Thinking about having a tweak? Consider this your simple reference guide for getting the best and safest results.

Long before “tweakments” were even called tweakments, the options for what you could have done to improve your appearance without surgery were fairly limited. Lasers were far more ablative, peels more aggressive, and fillers more permanent.

Then, 30 years ago, Botox™ (the first brand named botulinum toxin type A to gain FDA approval for frown lines) burst onto the market, igniting a new phase of innovation. Alongside hyaluronic acid (and, at the time, collagen) fillers and advances in device-based technology, it drove growth and demand for nonsurgical aesthetics.

As a result, the field of aesthetic medicine has come very far in a relatively short space of time, meaning that it’s now possible to address facial concerns openly and confidently, with more choices, less fuss and far better results than you could previously hope for.

Most importantly (more on this later), aesthetic procedures are safer, and the majority of the practitioners

Botulinum toxin

Botulinum toxin is still one of the most popular aesthetic treatments across the board. Since its wrinklerelaxing capabilities were first noted in the 1990s, it’s been considered the gold standard for addressing lines and wrinkles in areas including the forehead, crow’s feet (the outer corners of the eyes) and between the eyebrows. Top practitioners are now using it for lower facial contouring and even to smooth and refine the skin’s surface overall. Instead of injecting it into the muscles, toxins can be injected more superficially into the skin to help reduce oiliness and pore size and decrease inflammation, it’s reported. Fillers

Skin boosters

If you just want to freshen up your face, skin boosters might be for you. A relatively new injectables category, skin boosters are best thought of as a sort of injectable skincare, the benefits of which last for months. It’s a heavy hit of hydration (sometimes along with amino acids and/or vitamins, depending on the specific product) to nourish skin deep down, stimulating collagen production, smoothing fine lines and generally improving skin quality for all-round plumper, softer and healthier-looking skin. Once the numbing agent has taken effect, a fine needle or cannula will be used to inject the dermal filler into the targeted areas of your face.

STEP 7

Follow aftercare advice

The practitioner should provide after-care instructions, such as avoiding strenuous activities, excessive heat or sunlight exposure, and any medicines or activities that can increase swelling and bruising. They may also recommend ways to reduce swelling and discomfort, such as ice packs.

STEP 8

Follow-up consultation

Depending on the filer type and treatment area, a follow-up appointment may be necessary to assess the results and provide any touch-ups needed.

Maintenance and skin care

Just imagine a door to door travelling salesman, going from place to place and home to home with a treatment that claims to beautify, erase the signs of ageing, enhance and leave one bright eyed and bushy tailed. The shiny bottle is affordable, and everyone wants one. You take a closer look; the list of ingredients is long and in a foreign language.

What is the picture you imagine? A snake oil salesman in the wild west or the 18th century?

This is not far off modern-day cosmetic marketing, reaching our homes and our families, via the internet, tv, social media and consumer press. The cosmetics market is growing exponentially, ninety percent of women under 55 in the UK wear cosmetics. But this is not just the domain of women, since the 1980’s men’s personal care has become socially acceptable with the global men’s market being worth over $124 billion in 2020. The global eye makeup market alone is projected to be worth over $23 billion by 2028, with mascara being the most popular product. And it is not only adults that are interested in cosmetics. In the US, 54% of girls aged between 12 and 14 wear eye make-up and this practice is being mirrored in the UK.

As humans, we have always been preoccupied with the eyes, gaze tracking studies by the Russian scientist Alfred Yarbus in the 1970’s showed that when we communicate, we focus on the eyes of the face we are looking at.

The adornment and enhancement of eyes with malachite green dates back to 5500 BC Egypt, and Kohl black 3100 BC. The poisonous Atropa Belladona, meaning beautiful lady, is from the deadly nightshade plant and was used in Ancient Rome to dilate the pupils. Fast forward to the early 20th century and we see commercialization of products with Elizabeth Arden, Max Factor and Helena Rubenstein launching eyeshadows in 1910 and Mabel Williams (Mabelline)

mascara in 1933. Since then the beauty industry has grown to include lash extensions, lash serums, lasers, energy devices, hyaluronic acid fillers, botox and more, with non-surgical treatments aiming to achieve surgical results, in the hands of non-medical practitioners.

Cosmetics are poorly regulated, few ingredients have been assessed for safety. The US FDA estimate 12,500 chemicals are used in cosmetics with only 11 ingredients banned in the USA since 1938. In Europe there is slightly more regulation with 1300 ingredients banned. Less than 20% of ingredients, however, have even been reviewed for general safety, and eye safety is even less reviewed.

Not only are we potentially still being sold snake oil, cosmetics are potentially an alchemy of ingredients that may or may not be listed on

the packaging, read like a foreign language and may or may not have been assessed for safety. In 2023 Drs Roos and Murthy, with their surgical, aesthetic and academic expertise, were invited to be part of a global initiative of experts in eye care, to review all published literature on cosmetics and cosmetic procedures on the ocular surface and periocular area. This was the most read article in 2023 (The Ocular Surface, Volume 29, July 2023,Pages 77-130).

Commonly found ingredients in cosmetics can cause adverse effects, including allergies, skin dermatitis, dry eye disease, hormone disorders and some are linked to cancer. The top 10 make-up nasties include the commonly used preservatives Benzalkonium chloride (BAK) and formaldehyde releasing compounds

found in eyeliner, mascaras and BAK also in many eye drops. At concentrations, 20,000 fold lower than allowed in commercial products, this can cause allergy, irritation and damage to the eyelid oil glands resulting in dry eyes. Prostaglandin analogs originated as a treatment for glaucoma. In 2008 the eye drop, having had a desirable side effect of lash growth, was re-branded as Latisse a prescription only solution for lash growth. Synthetic prostaglandin analogs can now be found in many over the counter lash growth serums. Although the lashes may look darker and longer, they can be more wiry, there is also the possibility of damage to the eyelid oil glands, orbital fat atrophy giving a sunken, droopy eyelid appearance, discoloration of the eyelid and iris,

and dry eyes. Retinols, touted as an essential skin care item for improving collagen and reducing ageing signs, may not be suitable for the thin delicate skin of the periocular area with the thinnest skin in the body at less than 0.5mm. Retinols can damage the meibomian gland at concentrations thousands of times less than that allowed in topical products and should not be used close to the eyes.

Safe beauty practices include avoiding water lining with eyeliner on the wet-dry junction of the eyelid where the oil glands lie, changing mascaras within 3 months and avoiding waterproof makeup for daily use that requires more astringent and toxic alcohol-based removers. Consumers can now be empowered to check ingredient lists with useful websites such as skindeep www.ewg.org/skindeep and cosdna www.cosdna.com and apps such as Think Dirty, INCI Beauty and CodeCheck. For the report, we also reviewed cosmetic procedures which are much better studied and regulated, and also not applied on a daily basis with aggregate ingredients in multiple products. On the whole botox being performed by medical professionals and temporary, had low risk. Fillers in the UK are not regulated, and many non-medical professionals perform these treatments. It is important to seek a practitioner who has the medical background and experience to choose the right treatment for you and manage any complication, not just an aesthetic eye. Aftercare is just as important and as with going to the dentist you continue to brush your teeth; skin care can maintain in clinic aesthetic treatments.
