Lowering BMI Before Surgery Can Provide Better Outcomes
DEAR DR. ROACH: I’m 72, and I have had arthritis for three years: two years in both knees, and one year in my right hip and the base of my spine. I am 5’3” and weigh 221 pounds, with a body mass index (BMI) of 39. I lost 23 pounds from January to June.
What is the BMI needed for an orthopedic surgeon to do a hip replacement for someone with osteo arthritis — without a broken hip? I recently had an assessment of my hip X-rays. I was told I’m at the outer cut off of a BMI of 39, so I’m eligible for a hip replacement operation. Then, a few weeks later, I saw the surgeon, who ended up rejecting me — claim ing that 39 isn’t the outer limit, 35 is.
After a long talk, the surgeon said he’d do it for me, but I’d get a better result if I lose 20 more pounds and get my BMI down to 35. I felt jerk ed around, but said I’d wait and lose weight. I’m also trying to find a sur geon I like more than him.
So, what is the BMI needed for knee surgery: 35 or 39? — L.C.
ANSWER: There is no standard cut off BMI for joint replacement surgery. It is true that people with a BMI over 40 are at higher risk for medical com plications; however, it is also true that people with severe arthritis and a high BMI still get improvement in their quality of life with surgery. Denying a person a cost-effective surgery that can greatly improve their quality of life simply because of a BMI number (which, in itself, is a flawed metric) is inappropriate.

I can understand why you would want to see a different surgeon, as it sounds like you were not treated well. However, the surgeon was right that surgical outcomes are better if a person can lose at least 20 pounds, according to a 2019 study. This includ ed shorter lengths of stay in the hospi tal and a lesser chance of needing sur gical revision. I wonder, though, if part of the benefit seen in the study was due to increased exercise prior to surgery, which is known to be beneficial in sur gical outcomes. ***
DEAR DR. ROACH: My 45-yearold son has hemochromatosis. The treatment is phlebotomy of a pint of blood, sometimes every two weeks, until the iron level is normalized. My understanding is that the blood is discarded. Is there anything wrong with the blood? If not, I would think blood banks would welcome the donation. What are your thoughts on this subject? — E.B.
ANSWER: Hereditary hemochroma tosis is caused by the body absorbing too much iron due to a genetic defect. With no way to get rid of iron, it builds up and damages many organs, especially the heart, bone marrow, joints and adrenal gland. As you say, the best treatment is to remove iron in the form of red blood cells. This is effective at preventing most of the organ damage if started quickly enough.
There has never been anything wrong with the blood of people with hereditary hemochromatosis, and the FDA here in the U.S. has always allowed the blood to be donated. However, until recently, the American Red Cross has not accepted blood donations from people with hered itary hemochromatosis. I am pleased to say that they have just changed their pol icy and will now be using this precious resource to help others, so long as the person meets all the criteria for being a blood donor. The blood will undergo all the standard and rigorous testing prior to being pronounced safe for use.

Keeping it safe
With Winter’s ice, snow and cold upon us, there will be increased opportunities for injuries around the home.
let’s talk about some home safety hazards, how we can try to prevent them, and what to do if it happens.
Falls are one of the most common safety hazards around the home.
From babies falling off high places, or toppling as they learn to walk, to seniors who may be unsteady on their feet or tripping on unsafe paths throughout the home.
there are several things we can do to reduce the hazard.
Buckle babies up whenever possible. they can be quick little movers and it takes that one second for something bad to occur when you’re reaching for something or looking away.
as toddlers begin to walk, they usually start out with the unbalanced run. although they are as cute as can be doing so, they are running for danger towards that furniture corner and will get hurt.
Pad those hard pointed surfaces and remove whatever they can trip over until they are no longer a threat to child safety.
remove the pretty glass décor and dangerous items that kids can grab and injure themselves, let alone ruin on you. all the things that make the home beautiful can come back when the child understands the hazard. some learn quicker than others, hopefully not through experience. install safety gates at dangerous areas and stairs.
When reprimanding the child of what not to do or touch, do so with a stern voice and tell them why. they understand more than we think sometimes. When they listen, give them credit for it, showing them how you notice they listened and how proud you are.
take a walk around the home and identify safety hazards.
as we age our vision and balance isn’t what it used to be.
Falls occur when we’re trying to be the quick movers we used to be (ha, yes, i’m including myself in this category and from the voice of experience). those steps going up or down are our enemy. taking your time and holding on is a good start. if we shouldn’t be using the stairs, then wait for help and don’t be too shy or proud to ask for it. it’s better than needing help with the potential injuries incurred, right?

Check the thermostat to ensure it’s working properly by increasing the temperature by 5 F

Cornucopia Vase
This strange piece of art pottery is a cornucopia vase, popular in the 1930s. It was made by the Roseville Pottery Company. The pattern name is White Rose.
Roseville Pottery Company started in Roseville, Ohio, in 1890. A sec ond plant was opened in Zanesville, Ohio, in 1898. White Rose pattern was introduced in the 1940s. White Rose cornucopia vases were made in two sizes, 6 inch (143-6) and 8 inch (1448). By the late 1940s, there were over 50 shapes in the White Rose pattern glazed in different colors such as sea blue, coral or autumn brown. Repro ductions of Roseville pottery have been made in China and sold since the 1980s. The fakes are a different size, so they can be identified. A real, blue Cornucopia White Rose vase that is 8 inches tall sells for $100 to $125.

Matchbook collectors are called “phil lumenists.” There are local, regional, national and international clubs for collectors. The Rathkamp Matchcov er Society is the world’s oldest phil lumenic organization. The Society’s website, matchcover.org, lists swap meets and conventions. Collectors look for unused matchbooks with old ads, travel themes and other subjects. If there is a swap meet near you, you can see what covers are selling for and you might be able to find a buy er. Don’t let someone pick out the best ones and leave you with a bunch that are not worth much and will be hard to sell. If you plan to ship your collection to a buyer, remove the matches before shipping.

Lowering BMI Before Surgery Can Provide Better Outcomes
DEAR DR. ROACH: I’m 72, and I have had arthritis for three years: two years in both knees, and one year in my right hip and the base of my spine. I am 5’3” and weigh 221 pounds, with a body mass index (BMI) of 39. I lost 23 pounds from January to June.
What is the BMI needed for an orthopedic surgeon to do a hip replacement for someone with osteo arthritis — without a broken hip? I recently had an assessment of my hip X-rays. I was told I’m at the outer cut off of a BMI of 39, so I’m eligible for a hip replacement operation. Then, a few weeks later, I saw the surgeon, who ended up rejecting me — claim ing that 39 isn’t the outer limit, 35 is.
After a long talk, the surgeon said he’d do it for me, but I’d get a better result if I lose 20 more pounds and get my BMI down to 35. I felt jerk ed around, but said I’d wait and lose weight. I’m also trying to find a sur geon I like more than him.
So, what is the BMI needed for knee surgery: 35 or 39? — L.C.
ANSWER: There is no standard cut off BMI for joint replacement surgery. It is true that people with a BMI over 40 are at higher risk for medical com plications; however, it is also true that people with severe arthritis and a high BMI still get improvement in their quality of life with surgery. Denying a person a cost-effective surgery that can greatly improve their quality of life simply because of a BMI number (which, in itself, is a flawed metric) is inappropriate.

I can understand why you would want to see a different surgeon, as it sounds like you were not treated well. However, the surgeon was right that surgical outcomes are better if a person can lose at least 20 pounds, according to a 2019 study. This includ ed shorter lengths of stay in the hospi tal and a lesser chance of needing sur gical revision. I wonder, though, if part of the benefit seen in the study was due to increased exercise prior to surgery, which is known to be beneficial in sur gical outcomes. ***
DEAR DR. ROACH: My 45-yearold son has hemochromatosis. The treatment is phlebotomy of a pint of blood, sometimes every two weeks, until the iron level is normalized. My understanding is that the blood is discarded. Is there anything wrong with the blood? If not, I would think blood banks would welcome the donation. What are your thoughts on this subject? — E.B.
ANSWER: Hereditary hemochroma tosis is caused by the body absorbing too much iron due to a genetic defect. With no way to get rid of iron, it builds up and damages many organs, especially the heart, bone marrow, joints and adrenal gland. As you say, the best treatment is to remove iron in the form of red blood cells. This is effective at preventing most of the organ damage if started quickly enough.
There has never been anything wrong with the blood of people with hereditary hemochromatosis, and the FDA here in the U.S. has always allowed the blood to be donated. However, until recently, the American Red Cross has not accepted blood donations from people with hered itary hemochromatosis. I am pleased to say that they have just changed their pol icy and will now be using this precious resource to help others, so long as the person meets all the criteria for being a blood donor. The blood will undergo all the standard and rigorous testing prior to being pronounced safe for use.

Keeping it safe
With Winter’s ice, snow and cold upon us, there will be increased opportunities for injuries around the home.
let’s talk about some home safety hazards, how we can try to prevent them, and what to do if it happens.
Falls are one of the most common safety hazards around the home.
From babies falling off high places, or toppling as they learn to walk, to seniors who may be unsteady on their feet or tripping on unsafe paths throughout the home.
there are several things we can do to reduce the hazard.
Buckle babies up whenever possible. they can be quick little movers and it takes that one second for something bad to occur when you’re reaching for something or looking away.
as toddlers begin to walk, they usually start out with the unbalanced run. although they are as cute as can be doing so, they are running for danger towards that furniture corner and will get hurt.
Pad those hard pointed surfaces and remove whatever they can trip over until they are no longer a threat to child safety.
remove the pretty glass décor and dangerous items that kids can grab and injure themselves, let alone ruin on you. all the things that make the home beautiful can come back when the child understands the hazard. some learn quicker than others, hopefully not through experience. install safety gates at dangerous areas and stairs.
When reprimanding the child of what not to do or touch, do so with a stern voice and tell them why. they understand more than we think sometimes. When they listen, give them credit for it, showing them how you notice they listened and how proud you are.
take a walk around the home and identify safety hazards.
as we age our vision and balance isn’t what it used to be.
Falls occur when we’re trying to be the quick movers we used to be (ha, yes, i’m including myself in this category and from the voice of experience). those steps going up or down are our enemy. taking your time and holding on is a good start. if we shouldn’t be using the stairs, then wait for help and don’t be too shy or proud to ask for it. it’s better than needing help with the potential injuries incurred, right?

more so for seniors. they can be slippery as well. remove any rugs that can pose a problem and use slip proof backings. use that cane or walker if needed even for short trips from one room to another.
if you are thinking about climbing on that ladder, think again… Only Clarke Griswold can survive a ladder accident without landing in the er with injuries. the rest of us will have broken something or worse as you can imagine if the ground appears slippery, don’t take any chances. if there is no opportunity to salt the area, and your shoes are not slip proof, avoid the hazard. Wait for help.
We may have loved to shovel snow for exercise and the need to get out of the driveway, however that is a recipe for trouble as we age. Falling and exerting the heart can lead to a trip to the hospital.
it can be frustrating to have to wait for help and costly to hire someone, however a wise decision for the most part.
Wear an alert item or carry the phone with you should you need help in an emergency. Fire is another top hazard around the home.

seniors may not have the tooth strength to chew food totally or have throat issues and end up in a choking situation.
Keeping food moist and cut in small portions can help.

Household tools can become hazards if not used safely or maintained properly.
Keep sharp tools safely stored away from children’s reach.
if you think it’s not wise to trim those shrubs or cut that branch that’s beyond your reach, let it be until you are not alone and again, ask for help.
Pool drowning can happen even after summer is over.
Beyond keeping the typical pool safety rules in swimming season, it is also dangerous in the off season when the pool is covered. educate the children of the dangers of going near the covered pool and keep the area lit so it can be seen in the mostly dark hours of Winter.
as these hazards are particularly popular causes of injury, consider having talks with loved ones of safety precautions and how to react when something happens.

Cornucopia Vase
This strange piece of art pottery is a cornucopia vase, popular in the 1930s. It was made by the Roseville Pottery Company. The pattern name is White Rose.
Roseville Pottery Company started in Roseville, Ohio, in 1890. A sec ond plant was opened in Zanesville, Ohio, in 1898. White Rose pattern was introduced in the 1940s. White Rose cornucopia vases were made in two sizes, 6 inch (143-6) and 8 inch (1448). By the late 1940s, there were over 50 shapes in the White Rose pattern glazed in different colors such as sea blue, coral or autumn brown. Repro ductions of Roseville pottery have been made in China and sold since the 1980s. The fakes are a different size, so they can be identified. A real, blue Cornucopia White Rose vase that is 8 inches tall sells for $100 to $125.

Collectors look for unused matchbooks with old ads, travel themes and other subjects. If there is a swap meet near you, you can see what covers are selling for and you might be able to find a buy er. Don’t let someone pick out the best ones and leave you with a bunch that are not worth much and will be hard to sell. If you plan to ship your collection to a buyer, remove the matches before shipping.

The Friends of the West Warwick Public Library will hold their Annual Holiday Basket Raffle from November 28-December 10. More than two dozen themed baskets for adults and children will be featured. Tickets are $.50 each or 3/$1 and may be purchased at the Circulation Desk of the library, located at 1043 Main Street. The winning tickets will be drawn on December 10 at 1PM. The library will be open that Saturday from 12-4PM for a special Food Festival. All funds raised will help support library programs and services.
