Bereavement Services ‘In Your Loss Leaflet’ during Covid-19 May we offer our sympathy to you, your family and friends during this very sad time. We understand it may be that your loved one passed away without family beside them and for this we can only offer our deepest condolences. Please accept this leaflet as a guide in replacement of our usual booklet, this is due to the changes taking place with regards to Covid-19 and registering deaths. The booklet is still available for more information and a copy can be found with this leaflet, or please let Bereavement Services know and they will send one to you through the post. We hope that you will find this sheet helpful during the next few days with all the arrangements that need to be made regarding registering a death in the current climate and the impact of Covid-19. Our Bereavement Services Team are still available for you between the hours of 9am - 4pm, Monday to Friday and 9am - 12pm Saturday. The team can be contacted on 0151 604 7346. May we request you do not attend the Bereavement Services office unless advised to do so by a member of the Bereavement Services Team.

What you need to do next Please phone the Bereavement Services at Arrowe Park Hospital after 12noon the day after the death in order to give your details to the team. The Bereavement Services Team will take the following details from you: Name: Address: Contact number: Deceased name: Funeral Director: Is it a cremation or burial? Once the Medical Certificate of Cause of Death has been completed on the ward and brought to the Bereavement Services office, the team will call you back and go through the registrar’s pro-forma and all information you may need.

The Registrars in Birkenhead are now closed, however they are registering deaths over the telephone with next of kin once they have had the relevant paperwork from the Bereavement Services. They will contact you in a day or two after receiving the paperwork to complete the registration over the phone. The usual fee of £11 per Death Certificate will still be applied, sometimes families require more than one certificate, Bereavement Services will advise you when they speak to you, this is payable to the Registrars over the telephone when you have your telephone appointment. At your telephone appointment the Registrars will need to know the following information about the deceased: •

Date and place of death

Last usual address

First names, Surname (and maiden name where appropriate)

Date and place of birth

Occupation

Marital Status

Once this information has been collected they will send you the Death Certificate(s) through the post to you. You can contact a Funeral Director at any time during this process to start making arrangements. Unfortunately at this time we are unable to allow visiting of your loved one in the Hospital, however you may be able to visit your loved one in the Funeral Directors. This is at your Funeral Directors’ discretion. Your loved one’s GP will be automatically informed of their death so you don’t need to inform them. If you have any belongings the Bereavement Services Team will advise you on collection, depending on the circumstances of the death. If you require any support during this time please do not hesitate to contact the Bereavement Services Team on 0151 604 7346 and they will be able to direct you further. We are here for you at this time. The Trust has introduced a Family Support Team during this time, if you would like further information and contact details please let Bereavement Services Team know and they will organise this for you.
