Avante Care & Support is robust. Sub committees for Finance and Resources and for Care and Quality overseeing the finance and care strategies respectively give the Board of Trustees assurance on performance and sustainability.

This impact report covers our work during 2018, which proved to be a very busy year for us. We implemented the new corporate strategy which has three key goals: improving the quality of care we provide; improving our financial performance; and developing our services.

Avante Care & Support is a people organisation. Our staff are our ambassadors, representatives and underpin our reputation. The dedication of our teams is strong and enduring. We recognise the importance of retaining the best staff and this year saw progress in improving terms and conditions and importantly training and support. Our reliance on external agency has reduced over the year as recruitment improved, but this area requires on-going effort and leadership and remains a key priority for the executive team.

The year saw greater financial stability and we are therefore in a good position to develop and invest in our services. Following the rationalization of our estate to nine care homes, we have a program to ensure these are maintained to a high standard, most recently completing work to Weybourne.

As the new Chair I joined Avante Care & Support at a time of strength and opportunity. The Charity now has in place a stable and experienced senior team, well able to lead service developments and quality improvements, delivered from a stable financial base.

Work continues with improving quality with both Home Care services and eight Care Homes rated Good or Outstanding. We do however have more to do to ensure all services are at this level on a sustainable basis. Consequently, the governance and audit capability has been increased during the year to ensure we embed improvement and are able to objectively assess performance. We will continue to improve what we do so that people in our care live well and enjoy our services.

I would like to thank our Managing Director, Stuart Cross, the Executive Management Team and all the staff at our Head Office in Faversham, for their hard work over the year to secure Avante Care & Support’s position as a quality provider of care services.

This year has seen strong performance in our two Home Care Services. Positive working relations with the two Local Authorities has supported service improvements, so we are able to provide positive support for people at home. This is in line with the national care agenda and will continue to be a development area for Avante Care & Support.

Finally, I would express my gratitude to the Board of Trustees for their hard work over the last twelve months.

Managing Director Foreword In our Impact Report last year we reported on the work we did around setting a Vision for Avante Care & Support. Staff, managers, senior management and Trustees were all involved as we discussed the reason why we provide care, and what motivates us both individually and as an organisation. As the key themes emerged, a Vision Statement was agreed by the Board – ‘Communities where everyone has a vibrant and fulfilling life’. This year we have made great progress in making this Vision a reality for the people we care for across our care homes and home care services. We have done this by encouraging our Registered Managers to provide care in their homes and services in line with their particular residents’ and service users’ needs; by providing care in line with our Philosophy of Care; by investing in our front line staff through improved pay and training; and by investing in our homes. As you go round our homes and services, you will find a variety of different visitors and activities, depending on the needs of the local home or service. In August, residents at Parkview enjoyed a two week summer ‘Staycation’ - a summer holiday brought to the residents with activities including a spa day, BBQ party, outdoor cinema night and picnic on the beach with our very own sandy beach. In our home care service, we are always looking for opportunities to enable our service users to live vibrant and fulfilling lives, including organising trips to the cinema and taking part in sponsored walks. Our Vision fits in well with our Philosophy of Care, the Eden Alternative. Many of our staff have now been on a three day training course to become Eden Associates, and we have five of our nine homes accredited on the Eden Alternative register. This year,

we hope to extend that to all nine homes and our two Home Care branches as well. Indeed, with our homes and Home Care branches registered, we believe we will be the first organisation in the country to achieve this distinction. Our regulator, the Care Quality Commission, has also recognised the progress we have made, rating seven of our nine homes as Good, and Amherst Court in Chatham as Outstanding. With only 2% of services in the country rated Outstanding, this is a tremendous achievement. Our two Home Care services are both rated Good meaning that over 90% of our services are Good or Outstanding. At the same time, we have continued to invest in our buildings so that our residents enjoy a safe, warm and homely environment. This year, we have completed a £1m building project at Weybourne which involved replacing the roof, making some internal alterations and redecorating the communal areas. Our gardens are an important feature of our homes, and at Pilgrims View, relatives have been hard at work planting a variety of plants and shrubs to brighten the outside space. There was also an official opening of ‘Brian’s Café’, named in honour of resident Brian, who has been living at Pilgrims View for over 27 years. Finally, in 2018, we were pleased to announce the appointment of Jackie Churchward-Cardiff, as our Chair of Trustees, succeeding Peter Smallridge CBE. Jackie’s background and wealth of experience will maintain the excellent standards of governance we have enjoyed at Avante Care & Support, and will enable us to move forward and take opportunities with confidence.

My mother attended the day centre for a number of years. All the staff gave her the support she needed. She enjoyed the freedom of getting out of her home to be with friends in a safe and secure environment.

Our staff During the course of 2018, we have supported around 1,260 staff across 11 locations through consultations for changes to staff structures, the implementation of a Pay Strategy, a continual focus on recruitment, learning and development, employee relations and health and safety.

What we achieved: With workforce planning and effective consultation, we implemented changes to Care Home staff structures to improve the quality of services and training we provide.

Registered Managers completed the Skills for Care “Well-Led” Leadership Training. We retained the Investors in People Silver Award. We developed and implemented a Pay Strategy to improve rate of staff recruitment and retention of the right people to deliver high quality services. We supported recruitment and development and reduced staff turnover.

We held consultation meetings to update staff on our business objectives and to receive and act on staff feedback.

Learning and development Our support to our Care Operations continued to evolve in 2017/18. 512 internal courses were delivered by our internal trainers. These provided 3058 training places for staff, ensuring Home Care and Support and Care Home services achieved their 90% course completion targets. The number of available courses increased to 42 and there are new courses in development as standards and clients’ needs change. 52 induction programmes were delivered to 504 new recruits and 193 employees also achieved a QCF Health & Social Care qualification in 2017/18. 107 Carers achieved their level 2 certificate in the Safe Handling of Medication. We secured funding of £6,345 from the Workforce Development Fund to offset staff costs for eligible qualifications.

Future aims: We continue to aim for consistent quality and customer focus in delivering effective and comprehensive support across all areas of the business. We achieve this by partnering with our operational and functional colleagues to provide the processes and services required to effectively manage and deliver high quality services to vulnerable older people.

We inspected and audited Avante Care & Support’s Health & Safety systems and our premises to ensure our compliance with health and safety law.

Philosophy of Care The Avante Care & Support Philosophy of Care is the Eden Alternative. The philosophy provides a framework for all staff to understand and practice person centred care. We have followed the Eden Alternative in part since 2006, but have relaunched it across the organisation. The Eden Alternative is not a project, as there is no beginning and no end. Rather, it is an ongoing philosophy. The aim of the Eden Alternative is to change the task based model of care to one where care is provided on a person centred basis. The goal is to improve the wellbeing of residents and service users, and those who care for them, by transforming the communities in which they live and work. Our aim is to eliminate loneliness, helplessness and boredom, which are the three prevalent states of mind most observed in older people and those living with dementia. Through training and support, staff apply their learning through their delivery of care. Staff become more aware of how their communication and actions are perceived by someone living with dementia. Also, staff look at how they can enrich the lives of the people they care for by ensuring they are helped to engage in useful activities, to always be cared for in a sensitive and respectful manner and to feel safe in their surroundings.

9

It is essential that all staff working for the organisation have a working knowledge of this philosophy. Therefore, we have added an introduction to the Eden Alternative to our induction programme for new staff. In addition, for a service to be on the registry, over 10% of staff must be Eden Associates. The comprehensive three day Eden Associate course enables staff to gain a full understanding of the Eden Alternative philosophy. At present there are 70 Eden Associates across the organisation. Eden Awareness training continues to be provided twice monthly, in London and in Kent. Feedback from staff is always positive and staff feel encouraged to give the care and support each individual resident or service user needs. Avante Care & Support is fully committed to ensuring all services achieve accreditation and join the Eden UK Registry.

Mum has developed friendships with the residents and staff, she enjoys being involved in activities and outings. It's a wonderful environment for her and for us, her family.

Bridging the generations In July, Bridge Haven welcomed three little visitors from a local playschool. Magpies, a village community charity run playschool based in Boughton, Kent, took three of their children along to Bridge Haven to meet with residents and staff for the morning.

“It was such a success we have already started talking about our next visit

Our care homes 2018 has been a good year for our care homes, particularly in terms of positive feedback from residents and relatives about the care provided by our excellent staff. Our CQC ratings across our nine care homes overall, evidence the provision of caring, safe, effective, and responsive services which are well-led by committed and competent managers. To improve even further we have employed a new Head of Care Homes. We are proud of what we have achieved and continue to work hard to grow, learn and improve so that more of our care homes are seen as “Outstanding” services.

What we achieved: Care Quality Commission ratings maintained and improved across the services.

Future aims: We have built on our robust baseline of quality across the homes. While we have seen solid and consistent performance across our services, we recognise that we have more to do in areas such as the use of technology, for example, the introduction of electronic care plans and interactive activities for residents with dementia. This year we are consulting with all our stakeholders to develop a new Care Strategy. As we look ahead, the strategy will focus on residents’ wellbeing, the care home environment and the systems we use so more of our homes can move from ‘Good’ to ‘Outstanding’ CQC ratings. With a “can-do” approach we will adapt to the challenges in the industry and evolve to continue to drive improvements in all our care services.

Increase in positive reviews and scores on Carehome.co.uk over the previous year.

Occupancy increased across care homes. Increase in recruitment of permanent staff. Reduction in agency costs in our care homes

from the previous year saving a total of £281,936.

Recruitment of Head of Care Homes to manage and oversee operations.

Refurbishment of Weybourne completed.

Our home care service Our two branches of home care and support, located in Kent and Bexley, provide services on behalf of the Local Authority, NHS Continuing Health Care and self-funding service users. Our services care for and support over 390 service users at any one time, delivering 3800 hours of home care per week, employing over 200 front line care staff and over 30 staff within our office teams. The introduction of the new Vision has provided an opportunity to reflect on how we bring our vision to life for the individuals we provide care and support for.

What we achieved:

Future aims: Our 2017 Annual Customer Survey provided us with important feedback in line with our Vision. However there are areas which we can improve on, one of which is ensuring that people who use the service are aware of our Philosophy of Care. To address this, throughout 2018 we have embedded the Eden Alternative philosophy within the service. This philosophy believes that no matter how old we are or what challenges we live with, life is about continuing to grow. We hope that adopting the Eden Alternative philosophy will help us to achieve our Vision and we are looking forward to the journey this year and beyond.

CQC inspections for Kent and Bexley have rated the services as ‘Good’ across all five key lines of enquiry. Achieved ISO14001 accreditations for our commitment to reducing environmental impact. Retaining the Investors in People SIlver award. 2017 Annual Customer Survey displayed

increased satisfaction across the service with 97% of service users satisfied with the service overall.

Our Bexley service was awarded ‘A Top 20 Home

Care Provider Award 2018’ by Homecare.org.uk.

Our service overall was awarded ‘A Top 20 Home Care Group Award 2018’, highlighting the most recommended Home Care Groups in the UK.

My sister and I are more than happy with the care our dad receives. If at any time there have been questions or any issues the carers have notified us. The ladies that care for our dad really are amazing.

The service I receive from Avante is outstanding. Personal care is always given with respect for my privacy and dignity. I am extremely pleased and don't know now what I would do without them!

My very first impression was wonderful, a friendly smile and reassurances from staff members that my mum would be really well looked after. When you are making this huge decision that means everything.

Quality standards Our quality team work closely with senior managers in the organisation to continuously improve the quality of our services and to ensure compliance with regulators and the commissioners. In 2017/18 the Quality Team reviewed our current tools and systems for measuring quality assurance across the services, and as a result developed and then implemented a new Quality Improvement Programme, launched in April 2018. The Quality Improvement Programme is the overarching title given to Avante Care & Support’s entire quality programme, encompassing the management and review of our quality systems to ensure they remain fit for purpose. We do this through research and best practice, auditing our homes, services and departments, using individual Continuous Improvement Plans; researching work based practice development and overseeing policy review and ratification. Two new positions were created in the Quality Team to support the Quality Improvement Programme. The Quality Improvement Practitioner works alongside Registered Managers and their teams to help them successfully deliver their objectives, review their Continuous Improvement Plans, to maintain sustained improvement and coach staff and teams through practice development to deliver improved outcomes for residents. The Quality Administration Apprentice supports the Quality Assurance Officer with the collection and reporting of Key and Quality Management Indicators for the organisation. She also researches guidance and best practice to share with Managers, collates information and intelligence for internal audit and supports policy review.

Avante Care & Support remains committed to the Eden Alternative as our Philosophy of Care and to having all of our services on the Eden Registry by 2019. In the year 2017/18 5% of our workforce were trained as Eden Associates. In addition 110 staff members attended one day Eden Awareness training, making a total of 141 staff members trained in the principles of Eden during 2017/18. Amherst Court, Court Regis, Parkview, Pilgrims View and Puddingstone Grange all became, or retained their Eden accreditation. During 2018/19 the four remaining homes Bridge Haven, Northbourne Court, Riverdale Court, Weybourne and our two Home Care & Support Services in Kent and Bexley will be working towards gaining their Eden accreditation. The national Your Care Rating satisfaction survey was completed for both residents and relatives in December 2017 with Avante Care & Support receiving an overall satisfaction ratings of 82% from residents and 84% from relatives. We are working hard to improve our results and in 2018 we have been focusing on our weakest areas to improve our overall ratings in the latest survey. The organisation improved our ratings with the Care Quality Commission, with eight care homes rated Good or Outstanding and both of our Home Care and Support Services rated Good. Our Quality Standard Review Group continues to meet every six weeks in order to review quality across the services and update policy and procedures to ensure we remain a learning organisation.

Support and companionship Avante Care & Support provides Home Care and Support across Kent and Bexley allowing those living in their own homes the independence they desire whilst receiving care and support. The Home care and support service provides a range of services which include support with shopping, walking the dog through to personal care and medication assistance. A recent cinema trip with a home carer and client is a perfect example of the support that can be provided at Avante Care & Support. Vera who uses the home care and support service asked her carer if she could accompany her and her husband to the cinema to see the new Mamma Mia film ‘Here we go again’. The couple are both in their eighties, Vera is in a wheelchair and day trips out together are not as easy as they used to be. They had not visited the cinema since the first Mamma Mia film and that was over 5 years ago! The call was booked at head office through the normal system but this time instead of the carer supporting Vera at home the call was booked for the carer to collect them both and take them to the cinema at Lockmeadow, Maidstone. Sue Jarrett, Home Care and Support carer said ‘I sat across the aisle from Vera to allow them both some space and independence and to let them enjoy the film with each other. They were both in my view in case I was needed but what was important they could enjoy an afternoon out with each other, like they used to. Throughout the whole film they held each other’s hands and tapped their feet to the music.

17

When the film was over the couple were beaming. It was amazing to see how a simple cinema trip had made such a difference to Vera and her husband. It was evident that a simple day out gave them the opportunity to appreciate each other’s company in a different environment. Since this trip out Vera has been talking about future day trips including lunch at the local pub!’

Pet interaction for Parkview residents In August 2017 Parkview in Bexleyheath, was visited by Nightingale dogs, a company that specialise in therapeutic dog activities for people living with dementia. When the dogs arrived in the home there was an instant positive reaction from residents who engaged with the dogs which in turn stimulated conversations with each other. During the day, there was a conversation between staff and residents about the idea of having a pet dog join the home.

Meet Rosie Rosie is a Jack Russell and belongs to Parkview’s Activity Coordinator Jo Thompson. Jo was looking to rehome Rosie as her husband had changed jobs, which meant leaving Rosie on her own at home for longer periods.

this. Rosie is now part of the Parkview family and visits during the day and goes home with Jo after each shift. It works really well and the residents love her.” Not only has Rosie become a loyal friend to many residents but she is also a great form of exercise for residents and staff as she needs daily walks. Some residents who haven’t been so keen to leave the home before are now seen walking with Rosie and staff to the local park in Bexleyheath. Like all the pets at the home Rosie has become part of everyday life at Parkview and takes parts in the many games and activities throughout the week. She also likes a day trip out on the minibus!

Jo had previously brought Rosie into Parkview a few times and she was always welcomed by residents who would feed and stroke her. Before looking to rehome Rosie, Home Manager, Pat Collis, explored ideas with some of the residents on how Rosie could become part of the family at Parkview. “We have a few pets at the home including cats, goldfish and guinea pigs and we were all really keen to introduce another pet to join the family. When Jo started to bring Rosie into the home she was so popular with our residents it made sense to see if we could look at other options with

Property and development The Weybourne improvement programme was completed in January 2018. Having invested ÂŁ1.25m and after 16 months of work, construction of a new pitched slate roof over the existing flat roof was finished, greatly improving the aesthetics and thermal insulation of the building.

What we achieved: Re-locating the main Weybourne entrance and creating new reception and administration area. Upgrading of the heating system at Weybourne to

improve heat retention within the home.

Upgraded and modernised the bathroom, wet room and toilet areas at Weybourne.

Lounges and dining areas completely

re-furbished with upgraded safety systems throughout the home.

Modernised the laundry and linen storage area and installed new washing machines and dryers at Pilgrims View.

Future aims: We are looking forward to starting the Pilgrims View improvement programme. Lounge and dining areas will be re-furbished complete with decorations, curtains and furniture. Social spaces will be upgraded, installing new lighting and fire alarm systems. As part of the Bridge Haven improvement programme we are currently working on a corridor extension attached to four bedrooms, together with food servery alterations and the conversion of an assisted bathroom to an assisted shower wet room. In line with the Eden Alternative, our Philopsohy of Care, we have been busy working with the Home Managers to identify new areas at the homes to encourage more involvement with food preparation and cooking. Amherst Court are currently having new kitchen serveries which will be followed with similar refurbishments to both Northbourne Court and Riverdale Court, complete with decorations, furniture and curtains in the dining areas. We continue to make other improvements which this year has included a major refurbishment of the laundry at Pilgrims View. Our goal was to upgrade and create an efficient laundry washing area. The addition of reliable and durable machines that could be repaired easily was a priority for us. Miele, a manufacturer of high-end domestic appliances and commercial equipment, who supplied the laundry washing facilities, were particularly impressed with the projects outcome and what was achieved, that they wrote an article in the national magazine FM Industry.
