Parents MUST bring a certified copy of the child’s birth certificate and immunization records. They will also be asked to furnish some directory information such as date of birth, address, and telephone number. Besides enrollment information, school staff will be using the DIAL-4 to gain information about each child’s development. Immunizations can be received at your doctor’s office or the Community Health Nurse’s office (605-892-2523)

Appointments required - call North Park at 723-3379 to schedule an appointment ASAP. Appointments generally last one hour. Current Kindergarten students will not have school this day.
