Westchester’s first “Integrated Therapies” program for children to open in popular sensory gym The launch of personalized clinical program for children - ACCESS Sensory Kids at WeeZee - The Science of Play ACCESS Sensory Kids (“ASK”) is now being launched at WeeZee - The Science of Play as its new therapy division and the first clinical program for children that integrates key therapy services – physical therapy, occupational therapy and speech therapy. Located at 480 N. Bedford Rd. in Chappaqua, WeeZee features an environment of therapeutic play with an 18,000 square foot open-gym facility and hundreds of multi-sensory challenges. Physical therapists, occupational therapists, speech and language therapists, tutors and sensory coaches will be available to work with each child individually, within a group or with teachers and families to create a Personalized Sensory Integration Plan that is proven to improve academic, athletic and social skills. Westchester County consists of so many different communities, spreading out from Scarsdale, to White Plains, to Bedford, to Pelham. But at the end of the day, we are all one community, which cares for each other. Especially when tragedy hits – because it touches us all on some level. And, sometimes, brings us closer together. Witness the fiery crash caused earlier this month when an SUV driven by Ellen Brody, 49, of Edgemont collided with a Metro-North train. Brody and five people on the train were killed on that chilly evening. Fifteen people were injured, many seriously. And our communities continue to mourn their loss. Also lost in the accident from Westchester County were Walter Liedtke, 69, of Bedford Hills, Chappaqua’s Robert Dirks, 36, Eric Vandercar, 53, of Bedford and New Castle’s Joseph Nadol. Aditya Tomar, 41, of nearby Danbury, Conn. also perished in the accident. With a heavy heart, Healthcare News would like to take this opportunity to wish our condolences to everyone touched by this horrific tragedy. There has been much speculation about the tragedy on the Harlem Line that night. Details of the crash will, in time, emerge. The National Transportation Safety Board has begun its intense investigation. Different experts and the public will continue to offer their opinions. In a difficult time, perhaps we should focus instead on the beautiful lives lost, and the grief and pain felt by their loved ones. There will be many tough days ahead, but we will get through this tragedy together as one community. That is one of the many things that makes Westchester County special. Esperanza Steinberg began her graduate studies at Seton Hall University to become an occupational therapist. However, her course of study took a change after she was inspired by a patient she worked with during vocational training. That set the stage for Steinberg becoming Dr. Esperanza Steinberg, psychologist. She owns her own growing practice, White Plains Psychology. “That patient told me that she was significantly depressed,” Steinberg said. “She said she woke up every morning not feeling very functional. She had a stroke and I was moved that she felt depressed. I became fascinated by the emotional aspect of disability and wanted to help people.” Steinberg went on to earn a M.A. in counseling and Ph.D. in counseling psychology from Seton Hall. She completed her doctoral internship at Hagedorn Psychiatric Hospital in Gardner, New Jersey. Steinberg has provided psychotherapy to adult, adolescent and geriatric patients presenting an array of psychological disorders, such as anxiety, bipolar, depression, dementia and schizophrenia. She has worked at Staten Island University Hospital Rehabilitation Department, where she conducted neuropsychological assessments across all ages. In addition, Steinberg has provided psychotherapy to adult and geriatric patients to assist them in coping with traumatic brain injury,

stroke, Parkinson’s Disease, and hip and knee replacements. She consults at various nursing homes in New York City and Connecticut. Steinberg is a licensed psychologist in New York, New Jersey and Connecticut. Steinberg, a native of Fairview, N.J., said she was also inspired to become a psychologist by her own personal experiences. Her father, Fernando Hernandez, sustained a traumatic brain injury when Steinberg was a kid – the result of a construction accident. Hernandez fell off a scaffold and almost died. He suffered, among other serious injuries, a severe skull fracture. “My dad really struggled and it had an impact on the entire family,” Steinberg said. “That was the 1980’s, before people knew a lot about head injuries. We never really knew what he was going to be like when he got home. That always stayed with me.” Steinberg said that her dad still deals with issues due to his accident, including dementia. Her mother, Maria Hernandez, helps take care of him back home. Of course, Steinberg, moved by her father, helps with his care as well. “I just want to help families that suffer losses like that,” Steinberg said. “It can just be such a traumatic experience. I just hope to make a difference in people’s lives. That is really what encouraged me to go into this field.”

Defer Your Minimum Required Distributions with QLACs By Andrew J. Cavaliere, CLTC Do you want to defer unwanted Required Minimum Distribution (RMDs) from your IRA after age 70½…? Well, now you can and at the same time avoid those unnecessary federal and state income taxes. RMD rules require all individuals with qualified money to take distributions from those accounts beginning at age 70½, with respect to a portion of their total retirement savings, whether they need the income or not. In July 2014, the U.S. Treasury Department removed a significant impediment to the ability of qualified plan participants and IRA owners to balance their use of annuities and other investments in their retirement plans by issuing final regulations that allow individuals to bypass Required Minimum Distribution (RMD) rules. The new regulations allow individuals to essentially defer a percentage of their RMD distributions and the taxes associated with those distributions with no penalty! The individual establishes a Qualified Longevity Annuity Contract, or “QLAC,” a guaranteed deferred annuity, under which the amount of annuity payments is locked in at purchase, and those payments can be deferred at a later date up to age 85. These annuities are also referred to as Deferred Income Annuities, or “DIAs.” DIAs have been available in the marketplace for some time before these regulations became effective; however, when they were purchased under a qualifying employer-sponsored plan or Individual Retirement Account (IRA), the annuity payments generally needed to commence by age 70½, or the individual’s actual date when RMDs must commence. Thus, QLACs generally encompass such DIA contracts with income start dates after age 70½ and at or before age 85, while non-QLAC (or NQLAC) DIAs remain available for income start dates up to age 70½ are not subject to the new QLAC limitations. Limits on premiums / purchase payments ➢ Over their lifetime, individuals cannot allocate more than $125,000 of their qualifying retirement plan [401(a)/(k), 403(b), governmental 457(b)] and IRA (traditional; not Roth nor inherited IRAs) savings to QLACs. ➢ Additionally, no more than 25% of the participant’s account in any one qualifying retirement plan may be allocated to purchase one or more QLACs. This limit also applies to traditional IRAs; however, rather than applying to each separate IRA, the limit applies to the aggregate of the individual IRAs. Unlike Roth IRAs, Roth accounts within retirement plans are eligible for QLACs because they are also subject to RMD requirements. ➢ The 25% limit applies differently to plans and to IRAs. Under a plan the 25% limit applies to the most recent valuation of the plan, and under an IRA it applies to the prior 12/31 aggregated IRA balance. In both cases the value includes the present value of any previously purchased QLAC or NQLAC under the plan or the IRAs. The plan account value is also adjusted for any subsequent contributions or distributions. It is unclear whether this same adjustment applies to the 12/31 IRA value. The bottom line here is that individuals can avoid thousands of dollars in unnecessary taxes every year. They can defer their unwanted RMDs for very long periods of time without penalty. These RMDs can be deferred until age 85 allowing the IRA to continue to grow and compound overtime creating even more money for later years and for your family. What do doctors do to treat their own pain? As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable. I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked. I could walk, bend down and I had almost no pain. Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance.
