Where do doctors go to treat their own pain? Dr. Robert Sidlow MD, MBA—Internist, New York, NY “As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes-those alltoo-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of "conservative" therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I have personally discovered Dr. Jonathan Donath's outstanding practice. After beginning a vigorous exercise regime, I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I was literally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath's office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become "believers" in Dr. Donath's abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.”

It might seem hard to believe, but the celebration that is Thanksgiving Day is almost here. Thanksgiving is a U.S. holiday, but giving thanks is universal. It brings with it a reminder to stop and give thanks for all of the things in our lives that we have, and the amazing friends and families we are all fortunate enough to have in our lives. Thanksgiving is not just a placeholder between Halloween and Christmas: It is a time to express gratitude for everything that you have. Thanksgiving traces its origins to ancient harvest festivals. It became an official U.S. holiday in 1863 under President Abraham Lincoln. In 1941, President Franklin Roosevelt signed a law fixing Thanksgiving on the fourth Thursday in November. From there, it has been feasts, football and family. Thanksgiving is a time to express gratitude for everything that you have. Take a look around: You have so much to be thankful for! The theme for National Family Caregivers Month November 2016 is “Take Care to Give Care”

The first rule of taking care of others: take care of yourself first. Caregiving can be a rewarding experience, but it is also physically and emotionally demanding. The stress of dealing with caregiving responsibilities leads to a higher risk of health issues among the Nation’s 90 million family caregivers. So as a family caregiver, remember to pay attention to your own physical and mental wellness, and get proper rest and nutrition. Only by taking care of yourself can you be strong enough to take care of your loved one. You really do need to “take care to give care!”

• Caregiving can be a stressful job. Most family caregivers say they feel stressed providing care for a loved one. With all of their caregiving responsibilities – from managing medications to arranging doctor appointments to planning meals – caregivers too often put themselves last.

• The stress of caregiving impacts your own health. One out of five caregivers admit they have sacrificed their own physical health while caring for a loved one. Due to stress, family caregivers have a disproportionate number of health and emotional problems. They are twice as likely to suffer depression and are at increased risk for many other chronic conditions. • Proper nutrition helps promote good health. Ensuring that you are getting proper nutrition is key to help maintain your strength, energy and stamina, as well as strengthening your immune system. Maintaining a healthy diet is one of the most powerful things you can do to take care of yourself and keep a positive attitude overall.

• Ensuring good nutrition for your loved one helps make care easier. As many as half of all older adults are at risk for malnutrition. Good nutrition can help maintain muscle health, support recovery, and reduce risk for re-hospitalization – which may help make your care of a loved one easier.

• Remember: “Rest. Recharge. Respite.” People think of respite as a luxury, but considering caregivers’ higher risk for health issues from chronic stress, those risks can be a lot costlier than some time away to recharge. The chance to take a breather, the opportunity to re-energize, is vital in order for you to be as good a caregiver tomorrow as you were today.

Diabetes prevention: 5 tips for taking control PAGE 12

Diabetes prevention is as basic as eating more healthfully, becoming more physically active and losing a few extra pounds — and it's never too late to start. Making a few simple changes in your lifestyle now may help you avoid the serious health complications of diabetes down the road, such as nerve, kidney and heart damage. Consider the latest diabetes prevention tips from the American Diabetes Association.

Tip 4: Lose extra weight If you're overweight, diabetes prevention may hinge on weight loss. Every pound you lose can improve your health, and you may be surprised by how much. Participants in one large study who lost a modest amount of weight — around 7 percent of initial body weight — and exercised regularly reduced the risk of developing diabetes by almost 60 percent.

Tip 5: Skip fad diets and make healthier choices Low-carb diets, the glycemic index diet or other fad diets may help you lose weight at first, but their effectiveness at preventing diabetes isn't known nor are their long-term effects. And by excluding or strictly limiting a particular food group, you may be giving up essential nutrients. Instead, think variety and portion control as part of an overall healthy-eating plan.

Tip 1: Get more physical activity There are many benefits to regular physical activity. Exercise can help you: • Lose weight • Lower your blood sugar • Boost your sensitivity to insulin — which helps keep your blood sugar within a normal range Research shows that both aerobic exercise and resistance training can help control diabetes, but the greater benefit comes from a fitness program that includes both.

Tip 2: Get plenty of fiber It's rough, it's tough — and it may help you: • Reduce your risk of diabetes by improving your blood sugar control • Lower your risk of heart disease • Promote weight loss by helping you feel full Foods high in fiber include fruits, vegetables, beans, whole grains, nuts and seeds.

Tip 3: Go for whole grains Although it's not clear why, whole grains may reduce your risk of diabetes and help maintain blood sugar levels. Try to make at least half your grains whole grains. Many foods made from whole grains come ready to eat, including various breads, pasta products and many cereals. Look for the word "whole" on the package and among the first few items in the ingredient list.

When to see your doctor The American Diabetes Association recommends blood glucose screening if: • You're age 45 or older and overweight • You're younger than age 45 and overweight with one or more additional risk factors for type 2 diabetes — such as a sedentary lifestyle or a family history of diabetes Share your concerns about diabetes prevention with your doctor. What is Palliative Care? Palliative care is a vague and often confusing term to describe a type of treatment model. People of ask: Is it end-of-life care? Is it the same as hospice? What is palliative care? Palliative care is treatment aimed at relieving the symptoms associated with serious illnesses and improving a patient's quality of life. Contrary to popular belief, it is NOT hospice care. Unlike hospice care, palliative care can begin at any time during a serious illness and ideally at the time of diagnosis.

Palliative Care for Cancer Palliative care is most often utilized by cancer patients in the form of treatments such aspalliative chemotherapy and palliative radiation. Chemotherapy and radiation treatments that are considered "palliative" are those that will not cure the cancer, but offer relief of symptoms. An example of this is a woman with a brain tumor causing severe headaches and blurred vision who undergoes radiation to shrink the tumor and relieve her symptoms. Cancer patients who are undergoing curative cancer therapy are also prime candidates for palliative care. Aggressive palliative care can relieve distressing side effects from cancer treatment— nausea and vomiting, fatigue, pain, and thrush are just a few of the symptoms palliative care can address. Palliative care is not only for cancer Other serious illnesses can also benefit from palliative care. Heart failure patients can get relief from chest pain, fluid retention (edema), and shortness of breath.

Liver failure patients can receive intensive treatments for abdominal pain and swelling, itching, and nausea. Patients with respiratory disease can manage shortness of breath and improve quality of life. Any serious illness that negatively impacts a patient's quality of life can benefit from palliative care. And perhaps the biggest benefit of palliative care? Patients who receive palliative care livelonger than patients who receive standard care. If patients can live a longer life with improved quality of life, why isn't palliative care more popular? It's partly due to the misconception that palliative care is the same as hospice care.

Palliative care doesn’t equal hospice care Hospice care is simply one form of palliative care that is appropriate for patients in the final stage of life—those with a life expectancy of six months or less. Like palliative care, hospice care is designed to relieve symptoms and improve quality of life, but hospice is restricted to patients who are terminally ill. Palliative care can be given regardless of life expectancy. Palliative care can begin as early in a patient's illness as is necessary to control symptoms and improve quality of life.

Palliative Care Goals The goal of palliative care is simply to relieve suffering and improve quality of life. Further goals are then based on the patient's wishes in consultation with the palliative care team. Palliative care is more than just treating physical symptoms, however. Palliative care also addresses emotional suffering and spiritual care needs. The Palliative Care Team Palliative care is often carried out by a team of professionals who can address multiple concerns. The team may include: • Palliative care physicians • Palliative care nurse practitioners • Nurses • Social workers • Chaplains • Therapists (occupational, speech, physical, etc.)

Each November, the National Association for Home Care & Hospice (NAHC) celebrates National Home Care Month, National Hospice Month, and Home Care Aide Week to honor care giving heroes who make a remarkable difference in the lives of patients and the families they serve. November is officially cited as being National Home Care and Hospice Month, making it a great time to get the message out as well as give back to those who help your home care agency be successful. Here are a few ways that your agency can help celebrate during this month:

• Show your support. If you haven’t been doing it all along, this month is the perfect time to show your home care workers and volunteers that you appreciate them. You can do this by hosting and inviting them to a breakfast or lunch, and by telling them “thank you” for what they are doing.

• Spread the word. Help raise awareness about home care and hospice by sending out a press release, writing a letter to the editor, or asking the library to set up a display. These small measures can help to raise awareness about home care and hospice. • Share with patients. Let your patients know that it is a month to celebrate getting quality care right in the comfort of their own home. Send out a little newsletter, write up a page for your Website, or send out each home care professional with a small gift of gratitude to be given to each patient. • Give an award. If you have a home care worker and volunteer who stands out and who has went above and beyond this year, give him or her an award to recognize the important contribution.

Home care and hospice are health care options that we should celebrate throughout the year. They allow people to get the help they need without having to go to a hospital to get it. 5 steps to becoming a Volunteer for Hospice Care Because a hospice agency spends significant time and money to train their volunteers, they generally like to get at least a one year commitment from volunteers. This way overall volunteer turnaround is limited.

Hospice volunteers are unpaid workers who have donated their time and resources to support hospice patients, caregivers, and staff.

Importance of Hospice Volunteers Hospice volunteers are an essential part of a well-run hospice program -so essential in fact that hospice agencies receiving Medicare or Medicaidfunding have to prove that at least 5% of hospice work is being done by volunteers in order to be paid. Hospice volunteers may do a number of important tasks that range from assisting in office work to vigiling at a dying patient's bed side. A hospice volunteer typically dedicates four hours a week to a hospice agency but most agencies are flexible regarding the exact number of volunteer hours.
