About Calvary Community Care Contents

Calvary Community Care can support you to live independently at home and actively participate in the community.

We provide flexible services tailored to suit your lifestyle. Our experienced team of Support Workers are certified in home and community care, first aid and manual handling. They undergo reference and police checks and receive specialist training. Calvary is a value based, not for profit organisation with a rich heritage of caring in the community. Our services are available 24 hours a day, seven days a week. Calvary will always be there with you. It’s our mission.

Home Care Packages 1 Level One Support for people with basic care needs

2 Level Two Support for people with low level care needs

A Home Care Package is an allocation of funds enabling you to purchase products and services that support your independence. Packages are flexible so they can meet your requirements today and adapt to the future.

There are four levels of packages available. Please check with your Case Manager about which package levels are available in your area.

3 Level Three Support for people with intermediate care needs

What is Consumer Directed Care? From July 1, 2015 all Home Care Packages will be delivered on a Consumer Directed Care (CDC) basis. This means that you have choice and control over how your Home Care Package funding is allocated and how your services are delivered.

What services and products are available? Your Home Care Package can be used for services and products that support your independence and your social connections within the community. These include: • personal care – showering, grooming and mobility assistance • social support – companionship • transport to shops, appointments and events • nutrition – menu planning, meal preparation and cooking. • domestic assistance – cleaning, laundry and shopping • clinical care – nursing and other health support • house maintenance and modifications

With a CDC Home Care Package you can • select the services and products that will best support your independence • plan services to suit your schedule • decide your level of involvement in the management of your Home Care Package budget and service.

Setting your Goals When designing your package it is useful to think about your needs and goals. Your Case Manager can work with you on identifying goals and strategies by linking your services to outcomes. Goals could be related to your mobility, health and well-being or social activities. What do you enjoy doing most? What could improve your daily living? What lifestyle do you want to live?

Managing Your Budget Your annual budget is made up of: • funds from the government • your contribution or co-payment • income tested fees (determined by the government)

Everyday

Calvary Community Care is responsible for holding the budget funds on your behalf. You can also “top up” your budget with additional private funds if you choose.

Once a week

This plan will allocate funds for direct services, products such as personal alarms as well as costs for additional services such as service coordination, administration and management.

Everyday

Together we will put together a Home Care Agreement and Care Plan in line with your budget.

Each month we will provide you with a statement which will list your delivered services. It will also display the balance of funds available in your budget that will be carried forward into the following month.

Understanding your Statement Your monthly statement outlines • what you have spent • the balance of funds available

Government Subsidy: The funds provided by the government for your home care package.

Client Contribution: The government expects Home Care Package recipients to make a contribution towards the cost of their services.

Balance of Funds: The balance of available funds that will be carried forward into the following month. 6

Income Tested Fee:

Contingency: A portion of your funds separated out from your annual spend and retained for unexpected events or emergencies.

The Department of Human Services works out the income-tested care fee based on your financial information. (This applies to clients whose Home Care Package started after July 1, 2014)


Home Care Package Fees Personalised Care Coordination and Support We coordinate and organise services to suit your ongoing support plan. This includes booking appointments, rostering your support staff, rescheduling visits, hiring equipment, managing personal preferences and 24 hour customer service support.

Reporting and Quality Assurance

Your Home Care Package funds consist of services and fees that include:

We have a legal responsibility to make sure your services are always of a high quality and meet government standards. To do this we must have up to date quality systems and processes. These include your monthly statements, reports to government, regular audits, training for staff, health and safety policies and police checks.

Assessment and Management

Administration Fees

Together we consider your needs, personal goals, lifestyle and budget. We design a detailed plan of support so you can live in your own home and stay active in your community.

Administration associated with service delivery including insurances, office costs, payments to suppliers, invoicing, IT, payment and management of workers.

We provide support to navigate the health and community care system by organising referrals, expert advice and advocating on your behalf. We monitor your progress and adjust your plan as your needs change.

Self Management Calvary Community Care can manage your package and assist with referrals, advocacy and expert advice. Alternatively you can choose to self manage. It is your choice.

There are a range of clinical services that Calvary Community Care may be able to assist you with. Our aim is to work with you and the other health professionals involved in your care, to assist you to obtain optimal health and be as independent as possible at home. We will assist you in identifying ways in which you can improve your health.

You can also use your package to make your home more accessible and easier to get around. Rails and ramps can be installed both inside and out the house. Personal alarms, fall detectors and motion sensors can also improve your safety. They can be worn around the house or wall mounted in bathrooms or kitchens. Electronically linked to a 24 hour monitoring services these telecare products offer peace of mind and ensure help is just a push button away. Calvary can also help source other independent living aids that make living at home easier.
