Measuring Quality of Life

As part of this commitment, At Home clients are given the opportunity to participate in the new Quality of Life screening.
What is the Quality of Life – Aged Care Consumer screening (QoL-ACC)?
The Quality of Life – Aged Care Consumer Instrument (QoL-ACC) is a new tool developed by Flinders University. The tool was developed in partnership with older people receiving aged care services, and their families. The QoL-ACC screening reviews six key areas that impact quality of life including mobility, pain management, social connections and leisure activities.
What are the benefits?
The Quality of Life – Aged Care Consumer screening will help you to identify goals in areas that are important to you. Your Care Facilitator will then support you to design services that will help you achieve those goals.
Outcomes from QoL-ACC screenings also help BaptistCare to better understand how we can improve quality of life for our clients.
What does the screening involve?
Your Care Facilitator will complete the QoLACC screening by asking you a short number of questions. After completing the screening, a score is generated and used to identify areas you may choose to focus on. This score also helps to measure changes over time.
The QoL-ACC is completed at the commencement of your BaptistCare service and repeated as part of the ongoing review of your goals and care need. This will show progress made towards your goals, help identify new strategies you would like to try, or different areas you might choose to focus on.
Participation in the QoL-ACC screening
Our Care Facilitators have been trained to complete the QoL-ACC with you as part of our assessment and care planning process. You can also choose to complete the QoL-ACC independently and discuss the results with your Care Facilitator afterwards. If you would prefer to complete the QoL-ACC independently, speak to your Care Facilitator.
