*Applicants must supply the following documents when bringing in their completed application packet:

- ORIGINAL birth certi cates for entire family – adults and children

- ORIGINAL social security cards for the entire family – adults and children

- Picture ID for all family members 18 years of age and older

- All family members 18 years old and older must appear to provide picture ID and sign application documents
