For the Leader When it comes to breast health, breast radiologist Ajanta Patra, M.D., believes knowledge is power. Fellowship-trained in breast radiology at Weill Cornell Medical College in New York City, Dr. Patra provides a full range of screening and diagnostic mammography services as an affiliated breast radiologist at the Memorial Hermann Breast Care Center at Memorial Hermann Northwest Hospital located in the Heights. She works closely with her patients to ensure a positive experience and quick turnaround on procedures and results. “Women who come to the Breast Care Center for their annual screening mammogram have the advantage of knowing their results will be read by a dedicated breast radiologist and that they will receive highly personalized care,” said Dr. Patra, who graduated from the Johns Hopkins University School of Medicine, where she also completed her residency in diagnostic radiology. The full range of services at the Breast Care Center includes digital mammography with the most advanced computer-aided detection system, breast ultrasound, breast MRI, stereotactic breast biopsy and ultrasound-guided needle biopsy. “We bring women who receive news of an abnormality on a screening mammogram back for a more advanced follow-up exam within days,” said Dr. Patra, who personally reviews the results with women called back for follow-up. “We’re very sensitive to the fact that this is a frightening time when women want answers fast. I’m often the first doctor a woman sees if she has an abnormal finding on a mammogram so I feel that it’s important to spend time with her, either explaining that it’s nothing to worry about or helping her through the next steps.” Although the breast cancer gene has received much media attention, only 5 to 10 percent of breast cancers are caused by genetic problems. Two genes – BRCA1 and BRCA2 – are the most common genes responsible for hereditary breast cancer. “Women in the general population face a 12 percent risk for breast cancer over their lifetimes,” Dr. Patra said. “Normally, BRCA1 and BRCA2 keep cells from growing out of control. But women with defective copies of these genes have a much higher chance of developing breast cancer, often before the age of 50. Garden Oaks resident: survivor, advocate and support system for breast cancer patients By Christina Martinez christina@theleadernews.com Garden Oaks resident Shirley Cato says she never paid much attention to her health or nutrition until after she was diagnosed with breast cancer and then later diagnosed with lung cancer and a second breast cancer diagnosis. Cato says early detection and having annual screenings are the reason she will get to celebrate her 79th birthday this year at Christmas, and many more to come. “I was first diagnosed with breast cancer in my late 50s,” Cato said. “I never paid much attention to breast cancer, nutrition or health. I was just busy with life. When I was in the hospital with my first breast cancer surgery, I got a visit from two ladies from the Reach to Recovery program with the American Cancer Society. They really impressed me. It was something that I wanted to do.” Twenty years later, Cato can be found in the American Cancer Society Reach to Recovery office at Memorial Hermann Northwest off of Ella Boulevard, helping women much like herself a few years back. The program is an outreach initiative for breast cancer patients to meet with breast cancer survivors to help with resources in their time of need. Much of those resources consists of beautification products like wigs, scarves, mastectomy bras, prostheses, knitted hats or comfort pillows, which the pillows and hats and are handmade by St. Rose of Lima in Garden Oaks. Along with beautification resources, patients get to meet with someone who has walked their same path
and share and listen to stories that might provide hope. Volunteers offer resources and information provided by the American Cancer Society and are trained in fittings for wigs, the prostheses and mastectomy bras. Before these resources were readily available at Memorial Hermann, Cato recalls back to when she first started with Reach to Recovery. Cato says she would visit patients in their home, because catching the patients after surgery in the hospital was so difficult. “It was nice to be able to go into their home and visit with them,” Cato said. “All of the ladies were always so gracious. Everyone I have ever visited has been gracious. Now that I am older, I love the resource center. I live close and come down mostly once a week. We have everything right here at our disposal. When we were visiting in homes we would have to go down to the Richmond office and take a bag and fill it with our literature, bras, everything that we needed to go and make that visit. Now it’s right here at our fingertips.” Cato finds that sometimes doctors might be too busy to explain the answers to questions that the patient might forget to ask, or even not know to ask. “Sometimes doctors are busy and they don’t always have time to explain,” Cato said. “I had one lady that I visited tell me that her arm was numb. She didn’t know that those nerves in her arm had been cut and that her arm was going to be numb. Mine is still numb from my surgery over 20 years ago. Little things like that can be very helpful when we
meet and talk to patients.” Bits and pieces of knowledge that Cato has personally dealt with is something she enjoys sharing. Something Cato enjoys even more is seeing the transformation in the women she fits for wigs, prostheses or mastectomy bras. “When you fit one of the ladies with a wig or with a bra, you can just see their transformation in their face,” Cato said. “It’s just evident. They feel beautiful again, and they look good in their clothes. That alone is really great.” Cato had to pause before continuing, as emotions of joy got the best of her. “The ladies are so courageous,” Cato said. “They can’t help but inspire me. Because they are all so positive. It’s amazing. Young or old and they just have a positive attitude. I think sometimes just seeing us, when we aren’t crying anyway, the way they are is encouraging. I think it gives them hope to get through. If we can just touch in a small way to give them hope. I’ve felt very comfortable doing it and I’ve never once looked back or regretting not doing it. It’s just something that I do.” As Cato celebrates her 79th birthday this year, she is reminded everyday of how important health, nutrition and annual visits are. Now, Cato is an advocate and leader for living a healthy lifestyle and helps women see and understand choices she overlooked years ago. Shirley Cato is a standout volunteer for the American Cancer Society, a healthy leader in the community and a support system for women to lean on when they need it most.
Common bonds: How the power of friendship is helping one woman in her fight against cancer By Jonathan Garris jgarris@theleadernews.com For over thirty years, Liz Spillars and Rosie Busa have called each other friends. The two first met in the mid-1970s while living in the same apartment. Both Busa and Spillars recall fond memories of their time spent living near one another, particularly Busa who met her late husband while living there. “They were always out having parties around the pool,” Busa said with a laugh. Even after the years passed, both Spillars and Busa made sure to keep contact with one another.

Spillars through occasional greeting cards around the holidays and other
conversations. “We’ve always talked about doing things, but we just never had the time or the money,” Spillars said. “We both had our families and our lives but we always said hello to one another over the years.” After returning to the Houston area, Busa reconnected with Spillars, who has been working at Memorial Hermann Northwest Hospital for nearly 15 years and is currently a mammography technician. Each year, Busa requested Spillars to help with her mammogram. Last year, their lives took an unexpected turn when doctors found Busa had breast cancer. Spillars See Bonds, P. 2B

From diagnosis to survivorship Memorial Hermann Oncology Navigation Nurse Angela Sisk is there each step of the way By Christina Martinez christina@theleadernews.com When diagnosed with an ailment, several questions usually follow. However, when diagnosed with an ailment as severe as breast cancer, several questions follow along with different emotions that may cloud how to respond with the diagnosis. Angela Sisk, an Oncology Nurse Navigator at Memorial Herman deals with these types of cases and questions everyday. Nurse navigators follow patients when they are newly diagnosed with cancer through treatment and beyond. Sisk knows when the patient is going through treatment, what kind of treatment they are having or when they are going to have a procedure. “As nurse navigators we follow patients and there are a lot of programs we refer them to, with Reach to Recover being one of them,” Sisk said. “Most of the time, what navigators have to do is send a referral out to the main Reach to Recovery office in Richmond and someone will have to go to their home to see them. Reach to Recovery volunteers are breast cancer survivors and so they’ve been through this experience. They are able to go out and talk to them about their experience and talk to them about different kinds of exercise and nutrition. They usually give them a leisure bra, a pillow and some pamphlets. I’m blessed because I actually have two ladies who are here during the day and who can meet with patients if I need them to. I don’t even have to go through the main office and have the main office do home visits, they can just meet while they’re here. Or schedule accordingly.” As a navigator, Sisk says she offers different classes and support groups. Another class offered is the Look and Feel Better class that is offered once every other month. Sisk
has a skincare consultant and wig specialist that put the class on free of charge for patients. Amanda Parrish has worked in dermatology for over seven years and worked for a Physician practice for two years. While obtaining her cosmetology license for skin care, Parrish learned of the Look Good Feel Better program. After Parrish lost her grand mother to brain cancer, she learned first hand what the body goes through during treatment and how the skin is affected in treatment. After learning first hand, Parrish says she knew she wanted volunteer with the class and offer skin care tips to patients. “During the class, patients receive a bag filled with a bundle or products,” Parrish said. “We go through and give them about 12 steps. We touch on cleansing, moisturizing, applying makeup and skin maintenance. We talk about taking care of their skin while going through treatment and we give them tips and techniques.” The class size varies from month to month. But regardless of the class size, Parrish says the class offers support for the women. “Some women are just starting treatment and some have just finished,” Parrish said. “Some families don’t quite know or understand what these women are going through. In class these women talk about everything, like what to expect. It’s very uplifting to see and we’ve gotten great feedback from the class.” Lisa Amosu-Smith, Owner of My Trendy Place on Post Oak has volunteered with the American Cancer Society for five years. She started her volunteer work by washing wigs for the society and later was offered the instructor position with the Look Good Feel Better class. “I bring in wigs that are donated from my place or by American Cancer Society,” Amosu-Smith said. “We make

the ladies look and feel better. We show them how to wash their wigs and how to tie scarves and turbans.” Amosu-Smith says she is asked often if her volunteer passion comes from the loss of a loved one. Although Amosu-Smith has mourned the loss of a brother-in-law to cancer, she says this has been her passion and calling for some time now. Amosu-Smith shows the women how to wear their wigs and at the end of the class asks the women who enjoyed wearing and learning about the wigs. She says the women always answer graciously and Amosu-Smith announces that they get to keep their wig of choice. Another resource that was already offered, but is new to Memorial Herman, is the prostheses program. The American Cancer society offered the program for women who are uninsured or underinsured. These women can get a prostheses or mastectomy bra for free. Unfortunately, the program was cut from American Cancer Society, but Memorial Hermann picked up the service to offer to their patients. “Luckily one of our advance practice nurses was able to work with our foundation and take it over,” Sisk said. “Now we are in charge with writing the grants and getting the funding.” One of the psychological services Sisk uses that is more medical based and something Sisk is proud of is Memorial Herman’s National Accredita-
tion Program for Breast Centers. NAPBC is an accredit that can only be reached by meeting certain standards, Sisk says. “Every single breast cancer patient that is diagnosed here, we present their case at a multidisciplinary breast cancer conference once a week,” Sisk said. “For all of the doctors dealing with breast cancer, we all meet every week to present the case. We look at the pathology, the radiology and determine the best treatment for the patient. You get multiple sides of input. I think the patient gets really great care when you have all of those heads talking about their particular case.” Sisk says that’s the kind of care Nurse Navigators make sure their patients get or should get. “When you have cancer there are a lot of doctors you see and a lot of steps to the puzzle,” Sisk said. “We make sure all of those steps fit together and make sure that they’ve seen everyone they need to. And most importantly, that they don’t fall through the cracks. Falling through the cracks is something Sisk says happens when communication fails. Making sure communication is met and the best resources, programs, volunteers and recommendations are given from diagnosis to survivorship is the quality of care Sisk and other nurse navigators provide to several patients on a daily basis.

Bonds from P. 1B struggled to hold back tears as she talked about how it affected her. “That’s the closest it has ever been to me,” Spillars said. “It caused us to really reconnect in a good way. To make a bad thing become something so good is why I believe it’s so special. Fate has brought us back together.” Busa remembers fondly how Spillars would use her lunch hour to sit with Busa during her biopsies. Busa also struggled to hold back tears as she jokingly spoke about how much the procedures hurt. “I remember telling her she had better place her hand on top of mine because if I hold it I’m going to break it off,” Busa said with a laugh. Unfortunately, this hasn’t been Busa’s first experience with cancer. Two of her sisters had previously been diagnosed with cancer, and her husband lost his battle with cancer in 2001. Having people like Spillars, and her son, has made a world of a difference in her fight with cancer. “Having someone you can be close with and feel like you’ve got that love and support is so important,” Spillars said. “It’s a very scary situation, and not knowing the outcome, you feel like you’re in another zone when you first get the news.” Busa also said she was surprised to learn what doc-
tors believed her cancer developed from. “I had a hysterectomy years ago and I had been taking hormone medication for quite a long time,” Busa said. “I had to get off of them because that’s basically what they determined caused the cancer.” Getting routine check-ups also helped Busa’s outlook once the cancer was diagnosed. “I really recommend having the ultrasound done as well,” Busa said. “It’s more expensive for the insurance companies, but it can look a lot deeper into potential issues than some of the other exams.” Spillars also emphasized the importance of following the “triangle” of breast cancer prevention – self exams, clinical exams and mammograms. “All three go hand-in-hand with each other, as one may not be as transparent as the other,” Spillars said. As an Oak Forest resident, Busa says she enjoys having the Cancer Center at Memorial Hermann Northwest because of its proximity and its quality of care, but having Spillars around as a guide and, most importantly, a friend has been a game changer. “There’s a connection strong enough now that I don’t think we’ll ever separate,” Spillars said. “We’re like two loose ends that came together and tied in a knot.”

But you can reduce your risk by exercising regularly, maintaining a healthy

For one survivor, a second opinion meant a second chance By Jonathan Garris jgarris@theleadernews.com
Cancer survivors like Ellen Frieze know all too well what it means to trust gut instincts. The 62-year-old survivor and mother of local real estate agent, Allen Frieze, was first diagnosed two years ago in May 2012. Before her diagnosis, Ellen stressed that she felt great and wasn’t worried about anything. Cancer had been a part of her mother’s side of the family, across four generations, however she felt fine. Ellen said she felt a bit uneasy when she was asked to repeat her yearly mammogram a day after her appointment. “They said they needed more photos because what they saw looked kind of cloudy,” Ellen said. “That worried me a bit, but it happened once or twice before so I was OK with it.” Two days later, Ellen had another appointment and the pictures came out clearer and her doctor knew something was there. An MRI later showed that something was definitely there. Despite the serious situation, Ellen said she was less than thrilled with the way her doctor handled the situation. A nurse serving under the doctor contacted Ellen to tell her the news, but with only small pieces of information about
the cancer in her breast, and the nurse admitted she didn’t have much information “I felt like he should have made the call,” Ellen said. After speaking with the doctor, he assured Ellen that what was there was very small and that she could wait several months and have another mammogram done. Ellen instead opted to see a doctor at St. Lukes Episcopal Health System. Following an ultrasound and biopsy, doctors told her the cancer was aggressive but fortunately at stage zero. Further research also revealed that her cancer wasn’t hereditary but was actually hormonal. “The cancer itself was aggressive but it was still confined,” Ellen said. “We originally thought it was three different spots but it was just two.” Calling her situation scary would have been putting it lightly. “All this time I had been coming to my gynecologist and I had full trust in him,” Ellen said. “I really liked him but I immediately lost confidence in him. I feel like he should have said that something was there and you should go as soon as possible.” Had Ellen waited six months, she said there’s no telling what might have happened. Ellen would go on to have a total mastectomy with immediate
reconstruction several months following her diagnosis. Her son Allen said he was devastated but also relieved to have caught the cancer as early as they did. He was reminded of how lucky his family was at a luncheon supporting breast cancer awareness hosted by the Houston Area Realtors. “One of the things I keep telling her is to be very open with her story,” Allen said. “She comes from a generation where things were held very privately. I’ve encouraged her to talk often about her process and her experience and how people should do what needs to be done.” “I just thank god every day that they found it early and that I’m still here for my family,” Ellen said. “Women absolutely need to have their mammogram done yearly. I know there are some women who haven’t had it done for several years and if I was one of them I would have never known.” Ellen also encouraged those concerned about costs for procedures or tests that need to be completed to move forward with them regardless. One test for Ellen in particular would have totaled around $1,400 and her insurance wouldn’t cover. “$1,400 is a small price to pay to have the rest of your life,” Ellen said.

every year for Breast Cancer Awareness Month, with a portion of funds going towards the Susan G. Komen Foundation. Cupcakes, large cakes and cookies can be decorated with pink from top to bottom. “If you look at some of the information regarding when Susan G. Komen was created, the survivorship rate was somewhere in the 75 percentile,” Jucker said. “You see a direct correlation between Komen growing and the survivorship rate increasing as well.” Three Brothers Bakery will also host a Shred for Komen event alongside Texas Security Shredding at its location
at 4036 S. Braeswood Blvd., Jucker said. Texas Security Shredding will have a truck at the bakery with pink shredding bins where residents can bring documents and other papers to be shredded along with a suggested donation of about $5 to $10. Those that bring items to shred also receive a free cupcake, Jucker said. Three Brothers Bakery has three locations, including its Braeswood bakery at 4036 S. Understanding rare cancer types important for loved ones at risk For The Leader
Many people have been touched by breast cancer. Whether you have dealt with your own diagnosis or that of a friend or family member, the prevalence of breast cancer has left few people without a story to tell about a personal experience with this potentially deadly disease. According to Breastcancer. org, a nonprofit organization dedicated to proving reliable, complete and up-to-date information about breast cancer, roughly one in eight women in the United States will develop invasive breast cancer over the course of her life, while the Canadian Breast Cancer Foundation notes the figures are slightly more optimistic in Canada, where one in nine women is expected to develop the disease during her lifetime. While women and even men who develop breast cancer are most likely to be diagnosed with invasive ductal carcinoma, or IDC, there are less common forms of breast cancer, and learning about these rare cancers may help save your life or that of a loved one. Medullary carcinoma Though medullary carcinoma diagnoses are rare, this is an invasive type of breast cancer that begins in the milk duct and spreads beyond it. The tumor that forms when a person has medullary carcinoma is a soft and fleshy mass. Medullary carcinoma cells typically appear like aggressive, abnormal cancer cells, but these cells do not grow quickly and often do not spread outside the breast to the lymph nodes, which makes medullary carcinoma a relatively easy cancer to treat. Medullary carcinoma can occur at any age, though it typically affects women in their late 40s and early 50s. Tubular carcinoma Tubular carcinoma is a type
of IDC that starts as small, tube-shaped structures known as tubules that appear similar to normal, healthy cells. Tubular carcinoma cells grow slowly and are unlikely to spread outside of the breast, and, for that reason, tubular carcinoma typically responds well to treatment. Though tubular carcinoma once accounted for a very small percentage of breast cancer diagnoses, they are now being diagnosed more often, thanks in large part to the prevalence of screening mammography that has been catching cases of breast cancer before doctors feel a lump. Tubular carcinoma may feel less like a lump and more like a cushiony area of breast tissue. Mucinous carcinoma Mucinous carcinoma is also a rare type of IDC in which tumors are made up of abnormal cells in mucin, which is a part of mucus. Many types of cancer cells, including the majority of breast cancer cells, produce some mucus, which lines most of the inner surface of the normal human body. But when a person is diagnosed with mucinous carcinoma, the mucin has become part of the tumor, so when examined under a microscope, the cancer cells appear to be scattered in pools of mucus. Mucinous cancer can affect people at any age, though it is especially rare in men and most often diagnosed in women after they have gone through menopause. Mucinous carcinoma is a less aggressive type of breast cancer than others and is less likely than other types to spread to the lymph nodes. As a result, it typically responds well to treatment. Paget’s disease of the nipple Paget’s disease of the nipple is characterized by breast cancer cells that collect in or around the nipple. The cancer typically affects the ducts of the nipple first before spreading to the nipple surface and

While Paget’s disease of the nipple is rare, the National Cancer Institute notes the importance of recognizing its symptoms, as 97 percent of people with Paget’s disease also have cancer elsewhere in the breast.

While area businesses may have no lack of items sporting pink ribbons, the co-owner of Three Brothers Bakery has a much more personal connection with the pink confections being sold inside her stores. Janice Jucker said she first started promoting special pink colored and pink ribbon-laden bakery items following her breast cancer diagnosis in 2006. The businesswoman would also discover she was one nine people diagnosed in her family throughout the years, and felt it wasn’t a question of if she would have cancer but rather when she would. “I found out on the eve of a Jewish holiday here in the bakery,” Jucker said. “I worked all day and then told my husband about it at midnight after a long trip. It was a long time to kind of keep that to yourself but I was scared.” Like many people facing cancer, Jucker had plenty of help from friends and family. “You have no idea how many people love you until you’re faced with adversity,” Jucker said. “Six weeks after my surgery, my husband and I wrote over 100 thank you notes to people that had come by to help me and my family.” Jucker has since taken up baking up pink concoctions

Health experts examine vitamin D, breast cancer link Vitamin D and breast cancer link
Try as they may, the thousands upon thousands of medical researchers, scientists and doctors around the world have not yet figured out a way to eradicate cancer. Ongoing studies examining the disease as well as its risk factors may not have yielded a cure, but they certainly have helped shape a number of different methods of reducing risk and improving treatments. Vitamin D may help reduce breast cancer risk, and it may even help women already diagnosed in their fight against the disease. According to a meta-analysis of five studies published in the March 2014 issue of Anticancer Research, patients diagnosed with breast cancer who had high vitamin D levels were twice as likely to survive when compared with those who had low levels of vitamin D. This analysis studied more than 4,500 breast cancer patients over a nine-year period. The study’s authors also found that a vitamin D level of 50 ng/ml is associated

Genetic history may hold answers for some breast cancer victims, survivors ing cancer, whereas strictly hereditary cancer is developed primarily from abnormalities in genes. “Familial cancer is where most of our patients end up,” Newman-Dunnington said. “We might know there are people in the family with cancer, but these people not only share genes, they’re also exposed to similar things during their lifetime,” Dunnington said. “They share food, may drink the same water and breathe the same air.” Those most at risk of developing hereditary cancer might have a family history of rare tumors, male breast cancer, young onset of cancer and other rare diagnoses, Dunnington. “We see those things as red flags that people need genetic testing,” Dunnington said. Dunnington said the best thing people can do is to arm themselves with information about their own family’s medical history, even if the topic can be intimidating for many women. “One misconception for breast cancer is that only the mother’s side matters,” Dunnington said. “Both sides of a person’s family contributes equally to the risk, and it’s important to talk with a doctor to see about qualifying for genetic testing or extra screening or something that might catch cancer early.” Genetic testing isn’t necessarily appropriate for all people but it is an effective tool for those who have an increased risk or who may already have

For those who might come from a family with a strong history of cancer, it may be tempting to dismiss a potential cancer diagnosis as something that unfortunately “runs in the family.” Genetics counselors like Leslie Newman-Dunnington, who works at Memorial Hermann Northwest Medical Center, stresses that there may be numerous factors contributing to cancer, and arming oneself with genetic information can be a powerful tool. “It’s an effective tool,” Dunnington said. “Only about 10 percent of women with breast cancer have a mutation in one of those genes that contributes to developing cancer. Even if they have a really strong family history with cancer, most of these women will have fine genes.” Genetic testing can take between three to four weeks to complete generally, and offers an opportunity for doctors to help pinpoint what might be causing cancer to develop. Most of the testing is performed with a saliva test, and a variety of specific genes are examined for any abnormalities in special laboratories out of state, Dunnington said. The development of cancer is broken into three categories – strictly hereditary, familial cancer and multi-factorial. The latter two take into account different models and elements that can potentially increase the risk of develop-
cancer to determine contributing risk factors, Dunnington said. Women should be ready and willing to explore all options when applicable to their health. “It’s definitely a scary topic for women,” Dunnington said. “Sometimes they might say they don’t even want to know. ‘Why would I want to know if there is nothing I can do about it?’ I hear that a lot but there is a lot that can be done, and not just surgery, in treating women with hereditary predisposition to cancer. Knowledge is power and it’s certainly better to be prepared than not.”
