St. Joseph’s Regional Medical Center Physician implants smallest defibrillator St. Joseph’s Regional Medical Center’s cardiac electrophysiologist Satish Tiyyagura, MD, FACC, Director, Electrophysiology Laboratory, is among the first in New Jersey to implant the newly FDAapproved mini implantable cardioverter defibrillator (ICD), an automated treatment of lifethreatening ventricular arrhythmias. Ventricular arrhythmias, are a leading cause of sudden death in the United States, especially among elderly people. It can be treated with medication, but many times an ICD is needed to protect the heart and prolong life. “This smaller device is particularly beneficial for thinner patients or patients with less fat and soft tissue in the upper chest, like older patients,” says Dr. Tiyyagura. “We’re committed to providing our patients with advanced technology that will help improve their quality of life; this is an example of that kind of technology.” ICDs are used for people who have had an abnormal, fast heart rate that caused them to faint or caused their heart to stop pumping properly. The new device, manufactured by Boston Scientific Corporation, is 20 percent smaller by volume and up to 24 percent thinner than others on the market. The ICD is implanted beneath the skin, near the collarbone. Leads are placed inside the heart and are attached to the ICD. Every 39 seconds, an adult dies from a cardiovascular disease such as a heart attack or stroke, according to the Centers for Disease Control and Prevention (CDC). The use of Echocardiography— a diagnostic test that uses ultrasound waves to create an image of the heart muscle—makes early detection of life threatening heart disorders and other diseases possible. In this test, the ultrasound waves rebound, or echo, off the heart and show the size, shape and movement of the heart's valves and chambers as well as the flow of blood through the heart. An echocardiograph may also show abnormalities such as poorly functioning heart valves or damage to the heart tissue. Home to the renowned Heart and Vascular Institute, Saint Michael’s Medical Center (SMMC) received three-year accreditation in Echocardiography in the area of Adult Transthoracic by the Intersocietal Accreditation Commission (IAC). Accreditation by the IAC means that SMMC underwent a thorough review of its operational and technical components by a panel of experts. The IAC grants accreditation only to facilities found to provide quality patient care in compliance with national standards, through a comprehensive application process including detailed case study review. “This accreditation demonstrates that Saint Michael’s Medical Center has met the rigorous requirements by the IAC in regard to all aspects of its operations considered relevant by medical experts in the field of Echocardiography,” said David A. Ricci, president and CEO, SMMC. IAC accreditation is widely respected within the medical community, as illustrated by the support of
the national medical societies related to Echocardiography, which include physicians and sonographers. In fact, Echocardiography accreditation is required in some states and regions by the Centers for Medicare and Medicaid Services (CMS) and by some private insurers. “There are many facets that contribute to an accurate diagnosis based on Echocardiography testing,” said Irvin D. Goldfarb, M.D., FACC, medical director, Non-Invasive Cardiology, SMMC. “ The skill of the Echocardiography sonographer performing the examination, the type of equipment used, the background and knowledge of the interpreting physician and quality assurance measures are each critical to quality patient testing. Hunterdon Regional Cancer Center: The only Cancer Center in New Jersey to offer immuno-therapy clinical trial for lung cancer According to the American Cancer Society, lung cancer is the second most common cancer among men and women and the leading cause of death in the United States. In 2014, it is estimated that 224,210 people will be diagnosed with lung cancer. Hunterdon Regional Cancer Center was selected as the only cancer facility in New Jersey to offer a clinical study on a new lung cancer investigational drug called Nivolumab, which is made by Bristol-Myers Squibb. “This clinical study uses immuno-therapy as a new approach for treating lung cancer. This therapy uses the patient’s own immune system to attack the cancer cells,” stated Myron Bednar, M.D., Medical Oncologist, Hunterdon Regional Cancer Center. Dr. Bednar further explained, “Nivolumab works by blocking the protein called PD-1. The PD-1 antibodies stop lung cancer cells from blocking the body’s natural immune response to cancer. A drug that can inhibit PD-1 may be able to treat a variety of cancers, which is very exciting.” Patients who may be eligible for the study have been diagnosed with advanced stage lung cancer and have undergone at least one therapy prior to entering this trial. “The benefit from standard therapy is limited for this patient population. Our hope is that this drug will keep the cancer from growing and have the patient’s immune
system do the work of killing the cancer cells, versus drugs that may have many side effects, said Kenneth Blankstein, Medical Oncologist at Hunterdon Regional Cancer Center. Dr. Blankstein also mentioned, “We are pleased to be able to provide this trial in our community. We are not only the only cancer facility in New Jersey testing this drug in a clinical trial, but the only one in the tri-state area. The next closest facility is in Tennessee.” Daria Shepherd of Kingwood Township was diagnosed with advanced stage lung cancer in December. Ms. Shepherd had gone to her family doctor because of a pain she was having on her side. An ultrasound showed that the pain was the result of a cyst on her ovary, but the test also found a spot on her lung. A chest scan and a biopsy determined it was cancer. “I started chemotherapy, but it wasn’t effective for my type of cancer, so Dr. Bednar thought I would be an ideal candidate for the Nivolumab clinical trial,” stated Ms. Shepherd. Shepherd added, “This is my second week receiving the drug and I feel good, it has not had any side effects that have interfered with my life.” In fact, Ms. Shepherd was heading out to Spruce Run to fish with her family after her treatment. AHRMM, CQO: Turn Supply Chain into Value Chain Hospital supply chain professionals are gearing up for our annual conference, hosted by the industry’s professional society, the Association for Healthcare Resource & Materials Management (AHRMM). The theme of AHRMM14 is "Initiate. Collaborate. Innovate." It's a really grand expectation that shows just how far supply chain has come from box counting in the loading dock. What’s driving the transformation of supply chain? Health reform, of course, but also AHRMM’s response to health reform: the Cost, Quality, Outcomes (CQO) Movement. One of the biggest impacts of the ACA is performance-based reimbursement, meaning that clinical and fiscal teams must collaborate now more than ever to ensure their capacity to provide quality care at a more affordable cost. Accordingly, the hospital supply chain needs to adopt to playing a more significant part in synthesizing input costs in relationship to outcomes data. In January 2013, AHRMM launched the CQO Movement to help industry supply chain professionals navigate the developing healthcare space and to succeed in their new, expanded role. Over the years, supply chain professionals have created key strategies to optimize operations and resources. But targeting the supply chain and its expenses is no longer enough. With supply chain leading the way, understanding the total cost of supplies, procedures, and delivered care—as well as how all of these costs intersect to affect reimbursement levels—stands within the purview of the supply chain as well as all departments within hospitals and healthcare systems. Using the CQO model, everything (both inside and outside of the supply chain) is focused on patient outcomes, which not only depend upon cost and quality, they drive reimbursement. The CQO Movement centers on the correlation between cost (supplies, services, and other expenditures in the supply chain, as well as the total cost of care), quality (patient care in line with achieving the best possible clinical outcomes), and outcomes (financial reimbursement driven by exceptional clinical care at the appropriate costs) as opposed to viewing each component separately. AHRMM14 will lay out the actions that supply chain professionals must take in order to thrive and that AHRMM will facilitate. In addition to continuing professional education (CPE) credit hours, the conference offers opportunities to discover and share cutting-edge solutions for organizations—large or small. AHRMM14 aims to help supply chain and other healthcare professionals to initiate a paradigm shift from cost-based materials management to a supply chain focused on cost, quality, and outcomes. collaborate across healthcare functions to reach new frontiers of performance. innovate supply chain management strategy to ensure financial reimbursement driven by outstanding clinical care at the appropriate costs. Brian Walsh, MD, MBA, presented two research projects at the National Committee of Residency Directors in Emergency Medicine (CORD-EM) in New Orleans on March 31, 2014. Patient-centered communication and disposition are two of the milestones established by the Accreditation Council for Graduate Medical Education and American Board of Emergency Medicine. Since these milestones can be difficult to assess because of variability in individual patient interactions, Dr. Walsh’s research sought to determine how assessing 24-hour return visits can make the evaluation of these milestones more objective. His presentation, titled, “The Use of 24-hour Return Visits to Assess Residents’ Patient Centered Communication and Disposition Ability” concluded that evaluation of 24-hour return visits provides an objective way to begin assessment of patient-centered communication and disposition. The second presentation was titled, “Assessment of Pre-hospital Care with the Emergency Medicine Milestones.” Advanced Life Support (ALS) patients receiving online medical control or base contact who are triaged by emergency medicine residents for Basic Life Support (BLS) tend to be complex and are associated with increased liability. The research evaluated whether a review of these patients could provide a simple and efficient way to identify proficiency gaps in emergency pre-hospital care. The study concluded there are relatively few ALS patients who are triaged to BLS by emergency medicine residents. These patients tend to be complex and many are admitted to a monitored bed from the ED. Dr. Walsh is an active researcher in the fields of pre-hospital and emergency medicine. He is a frequent regional and national lecturer on topics pertaining to emergency medicine. He is an attending physician at Morristown (N.J.) Medical Center and a partner of Emergency Medical Associates. Meadowlands Hospital Medical Center During his busy shifts at the Meadowlands Hospital Medical Center Emergency Room (ER), Arthur Kharonov, RN, BSN, Director of ER, is stopped often by patients leaving the facility. They want some quick face time to thank Kharonov, and the rest of the talented Meadowlands ER staff, for the quality treatment and attentive care they received. More importantly, patients appreciate that their care came in a timely, precise manner. The waiting room at many hospital ERs can often be crowded and backed up, leaving patients, including many who may need prompt care, waiting for hours. That certainly isn’t the case at the Meadowlands ER, where the commitment to the prompt evaluation, diagnosis and treatment of each and every patient is a priority. Eight months ago, Meadowlands initiated a program that ensures that every patient is seen by a nurse within five minutes of arrival and an ER physician within 15 minutes. Translation: Instead of waiting for hours, patients are evaluated quickly, moving on to the next stage of treatment or discharge in a fraction of the time of other hospital ERs.
“It’s very satisfying to get that kind of feedback because then we know our team did its job,” said Kharonov. “We want to retain patients and make them part of our family. To date, the program has been a success and word is spreading, and we are getting a lot of referrals through word of mouth. It has been amazing. A patient comes in and they tell someone else of their positive experience.” The ER can be hectic, with a potential emergency crisis at any moment. However, the transition at Meadowlands has gone smoothly, with the hospital proudly living up to the five/15-minute guarantee. Meadowlands Hospital provides 24-hour, comprehensive emergency care services at the hospital campus on Meadowlands Parkway in Secaucus. The ER is staffed by board-certified emergency physicians and emergency-certified and paramedic nurses. These clinical professionals, along with support staff, utilize state-of-the-art equipment to treat a variety of patient needs. Meanwhile, a fleet of modern ambulances with EMTs supplements the Meadowlands ER. They respond to calls throughout Secaucus and surrounding communities in an average time of four minutes.

For Better or For Worse You walk onto your unit for a new shift with a fresh start. Routinely, you glance at the list of patients who have been assigned to your care, and then you gear up to receive shift-report from the departing nurse. This crucial time allows the leaving shift to paint a picture of the patients you will be spending the next twelve hours caring for and their condition. The departing nurse is laying the groundwork for your day via this synopsis. However, this change-of-shift report also has the ability to affect your morale, if you let it. Though change-of-shift hand-off communication lasts approximately thirty minutes or less, this time can truly transform your attitude at the start or end of your shift for better, or for worse. For instance, envision the shift that winds up like a nightmare. New and old nurses have all experienced it. This is the type of shift when something unimaginable happens, and then everything else crumbles to pieces. You did not stop to use the restroom, to eat, to chat with co-workers, or to sit down for twelve dreadful hours. The end of your shift can go in two very different directions. The on-coming shift can support you in your efforts and praise you for trying your best in light of the situation. Or, the on-coming shift nurse can reprimand you for unfinished tasks and a scattered report. One of these scenarios ends in patting yourself on the back, the other, undoubtedly ends in tears. Nursing is undeniably one of the most challenging professions. Nurses need to support each other in an effort to keep morale high and prevent burn out from occurring. Yet, it's easy to notice that other nurses have the ability to quickly make you feel inferior, especially for the unseasoned new nurses. Rather than rolling eyes when a shaky hand-off report is given, try constructive feedback for your co-worker. Foster confidence and success in co-workers, rather than devaluing others. Throw snide comments and sighs at the wayside as much as possible in your professional role. Let us replace this lateral violence with encouragement and empathy. Nurses have enough grief to leave the unit with after a shift with, for instance, a death of a patient, poor prognoses, or even hurtful remarks from patients. Other nurses should not be adding to fire. Instead of burning out fellow nurses, let us keep each other's flame ignited for the passion to provide competent care for patients. There will never be enough time in a shift to make everything perfect. However, beating each other up for mistakes will never make things better.

“Finally, it’s not pouring rain or 51 degrees. You earned this.” With those words, Wayne Cabot, news anchor for WCBS Newsradio 880, sent the 5K runners on their way during the 17th annual Miles for Matheny, the Matheny Medical and Educational Center’s annual fundraiser and community event, held Sunday, June 1, at Liberty Park in Peapack. Matheny is a special hospital and educational facility for children and adults with medically complex developmental disabilities. The weather, as Cabot indicated, was perfect. But so was the atmosphere and overall sense of community spirit, highlighted by the festivities at 23 Mendham Road, located along the route of the Lu Huggins Wheelchair Walk in Peapack-Gladstone. Spearheaded by Heather Santoro, who lives there with her family, that stop, just

before the home stretch of the 1.5mile journey, served as an incredible rallying point for the wheelchair participants, their friends, families and caregivers when they saw signs such as, “We Love Matheny” and “Roll On Matheny”, and heard the enthusiastic cheering and noisemaking. The day, which also included five cycling routes and a kids fun run, attracted an outpouring of supporters from communities throughout New Jersey. And, all the running, walking and cycling, as Cabot and WCBS-TV meteorologist Elise Finch reminded everyone, was being done for one purpose: to support the Matheny Center of Medicine and Dentistry where more than 800 of New Jersey’s children, teens and adults with developmental and intellectual disabilities have access to the best possible medical, dental and therapy care.

education & careers Trinitas School of Nursing Awards Nursing Diplomas and Associates Degrees to 74 graduates Men continue to attain entry into a once female-dominated profession According to a U.S. Census Bureau study released in 2013, the proportion of male registered nurses has more than tripled since 1970, from 2.7 percent to 9.6 percent. The report went on to say that of the 3.5 million employed nurses in 2011, about 3.2 million were female and 330,000 male. With its most recent graduating class, the Trinitas School of Nursing reflects the growing number of men gravitating to the profession. At its commencement exercises last month, 74 graduates, including 11 men, were awarded their nursing diplomas as well as an Associate of Sciences degree from Union County College, its academic partner in the Cooperative Nursing Program. Trinitas School of Nursing is one of the largest nursing schools in the country and is a member of the National League for Nursing and the Association of Diploma Schools of Professional Nursing. In addition, its graduates consistently perform in the upper percentiles in the National Nurse Licensing Exam, the NCLEX, frequently passing the rigorous examination on first sitting. The National League for Nursing (NLN) has also recognized the School with a designation as a Center for Excellence in Nursing Education Creating Environments that Enhance Student Learning and Professional Development through 2015. Trinitas was the first school in New Jersey and the first Cooperative Nursing Program in the nation to receive this recognition. In September 2011, the School was redesignated as an NLN Center of Excellence through 2015 based on its efforts to maintain excellence through a series of long-range strategic plans. education & careers New Jersey League for Nursing hosts 5th Annual Practical Student Nurse Education Day The New Jersey League for Nursing (NJLN), an Affiliate of the National League for Nursing, hosted its 5th Annual Practical Student Nurse Education Day, “A Workshop for Practical Student Nurses: Journey into Practice...Taking the First Steps.” The program was developed by nurse leaders in the practical nurse setting to help Senior Nursing students as they plan their transition from academic studies to nursing practice. Over 275 LPN students participated, representing eleven Practical Schools of Nursing from across New Jersey. Students had the opportunity to network with fellow students and exhibitors, plus listen to presentations geared specifically for their needs. Kathleen Gialanella, RN, Esq., Counselor at Law in Private Practice, spoke about legal aspects of nursing documentation; NJLN President Patricia Castaldi, DNP, RN, ANEF, Director of Practical Nursing & Allied Health Liaison, Union County College, provided strategies to keep your own energy high
while giving patient care; and John Lanier, RN, MS, NE, BC, Manager, Bloodless Medicine & Surgery Program, Trinitas Regional Medical Center, discussed how to navigate the bridge from being a Licensed Practical Nursing to become a Registered professional nurse if that is a future career goal. NJLN awarded eleven Nursing Scholarships to students nominated by their school administrator and faculty. Four special awards were presented from the Dorothy Lambert Memorial Scholarship Fund. Lambert was a nurse educator with the Somerset County VocationalTechnical School’s Practical Nursing Program before her death. The New Jersey League for Nursing supports and implements the mission of the National League for Nursing to promote excellence in nursing education to build a strong and diverse nursing workforce to advance the nation’s health at the constituent level.
Bergen Regional Medical Center named Mentor Hospital by New Jersey Hospital Association New Jersey hospitals searching for examples of how to make improvements in reducing patient harm can now turn to Bergen Regional Medical Center (BRMC) for support. BRMC has been named a Mentor Hospital by the New Jersey Hospital Association - Hospital Engagement Network Project (HEN). As a Mentor Hospital, BRMC will share with other hospitals its strategies that have proved effective for reducing patient harm. BRMC joined HEN in 2012 and worked with the Project to improve patient safety by addressing patient injuries that result from adverse drug events, various types of infections, injuries from falls, pressure ulcers and more. “We’re honored to have been asked by HEN to elevate our participation in the program and share our successes with other hospitals. Our staff are leaders in providing specialized care for vulnerable populations and they look forward to ex-
panding their effect on creating substantial and positive outcomes on patient care,” said Kathleen Divers, BRMC Associate Vice President, Quality/Outcomes Management. Hospital Engagement Networks (HENs) work at the regional, State, national or hospital system level to help identify solutions already working and disseminate them to other practitioners to assist them with designing systems of care that are safe, efficient, evidencebased and effective. Learning from other healthcare practitioners’ successes and challenges will help others apply those lessons and continuously improve performance. The HEN Project is part of the U.S. Department of Health and Human Services’ Partnership for Patients campaign. The principal objective of this campaign is to reduce preventable conditions acquired in hospitals by 40% (with a view to eventually eliminate them altogether).
