Where you belong, is where we want to help you stay. This booklet is designed to help you identify your personal choices for care. It’s the starting point for helping you through the next steps of government assessment for a Home Care Package. We are happy to sit with you to develop an understanding of your unique day-to-day needs, what makes you happy, and what can assist you to remain supported and independent. You can then use the following easy checklist as a reference for your Home Care Package moving forward. Best of all, your plan can be as flexible as you need it to be, so you can change it at any time if you need a little more support, or if you’d like to engage in a different type of service or activity.

1. Your Lifestyle With each year that passes, some daily tasks or activities can become a little more challenging. But that’s okay; you may just need some additional support. We are committed to helping you maintain your independence and freedom at home so that you remain close to family and friends, and the wider community. Below are some suggestions on services that we can help you with that will keep you independent and happy at home.

2. Your Wellbeing Managing your emotional, intellectual, mental and physical health are paramount to your wellbeing. Maintaining your health, recovering from an injury, or managing an illness can require some special services. Below are services that may make life a little easier.

3. Your Relationships Your independence and relationships play a vital role in your happiness and connection to your past and future. Some of the following services can help engage personal and social relationships you hold dear and foster new ones as well.

Culturally Appropriate Support Workers • A culturally sensitive support worker where available who can communicate in your native language • Engaging interpreters for phone calls and appointments

Companionship for Meals or Travel • A friendly support worker to chat to, enjoy a meal with, or an outing • A travel companion for support while away on holiday

Technology & Communication Assistance • Facebook or Skype assistance to contact loved ones who live interstate or overseas • Assistance with email or internet searching • Help with correspondence, letter writing and typing

Family & Carer Communication • Liaison and support for your family • We can act as an advocate for your needs • Care and mediation support

4. Your Home We all need to feel safe in our home. As things change you may wish to add some security and modify a few home items so life is a little easier. We can help with the following services that can give you added confidence in security around your home.

Home Modifications • Modifications so you can get around easier e.g. installing ramps, grab rails, widening doorways or installing aids in the bathroom • Home assistance with a multitude of aids

Personal Alarms & Monitoring • Personal alarms and fall detectors attached to 24 hour support services

Mobility Aids & Care Products • Walking frames, crutches, wheelchairs, adjustable beds, lifting devices, pressure relief products and absorbency aids • Access to reduced pricing on all mobility aid purchases

Key Box & Emergency Planning • Family and friend emergency contact planning • Security key box planning

Daily Phone Calls • A daily phone call and chat to check everything is okay

5. Your Fun Being around your community and activities that connect you with family, friends and interests is fulfilling and helps the body and mind. It’s also a great way to make new friends. We can help you with the following pursuits so you can stay connected.

Events and Outings • Participation in group or individual outings, e.g. festivals, garden walks, museums, fishing, dancing and sport

Exercise Programs • Engagement with fun health activities like water aerobics, dancing, gym and weights programs • Arranging personalised strength building and mobility sessions

Interest Groups • Participation in special interests like bridge groups, bingo, woodwork, quilting, book club, chess • Being part of a volunteer program or clubs such as Probus

Planning Special Days with Grandchildren • Planning activities for grandchildren e.g. Easter egg hunt, cooking, painting, reading, games • Planning and assistance with gift buying

Build a Music Collection • Assistance on the web to build a music collection that has all you favourite tunes

6. Your Spirituality Freedom to express your religious beliefs and spirituality lie at the core of our mission to all people. We encourage diversity and freedom in all forms of religion and are happy to support and nurture the expression of your beliefs.
