Health: Post-Partum Problems

Some mares might need to be evaluated several times after foaling to ensure they are ready for a return breeding

POST-PARTUM PROBLEMS

POST-PARTUM EXAMS WITHIN 24 HOURS AFTER FOALING IS A MUST HAVE FOR ALL MARES

omplications sometimes occur at foaling, and C these can be minor or serious. Dr. Ahmed Tibary, professor of theriogenology at the Department of Veterinary Clinical Sciences, Washington State University, says professional breeders usually have a standard protocol for post-partum examination of the mare by their veterinarian to fnd complications as soon as possible.

“Tis examination is important for both the mare and foal and should occur within the frst 24 hours,” Tibary said. “Even if the foaling occurred quickly and with no observed complication, this does not mean there are no problems in the mare. You cannot see cervical or vaginal injuries, and these may occur even without a dystocia.

“No post-partum examination is complete without saving and examining the placenta. Breeders and mare managers can look at the placenta and consult the veterinarian if they see something abnormal, or save it for the veterinarian to examine. Just assuming it is complete and throwing it away is not a good idea; the placenta is where a lot of answers can be found.

“Tere are two types of mares that we examine. One is a mare whose pregnancy has been normal, with no

health problems, and she would undergo a routine post-partum examination. Ten there are mares that we know had complications during pregnancy, and we do a thorough post-partum exam—not only for her, but also for the health and safety of the foal. We want to avoid or head of any problems that we might be able to tackle immediately. It is also the frst step in making a decision on whether we can breed that mare on foal heat or not.

“Regarding planning, there are two situations we need to look at. Te frst is when parturition or the immediate post-partum period requires emergency assistance, within the frst 12 hours. It could be as immediate as during the foaling itself. If we know there is something wrong, we need to make sure the mare is seen by a veterinarian as soon as possible.

“Any mare that shows anything abnormal in the frst few hours becomes an emergency. Tis would include anything from profuse bleeding to colic, to abnormal appearance of vulva and/or rectum, a mare that is shivering or shocky, rejecting the foal, a foal born dead, or any other abnormalities. All of these factors can signal a serious problem.”

RETAINED PLACENTA POST-PARTUM EXAMINATION

A situation that requires attention is a mare that delivers her foal normally, but does not shed the placenta on time.

“Usually, we defne retained placenta as one that hasn’t been delivered completely by three hours post foaling,” said Tibary. “If it is still hanging there or we know there is a possibility of part of it being still within the mare, this problem must be tackled.”

Many breeders have had mares that retained the placenta with no adverse efects on the mare, but this is not a reason to do nothing.

“I wouldn’t take a chance,” said Tibary. “Te overwhelming majority of mares that retain the placenta have severe complications, including endotoxemia and laminitis.

“One caution with retained placenta is overuse of oxytocin in these mares. A lot of people use oxytocin on every case of retained placenta, without knowing why the placenta was retained, or use large doses. A plan for treatment should be devised after examination of the mare to determine why the placenta was retained. Occasionally, the problem may be hypocalcemia (shortage of calcium). Without treating this situation, you are not addressing the real problem.”

It is important to examine the placenta to determine if the mare has a small piece still within the uterus.

“Tis can cause a problem that’s just as severe as if the whole placenta was retained,” Tibary explained. “It is easy to see the problem when the placenta is hanging there, but more difcult to make that judgment when the placenta has already been passed—particularly if foaling was unobserved at pasture and the placenta is missing or has been torn apart.” Another problem with feld foaling or any foaling in which the mare has not been constantly monitored is that you don’t know when the mare actually foaled. “If the placenta is still hanging there, we don’t know whether it’s been one hour or three hours,” Tibary said. “If the placenta is retained for more than six hours, the management of the case must be much more intensive than when it is still fresh.” Length of time elapsed since foaling is crucial to know whether there’s a problem with the placenta, the foal, or the mare. “If the foal is unable to rise and nurse within a certain length of time, it also needs attention,” said Tibary. “Also, we may have a mare that foaled normally, but now is displaying systemic signs of trouble.” “Mares that appeared to have a normal foaling and a normal foal should be examined within18 hours, or sometimes as late as 24 hours, for a general post-partum and foal check,” said Tibary. Tis frst check would include general evaluation of the foal and an IgG to determine passive transfer from colostrum. “In that examination, I look at the mare’s external appearance, trying to determine if she is doing well, such as defecating and urinating normally,” Tibary said. “Generally, if she seems to be very healthy, with no history of problems, I limit my examination to checking the placenta (which I hope the owner saved for me and kept refrigerated) and examining the mare’s vagina and cervix.” Cervical tears might not be accompanied by severe signs, and an observer might not be aware there’s a problem. “Tese strategies might address any major emergencies in the mare or foal before they get out of hand,” said Tibary. “Another aspect of this post-partum exam is to discuss any true emergencies that might occur in the frst 24 hours. So the people monitoring the mare and foal could be on the lookout for signs that should be considered emergencies that require treatment, or even hospitalization.” A retained placenta, such as this one shown Emergencies would include a prohere decomposing after 24 hours, can cause lapsed uterus, prolapsed rectum (both serious problems for the mare of which are rare), prolapsed or invert-

ed bladder, severe rectal-vaginal tears (as when the foal’s foot pushes through the vagina into the rectum), or severe bruising or laceration of the vagina.

“Tese can be complicated if the mare becomes infected, or can become aggravated by severe infammation,” said Tibary. “For example, simple vaginal bruising that is painful may prevent the mare from urinating or defecating because it hurts.”

Te mare then becomes impacted, and this secondary problem might be worse than the primary problem.

“Some of the preventative treatments for mares before foaling would include laxative feeds like bran mash, so that the bowel movements will be softer and more readily passed without causing pain,” Tibary said.

Some broodmare farms and practitioners like to administer mineral oil to all post-partum mares, particularly those that have had some trouble during foaling in the past.

One of the biggest emergencies in the mare after foaling is bleeding.

“Tis may take hours to show up, or a few days, depending on the extent of the hemorrhage,” said Tibary.

Just because a mare seemed fne after foaling doesn’t necessarily mean she’s out of danger.

“Tis is why it is important to do the post-partum examination during the frst 24 hours,” Tibary said. “Even if everything seems normal, the examination can help classify the mares as normal or needing more follow-up examination.”

Such exams can alert the owner and veterinarian to something that could be a problem.

“Post-partum protocol involves two things,” said Tibary. “Going into foaling, we need to have an idea of what would constitute an emergency during foaling, because most of the post-partum problems will be linked to that. If breeders have a very good protocol about problems with foaling, they would also have a good protocol on post-partum care. When they get to the post-partum examination, they should realize that if everything has gone according to schedule, this doesn’t mean there will be no problems at all. Tey still have to do the post-partum check between 12 and 18 hours (24 hours at the latest). Tey should also try to identify emergencies as quickly as possible.

“I cannot emphasize enough the importance of a post-partum exam. Last spring I had mares referred here that had retained placenta—that had gone on too long. One had gone on for 18 hours and one was close to 30 hours. We know how disastrous those can be.

“Tere are also some clients who feel that if everything goes well with foaling and the mare seems fne and shed her placenta on time, etc., they don’t need to spend money to have a post-partum exam,” said Tibary. “I tell them that no one (except the mare herself ) knows whether the foaling was easy or not.”

Owners should have a plan in place to make sure the foal gets the all-important colostrum

After an initial post-partum examination at the farm, some mares need to be transferred to a veterinary clinic for further evaluation and treatment

Tere are many things you cannot see, such as internal trauma like a torn cervix.

“Complications can arise if the mare is in pain and reluctant to defecate or urinate,” said Tibary. “Ten suddenly she has colic.”

While veterinary advances have helped breeders and veterinarians deal with some of these problems and get better results, nothing is as important as early detection and diagnosis and thorough intervention.

FOALING INJURIES AND PROBLEMS

Sometimes during a difcult foaling or manipulation of the foal to assist delivery, one of the foal’s feet may scrape or tear the uterus.

“Te list of injuries that can occur during foaling is long,” said Tibary. “Tese need to be classifed as to seriousness.

“If I were to rank various post-partum problems by frequency, the most common would be retained placenta— either total or partial. Tis could lead to complications such as toxemia.

“Te second most common problem that is not quite an emergency but needs to be taken care of is rectal-vaginal tearing, with diferent degrees of seriousness. Some are more dangerous than others.

“After that, in almost equal proportion (and increasing in number when there’s been severe dystocia) are hemorrhaging, uterine prolapse, uterine tears, and vaginal/bladder prolapse (the bladder is pushed out during or immediately after foaling).

“In mares, bladder prolapse is relatively common after severe dystocia, compared with other species, because the urethral opening in the mare is so large. Sometimes it is so large that you can put your whole hand into the bladder. Te minor problems that may become complicated later are bruising of the cervix, etc.”

Some of these problems are not very common, but they do happen occasionally.

“Tis is why every mare needs to be looked at after foaling, to pick up on those situations,” said Tibary. “Even if it is not life-endangering, it may still be a problem for the mare’s future fertility.”

You might be able to treat or correct something now and prevent problems later. Some situations might not be correctable. You might have to make the hard decision about putting down the mare and ending up with an orphan foal.

WHAT IF THE MARE REJECTS THE FOAL?

“Often the cause of foal rejection— not allowing the foal to nurse—may be due to pain in the udder,” said Dr. Ahmed Tibary of Washington State University. “The mare may have mastitis or severe edema. This can jeopardize the life of the foal if he can’t obtain colostrum.

“Breeders should always have a plan, in case of this emergency—to be able to supply substitute colostrum to feed the foal or to make up for lack of quality of the mare’s colostrum. They should also be aware of the need to examine colostrum quality.”

The welfare of the foal is always part of the equation when a mare has a problem.

“Anything that affects the mare can have an adverse affect on the foal, and vice-versa,” said Tibary. “For example, whenever a foal is hospitalized for some reason, this will affect the mare’s uterine involution.”

If the foal can’t nurse, this can be a problem, since suckling stimulates contractions and shrinking of the uterus.

“When we hospitalize the mare and foal, she also doesn’t get the exercise, which is crucial for post-partum involution of the uterus,” Tibary said. “Involution is activated by nursing and by exercise. Normal involution is hindered if the mare is just standing in a stall.”

People are often so focused on the medical crisis of the mare or foal that they don’t think about the need for exercise for the mare. Post-partum emergencies are responsible for about 40% of mares that have problems with timely uterine involution.

When talking with his veterinary students, Tibary tells them never to separate three important things when performing post-partum examinations—the mare, the placenta, and the foal. These are all very closely interrelated, and they all affect the health of the other.

Mare owners need to have a plan for what they might do if they lose a mare and end up with an orphan, or have a foal that must have another source of colostrum or milk.

“Things as simple as connecting with other breeders may be helpful, to see if there might be access to substitute colostrum, for instance,” Tibary said.
