Clinical Services and Community Health Programs:

Adult & Child Primary Care Clinic 850-595-6500, option 2 Primary care services are available to Medicaid Managed-care adult and pediatric clients who are assigned to/have selected FDOH-Escambia as their provider. Call to schedule an appointment. AIDS Drug Assistance Program (ADAP) 850-595-6767 ADAP assists uninsured and underinsured individuals living with HIV/AIDS in obtaining access to life-saving medications. Participants must meet eligibility requirements, have at least one prescription for a medication on the ADAP formulary, and have viral load and CD4 test results no more than six (6) months old. Area 1 HIV/AIDS Program 850-595-6500, extension 1505 The Area 1 HIV/AIDS program provides services to persons living with HIV and AIDS in Escambia, Santa Rosa, Okaloosa and Walton counties in Northwest Florida. Services may be provided directly by program staff or through contracts with area providers. Call for more information.

Breastfeeding Program 850-595-6670, option 2 The breastfeeding program provides lactation education and support, including breast pump loans, to eligible mothers. Call for more information.

Child School Physicals 850-595-6500, option 2 Physical exams required for school attendance are available for pre-K through 12th grade students. Services are provided for a set fee. Call to schedule an appointment or inquire about fees.

Epidemiology 850-595-6683 The epidemiology program monitors incidences of communicable diseases and conditions that significantly affect public health. Call for more information.

Family Planning Clinic 850-595-6500, option 2 The Family Planning clinic offers a variety of birth control methods and makes appropriate referrals. Family Planning also provides abstinence counseling and information about natural family planning. Participants will receive a clinical exam by a nurse practitioner or doctor which may include STI testing. The cost for a Family Planning visit is based upon ability to pay. Florida Breast and Cervical Cancer

& Early Detection Program

The Florida Breast and Cervical Cancer Early Detection Program (FBCCEDP) promotes early detection through connecting at-risk populations to resources for screening and treatment. The program targets women 50 to 64 years of age without Medicaid or insurance who live in Florida. Call for more information.

HIV Prevention -- Pre-Exposure Prophylaxis (PrEP) Services 850-595-6345

and Post-Exposure Prophylaxis (PEP) Services

Screens candidates and coordinates treatments to prevent HIV infections in Escambia County residents who are at high-risk for exposure, or have been exposed, to HIV.

Immunizations 850-595-6500, option 2

FDOH-Escambia offers immunizations for children and adults. All childhood and adolescent immunizations are offered in accordance with the Bureau of Immunizations Schedule and the Florida School Guidelines. No-cost vaccinations are available to eligible children from six weeks through 18 years of age. Clients who are 19 years old and older may qualify for the Adult Vaccine Program that offers a certain vaccines for $18 each. A separate fee schedule exists for adults who do not meet the eligibility requirements for the Adult Vaccine Program. Call for a same-day appointment. Sexually Transmitted Disease (STD) Clinic 850-595-6500, option 2 The STD clinic provides testing and treatment as well as education and counseling in a confidential setting. Tuberculosis 850-595-6500, extension 1210 FDOH-Escambia’s tuberculosis program collaborates with clinicians to ensure patients receive effective and timely treatment and that contact investigations are performed to minimize transmission. Call for more information.

WIC 850-595-6670 The Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides food items that support nutrition for growth and development, as well as nutrition education and breastfeeding support, for pregnant and breastfeeding women, infants, and children under 5 years old. Diabetes Prevention Program 850-595-6500, extension 1814 Certified Lifestyle Coaches deliver the CDC’s evidence-based Prevent T2 curriculum. Class dates, times, and locations vary. Call for more information or to request materials for your practice or waiting room.

Healthy Start 850-595-6641 Healthy Start coordinates services, for pregnant women and children ages 0 to 3 years, to support healthy birth outcomes and early childhood development. Services consist of home visits by health professionals, childbirth education, breastfeeding support, and facilitating access to local service agencies and other resources. Public Health Preparedness 850-595-6500, extension 2201 The public health preparedness (PHP) program provides operational and tactical level planning to support preparation, response, and recovery from natural and man-made disasters.

School Health 850-484-5100 School Health nurses and health educators support and deliver education on health topics in Escambia County District Schools. Call for details. Tobacco Free Florida in Escambia County 850-595-6500, extension 1831 The Tobacco Free Florida in Escambia County (TFF-Escambia) program spearheads tobacco use prevention, social norm and policy change, and cessation initiatives for the entire Escambia County community. Vital Statistics (Birth and Death Records) 850-595-6500, option 3 Vital statistics issues birth certificates for persons born in Florida and death certificates for persons who died in Florida from 2009 to the present. We Care 850-595-6500, extension 1082 Connects financially-vulnerable Escambia County residents requiring medical specialty care to medical and auxiliary care professionals who are willing to donate their services so that individuals with needs have access to better health.
