Access to Healthcare records

Personal representatives of patients are entitled to access to their health records under the Access to Health Records Act 1990 where a request is made to access the Medical Records of a deceased patient. Enquiries about access to health records can be made contacting the Access to Records Department and ask for an application form to be posted to you. Alternatively, an applicant may send their request by letter rather than using the Trust’s request form, as long as the letter contains enough information to identify the correct patient record and confirms what information is required.
