WELCOME FROM THE EDITOR Dear Reader and Supporter, Welcome to the first special issue of BEAUTY, a publication launched by The Safety in Beauty Campaign, to mark our three year anniversary as a public safety campaign.

For those of you unfamiliar with Safety in Beauty, the campaign was launched in October 2013, as a response to the growing dissatisfaction and devastation caused by dubious beauty and cosmetic treatments and providers. As a campaign, our endeavour is to help raise awareness on social media, of unsafe practices and activities that may potentially pose harm or risk to members of the public. Our mission is to empower you to make safer and better informed choices. We all deserve to look and feel our best, but not at the price of unscrupulous practitioners or unsafe activities. We are proud to launch this first special issue in the centre of England, namely, the beautiful

West Midlands, home to a vast array of elite beauty, cosmetic, aesthetic, and surgical specialists, all dedicated to passionately helping you look and feel your best. About Safety In Beauty

ABOUT SAFETY IN BEAUTY l Launched

in 2013, The Safety in Beauty Campaign was created to protect those seeking and undergoing cosmetic and beauty procedures, by empowering safer and better-informed choices, and by giving the public a place to turn to if standards or services proved harmful. There is very little regulation currently in the UK to oversee the beauty and aesthetic industry. Everybody has the right to look and feel their best, but not at the cost of unscrupulous activities or individuals. Our campaign is working tirelessly to influence change, so that a safer, and more transparent industry can exist in the near future. We have worked hard to give the public access to legal, emotional and physical support. There is currently no other resource Botched Surgery Abroad

in the UK, that can offer the public such a pathway, and we are proud to have created one. The campaign is supported by volunteers, all of whom are professionals in their fields of respective expertise, ranging from medical, psychological, and legal specialty. We are proud to state that our organization is entirely self-funded; we do not rely on the public to sustain our work or endeavours. Our Campaign received 187 complaints between October Botched Cosmetic Injections

2014 and October 2015, with lip injections and dermal fillers being a primary area of concern. Complaints also covered a broad range of concerns, from unhygienic premises, fake machinery and devices, to substandard treatments, by under qualified individuals. n the ever growing beauty industry (Transparency Market Research) predicts that the market will reach over $7.1bn by 2020 alone), consumers are faced with more and more options when it comes to everything. e doubt Kylie Jenner herself could have imagined that the world would become obsessed with lip fillers after she decided to have her lips injected at the tender age of 17. Her pout is now so famous that it is coveted by women everywhere and has even resulted in her launching a range of uber successful lip kits. There is no denying that the teenager has considerable influence, in particular on young girls who admire her flawless appearance and glamorous lifestyle. Her lips even sparked the dangerous ‘shot glass’ craze, where those wanting the bee stung lips effect would place their mouth over the opening of a cup or glass and suck in until the air vacuum caused their lips to swell up. It could be said that her lips were the making of her - they certainly contributed to her being catapulted into the limelight. As a consequence of her lips she is now as widely followed on social media as her older sisters the Kardashians, and attracts almost as much press attention after previously being somewhat in their shadow. But what would make such a young girl want to alter her appearance in that way? Kylie was already beautiful, but seemed to want a more unnatural appearance to her lips. Her lip fillers were not subtle ones and yet for a year she denied she had undergone any injections at all until she finally admitted to the procedure on an episode of

Although the shot glass craze was of course not Kylie’s fault, her denying she had lip-fillers possibly lead some to believe that her pout was achievable via less invasive methods. Kylie’s decision to have lip fillers no doubt left many parents despairing, as their similar- age daughters were desperate to emulate her. Lip fillers, as with any cosmetic intervention, are not without risk, and of course, her

decision to undergo them at such a young age was controversial. There was much debate as to whether Kylie was old enough to make the decision for herself and also much criticism of her new look. Kylie has since admitted that she went overboard when she first underwent fillers in 2014, admitting her pout was initially way over the top. That did not stop thousands of women wanting to replicate her look however.

After that more injections are needed to keep the volume of lips. There are several hyaluronic acid fillers on the market, such as Restylane and Juvéderm, Teoysal Kiss. All hyaluronic acid fillers are injected the same way and give similar results, some also contain lignocaine, a local anesthetic. The benefits of Hyaluronic Acid Fillers are that once injected,

the gel in the filler supports and shapes the tissues of the lip enabling the practitioner to control the amount of lip volume that is created. These types of fillers can also be dissolved should something go wrong - using a special enzyme called Hyalase. This does not mean that there are no side effects: it is not unusual for those undergoing injections to suffer bruising and a degree of swelling, and some people find that lip fillers can be painful. It is imperative to check the credentials of anyone you allow to perform an aesthetic intervention. They must be skilled, qualified and experienced. Avoid the temptation to save money by opting for a special offer. It is possible that someone offering a too good to be true offer is not qualified or experienced to carry out the procedure. It is always better to opt for someone reputable. Saving a few pounds is not worth the risk of having your face permanently disfigured. SURGERY n For a more permanent fix, implants made from saline or silicone can be injected into the lips, or alternatively fat can be grafted from elsewhere on the body and injected into the lips. Costing much more at around £2,000 - £3,000 these implants can be more problematic to remove should something go wrong. There is a possibility that the lip might become infected or that the graft may not take and may have to be removed. There is the possibility of infection, hardening and scarring with the silicone implants, and also the possibility of an allergic reaction leading to redness, itching and tenderness. This is rare and can be treated by removing the implant. Implants are beneficial to those who don’t want to spend few hundred pounds once or twice a year on fillers. Again, you must thoroughly research the procedure and your surgeon before undergoing any lip implant surgery. Recovery from surgery takes around a month. Although the implants are “permanent” some reabsorption will likely occur over time but it is likely that more than half of the implant will remain.

What is breast augmentation? Breast augmentation, also known as breast enlargement or a boob job, is enhancement of the breast and this is usually done using implants, although more recently another technique where fat is transferred from other areas of the body has been used to enhance the breasts. However, this technique is not well established at the moment due to the modest results and relatively high cost.

Preparing for breast augmentation Breast augmentation can dramatically affect quality of life and improve self-esteem. However, it is important to consider the emotional impact of any surgery that will change your body shape, it is important that you are well prepared, both mentally and physically for the changes that you will go through. We have psychological and emotional support for all of our patients here at the Staiano Clinic and you will need the backup of your friends and family to help you through, it is important that they support your decision. You will also need support and guidance from your surgeon and clinic team and you should make sure that you are comfortable with the level of care and the expertise that you will receive from them. If you are going through an emotionally challenging time in your life such as a change in your job or relationship, then this may not be a good time to undergo breast augmentation and it may

be better to wait until things are more stable to allow you to cope better emotionally with the surgery. When operating on the breast it is always best to wait until the size and shape of breasts have settled and there are no significant fluctuations planned for the future in order to give you a more predictable result. This means that your breasts should have stopped growing and so ideally you should be at least 18 years old (it is important to consider whether breast implants are right for you if you are young as they are a long term commitment). Also if your weight is not stable then it is better to wait until it is before having breast implants because changes in your weight can have an unpredictable effect on the size and shape of your breasts. If you are considering having children in the future, then this can also have an unpredictable effect on your breasts and so ideally you should wait until you have finished your family before having breast augmentation. However, if you feel that this will be a long way in the future there is no problem with having a breast augmentation and then going on to have children afterwards, it will not affect your ability to breastfeed. If you have recently had a child, then it is best to wait to allow the size and shape of your breasts to settle before considering breast surgery. This should ideally be six months and preferably a year, this will also give your child a chance to grow up and perhaps be easier to manage when they are slightly older and able to walk. If you are a smoker then you

should give up as this can affect your ability to heal adequately, particularly in the first two weeks after surgery.

Types of implants Most people talk about implants in terms of their volume such as 250cc or 350cc but there is a great range of implants available and the volume is only one aspect. They can be categorised according to their shape, the contents of the implant or the coating of the shell.

The shape of the implant can be round or teardrop and each implant comes in a variety of profiles which means how far they stick out. This can be low, moderate, high or extra high. Each will give a different look and it is important to go through this with your surgeon as a low profile teardrop 250cc implant will look completely different to an extra high profile round 250cc implant therefore it is not enough to go on volume alone. The contents of the implants can be either saline or silicone. In this country we mainly use silicone filled implants as they give a more natural and longlasting result. Saline implants were used exclusively in the USA for some time while silicone implants were banned in that country. However, they are now back in use as there is no evidence to suggest they cause any systemic illnesses. Saline implants are more prone to rippling and deflation so not widely used. All implants have a silicone shell (even ones filled with saline) and this shell can be smooth, textured or polyurethane foam coated. Smooth silicone implants are not widely used in the UK in favour of the textured variety, which is said to form less capsular contracture, which is hardening of the implant over time. Polyurethane foam coated implants have an even lower risk of hardening over time but are controversial and so not all surgeons use them. However, if you are considering breast implants then it is worth doing your research and looking into all the available options.

Incision placements INFRAMAMMARY INCISION The most common placement for the incision in the UK is in the inframammary crease, which is the fold where the bra sits. The incision is usually 5cm long and can be slightly red and obvious to start off with but tends to fade well over time. This is the incision that I most often use. It gives good access to the breast pocket and once faded can be difficult to see. INFRA-AREOLAR INCISION This incision is a U-shape at the lower border of the areola of the nipple and again when faded can be difficult to see. However, if it does not leave a nice scar then it is in a more obvious position than the inframammary incision. There is a greater risk of alteration in the nipple sensation when using this incision. TRANSAXILLARY INCISION This is an incision in the armpit, which is well hidden and leaves no scarring on the breast. However, access can be limited and it is difficult to create a breast

pocket, particularly round the cleavage area and is not widely used in this country. However, there are surgeons who have expertise in the use of this incision and my advice to the patient is to find a surgeon that you trust who has good results and to allow them to do the incision that they are most comfortable with. If you have a strong view that you would like a particular incision, then you should find a surgeon who performs this regularly.

Who it is for? There are three groups of patients who most commonly seek breast augmentation. The first is the patient who has always had small breasts. They are out of proportion with their body and they are seeking surgery to feel more womanly and in proportion. The second group of patients are patients who have lost volume and/or shape in their breasts, usually due to having children or losing weight. This group of patients want their breasts to be restored to how they used to be, perhaps with a little bit of extra enhancement. The final group of patients are patients with a breast asymmetry with one breast naturally larger than the other who would like to regain symmetry and be able to wear bras that fit.

Who is it not for? Breast augmentation is not recommended if you are overweight. You will have an increased risk of complications and particularly if you are planning to lose weight then you should wait until your weight

is stable in order to get a more predictable and long-lasting result. If you are a smoker, then you are strongly advised to give up as this can cause a delay in wound healing and increase your risk of problems. Any surgery to change the shape of your body can have a profound effect on your mental health and wellbeing, it can be an emotional journey as you come to terms with the changes. It is important to have good support structures in place, and if you are going through any sort of emotional challenges, such as a change in your relationship or your work, or you do not have the full support of your friends and family, then it may be an opportunity to reflect on whether this is the right time for you. If you do not want larger breasts and it is mainly the shape of your breasts that you are unhappy with, then perhaps a breast lift may be right for you rather than breast implants.

What to expect? Breast augmentation is performed under general anaesthetic or local anaesthetic with sedation. The procedure itself lasts between an hour and an hour and a half. You are usually asked to attend the hospital at least an hour before the procedure starts. You will need to have nothing to eat or drink in the hours before your surgery (full details should be given by the hospital). You will meet with your surgeon and your anaesthetist prior to your surgery. Your surgeon will usually draw markings on your body while you are awake and sitting up before

the operation because the shape of your breasts will change when you are lying on your back on the operating table. The surgeon may also take preoperative photographs. There will be an opportunity to ask any questions and to go over things to ensure that you are completely happy with the plan although you should avoid asking major question like: “Can I have a different size of implant?’, these questions are best dealt with in the days and weeks before the surgery rather than on the day! You will normally wake up from your operation in the operating theatre, although patients do not usually remember this. You will be brought to the recovery area, which is a ward within the theatre complex where you will first become aware of your surroundings although things can be a little groggy to start with. When you are comfortable you will be transferred back to the ward so it is important to tell your friends and relatives that you may be off the ward for an hour and a half to two hours but this does not mean that the operation has taken all this time. Most surgeons do not use extensive dressings or bandages and you will usually come back from theatre wearing a good supportive bra, which will act as the main dressing. There will probably be a small dressing over your incisions. You may or may not have drains, which are tubes, connected to bottles to collect any blood. Within a few hours of your surgery you will be up and about and able to eat and drink and go to the

toilet. You would usually be well enough to go home at this stage although sometimes patients stay overnight in hospital and go home in the morning.

Healing and recovery You will usually see me in clinic about a week after your surgery when the dressings will be removed. You normally do not need any further dressings after this. You will need to check with your surgeon whether you can wash with the dressings on but once the dressings have been removed you should be able to wash and shower normally. For the first week things will feel uncomfortable and stretched and you can get unusual sensations such as tingling or hypersensitivity in your breasts, which usually settles. Occasionally there is trapped air in the breast pocket and this can give a squelchy sensation, which you can sometimes hear too, but do not be alarmed, as this will settle. You will not feel like doing a great deal in the first week after surgery but will be able to potter around the house and perform light chores. In the second week after surgery you will start to feel better and may be able to work from home although I normally advise two weeks before driving (check with your car insurance company in case they have any set rules). Most patients are able to go back to work after two weeks if they perform light duties or office work. You will be able to go back to the gym to perform gentle exercises after two weeks but I normally advise patients just to focus on the

lower body such as the exercise bike and the stepper (although avoid strenuous activity with your arms). If you need to do strenuous upper body activity either at the gym or at work, I advise that you wait four to six weeks before starting this and then you should ease in gently as if it is uncomfortable or things swell then you may need longer. Your scar may be red and obvious to start off with and after about a month I advise patients to start massaging with cream to help it to soften and fade. It can take several months to fully fade but when it does it can be hard to see.

How to find a surgeon? This is a very important topic and one I could write a book about. In fact, I am writing a book about it, which is due to be published this year. Personal recommendation is always a good place to start. If you know someone who has had a good experience and who can recommend a surgeon,

particularly in terms of how they were looked after, after the surgery then this is a great help. The internet is a wonderful place and there is a lot of information out there. All doctors should have their training and qualifications available for you to search. You can usually identify a plastic surgeon by the letters FRCS(plast) after their name and whilst your surgeon does not have to be a plastic surgeon in order to perform this surgery it is the mark of someone who has been through a formal and rigorous training scheme. The Plastic Surgery Associations BAAPS and BAPRAS all publish a list of their members on the Internet including their special interests. All plastic surgeons will perform breast implant surgery but if you have a complex problem, particularly in terms of an asymmetry, or problems with the shape of your breasts, then you should look for a plastic surgeon that specialises in breast surgery. It is perfectly reasonable to see more than one surgeon because you must feel comfortable with the surgeon and the team, there is no substitute for a face-to-face consultation with the person who will be performing your surgery. Do not be afraid to come with a list of questions and to ask about their training and experience as any surgeon who is not comfortable talking about this is probably one to avoid.

Trends in breast shape and size If you look over the years, the trend in breast implant surgery has moved towards slightly bigger implants and the average size that we are using today is larger than it used to be. Having said that, patients still want a natural result. There are only a small minority of patients who want to look fake despite

the public perception of this procedure. In my experience the interpretation of a natural result can vary between individuals and it is helpful to bring in photographs of your desired look. A truly natural result has a gentle slope to the upper part of the breast with a more rounded shape below the nipple. However, some patients do want some fullness in the upper part of the breast and this is something that I call the “supernatural look�. Whilst not desiring a fake look which has got a lot of fullness in the upper pole and a very rounded appearance, many patients want to feel fuller while keeping an attractive contour that is in keeping with the proportion of their frame.

Push up bras versus implants I show before and after photos

of my cases to all of my patients, which demonstrate the differences that the different shapes of implants can achieve, as well as the differences that the same implant can achieve in patients with different breast shapes preoperatively. It is important to stress that when looking at before and after photographs these are taken of patients who were not wearing a bra and when you have had surgery it is perfectly reasonable to wear a push up bra to give a fuller look if this is something that you want, for instance if you are going out or for a special occasion. However, there are some implants, which give this sort of look even when you are not wearing a bra. It appeals to some patients but not others. There are photographs in the media with people who have their breasts sitting very high up

on their chest. This is all done by the push up bra because it would be unnatural to have the breast in this position when they are out of a bra. It is important to consider what you will look like every day, you will still be able to wear clothes and bras to enhance your breasts after you have had surgery but you need to be comfortable with your body out of clothes.

Factors that cause breasts to look unnatural Patients spend a lot of time deciding what size implant they should have. I explain that as long as the implant is within the base width of their chest then the difference of one size bigger or one size smaller will not make a significant effect on their final result. For me the width of the implant is key to getting a good result because if the implant is too wide then you may be able to see or feel the edges of it and this can give an unnatural result. The other problem is putting an implant in that is too wide is that there can be too much fullness laterally, so-called side boob or a web between the breasts in the middle, so-called symmastia, both of which give an unnatural result. The really crucial thing when putting in breast implants that is actually more important than what size of implant you have is the position of the implant. That is why it is so important to choose your surgeon because the pocket that the implant is placed in is crucial. It needs to be centred on the nipple and the cleavage needs to be enhanced. If the implant is put in

too high or too low or too far to one side or the other, then this will give an unnatural result regardless of what the implant choice was.

Why breast implants may not give you the natural look you would like When choosing an implant, I use a sizing kit in the clinic to give an idea of the volume of implant that you may like, sometimes patients come with ideas about the volume that they would like based on a friend’s experience or photographs they may have seen on the internet. They look at the different shapes and profiles of implants and try to choose a suitable implant based on the diameter of their breast. Occasionally there is a situation where the patient wants a natural look without too much fullness and are perhaps considering a teardrop implant but the size that they would like is quite large for their frame. It may be that the friend they were talking to or the photograph they saw on the Internet is of a patient with a wider frame, which can accommodate that size using a natural teardrop implant. If for instance a patient decides that they want an implant of around 300cc because their friend has this and has got a very nice natural result it may be that their friend has a wide chest and so may have a natural low profile or teardrop implant of 300cc which fits them nicely. However, if your chest is narrower than your friend’s, then a 300cc implant on you may need to be fuller and more rounded in

order to fit your frame. This will give you a different look. This is why you cannot go on volume alone. I appreciate that this can be a difficult concept to grasp. This is why I always encourage patients to come for at least a second consultation to go over things. You need to develop a rapport with your surgeon, explain what you are trying to achieve, not only in terms of volume but also in terms of the look that you desire. I would encourage all patients to talk to their friends and family and to look on the Internet to get ideas and help with the implant choice, as it is an important decision and one that you must be happy with. However, it is also important to make sure that you are able to get your hopes and concerns across to your surgery and can listen to your surgeon’s advice so that between you, you can get the best result for you. Do not be daunted, it can be an amazing life changing experience but at the same time go in with your eyes open and listen to the advice you are given. Give yourself enough time and space to be able to reflect and make an informed decision that you are completely happy with. e have all done it, got a little tweezer happy, booked an appointment with a less than conservative therapist and have been left with pencil lines for eyebrows and wondered why we ever messed with them in the first place. The emphasis on beautiful brows is arguably at it’s peak and whether you have botched a home made attempt or succumbed to a dodgy therapists’ plucking misdemeanot, there are some solutions to minimize the stress of sorting out your brows:

1.Grow them back In the most severe cases it can take around 4 months to grow back your natural brows, but typically you can expect growth in around four weeks. In this time, try not to tweeze or you may risk getting carried away again. Wait as long as you can and then, once you think you have a good growth, seek a reputable salon to shape them properly.

2. Don’t use a magnifying mirror to magnify the problem Stop analyzing and leave well alone. You might feel like the world and his wife can see your tiny brow hairs sprouting their appearance, but in all sincereity they can’t. Resist the magnifying glass as it will only magnify the issue in your mind. If you need to, use a small amount of foundation and powder to conceal.

Resist the urge to use a magnifying mirror to analyse your brows – it will only lead to over-tweezing.

3. Disguise rogue hairs Filling in the brow is fine for the time being (you can always try HD Brows range of pencils and shadows for a more full and shapely brow) but don’t pluck the hairs that are growing outside of the shape you fill. Use some concealer to cover the tiny hairs but don’t go crazy as it can look cakey if you over do it.

4. Brush and groom Brush the brows when you fill them in. Before and after is the best protocol. This will give a much more natural look (the 60s Twiggy

look really is out) and leaving them un-brushed will only make them look more uneven. Use a pencil or powder to fake some wispy lines in the gaps where the hairs are not yet visible or may not be as thick. This is a much more natural tecnnique to use, rather than colouring in and leaving you with block caterpillar brows.

5. Maintenance is key Once your brows are back, it is important to maintain the shape and condition of them between therapist appointments. There are several brow conditioners on the market (most of which look like clear mascara) and Revitalash have also developed an advanced brow serum to speed up the growing process and to condition the hairs.
