Who can help? People ďŹ nding it hard to manage on their own, and their families, need to know where they can go for advice and help on the many community care services that are available locally. This Directory is an important source of that information. There are many appropriate services provided by dierent organisations, including councils, voluntary organisations, private care agencies and

the NHS. In this Directory, which covers a large area, three councils are responsible for Adult Social Care (ASC) services: • Dorset County Council (Adult and Community Services), • Borough of Poole (Adult Social Care), and • Bournemouth Borough Council (Adult Social Care).

Where do I start? Adult Social Care (ASC) assesses need and provides services to the people of Dorset, Poole and Bournemouth, 24 hours a day, 365 days a year. ASC supports adults of working age, older people who have physical disabilities, learning disabilities, mental health problems, a sensory loss or general frailty. The aim is to actively promote independence and choice and help people to be self-reliant and live in their own homes for as long as possible. When this is no longer possible they help people to choose good quality supported housing or care homes.

If you need any form of advice about help, to remain independent or your care options, you should phone your relevant Adult Social Care department to request an assessment. Contact details can be found below. Anyone can ask for an assessment, regardless of their ďŹ nancial situation. The assessment and advice is free. You may be asked to make a ďŹ nancial contribution towards services arranged for you, which could be the full cost of the service for people who can aord to pay.

Help and advice ASC will always provide support and advice, even if they cannot pay for the help you need. They will do their best to help get you back to living independently. One in ten people in the UK care for friends or family members. ASC supports unpaid carers in various ways, for example by providing training, links to support groups and assisting carers to get a break from their caring responsibilities. You can use Source Directory to ďŹ nd out about the choices available to help you keep healthy and active and to learn about care and support options. The Source Directory website is complemented by telephone help lines and face to face interaction. The directory is a partnership between Bournemouth Borough Council, Borough of Poole and Dorset County Council working with a cross section of adults with care and support needs and their carers. Your assessment Adult Social Care works with you to access your needs The assessment is based on your circumstances and will be dierent for each person. A social worker/care manager will look at ways to best meet your agreed outcomes and aspirations. The best way of meeting your goals will then be discussed with you, sometimes with an occupational therapist s help. If you already have a relative or friend who is a carer, they can also be assessed if they wish. Once your needs have been assessed and your desired outcomes established, Adult Social Care will decide if you are eligible for help. To do this, they will use a framework set by the Department of Health. Your risks to independence and needs will be assessed in four eligibility bands ranging from low to critical . A ďŹ nancial assessment will also take place, to determine how much you can aord to contribute towards the cost of your care. Before they can arrange care services for you, Adult Social Care needs to assess your needs. Social workers, housing sta and health professionals may work with you and your carer to identify what your needs are and how they might best be met. Am I eligible for an assessment? To be eligible for an assessment, you may have one or more of the following: • sight or hearing diďŹƒculties, • mental health needs, • frailty because of old age, • ongoing care needs when you leave hospital, • learning disabilities, • permanent, substantial physical disabilities, • a long term illness, or • problems related to substance misuse. Alternatively, you may provide regular unpaid care for someone and subsequently be eligible for a carer s assessment.

Who can refer you for an assessment? You can be referred by: • yourself or a relative, friend or neighbour, • a social worker or health professional such as a doctor or health visitor, or • any other agency. If you wish to apply for this service or ask for more information, telephone your local Adult Social Care oďŹƒce (contact details are on page 4). They will arrange for an interpreter if you do not speak English or if you use sign language. What does the process involve? The assessment of your needs will involve you completing the supported assessment questionnaire and your worker talking with you, your carer (if you wish) and other people who know you well, about dierent aspects of your life. These discussions may include your physical health, your emotional health, how you manage to look after yourself, what outcomes you are looking for from the care and support provided and your current family and support networks. Services may be oered to meet needs that pose a critical or substantial risk to your independence, in line with the Government s Fair Access to Care Services guidelines. Methods of best meeting your needs and achieving your outcomes will be discussed and agreed with you. Do I have a say in what services I get? After your needs have been assessed, Adult Social Care will advise you about services you may be eligible for and/or a Personal Budget to help you meet your care needs. They will complete a support plan for you and discuss with you: • the type of help you think you need to meet your outcomes, • when you think it is needed, • who will be providing the assistance, • how long they will be with you, • how it will be paid for, • your religious and cultural needs, and • when the service will be reviewed. Support plans are reviewed regularly and will include all references to any Personal Budget you may be eligible for, to help pay for your care needs.

Personalising Adult Social Care In the traditional system of social care, professionals made decisions about the support people could have and who would provide it. As a result, people often had little or no choice on what support they received. The central government directive Putting

People First has changed the way communities, organisations and individuals work to support people. It recognises that people want better quality services that are personal to them, and more control over decisions that affect them.

Self-Directed Support Part of the Putting People First directive, SelfDirected Support seeks to put people in control of the support they need to live the life they choose. It recognises that some people can manage their support on their own whilst others need help ‒ from family or friends or people who are paid to help. Self-Directed Support is for adults who are eligible for long term social care support (see page 6). It allows people to have more choice and control over

their lives because it lets them make decisions about the support that they need. It asks how they want to live their life, what they want to achieve and the type of support they need to do this. If you are eligible, Self-Directed Support allows you to have a Personal Budget. You can choose what social care and support services this money is spent on. You can be as creative as you want, as long as the money is spent to meet your needs.

Personal Budgets A Personal Budget is money given to adults of all ages who are recognised as needing long term support from Adult Social Care after an assessment of their situation. There s also a financial assessment to work out how much someone needs to pay toward their Personal Budget. Adult Social Care also check people are getting the benefits they are entitled to. A Personal Budget, for example, can be used to pay for a personal assistant if this is needed to support someone. The amount should be enough to meet the person s assessed needs. The assessment will look at things like how much help is needed to carry out practical tasks for daily living.

This money can be managed by the individual or Adult Social Care and is not classed as income ‒ it s paid to meet someone s needs, and having a Personal Budget will not affect their benefits. A Personal Budget can also be managed by someone the service user nominates to act on their behalf or they can ask the council to look after their budget and use it to buy and arrange support. People can also choose to use a mixture of these methods. Direct Payments explained

Direct Payments Direct Payments are financial payments made to individuals, who have been assessed as eligible to receive support from Adult Social Care, instead of councils arranging or providing services directly. You can then use this money to purchase support that you consider most appropriate to meet your assessed social care needs. Direct Payments allow you to make more of the decisions that affect your life. They give you more

flexibility and choice, by enabling you to buy support for yourself that better suits your individual needs. If you receive a Direct Payment, you can decide how your support needs will be met, by whom and at what time. You are in control. You can employ your own support staff, which means they will report directly to you. Or, you can have a contract with a local agency, in which case the agency will be accountable to you, not to >>

Direct Payments continued >> Adult Social Care. If you are a carer you may also be eligible to receive Direct Payments to help you in your caring role or to have some time for yourself. A Direct Payment can be used for all or part of your care or support package. Having a Direct Payment as an element of your support can be a useful and relatively safe way for you to see if Direct Payments are the right option for you.

Most Direct Payments are made to meet regular ongoing support needs however they can also be made as a single payment, for example, to purchase equipment or a short respite break. If you prefer, Adult Social Care can arrange services to meet some of your needs directly, they can oer you a combination of a Direct Payment and services that they provide or buy from another provider on your behalf.

Who can have Direct Payments? Adult Social Care can make a Direct Payment to most people who need support, including: • older people who have been assessed as needing community care services, • disabled people (including people with mental health needs) aged 16 and over, and those with short as well as longer term needs, • carers, and • parents of disabled children, for children s services.

What do I do next? If you are assessed as being eligible to receive support and you want to consider having a Direct Payment, you must be: • over 16, • able to legally consent to receiving the payment from Adult Social Care, and • able to manage the money to meet your needs. This may be on your own or with someone s help, as long as you are in control of how the money is spent. What can Direct Payments be used for? The money is for you to arrange support to meet your eligible needs. You may choose to employ people directly, to buy services from an independent or voluntary sector provider or purchase equipment to help you live more independently. Direct Payments enable you to choose and plan your support in more exible ways that ďŹ t in with your life. It is important that your plans meet your assessed needs in the best way for you. As long as the money is used lawfully, and your eligible needs are met, there can be a lot of exibility about how you spend the money. For this reason there is not a list of things the money can be used for.

Leaving hospital and staying independent Adult Social Care and the NHS work very closely together to ensure that hospital patients are discharged safely and are able to return to home life as quickly as possible. Social care staff are available to help with information and advice when you are ready to leave hospital. No matter which hospital you are in, you can request an assessment of your needs to help you stay independent and confident when you return home. The assessment involves asking you about your needs and what is required to help you live safely and independently at home. There will also be an explanation of any charges that may be made. A relative, carer, or a member of the hospital staff may also approach the hospital social care team and ask them to carry out an assessment, but this will not go ahead unless you agree. If you already have a social care service in place Adult Social Care will make an assessment of any other needs you may have after your stay in hospital. Carers should be offered an assessment and may also need advice and support and the hospital social care team is available to discuss any issues with them and also give them details of carers support groups. Adult Social Care can also introduce you to local voluntary organisations who can help, short-term, with a variety of services such as laundry and domestic cleaning, or sitting services. Help and Care offer a Hospital Discharge Support Service which is aimed at people who need to make their own arrangements for some extra help after leaving hospital. This service helps people who are 65 who: • Adult Social Care agree need some support, but who have been told they need to pay for some or all of the cost of any services required, and where Adult Social Care are not involved in helping to arrange the services needed when a person leaves hospital; • Require support but do not want help or a financial assessment from Adult Social Care; or • Need some support but whose needs are not substantial enough for Adult Social Care support, but who would like some help to purchase services.

Help and Care can help by visiting people in hospital and talking through what support people need in order to leave hospital. They will provide useful information about the services available together with the costs, to help people make decisions about what they need. They can also help to arrange and contact people who provide services on a person s behalf so that everything is in place when a person leaves hospital. Help and Care will also contact people once they have returned home to ensure that the person is happy with the services and support they have in place. Bournemouth residents who are being discharged from Royal Bournemouth or Christchurch hospitals following a stroke are supported by the Stroke Coordination Team. They offer practical support to help people return to living as independently as possible. This could involve signposting to services for people who need ongoing care, helping people to join clubs and groups or assistance with finding work or volunteering. You can contact them on 01202 705440 or email: stroketeam@bournemouth.gov.uk Poole residents being discharged from Poole Hospital following a stroke are also supported by a Stroke Coordination service. This service provides information, support and signposting to ensure that people have the information they need to make choices which enable them to live as independently as possible after a stroke. The Stroke Coordination service can also refer clients to the care management team and psychological support services if additional care support is required, based on identified need. Dorset residents being discharged from Dorset County Hospital (Dorchester), Royal Bournemouth, Christchurch or Poole hospitals are also supported through a Stroke Coordination service. This service provides advice and information, support and signposting to help people to become as independent as possible.

Patient Advice and Liaison Service (PALS) The free and conďŹ dential Patient Advice and Liaison Service (PALS) aims to guide patients, their relatives, friends and carers through NHS services. PALS oďŹƒcers provide advice, support and information. They help to resolve concerns as they occur, tell you about the complaints procedure or provide you with information about the services available to the public. Rehabilitation and reablement Reablement is often described as helping people to do for themselves rather than doing it to or for them . It is short term, intensive support that enables people to optimise their independence. Dorset County Council s reablement service provides practical and emotional assistance to people who are at risk of not being able to remain independently in their own home because of frailty, disability or illness. The service is provided by our Adult and Community Services ‒ Community Support Service. The aim of re-ablement is to maximise independence, choice and quality of life, and reduce the need for support in the future. The service works with people, to enable them to do things for themselves, rather than having things done for them.

In Poole there is a Reablement service, which is a short term service designed to help people who have had a crisis (like a fall or spell in hospital) to regain the conďŹ dence and skills to live independently so they can help themselves and continue to stay in their own home. If you live in Bournemouth, are aged 65 or over, and qualify for adult social care, reablement may be one of the services you are oered. Reablement can support you to become independent after a fall or illness, or if you get to the stage when you feel you cannot manage without support. Sta work with you for a short time to regain skills and conďŹ dence to achieve manageable goals. They oer advice and information, make sure you have the right equipment, suggest daily living aids that might help you, and let you know about other sources of support.

Intermediate care This is usually a short-term service. The aim of the intermediate care service is to maintain or increase your independence through rehabilitation and reablement and allow you to live in your own home for as long as possible. You will be supported with the provision of short-term care support and/or therapeutic intervention for example physiotherapy or occupational therapy. The assessment is carried out by multidisciplinary teams of health and social care staff. The intermediate care service will promote your health and well-being and can help to prevent falls at home. The service also aims to reduce admission to hospital by providing a rapid response to you at

home. In addition, the service can enable you to be discharged home from hospital earlier by providing care support and intensive rehabilitation in your own home. The service helps prevent unnecessary or premature admission to long-term residential and nursing care. The service may last up to six weeks and during this assessment period social care services are free of charge. When this is completed a care plan for your future care needs will be discussed and you will be given details on how you can receive help to set up on going care arrangements. Ask your care manager or health worker for further details.

Continuing care Continuing care is the name given to the care needed by an adult who requires help over an extended period to manage their daily life because of illness, disability, accidents or the effects of getting older. It is a package of care that involves services and funding from both the NHS and Adult Social Care. Continuing NHS healthcare is provided solely by the NHS if you have extensive healthcare needs. This is free of charge, wherever it is delivered and can be provided in any setting. In your own home the NHS funds all care required to meet your assessed needs. In a care home, the NHS makes a contract with the home and pays the full fees for

your accommodation as well as all your care. NHS ‒ funded Nursing Care is the contribution paid to you by the NHS for care in a registered care home with nursing. The current amount is £108.70 per week. You can ask for an assessment by health and social care professionals who will visit you to identify and prioritise your needs. The assessment will be used to decide what services will be provided and where, and who funds them. If your needs change, regular reviews will be carried out and your care and/or funding arrangements will change to meet them.

Occupational therapy Occupational therapists can help you lead a more active and independent life using a variety of methods, including the use of adaptive equipment. They can assess your home for hazards and point out things that might contribute to falls. They also advise carers on techniques for moving and handling the person they care for. Adult Social Care may be able to help with their team of specialists. Visiting you at home, an occupational therapist will be able to suggest adaptations that will improve your mobility. Perhaps handrails up stairs would be useful? Would a ramp up to your front door help if you are in a

wheelchair? Major alterations such as fitting a downstairs bathroom or widening doorways can be paid for with a Disabled Facilities Grant (DFG). An occupational therapist will talk you through any new equipment you may be given, as well as help you apply for funding for major adaptations, if you need them. Occupational therapy continued Dorset The local authorities, NHS Dorset and hospitals in Dorset have collaborated to provide an integrated supply service, with Adult Social Care currently supplying 9,000 items of home equipment and adaptations to people with disabilities each year. A new prescription-based system is being trialled, aimed at providing greater choice and more buying power to the end user. Contact the Adult Access Team for details of how they may be able to help. Bournemouth and Poole In Bournemouth you can access a wide range of aids to daily living to meet your assessed needs through the Equip for Living Service. This is run in partnership with Borough of Poole, NHS Bournemouth and Poole and Nottingham Rehab Supplies (NRS). Telecare and equipment Assistive technology and Community Alarms Assistive technology or telecare services help vulnerable people, particularly older people, to live in their own homes with greater safety and independence. They use a range of technologies that raise alarms to get help for someone 24 hours a day and so provide help, security and peace of mind. Special equipment can be linked to your telephone system so you can get help quickly if you need it, even if you are unable to speak. For some of the services you do not even have to press a button to call for help as they work on sensors that detect movements like falls, gas or carbon monoxide escapes, or oods. There are dierent types of telecare service depending on your level of need. Services can be provided if you have been assessed as eligible for help by Adult Social Care, or equipment can be purchased privately. Dorset Those assessed as eligible for Telecare equipment in Dorset will be provided with the equipment free of charge. Equipment provided can range from a Lifeline unit and pendant only, through to a Lifeline unit and a whole range of additional sensors. Adult Social Care Adult Social Care is the name given to services which aim to help people live at home independently with a care support package. Services might include: • Reablement ‒ help to build up strength, re-learn skills and get ready to cope with day-to-day living at home after an illness, accident or stay in hospital. See page 10. • Personal ‒ a exible way of providing social care services which enables people eligible for social care to have money to choose and buy services, that suit their lifestyle ‒ see below chapter for further information. • Direct Payments - this scheme allows you to buy in the care you need for daily independent living ‒ more information is on page 7. • Community meals - usually supplied frozen to reheat yourself. • Day services - available in care homes or community centres. For more information, see page 27. • Housing adaptations - Adult Social Care can provide information about equipment and adaptations to make it easier for you to continue to live in your own home ‒ see page 14 for more details.

• Home care - help with personal care such as washing and dressing. See below for more information. • Supported housing or extra supported housing provided by your district or borough council. See page 29. • Respite care - residential care or care provided in your own home to give you or your carer a break. Further details are on page 27. To decide what sort of help you need, and what kind of outcomes you are looking for, Adult Social Care will work with you to complete a supported assessment questionnaire. They will then complete and talk through your support plan with you. This sets out how you can meet your needs and what help can be provided to help you manage at home. It is likely that Adult Social Care will ask you to make a ďŹ nancial contribution towards the services you receive, based on your ability to pay. You should be given clear information about what you can expect to pay. Support plans may change if your needs change, leading to another assessment, which is often called a review .

Independent home care providers In order to enable people to live in their own home for as long as possible, a range of home care services are available. A range of help is on oer from day-to-day tasks such as cleaning, shopping and food preparation/cooking through to personal care that helps with tasks like dressing, bathing, toileting and prompting the taking of medication. Carers need to be properly trained, particularly in moving and handling and the use of hoists for some tasks. Before you make any decisions regarding the provision of care, be sure to contact several providers and ask for a Service User s Guide and their charges and a draft contract between you and the care provider. Daily care/ domestic support Carers can call in on a daily basis to assist with any of the tasks described above. Depending on the level of help required their visits can be just half an hour or up to several hours. Generally, visits are

available from 7am until 10pm. Some people will need multiple visits per day. Night services can also be provided. The hourly rate for these types of services typically range from ÂŁ9 to ÂŁ15 per hour depending on the services required, the time of day and the location. Rural areas may present particular diďŹƒculties and a travel charge will probably be made in addition to the normal hourly rate. Live-in care 24 hour live-in care can accommodate people with a very high dependency on a permanent basis. It can also provide respite breaks for regular carers and short-term support following hospital discharge. In some cases it s preferable and more economic to have a carer actually living in the home. This can be for a short period e.g. a week, or on an ongoing basis. Typical charges for this service range from ÂŁ520 to ÂŁ750 per week depending >>

Independent home care providers continued >> on the amount of care and the particular skills required. Live-in care is also available to people with permanent physical or mental disabilities who require long-term ongoing care. All home care providers are regulated and

inspected by the Care Quality Commission who publish inspection reports on their website www. cqc.org.uk. When considering a service it s always a good idea to check their report.

Home care agency checklist © 2013 Care Choices Ltd These questions may be useful when considering using the services of a home care (domiciliary) provider to help you build up a picture of how your care needs will be met in your own home. People arranging and funding their own care either privately or through a Direct Payment or their Personal Budget are able to choose their own worker or care agency. Lists of local home care providers begin on page 21.

Carers • You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding? • Carers should be fully trained or be in ongoing training. Ask the agency about their policies on this.

Agencies • What experience does the agency have in your particular field of need? Can they supply references for you to check? • Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate. • How long has the agency been operating? • How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending?

• You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency. • Carers must be checked with the Criminal Records Bureau (CRB) and have a criminal records disclosure ‒ make sure this is the case. Paying • If your care needs do not meet Adult Social Care s eligibility criteria and have been arranged privately you should check carefully the fee rates charged and exactly what the payment you make covers.

• How can you contact the agency in an emergency or outside office hours? • How hard or easy would it be to make a complaint and how are things then put right? • If this is a private contract ask for a copy of the agency s contract terms and read these carefully with someone you trust. Further local help Handy Van Repair Service East Boro Housing Trust s Handy Van Repair Service provides a friendly, professional repair service, providing practical, useful help around the home. If you are aged 60 or over, own or rent your home in Bournemouth or Poole and receive benefit support, eg pension credit, attendance allowance you are eligible for the service.

Contact the service on 01202 883503 and a member of staff will be happy to help. There is no charge for labour, no membership fees and no hidden costs. All you pay for is the cost of materials needed for your repair as the Handy Van Repair Service is funded by the Borough of Poole and Bournemouth Borough Councils.

Day centres Day services are provided in a variety of locations, ranging from residential and nursing homes to community and specialist centres. The services aim to help people live as independently as possible. If you have a learning or physical disability or if you are an older person, Adult Social Care will help to support you in finding activities which will help

promote independence. To be eligible for day services Adult Social Care will have to assess your needs. This means they will have a discussion with you to agree with you the best way of organising support. They will consider your needs and decide with you what services might be suitable for you.

Respite care and short stays Carers sometimes need a break and this can be provided in several ways: you may wish to go into a care home yourself for a week or two; or arrange home-based respite care where another carer moves into your home; or family-based respite care where you move into another family s home for a period. The best option for you will depend on your needs, the urgency of the situation and personal

choice and cost. If you are thinking about long-term care, but have had no experience of living in a care home, you may wish to book in for a short stay to see what it s like. This will help you make an informed decision. If you have nursing needs, ask for a nursing determination as a contribution to your fees ‒ see Care homes with nursing on page 37.

Supporting carers If you look after, help or support a person who is ill, frail or disabled who cannot manage without support and do not receive pay to do this then you are a carer. The person might be a child with a disability, an adult member of your family, or a friend. You do not need to be living with the person. You do not have to be giving physical care. The care you provide might be: • help with washing and dressing, • feeding or cooking, • getting about outside, or • emotional support and company. Whilst looking after another person is mostly very satisfying, it can also leave you isolated, tired, resentful or angry. Caring can affect your personal

life, your health, leisure time and finances. As a carer you need to think about things like: • the type of help that you need to continue caring, • whether you are willing or able to carry on caring, • planning for the future, and • how to meet your own needs. Dorset A sitting service for carers is available from Dorset County Council. In order to qualify for this service, carers will need to meet set criteria and will then be registered. Those approved to receive the service will then be referred to Help and Care, a local independent charity, who have been commissioned to provide this service on behalf of Dorset County Council. Supporting carers continued should contact the Adult Social Care s Access Team (see page 4 for contact details). A carer s caseworker or care manager will contact you to complete a carer s assessment. The sitting service is provided through approved care agencies and there is a maximum hourly allowance and a small hourly administration charge payable for this service (full details will be provided during the assessment process). Poole and Bournemouth The Carers Information Service for people caring for someone living in Bournemouth and Poole, keeps families and friends informed of helpful news and events. Regular events enable carers to meet up and enjoy an activity, some of these with

the person they support. Carers will receive a free welcome pack when they join, copies of the local Caring Matters magazine, regular newsletters and a friendly team to call when they need help. There is a short break sitting service in both Poole and Bournemouth that runs in a similar way to the service in Dorset and is available to unpaid carers of cared for people who are over 18. It is a exible service that provides a care worker from a local approved agency to stay with the cared for person. The service in both areas is subsidised by the local authorities, but they are all run slightly dierently.

Home from Home day respite This is a new service for carers of older people in partnership with East Boro Housing Trust, Bournemouth Borough Council and Borough of Poole. It provides an alternative to day centres. Home from Home oers a day sitting / respite service in the Home from Home host s own home. Each host undertakes training, has an enhanced CRB police check and their home has been inspected to make sure it reaches appropriate health and safety standards. As well as needing a break, carers worry about what would happen to the person they support if the carer is unwell, or has an accident. Carers in Crisis is an emergency back-up scheme, available to all carers of adults over 18, that has been set up to give carers peace of mind by alerting people to keep the person being cared for safe. To ďŹ nd out about the support for carers, and to join the Carers in Crisis scheme or Carers

Help for carers in Dorset Dorset County Council carer s caseworkers provide assistance to family carers to support them in their caring role. Carers may require help in ďŹ nding out information on how to ďŹ nd local support groups and carers caseworkers can assist them with this. Carer s caseworkers may be able to support family carers to access local services which may give them a much needed break. The carer s caseworker will carry out an assessment with carers who provide a substantial level of care. This may lead to a support plan for the carer. Carers Direct A one-stop information and advice service for carers who look after ill, frail or disabled friends and relatives has been launched by the Government. Housing with care Sheltered (or retirement) housing This is usually provided by councils and housing associations across the region. Sheltered housing is a practical step if you would like more security and companionship but don t need to move into a care home. In a sheltered (or retirement) housing scheme you would live independently in a flat or bungalow built especially for older people within a larger complex, sometimes around a garden or communal facility and linked to a central control centre for security. Some schemes have access to a resident or non-resident manager who provides support, advice and a link to services for older people. Dorset East Dorset Housing Association has a range of housing specially designed to meet the needs of older people consisting of flats, a communal lounge, a laundry and gardens. Schemes are especially suitable for very frail older people with flats and bungalows built

with wheelchair access and level deck showers. All schemes have a visiting sheltered housing officer or scheme manager. There are schemes in Bournemouth, Wimborne, Ferndown, Corfe Mullen, West Moors and Verwood. Poole Poole Housing Partnership has a dedicated team of housing officers, who provide support to residents at the signing-up stage and throughout the duration of their tenancy. They spend a lot of time having direct contact with residents, visiting them at home and dealing with enquiries. You can contact the housing officers by calling 01202 264444. Bournemouth Adult Social Care can help you decide if sheltered housing can meet your needs and liaises with Bournemouth Borough Council s Housing Services which manages sheltered housing schemes.

Extra Care housing Extra Care housing is a form of housing, similar to sheltered schemes, with self-contained accommodation together with some communal facilities. In an Extra Care scheme, instead of low level support traditionally provided by wardens in sheltered schemes, higher levels of care and support are provided on site for people with assessed needs by a designated team of care workers, who can be available 24-hours a day. Extra Care allows people to retain their own tenancy and have care services delivered ensuring that they are able to remain safely in their own selfcontained accommodation. Dorset A new Extra Care housing scheme has been developed in Christchurch that will help older and disabled people to live independently. The scheme at Bure House, has been devised by East Dorset Housing Association in partnership with Christchurch Borough and Dorset County councils. There are 41 one- and two-bedroom flats as

well as three specially adapted flats for disabled people available to rent. The scheme is designed to allow people who need physical help to live independently while having 24-hour care support. Every resident has their own self-contained flat with their own front door, care and support when needed and the means to install safety devices - for example, to detect if a person has a fall. All flats have been designed to wheelchair standards and include fitted kitchens, walk in showers, central heating, double glazing and smoke alarms. Anyone interested in living at Bure House should contact Christchurch Borough Council on 01202 495000 to see if they are eligible. If so, they will be invited to make an application. Part of the application process will include an assessment of their needs. Poole Most of the schemes available to rent in Poole are provided by Borough of Poole or local Housing Associations. Extra Care housing continued usually through the Council s Housing Register. You will need to speak to Housing and Community Services on 01202 633805 to establish whether you are eligible for consideration for this type of housing. People are expected to show a degree of housing need, this can be due to unsuitability of their present home for medical or social reasons. In addition, Adult Social Care will need to consider your ability to secure alternative accommodation by taking into account your savings and equity, if you are a homeowner. Some housing associations will take an application from you directly and may help you even when Adult Social Care may not. A list of local housing associations is available from Housing and Community Services. Properties are also available to buy. The price and types of property available to buy can vary considerably. In early 2011 a new Extra Care housing facility opened in Poole - Delphis Court has been developed as part of the new Poole Quarter developments and provides a purpose built facility

for people who need a higher level of care and support and want to maintain their independence in their own home. Delphis Court oers 45 ats in a mixture of one and two bedrooms, every resident will have their own self-contained at with their own front door. Support is available on site 24 hours a day and where appropriate, safety devices can be installed within ats to support people to live independently. Anyone interested in living at Delphis Court should contact Housing and Community Services within the Borough of Poole on 01202 633805. Bournemouth Self contained apartments with extra care and support on site are located in several developments in the town. High standards of accommodation complement communal facilities where you can relax and meet other residents. Specialist care sta are available to provide services to meet assessed needs. The latest technology is also available to ensure your safety and security. Bournemouth Borough Council works in partnership with housing associations, the NHS, local doctors and pharmacies to support your wellbeing. Housing and support for younger adults There is a range of care homes catering for younger adults with learning diďŹƒculties, physical or sensory impairments or mental health issues. There is also a range of housing options you could explore. If you are thinking about a care home placement, seek advice from social and health care professionals about your needs. Does the home provide opportunities to learn and maintain skills, and opportunities to move on into living in the community? What are the home s links with specialist sta in the community services Are you happy living at home? Would you like a little more independence with help when you need it? What are your housing options? You could: • Continue living with your family ‒ If your home is specially adapted and you receive the practical and emotional support needed, you may prefer to stay at home. It s also convenient if your college or job is nearby.

• Move into supported living accommodation ‒ This oers a level of independence and the chance to meet other people in similar situations. Help from a visiting care assistant is available and usually nursing care is provided nearby. • Rent a place ‒ You can apply for council housing which is usually cheaper than renting privately. There could be a waiting list and you must fully explain your housing needs. Local housing associations may also be able to help. Ask your council housing department for a list. Housing and support for younger adults continued • Move into a care home ‒ If you want to leave home but are not ready to live alone, care homes can meet your personal needs and offer appropriate support. Adult Social Care and Health will contribute if you are eligible and if they agree the care home meets your needs. If your chosen home costs more than usual, you may get some help but you must pay the rest. When living independently you may require some adaptations around the house. You can choose housing that s specially adapted. There are organisations that specialise in providing this sort of accommodation including Livability (formerly

John Grooms) and the Accessible Housing Register. New homes that have been built since 1999 should be fully accessible to disabled people. However, if you choose an older home, there are a number of adaptations that could make it easier for you to use. These include wider doorways; a downstairs toilet; bathroom equipment including handrails, a walk-in bath, electronic bath seat; kitchen adaptations including lower working surfaces; lower level light switches; easy-to-use controls for heating; a stairlift; security intercom at the front door and ramp access to the garden. You should also think about the general access to your home.

Specialist care services Physical disability The support you require if you have a physical disability must be tailored to your specific needs and can be provided by the independent and not for profit sectors as well as by Adult Social Care. Help at home with personal care is available according to need, during the day and night, 7 days a week. There are also private agencies offering help at home with personal care and housework. Services from Adult Social Care include: occupational and paediatric occupational therapists; help with adapting your home; the Handyperson Scheme ; activities within the community; help at home, with meals and leaving hospital; respite care; supported housing, and help for carers. In Dorset, the Handy Person Scheme will be provided under new contracts which are integrated with Home Improvement and Housing Options for Older People (HOOPS) services. West Dorset District Council Care and Repair will provide a Home Improvement Agency /

Handyperson / HOOPS service for the West Dorset District Council area. Telephone: 01305 252405 for further information. Learning disability Learning disability services are run by a partnership between Adult Social Care and the NHS. Community learning disability teams are the ďŹ rst point of contact for advice, information and guidance on services for people with a learning disability. You can contact them through your local Adult Social Care oďŹƒce, listed on page 4. You can also ask your local oďŹƒce for copies of leaets or factsheets which tell you more about day care services, care home provision and services for carers available locally. Adult Social Care provides care where necessary and gives support to enable and encourage service users to reach their full potential in all areas of their lives. Access to services is via an assessment, explained further on page 6. An important change aecting people with

learning disabilities is the introduction of Personal Budgets (explained on page 7), putting people at the centre of how they choose to pay for their support. This may be in their own at or house, a supported living scheme or a care home or care home with nursing which specialises in this type of care. Valuing People Now is a cross government strategy for people with learning disabilities. It states that all people with a learning disability are people ďŹ rst with the right to lead their lives like any other. The website www.valuingpeoplenow. dh.gov.uk is for people with learning disabilities, family carers and anyone who works with people with learning disabilities. It can help with many subjects, including health, housing, employment and relationships.

Mental health Your local Community Mental Health Team (CMHT) should be your ďŹ rst point of contact if you have mental health issues. You can ask your GP to refer you or ask someone who knows you to contact the CMHT on your behalf. If you have mental health needs, residential care can help improve the quality of your life - the structured environment and activities provided can build up your conďŹ dence. The care providers should assess you carefully to make sure the placement is

right for you. A qualiďŹ ed member of the residential care sta may visit you at home, talk to you and look at your assessments of health/social care need. To ďŹ nd out more about mental health care in your region, organise visits to prospective care homes. Ask the residents about the care they receive. Are psychiatric nurses available if needed? Do the activities provided assist rehabilitation? What are the attitudes to medication and other therapies?

Dementia care Usually, the initial point of contact to access care services is either your GP (for health care solutions) or your local Adult Social Care oďŹƒce (for most other services). Your GP, as well as discussing your concerns, will be able to carry out a series of tests which will establish the nature of your condition and advise whether treatment is possible. Some forms of dementia are not permanent and can be caused by a range of emotional, physiological and physical ailments, which bring on dementia-like symptoms. It is important that any diagnosis eliminates the possibility of a reversible dementia before a medical prognosis is made. Following a diagnosis of dementia, your GP may refer you to a specialist like a consultant in old age psychiatry, a community psychiatric nurse or an

occupational therapist with a special interest in dementia care.

Dementia care continued Support in the community As it can be best for the person with dementia to stay in their own home for as long as possible, a range of adult social care services is available to facilitate this.For example, they may provide access to: home care (discussed on page 18); meals in the home; sitting services (which provide a break for any carer); advice on equipment and adaptations; day care centres and respite care. These services can be provided directly by registered providers to those who do not require Adult Social Care ďŹ nancial help, however Adult Social Care will still be willing to oer an assessment of need. You may have a choice of which agency you receive your care from whether or not Adult Social Care assist with the funding of your service. People are entitled to receive an assessment of their care needs and if eligible Adult Social Care may fund services to meet these care needs or provide a Direct Payment so that people can arrange their own care services in the way that best suits them.

The Department of Health has published a ďŹ ve year Dementia Strategy which aims to transform the lives of millions of people with dementia and their carers. Health and Adult Social Care organisations are working together across Bournemouth, Poole and Dorset to implement the requirements of the Dementia Strategy and to ensure that people living with dementia have access to good quality care services. The Dementia Strategy identiďŹ es how crucial it is for people who may be experiencing the early symptoms of dementia to be able to access good quality advice and support and receive a diagnosis as to whether in fact they have a dementia illness. Alternatively people may want to initially access one of the memory clinics or Dementia Cafes that are run by the Alzheimer s Society, details of these can be obtained from the Alzheimer s Society national helpline on 0845 300 0336 or the local oďŹƒce on 01202 309084. Residential dementia care If the time comes when remaining at home is no longer viable, the beneďŹ ts of residential care need considering. Questions to ask of any home registered for someone with dementia would include: who is the senior clinician who can act as a single point of contact for people to approach for information and advice? What training programmes do sta participate in? Does the home oer a person-centred approach to their care planning? Is the home designed with the needs of its clients in mind, especially safety and accessibility? Is there plenty of natural light? Is moving around helped by easily-understood signage? How is technology employed to help alleviate falls and help orientation? Please also see the care homes checklist on page 39 and the residential dementia care checklist on the opposite page. Dierentiating one home from another can be diďŹƒcult but the quality and quantity of sta training is especially important, along with the turnover of sta as continuity of relationships in this type of home is critical. The use of medication is a highly emotive subject and any psychotropic medication must be very carefully prescribed. A ďŹ ne balance exists between acting in the best interests of the individual by prescribing medicine and the beneďŹ ts of normalisation where time and energy are spent maintaining familiar, comfortable surroundings which help reduce stress and possibly avoid the need for certain medication.

Types of care home There are two types of care home: Care homes offering personal care only If you are reasonably active, but would like greater security and care, subject to a needs assessment, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving and must be paid for if your capital/savings exceed £23,250. Care homes with nursing If you think you may need nursing care in a home, you will need to be visited by a social worker or a care manager to work out what care you will need.

This visit might be in your own home, or in hospital if you ve been ill, or in a care home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, the social worker will give you information to help you find a home which meets your care requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly: the current amount is £108.70 per week. The figures mentioned here may change after April 2013, check with Adult Social Care after this date. See page 42 for more information on paying for your care.

Out of area care options Did you know you can choose a care home outside the area you currently live in? You may want to be closer to friends, family members or you may want to relocate to another part of the country. If your care home place is state-funded, your local authority is still responsible for your care fees, even if you choose a care home in another area. The home you choose must be suitable for your assessed needs and comply with the terms and conditions set by the authority. The fees that your local authority will pay may vary. They may offer you the fees you would receive if you remained where you are, or they may offer you the fees that the local authority would pay in your chosen area. You must seek further advice before making your decision. If you re self-funded, obviously you have freedom of choice, however it is important that

you have considered the fees the home charges and whether the home will accept the local authority rates if or when your savings fall below the capital limit. If the home is unable to meet the local authority rate the possibility of moving to an alternative home increases substantially.

Paying for care Care fees can increase It is usual for the fees charged by care homes to increase each year. If you are not paying your own fees, the amount that Adult Social Care can contribute may not increase at the same rate as the home fees increase. Any shortfall in fees must be met by a third party (top-up) payment ‒ discussed on page 45. The third party element of the fee can only be increased once in any ďŹ nancial year and as a guide, the amount should never increase by more

than 25% a year. You should receive at least one month s notice that the third party payment is to increase. The person who makes the payment for you must sign a new contract with Adult Social Care if the amount of the third party payment increases. If you are already living in a care home, plan ahead and ďŹ nd out what help you will get from Adult Social Care when your savings fall below the ÂŁ23,250 limit, explained below.

Paying for your own care Even if you know you will have to pay for it yourself, if you need care in your own home or are considering moving into residential care, you still qualify for a social care assessment by Adult Social Care. The assessment will make sure that your needs will be met. Also, if you expect your capital to fall below ÂŁ23,250 as a result of paying for care, Adult Social Care may then help you with the cost.

This is called the property disregard period. Deferred payment agreements After the twelve-week property disregard period, Adult Social Care may be able to help you while your property is being sold. They will charge any financial help they give against the value of your home. This means they will take back the amount they have paid once your property has been sold. However, they may limit how much they will pay. It may affect your entitlement to Pension Credit if your property is not seen to be on the market and becomes treated as capital by the Department for Work and Pensions (DWP). What if you run out of money? If your capital is likely to reduce to £23,250 as a result of paying for care, you must tell Adult Social Care well in advance that this is going to happen. They will then be able to help with your care fees (provided your care needs assessment has shown that you need residential care). They will carry out a financial assessment to confirm the date from which your funds fell below £23,250, and the amount they, and you, will pay towards your care. If the home you have chosen charges more than

the local authority will pay, you must find someone to pay the difference ‒ a top-up payment. Whoever does this, whether a family member or a charity, they should understand that the cost could increase, and that they will have to pay the top-up for as long as necessary. The local authority will not meet the additional cost even if you have lived in the home for a number of years. If you cannot find someone to help you meet the extra cost, you may have to move to a home within the local authority s funding levels. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek financial and legal advice about the various options you may have before committing yourself. Attendance Allowance This is a non-means-tested, non-taxable benefit from the DWP. It s paid at the lower rate of £51.85 a week if you need care by day or night and at the higher rate of £77.45 a week if you need care during the day and night. Everyone who needs care can, and should, claim Attendance Allowance. If you are paying the full cost of residential care, with or without nursing care, you will be entitled to the higher rate. NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled to NHS Nursing Care Contribution towards the cost. It is not means-tested and is currently £108.70 a week. This is paid directly to the home.

What will you have to pay for your care? Do you have more than ÂŁ23,250 in capital and savings? The value of your home is included in the assessment of your capital unless one of the following still lives there: • Your partner (that is, your husband, wife or civil partner, or someone you live with as if they were your husband, wife or civil partner), or • A relative who is over 60 or disabled, or • A child under 16 who you, or a former partner, are responsible for. NO Adult Social Care will contribute ďŹ nancially towards your care costs after they have carried out an assessment of your care needs. How much? You will need to ask Adult Social Care for a social care assessment. This will deďŹ ne your care needs and how they will be best met. Any care homes you consider must be able to meet these needs. Adult Social Care will also make an assessment of your ďŹ nancial situation. This determines how much they will pay and how much you will have to pay. Your choice of care home will be limited to those that accept your local authority s funding level. If you want to choose a more expensive home you would have to arrange for a third party - such as a family member or charity - to top-up the dierence. You are not allowed to do this yourself if your capital is below ÂŁ23,250. If you have capital between ÂŁ14,250 and ÂŁ23,250 you will pay ÂŁ1 a week for every ÂŁ250 you have above ÂŁ14,250. If your capital is less than ÂŁ14,000, and the home you choose charges fees that are within your local authority s funding rate, your contribution will be assessed only on your income.

Third party payments for care home fees Some care homes and care homes with nursing charge fees that are higher than the maximum amount Adult Social Care can contribute. If they are contributing towards your care home fees and you choose to move into a home which charges a higher fee, the difference between the two amounts is paid by a third party, usually a relative. Third party payments are sometimes referred to as top-up payments. Before you sign any contract the home should give you written details of all the charges it intends to make in its Statement of Purpose document. If there is anything that is not clear or which you do not understand, you should ask for advice. As the name suggests, this payment must be made by someone other than yourself or Adult Social Care. It can be a relative or friend, but it cannot be a liable relative i.e. someone whose financial situation has been taken into account when your financial assessment was carried out by Adult Social Care. The general rule is that you cannot use your own money to fund a third party payment. However in certain specific circumstances you may make a third party payment from your own funds. These are:

Adult Social Care recommends that you seek independent financial and legal advice if you are considering these options. If you need further advice about how a top up to your care home fees might be arranged please discuss this with your care manager. If you move to a care home where a third party payment is required, the person who will make the payment on your behalf must sign a contract with Adult Social Care, before the contract with the home is signed. In doing so they must confirm that they have the financial means to make the third party payments (including any future increases) for the whole time you will live at the care home. If you are already resident in a care home, and no third party agreement was required at the time you became a resident, the home may seek to introduce a third party payment at a later date. Also if a change to your arrangements is made at your request or with your agreement, for example you move to a superior room, then an additional third party payment can become due.

Essential information Inspection and registration of care services All care providers must be registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the ďŹ rst time. The Care Quality Commission (CQC) is the independent regulator of health and social care in England. They regulate care provided through a range of methods including: • Registering providers to ensure they are meeting the essential standards of quality and safety. • Monitoring how providers comply with the standards. • Promoting improvement in services. • Using enforcement powers where necessary. • Publishing up to date information about the compliance of adult social care providers with essential standards of safety and quality.

Following an inspection, each service is given a report of how it rates against national essential standards of quality and safety. Comments, compliments and complaints All organisations need to know how they are performing. They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service. If you do need to make a complaint you should feel able to complain about any aspect of your care that aects your happiness or comfort. This could be about the way you are treated by a sta member or the quality of the food you are served. You can also make comments and suggestions about possible improvements to your surroundings and the services provided. Making a complaint should not be diďŹƒcult for you. Providers are required under essential standards of quality and safety to have a simple and easy to use complaints procedure that they will be happy to give you. If you are concerned about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any

complaints made. Have your say about local services In April 2013, all Local Involvement Networks (LINKs) in England will be replaced by new organisations called local Healthwatch. They will be the consumer champion for users of health and social care services. They will take over the LINks remit to give citizens and communities a stronger voice to influence and challenge how health and social care services are provided within their locality. At a national level, Healthwatch England was launched in October 2012 as the independent patient champion within the Care Quality Commission (CQC). It may be physical, sexual, financial, psychological or institutional, or may relate to neglect or discrimination. Legal help and advice It would be nice to think that your retirement and old age will be worry free. It can be. Advance thought and preparation can pay dividends for you and your loved ones at a time when you may be less able to cope by yourself. Some people are lucky enough never to need help with their financial or personal arrangements. For many, though, health issues or general slowing down in old age mean that decisions and action on day to day matters (which seemed routine when you were younger and fitter) need to be taken by someone else on your behalf. If you take no steps to make arrangements yourself then the only course of action may be the appointment of a Deputy through the Court of Protection. The Court will oversee the actions of your Deputy and complicated work is necessary to comply with the Court s requirements. A level of formality is needed which can seem harsh and intrusive, not to mention expensive.

The alternative is for you to make decisions yourself ahead of the difficult times which you may face in the future in the form of a Lasting Power of Attorney (LPA) for either your financial or welfare affairs or both. An LPA enables you to appoint people of your choice to handle your affairs and gives your personal authority to them to act with greater ease on your behalf. It is important to note that if you have already made an Enduring Power of Attorney it will probably still be effective, but it would be sensible to take advice on whether a welfare LPA would suit your future needs.
