Where do doctors go to treat their own pain? Dr. Robert Sidlow MD, MBA—Internist, New York, NY “As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes-those alltoo-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of "conservative" therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I have personally discovered Dr. Jonathan Donath's outstanding practice. After beginning a vigorous exercise regime, I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I was literally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath's office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become "believers" in Dr. Donath's abilities as well--- the results he achieves are real and lasting. Coping through holiday season anxiety

With the holiday season upon us, UConn Health’s Dr. Michael Kisicki, assistant professor of psychiatry, shares his best advice to help you and your family get through any potential stressful and anxiety-provoking holiday activities, including family get-togethers and post-election debates during dinner.

What is it about the holiday season that leads to higher levels of anxiety and stress? We all want that perfect Hollywood holiday, where everyone is happy and gets along with each other. We have so many high expectations about the holidays that just keep on rising until the big day comes. But inevitably we may be disappointed once the day actually arrives. Being with family can trigger hidden or past personality traits. For example, adult children can become childlike when they return to their childhood home, leaving their spouse wondering who the adolescent-like knucklehead dressed like a grown up is sitting next to them. I know firsthand about holiday stress. For myself, I change from a middle-aged man to the devious spoiled younger brother who gets away with taunting his brother, expecting my mother to take care of everything. Lastly, holidays can bring stresses about finances to the surface. Not being able to afford the presents that you eagerly want to give to your loved ones can be devastating. How much do interfamily relationships add to holiday anxiety? Love and commitment to family can bring together a motley grouping of socially mismatched individuals that have little in common aside from a last name. During the holidays we have to be around family that wouldn't necessarily be our first choice

for dinner companions. There may be the judgmental mother-in-law, the disrespectful daughter-in-law, the uncle who drinks too much and insists on talking about politics. Also, old history can be dug up and old conflicts can be rejuvenated when extended family gets together. Humans love drama and gossip and will look for it if there is nothing else to talk about. As we get together and see people we have not seen for awhile, the holidays can be a time when people compare themselves to siblings, cousins, and inlaws, thinking about how we measure up in terms of career, family and happiness.

How can we limit our holiday stress or anxiety, especially when interacting with a spouse or family member? Despite all of these conflicts, struggles and hangups, it’s still possible to have a good time and treasure our time with family. The most important thing is to focus on enjoying what you have, even the dry turkey or the hideous sweater. Our high expectations can hurt us, so try to minimize what you are expecting. A sense of humor, humility and a grateful mindset can deliver you safely through the holidays into January. It’s easy to focus on material things and on yourself during the holidays, but try to match every selfish thought with two thoughts about others. To manage stress, also make sure you are taking care of yourself. During the holidays, besides being stressed, we tend to eat more and drink more.

Minimizing your alcohol intake and getting some exercise will be good for stress relief. Get an early start on that New Year's resolution of walking or going to the gym more. Try learning to meditate. This is a great way to manage stress and there are great free ways to learn with free online videos or apps. There is something about making a commitment to putting time aside to take care of yourself that can do wonders for your spirit.

How do you advise parents prevent translating their own anxiety to their children? For those families that really have a hard time communicating or have nothing in common, try a destination holiday (like a camping or ski trip). If getting away is not possible, try home activities (baking a cake together or playing a board game) or small outings nearby, (miniature golf or a movie). Group activities give the family something benign to talk about, create a fun backdrop and make for good memories. It’s a great time of year to volunteer, try taking the whole family to work at a soup kitchen or shelter. This can be a particularly good idea if you have to be alone or away from family this time of year.

After this stressful election what is the best way to talk with family members who have a different political view? There is definitely going to be a lot of arguing over the egg nog about this past election. In my own practice, I have seen how this election, more than I have ever seen before, has drawn blue and red battle lines down the middle of the dinner table. For the inevitable political or religious arguments, focus on the subject rather than the person. As absurd as you may believe someone's beliefs to be, they must have a personal reason for having them. Try to find out what that is, figure out where they are coming from rather than trying to show you are right. No one is keeping score and winning an argument over a holiday meal is ultimately a loss for the family. You are more likely to get the silent treatment from your wife on the car ride home than respect for your genius debate skills.

What should people do if their anxiety lingers after the holidays – or their child’s anxiety does? Besides the stress of the holidays, some people can be very sensitive to the shorter days and colder weather of winter. I see patients that routinely have depressive episodes or worsening of anxiety this time of year. If you find that you or a loved have become persistently anxious without reason or are feeling uncharacteristically sad or irritable, you may need professional help. There are effective therapies to help with anxiety and depression. There is no shame in getting help, so you can be more resilient for your family.

Helping Physicians Focus on Patients, Not Paperwork “Doctors didn’t go to medical school to spend their time doing paperwork,” says Kelly O’Brien, President of Atlantic Cash Flow Solutions.

Make dessert a part of the meal Rather than serving dessert last, incorporate it into the holiday meal. This is helpful, because it is better to eat sugar at the same time as a balanced meal. The other more healthy foods will not only displace the sugar from your teeth, but they can also aid in neutralizing any acids from the ingested sugar.

Make toothbrushing fun While it is important to stress good dental care throughout the year, the holidays present a special opportunity to make dental health fun.

Chances are good that visions of cookies, desserts and candy canes may be dancing in your children's heads this holiday season. While you will do what you can to limit their intake of these sugary treats, your kids will probably be eating their fair share of sugar at your family holiday parties. Despite their consumption of sugar, there are ways to keep your kids' healthy teeth and gums in shape and to minimize damage to their dental health.

Why is sugar bad for dental health? Whether your kids are eating chocolate cake, sugar cookies or peppermint candy, they are ingesting sugar. According to the American Dental Association (ADA), naturally occurring bacteria in the mouth feed on this sugar, and the byproduct is acid. This acid can eat away at tooth enamel, which can lead to tooth decay and cavities.

Brush after eating a sugary treat In general, the ADA recommends that everyone brush their teeth and gums at least twice a day for at least two minutes each time. Flossing should also be done at least once a day. During the holidays, encourage your kids to brush and floss even more than this, particularly right after they finish dessert. Limit sugar time Hard candy is one of the worst offenders at wreaking havoc on your kids' healthy teeth because your child will be sucking on the candy for a long period. Additionally, limit the amount of soda and juice that your children drink since these wash over teeth and gums. If your children do eat hard candy or drink soda, encourage them at least to rinse afterwards. Sugarless gum is also a great way to keep your kids' mouths busy while boosting saliva production, which will help wash away sugar.

UConn Health’s Lung Cancer Screening Program is urging current and former heavy smokers to get screened for the disease with a lowdose computed tomography (CT) scan. “Low-dose CT is a very powerful screening tool for giving doctors higher-definition and clearer imaging of the lungs,” says Dr. Omar Ibrahim, the newly appointed director of thoracic oncology at UConn Health’s Carole and Ray Neag Comprehensive Cancer Center who specializes in interventional pulmonology care. “This screening tool can make sure a high risk patient’s lungs are clear from the disease, or detect it at its earliest stages,” says Ibrahim. “Early detection is a patient’s best chance for a lung cancer cure.” The biggest risk factor for lung cancer is smoking. According to the CDC, 9 out of 10 lung cancers are linked to the smoking of tobacco. Lung cancer is the leading cause of cancer death among men and women. In 2016, the American Cancer Society expects the disease to claim more than 158,000 more American lives and lead to 224,000 new diagnoses. “Not everyone realizes that more Americans die of lung cancer than of breast, colon, and prostate cancers combined,” says Ibrahim. Lung cancer lesions are often identified as an incidental finding when a patient undergoes a chest xray or another imaging test for a different medical need. Also, way too often the disease is diagnosed in its advanced stages and after symptoms such as a heavy cough, coughing up blood, or shortness of breath present. “Fifty percent of the time, when we find lung cancer it is already at an advanced stage and surgery to remove it is not an option,” says Ibrahim. “Our goal is to find lung cancer at its earliest stage so we can have options to treat it and cure it.”

If a low-dose CT scan catches a suspicious lung nodule or growth, Ibrahim leverages minimally invasive techniques to rule out lung cancer, or diagnose and identify what stage the disease is at. In the exam room he is able to use video-guided 3D navigational bronchoscopy technology and ultrasound technology to closely examine a patient’s lung tissue, using a thin, flexible tube via the nose or mouth. Greenwich Hospital’s Bonnie McGuire Wreschner, MD, has been awarded the prestigious Albert Schweitzer Award for Humanitarian Endeavors from the Fairfield County Medical Association. While IBD can affect anyone, Caucasians are more likely than other ethnic groups to have IBD.The diseases are especially prevalent in Jews of European descent. African Americans and Hispanics in the United States are increasingly affected. Crohn’s disease is a chronic inflammatory condition of the gastrointestinal tract and may affect any part from the mouth to the anus. Ulcerative colitis is a chronic inflammatory condition limited to the colon, otherwise known as the large intestine.

Shared Symptoms of Crohn’s Disease and Ulcerative Colitis It is often difficult to diagnose which form of IBD a patient is suffering from because both Crohn’s Disease and ulcerative colitis cause similar symptoms. Symptoms related to inflammation of the GI tract: • Diarrhea • Rectal bleeding • Urgent need to move bowels • Abdominal cramps and pain • Sensation of incomplete evacuation • Constipation (can lead to bowel obstruction)

General symptoms that may also be associated with IBD: • Fever • Loss of appetite • Weight Loss • Fatigue • Night sweats • Loss of normal menstrual cycle

Both illnesses do have one strong feature in common. They are marked by an abnormal response by the body’s immune system. The immune system is composed of various cells and proteins. Normally, these protect the body from infection. In people with Crohn’s disease, however, the immune system reacts inappropriately. However, in people suffering from IBD, the immune system reacts inappropriately, mistaking benign or beneficial cells and bacteria for harmful foreign substances. When this happens, your immune system can do harm to your gastrointestinal tract and produce the symptoms of IBD.
