End-of-life care coordination service

Personalised support focused on education, advocacy and access to services for people with a life-limiting illness.

What is endof-life care coordination service?

End-of-life care coordination is a service designed to support people with a lifelimiting illness to enhance their wellbeing. The care coordinator works with the person and their carers to assist them to receive the support they require during this stage of life.

• Assistance in defining and achieving personal goals

• Information about specialised services and support, to access these services

• Coordination of care between services

• Education about palliative care and symptom management

• Support to develop advance care plans and directives, and appoint legal representatives

• Referrals to service that assist with everyday activities

• Assistance when moving between care settings

• Support to access culturally appropriate services and resources.

This program is fully funded by the Hunter New England and Central Coast Primary Health Network and there is no cost to access it.

How do I access the program?

Who can refer:

• The person with the life limiting illness

• Family / friends / carers

• GP’s, nursing and allied health professionals

• Local palliative care services.
