Reablement
Reablement is a short-term service that helps you get back daily living skills that you may have lost because of an accident, illness or disability. For example, support after you’ve come out of hospital.
You may be offered a period of reablement, depending on your needs. Reablement is provided by a number of different professionals, who work with you to help you get back your confidence and daily living skills. This could include looking at alternative ways of managing everyday tasks. The team will also help you set goals and monitor your progress.
The support you receive can be in your own home or, if necessary, in another residential setting. The team aim to be flexible in order to meet your needs. Whatever the setting, reablement is always time limited. It’s normally between two and three weeks, but occasionally longer, depending on your needs and progress.
You will not be charged for the agreed period of reablement, but you may be charged for any care and support you need after this. What you will need to pay will be determined by a financial assessment. See page 55 for details.

The Joint Community Rehabilitation Service (JCR) is a home care service which can offer reablement and rehabilitation support if you meet certain criteria. Referrals are made via your health professional or Health and Social Care Connect, which can be contacted using the below details. The services support people with physical and mental health needs. Referrals are made via health professional or Health and Social Care Connect.
The Milton Grange Community Outreach Service is a therapy-led team offering specialist rehabilitation and assessment to people with mental health needs living in the community, who meet certain criteria.
Referrals are also made via health professional or Health and Social Care Connect (HSCC)

Brighton & Hove
There are two residential care homes specialising in rehabilitation, and it can also be provided in your own home. Nurses, physiotherapists and occupational therapists will work with you to help you become medically fitter and regain physical mobility and strength. Usually, people are referred to rehabilitation after a stay in hospital or after a long-term illness. Referrals will be made via your health professional.
