Welcome from Nottinghamshire County Council Welcome to the 2013/14 edition of the Nottinghamshire Care and Support Services Directory. The Directory is designed to help you understand the wide range of services that are available for people who have been assessed as needing social care. People tell us they wish to be as independent as possible and remain in their own homes rather than go into residential or nursing care. We want to help people to do this and allow them to have choice and control over their support. We have a wide range of services, in conjunction with the health service, to help people stay living at home safely for longer. If you think you need social care support, we can carry out an assessment to see if you qualify for these services. One example is our reablement service, which is a short term period of home care support which aims to return people to independence after a fall or a period of illness. Since April last year, 53% of people who received our reablement services subsequently no longer needed support. Another new approach is our care and support centres, which provide short-term support and practical services to help people stay living independently at home. The centres are at the heart of their communities which means they respond to local demand and offer what local people need. Examples of services include respite care, carers groups and bathing services. We know that some people will still need residential or nursing care and all local services are listed in this Directory, starting on page 41. If you pay for your own care and are thinking of going into a care home, we strongly advise that

you seek specialist information and advice and explore all of the funding options available to you. The funding of long-term care is likely to be one of the biggest financial decisions of your life ‒ lack of planning could result in depletion of your assets and inheritance for your loved ones. Specialist information and advice can also help you to understand your entitlement to state and local authority benefits and help you to make informed decisions about your care and your life. Visit www.nottinghamshire.gov.uk/ findacarehome for more details. In addition to this Directory, we are here to support you with further advice and guidance. You can give us a call on 0300 500 80 80 and talk to one of our specialist operators who will provide advice or assistance on the services available and can arrange for an assessment to be carried out for yourself and/or your carer. Further information and the latest news about services and initiatives are also available at any time by visiting www.nottinghamshire.gov.uk/adultsocialcare

Where do I start? Your assessment Contact us on 0300 500 80 80 or visit www.nottinghamshire.gov.uk/adultsocialcare to find out more about our services and support provided by the voluntary sector and other groups. If you need social care support, we will carry out an assessment to find out what you are entitled to. You have a legal right to an assessment whether you are directly in need of care yourself, or a carer who is supporting someone in need. If you are a carer you can be assessed in your own right. Being assessed means that a trained worker will talk to you about your circumstances and find out if you have particular difficulties which mean you may require support. We will discuss with you the level of advice and help you may need at an early stage. Many of the services highlighted in this Directory have a cost attached to which you may have to contribute following a financial assessment which considers your ability to pay. We follow guidelines (eligibility criteria) for

deciding who is able to receive community care services, which are based on Government guidance called Fair Access to Care Services (FACS). This aims to make sure that all councils make decisions about who gets help in a similar way. We assess the risks to a person s autonomy and freedom to make choices, health and safety, ability to manage personal and other daily routines and involvement in family and other wider community life. In Nottinghamshire this means that if the risk to your independence of not getting a service is substantial or critical you will definitely get a community care service. We are allowed to offer services which may not be your preferred option if we can demonstrate that they meet your identified eligible needs in the most cost effective way. If you want to know more about FACS or the eligibility criteria used, visit www.nottinghamshire. gov.uk/adultsocialcare or www.doh.gov.uk

Personal budgets A personal budget is an allocation of money that you use to buy the care and support you need. It gives you as much choice and control as possible over how your needs are met. You can use your personal budget to purchase a range of different services including domiciliary care. This might be support with personal care, help with cooking or other household tasks or support to go out into the community. To find out if you are eligible for long-term social care support we would need to carry out a community care assessment. The assessment is carried out with you and a social care worker. It helps you think about your needs and works out how much you will have to buy the support you need with your personal budget. To ask for an assessment or for more information phone us on 0300 500 80 80. If you are eligible for a personal budget we will offer you a financial assessment to see how much, if anything, you need to contribute towards it. Almost everyone has to pay something towards their personal budget, although there are some exceptions.

6

The amount you have to pay towards your personal budget depends on: • your income and savings • whether you have a disability • whether you are single or in a couple • whether you are a carer • the amount of your personal budget. There are some special situations where you can still get support even though you cannot afford the full amount you should pay. The person who did your assessment will be able to tell you more about this. continued on page 7 >>

Personal budgets continued There are three different ways you can choose to receive your personal budget: • Personal budget as a direct payment: a direct payment is a cash payment which can be used to buy and arrange support. It is paid to you or someone acting on your behalf. Direct payments are the most flexible way for you to manage your personal budget. You can use your direct payment to buy the support or activities you want to meet your needs. However, you cannot use a direct payment to buy services provided by Nottinghamshire County Council. If you need help managing your direct payment, we can give you the contact details for a direct payment support service. If you need help arranging your support, your social worker will be able to help you. • Managed personal budget: we will arrange care and support services to meet your needs. If you choose to have a managed personal budget we will look after it.

choice and flexibility, but how much depends on you. You may decide that you want us to organise some of your support (managed personal budget), whilst also having some of your personal budget as a direct payment to give you more choice and control. It is up to you how you choose to have your personal budget. Each option gives you a different level of choice and control over how your social care needs are met. For information about home care agencies see pages 17-21.

17-21

• Mixed personal budget: this option gives you

Independent living options You may not necessarily need residential or nursing care as there are a wide range of services,

equipment, aids and adaptations available to help you stay safe and independent in your own home.

Reablement (short term home care support) If you are struggling with everyday tasks at home (e.g. getting dressed or making a drink) or if you have had an accident or illness, our reablement team can help you to regain skills and confidence so you can live as independently as possible. Reablement is a free period of short-term home care support which can last for up to six weeks. Our team of staff can help you by: • suggesting different ways of doing things • offering small items of equipment to make tasks easier • telling you about other kinds of help you could get and different ways you can pay for it. Care in your own home If you are an older person or have a physical disability, learning disability or mental health needs, home care staff may be able to support you with daily activities such as getting in and out of bed, getting dressed and going to the toilet. They can also help with daily living or domestic tasks like laundry, shopping and some meal preparation. For those with additional needs, specially trained staff are available. If you are frail, have been ill, or have just come out of hospital, you may benefit from short-term home care support (see reablement section on page 7). Home care services can be provided by: • independent agencies • voluntary agencies • individuals (paid for using personal budgets ‒ explained further on pages 6 & 7). The amount of care you receive weekly will depend on your assessed needs (worked out with Nottinghamshire County Council first) and will be delivered by a small team of carers. They must be specially trained in areas such as manual handling, first aid and food hygiene. Carers will also have had

background checks with the Disclosure and Barring Service (DBS), previously known as Criminal Records Bureau checks. This new organisation will provide a joined up, seamless service combining the criminal records checking and barring functions. Further legislative changes will come into force during 2013 and 2014. These details will be published on the DBS website. For disclosure information and services visit the DBS homepage: www.homeoffice.gov.uk/dbs Some agencies offer a night sitting or even a livein service and some are also registered as Nurses Agencies, meaning they can provide registered nurses if needed. All home care organisations that provide personal care must register with and be inspected by the Care Quality Commission (CQC). They ensure that quality standards are met by the service and that staff are appropriately trained to do the job. Contact the County Council on 0300 500 80 80 for more information about home care and the assessment process. See pages 17 to 21 for information about home care agencies in Nottinghamshire.

Night Response Service If you have home care support for help with your personal care needs, you could be entitled to extra help at night. The Night Response Service provides additional support to existing home care so you can contact staff during the night for things such as: • personal care such as incontinence issues • dealing with sickness or medical conditions • prompting to take critical medication. The service is available for people over 65 and we will carry out an assessment to determine if you are eligible. In order to receive this Night Response Service you will need to have a lifeline alarm installed. A Lifeline is an alarm you press when you need help. If you do not already have a suitable Lifeline service, and are eligible for the Night Response Service, you will be provided with our stay at home kit,

which is linked to a 24 hour monitoring centre (see page 11). A weekly fee is payable for the provision of the Night Response Service. If you do not already have a suitable Lifeline service, a weekly fee of a few pounds is payable for the stay at home kit. We will carry out an assessment to determine how much you will have to pay based on your available income and your ability to pay for the service. For more information visit www.nottinghamshire. gov.uk/adultsocialcare or phone 0300 500 80 80.

Care and support centres Our six care and support centres around the county provide short-term support and practical services to help people stay living independently at home. Each centre is different. Being at the heart of their communities means they respond to local demand and offer what local people need. Examples of services include respite care, carers groups and bathing services. Staffed by a professional and caring workforce, backed up by the stability of Nottinghamshire County Council, these centres offer vulnerable people a haven of care and support in their community. For information about services available at your nearest care and support centre phone 0115 977 2189 or email livingathome@nottscc.gov.uk.

Respite care and short stays Many of you may have an informal carer living with you, or perhaps living nearby, who helps you on a regular basis. Carers sometimes need a break and this may be provided in several ways: you may wish to go into a care home for a week or two; you may arrange home-based respite care where another carer moves into your home; you may arrange for family-based respite care where you move into another family s home for a short time.

10

The best option for you will depend on your needs, the urgency of the situation and personal choice and cost. If you are thinking about longterm care, but have no experience of living in a care home you may wish to book for a short stay to see what it s like. This may help you to make an informed decision. For more information about respite breaks contact us on 0300 500 80 80.

Intermediate care This is a short-term, structured programme of therapy and treatment designed to rehabilitate you and make you as independent as possible. Courses of intermediate care usually last no more than six weeks and can be used in a number of situations. For example, if you have finished hospital treatment but cannot manage independently at home, intermediate care may enable you to get home faster.

Intermediate care is different to respite care. Where respite care provides full-time care on a temporary basis, the aim of intermediate care is to help you manage by yourself. Intermediate care can be provided in your home, or in a care home, depending on your circumstances. For more information about intermediate care contact the County Council on 0300 500 80 80.

Stay at home kit If you feel vulnerable in your own home, the stay at home kit can help you to summon help in an emergency. Sensors around the home can detect falls, inactivity, smoke, flooding, gas or extreme temperatures in the home. If the sensors are triggered, an alert is either sent to trained operators at a 24 hour call centre or directly to a relative or carer. The equipment can help you to remain independent and your relatives and carers are reassured, safe in the knowledge that should an incident occur, they will know about it. It could also help as a short-term measure during a period of convalescence. Some devices can also help monitor particular health conditions and reduce the need for hospital admission. The kit can seldom replace care and assistance from people ‒ it usually enables existing care and support to respond more effectively and can sustain carers in their role.

The stay at home kit can only be used where there are sufficient carers or there is sufficient care support available to respond quickly when an alarm is raised. For more information visit www.nottinghamshire.gov.uk/adultsocialcare or phone 0300 500 80 80.

Shared Lives We have a number of Shared Lives carers across the county who can support older people, people with physical or learning disabilities or mental health needs.

We will need to assess you to find out if you are eligible for this service. Contact us on 0300 500 80 80 or visit www.nottinghamshire.gov.uk/ sharedlives to find out more.

There are different types of support available: • Long-term accommodation and support: you could move into their home and live as part of their family for some time. • Short breaks: you could move into their home for a short period while the people who normally support you are having a holiday or have to be away. • Regular short-term support: carers could support you in the day to help increase your independence and help you become part of the community. Visit www.carechoices.co.uk for further assistance with your search for care

Support with Confidence If you receive a personal budget and wish to employ your own personal assistant to help with your care support, the Support with Confidence directory can help. The directory provides a list of potential employees who have received training and have been checked by the County Council. You can select who you want from the directory to provide a particular service and then contact them to offer them an interview. To find out more about Support with Confidence and to use the directory phone 0300 500 80 80 or visit: www.nottinghamshire.gov.uk/ supportwithconfidence

The Handy Persons Adaptation Service The Handy Persons Adaptation Service provides the help and support you may need to keep safe and secure in your home with low cost but high quality essential adaptations and small practical jobs. The service is available to Nottinghamshire residents aged 60 or over or who have a disability. The work is carried out by professional traders who have been approved by our Trading Standards officers. The scheme will carry out small jobs or minor adaptations to your home to reduce the risk of a fall or help you remain living independently, ranging

from fitting grab rails or half steps, to changing light bulbs, fitting door locks and putting up shelves. Often very small jobs such as fixing loose carpets can prevent a fall and avoid a lot of unnecessary distress. There is a nominal fee (£15 as of April 2013) and you can have work carried out up to the value of £250. For more information about the service or to access the scheme, please contact the County Council on 0300 500 80 80 or email: enquiries@ nottscc.gov.uk.

Buy with Confidence Our approved trader scheme ‒ Buy with Confidence ‒ can help you find local plumbers, builders and other traders who have been vetted by our trading standards officers at Nottinghamshire County Council. We carry out checks to ensure they are solvent, adequately insured and don t have a history of ripping people off. Buy with Confidence is free to use. Visit www. nottinghamshire.gov.uk/buywithconfidence or

phone the Citizens Advice Consumer Service on 08454 04 05 06 to find a local trader you can trust.

Meals at Home ‒ County Enterprise Foods Most of us take for granted our independent life style. There are stages in life when we can all do with a little help from time to time, even for a very short period. County Enterprise Foods Meals at Home offers a service helping people who want to remain in their own homes and within familiar communities. The service is available and ideal to elderly people, retired people and adults who are vulnerable. The service is able to deliver frozen or hot delicious meals to your door. In addition it can also deliver a freshly made salad or sandwich along with the main meal enabling you to have a satisfying tea and / or a breakfast for the following morning. If you would like to try the service or require a brochure or further information please phone on 01623 490015 and one of the trained caring staff will talk to you about which service best meets your individual needs. If you would prefer, a friend or family member can do this for you. The service caters for many dietary needs, including a diabetic

range, vegetarian, gluten free, soft food option and ethnic meals. The price of a main course and dessert is £3.95. Delivery is free. You may feel that you are not ready for this service every day, but would like to have some meals in your freezer for when the weather is bad and you are unable to go shopping. Meals can be on a temporary basis when recovering from an illness or returning home from a stay in hospital. If this interests you, please phone 01623 490015 and discuss how you will benefit from the service. Web: http://countyenterprisefoods.co.uk

Caring for carers If you provide unpaid support to help an adult live at home you are a carer. You might be caring for someone that is elderly and frail, has a long-term physical illness or disability, a learning disability, mental health issue or substance misuse problem. Caring may mean that you help with practical tasks such as washing, dressing, taking medicines, shopping or laundry, provide emotional support, keep an eye on someone to make sure that they are safe or look after someone when they go out. We work with the NHS and other organisations to provide support for carers. One of the services offered is a Carers Emergency Card. This is a free small card that identifies you as a carer in an emergency situation. It will alert anyone finding the card to the fact that you are a carer and that someone you care for may require assistance. Once issued with a card, in the event of an emergency you will have peace of mind that there is a 24-hour system in place to ensure that the person you care for is looked after by the people that you choose. If you would like to apply for an emergency card please phone the County Council on 0300 500 80 80. We also support a scheme enabling the person that you care for to be looked after on a short term

basis in your home should you have an unforeseen or emergency situation. Examples of when the service has been used are: • You are too ill to provide care for a short period of time • You need to attend the funeral of a family member/ friend • You have a hospital appointment or are due to be admitted to hospital yourself. There is no charge for this service. It is offered through Crossroads Care and you can contact them directly. For more details visit www.nottinghamshire. gov.uk/adultsocialcare We also pay the Carers Federation to provide support to carers. This includes free advice, information and support to all carers. The Carers Federation may be able to help you in a number of ways, for example telephone or face-to-face advice, information about local support groups where you can meet other carers and information about events and training courses. For more information please contact the Carers Federation on 0115 9629310, or view their website: www.carersfederation.co.uk continued on page 15 >>

Helping you to Choose and Pay for care • 0800 38 92 077 Advice for older people

Caring for carers continued If you need some help for the person that you care for and/or you want a regular break from caring the person that you care for will need a community care assessment. If you regularly care for someone you are also entitled to ask for a carer assessment, to look at the impact that caring has on you and make sure that your own needs are addressed. For information

regarding assessments you will need to speak to the County Council on 0300 500 80 80. Caring may have an effect on you, for example your health, work, social life, finance, education, family and personal life. It is important that you know what help is available to help you balance your caring life with a life of your own .

Extra care housing Extra care housing gives you the security and privacy of a home of your own, but with 24 hour care services available if you need them. There is usually a range of communal facilities on-site such as a hairdresser and schemes offer the choice of properties for rent, sale or shared ownership. The look of the housing varies. Some are new purpose-built retirement villages, others are modernised sheltered housing schemes with extra services on-site. Choosing extra care housing means that you are living in your own home and can remain there if your care needs change. Unless you require longterm nursing care, it could offer you a home for life.

• residents have a mixture of abilities • it offers a safe and secure home for people who need support; including people with learning disabilities • 24 hour care services are available ‒ if needed

Extra care differs from care homes and sheltered housing in the following ways:

• you get support to keep your independence

• you are living at home ‒ not in a home

• extra care aims to offer a home for life.

• you have your own front door so you can decide who comes in

Extra care housing is available to people aged 55 years or over.

• couples and friends can stay together

For further information, call us on 0300 500 80 80.

• you can join in social activities if you wish

Mobility aids You can find information on mobility aids, such as scooters and stair lifts, as well as advice on where to buy them from the Disabilities Living Centre. There is a comprehensive range of equipment on permanent display in the centre including beds, chairs, bathing and showering equipment, wheelchairs, scooters, stairlifts and small equipment. There is also information on all the equipment on display, and a comprehensive range of leaflets and brochures on many other items, facilities and services for people with disabilities. An appointment is essential if you wish to visit, so please telephone the centre first on 01159 855780 to arrange one.

Agencies • What experience does the agency have in your • How can you contact the agency in an emergency particular field of need? Can they supply references or outside office hours? for you to check?

• How long has the agency been operating?

• How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick? Will you be notified in advance that a different carer will be attending?

• How hard or easy would it be to make a complaint and how are things then put right?

• Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate. • If this is a private contract ask for a copy of the agency s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK (sometimes called Age Concern in some areas) can help with this.

Carers • You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding?

• Carers should be fully trained or be in ongoing training. Ask the agency about their policies on this. • You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency. • Carers must be checked with the Disclosure and Barring and have a criminal records disclosure ‒ make sure this is the case.

Paying • If you are eligible for support from the County Council you will be financially assessed to see if you must make a contribution to your care.

22

• If your care needs do not meet the County Council s eligibility criteria and have been arranged privately you should check carefully the fee rates charged and exactly what the payment you make covers.

Care home options Types of homes There are two types of residential home: Care homes (personal care only) If you are reasonably active, but would like greater security and care, subject to a needs assessment, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving but it must be paid for if your capital/savings exceed £23,250. See page 33 for more information on paying for your care. Care homes with nursing If you think you may need nursing care in a home, you will need to be visited by a social worker or a care manager to work out what care you will need. This visit might be in your own home, or in hospital if you’ve been ill, or in a home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, the social worker will give you information to help you find a home which meets your care requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly: the current amount is £108.70 per week for the mid rate and £149.60 per week for the higher rate. These figures may change after April 2014, check with the County Council after this date.

Availability in care homes Our online database shows the number of beds in each care home in Nottinghamshire as well as the number vacant at any one time. The database also includes contact details and the County Council’s quality rating for each home. Visit www. nottinghamshire.gov.uk/findacarehome or phone us on 0300 500 80 80.

What support can you expect from a care home?

What happens if my relative lives outside Nottinghamshire? If you are considering choosing a home outside Nottinghamshire because it is nearer to your family or friends then you may still be eligible for financial assistance with your placement. Your social worker will be able to give you more information about this.

What happens when a council has suspended its agreement with a care home/care home with nursing? While all care homes/care homes with nursing are required to be registered with the Care Quality Commission, if we are providing financial help then we will have a separate agreement in place with the care home/care home with nursing. This is done to ensure that the care home/care home with nursing and we know exactly what is required from the service. Occasionally it may be necessary to temporarily suspend placements into a care home/ care home with nursing. This may be done for a variety of reasons: • to investigate a concern • to allow time for improvements to be made • to take the pressure off a care home/care home with nursing if they are experiencing a problem. Sometimes a care home/care home with nursing itself may request a temporary suspension of placements so that it can make changes. While a placement suspension is in place no further local authority placements will be made to the care home/care home with nursing and alternative placements should be sought. If there are serious concerns about a care home/ care home with nursing, residents and/or their relatives/carers will be informed of the situation.

All care homes provide help with personal care and permanent accommodation with staff available 24 hours per day, 7 days per week on every day of the year. Some care homes also provide nursing and are known as ‘care homes with nursing’. These homes must employ qualified nursing staff so that they can give this type of care for 24 hours each day. Specialist dementia care Usually, the initial point of contact to access care services is either your GP (for health care solutions) or you can contact us (for most other services) on 0300 500 80 80. Your GP, as well as discussing your concerns, will be able to carry out a series of tests which will establish the nature of your condition and advise whether treatment is possible. If the time comes when remaining at home is no longer viable, the benefits of residential care need considering. Differentiating one care home from another can be difficult but the quality and quantity of staff training is especially important, along with

the turnover of staff as continuity of relationships in this type of home is critical. The use of medication is a highly emotive subject and any psychotropic medication must be very carefully prescribed. A fine balance exists between acting in the best interests of the individual by prescribing medicine and the benefits of normalisation where time and energy is spent maintaining familiar, comfortable surroundings which help reduce stress and possibly avoid the need for certain medication.

Residential dementia care checklist © 2013 Care Choices Ltd Things to look for in care homes specialising in dementia. Please use in conjunction with the care homes checklist on page 24. Residents

YN

The best indication of a good home is that the residents appear happy and responsive. • Are there rummage boxes around the home to stimulate residents?

Access If the person with dementia needs (or is likely to need) equipment or adaptations you may want to check: • There is adequate signage and cues for different parts of the home such as dining room and bathroom. • The home s policy about locking external doors.

Bedrooms You may want to find out whether the person with dementia can have a single room and whether: • Residents are encouraged to bring in some of their own furniture and possessions to increase familiarity.

Staff

YN

It is important to note whether staff seem friendly and caring towards residents and whether they treat residents with respect. • Do they have any training and experience in dementia care? • Will the person with dementia have a member of staff particularly responsible for their care? • Is there a member of staff you can talk to about your own worries about the person with dementia?

Manager/head of home A manager who is caring as well as efficient can make all the difference to a home. • Does the manager have a knowledge of dementia and can they deal with difficulties that may arise in an understanding way? • Is there a full assessment at home before a resident is admitted?

Activities Residents should be stimulated without feeling stressed. • Are reminiscence activities available? • Are residents able to compile memory boxes? • Do staff compile life story books including photographs and mementoes?

A list of care home providers can be found on 41-73 page 41-73.

Support for younger disabled adults Shared Lives We have a number of Shared Lives carers across the county who can support people with physical disabilities or learning disabilities. There are different types of support available: • Long-term accommodation and support: you could move into their home and live as part of their family for some time. • Short breaks: you could move into their home for a short period while the people who normally support

you are having a holiday or have to be away. •

Regular short-term support: carers could support you in the day to help increase your independence and help you become part of the community.

We will need to assess you to find out if you are eligible for this service. Contact us on 0300 500 80 80 or visit www.nottinghamshire.gov.uk/ sharedlives to find out more.

Disability Nottinghamshire Disability Nottinghamshire promotes the independence and choice of everyone with disabilities and impairments within Nottinghamshire (excluding Nottingham City). Disability Nottinghamshire provides free, confidential and impartial information, advice and advocacy in respect of any aspect of disability to people with disabilities and impairments, their carers, friends and families as well as professionals and students. The service is provided by people who either have a disability or personal experience of disability.

For free, impartial and confidential advice and information on any aspect of disability (both general disability advice and specific welfare and benefits advice) call 01623 625891. A drop-in information service is available at the Mansfield office: 1 Byron Street Mansfield Nottinghamshire NG18 5NX www.disabilitynottinghamshire.org.uk/services

Physical disabilities There is a wide range of support available from the community for people with a physical disability / long-term illness. We provide a range of support for people with a physical disability / long-term illness who are eligible for social care support. For many services you will need an assessment (discussed on page 6) to determine if you are eligible. Services include: support to live at home; equipment and adaptations; occupational therapy and short term support. The Short-Term Assessment and Reablement Team (START) can work with you to regain the skills and confidence to live as independently as possible. It can help with tasks such as getting dressed, using the bathroom, getting around your home and cooking. Promoting Independence Workers can also visit people at home to look at what support they need

28

to help them stay independent. They offer advice and information on a range of topics such as home safety and security, home adaptations, mobility aids, transport, social activities and benefits. You can contact the council yourself or get someone else (such as family, friend or GP) to do this for you. Telephone: 0300 500 80 80 Email: enquiries@nottscc.gov.uk

Housing options for younger adults Are you happy living at home? Would you like a little more independence with help when you need it? What are your housing options? You could: • Continue living with your family – If your home is specially adapted and you receive the practical and emotional support needed, you may prefer to stay at home. It’s also convenient if your college or job is nearby. • Move into supported living accommodation – This offers a level of independence and the chance to meet other people in similar situations. Help from a visiting care assistant is available and usually nursing care is provided nearby. Phone the County Council on 0300 500 80 80 or visit www.nottinghamshire.gov.uk/adultsocialcare to find out more. • Rent a place – You can apply for council housing

which is usually cheaper than renting privately. There could be a waiting list and you must fully explain your housing needs. Local housing associations may also be able to help. Ask your council housing department for a list. If you want to rent privately, look in your local newspaper for ‘To Let’ advertisements. Housing Benefit, if you’re entitled to it, might reduce your rent. Direct payments can also help, ask your local authority for details.

• Buy your own home – Can you afford it? Are you able to live on your own? You could think about ‘part buying’ and ‘part renting’ a place from a housing association. • Move into a care home – If you want to leave home but are not ready to live alone, care homes >> can meet your personal needs and offer

Housing options for younger adults continued appropriate support. Nottinghamshire County Council will contribute if you are eligible and if they agree the care home meets your needs. If your chosen home costs more than local authority limits, you may get some help but you must pay the rest. Paying for care is discussed on page 33. As your needs may change over time, discuss your ideas with family or friends. Residential care Adults with learning disabilities who need residential care may still be young when they enter residential/

nursing care and they will have a whole range of differing needs. Homes often specialise and a good home will provide opportunities to learn and maintain skills and opportunities to take part in enjoyable and stimulating activities. Social workers and community nurses in Community Learning Disability Teams can give information to enable people to make the right choice for themselves. For more information about care home placements and services for people with learning disabilities contact the County Council on 0300 500 80 80.

Mental health Mental health services in Nottinghamshire are accessed through your GP. If you are experiencing mental health difficulties it is important that you seek help from your GP. He or she will decide whether you need to see a specialist nurse based within the GP practice. These are known as primary care mental health services . There are a range of psychological therapies available within primary care. If your GP feels you need a more specialist psychiatric assessment, he or she can refer you to the Community Mental Health Team which covers the area you live in. These are known as secondary mental health services . In Nottingham and Nottinghamshire, secondary mental health services are provided by Nottinghamshire Healthcare NHS Trust. If you are referred to secondary services you will be offered an appointment to see a community psychiatric nurse (CPN), social worker (SW) or community consultant, depending on the problems you are experiencing. This is called an assessment and will help determine the kind of difficulties you need help with. You and the person who has undertaken the assessment will then work out a care plan which will look at how you can best be supported to address the problems you have discussed. If you require medication for your symptoms you will need to see the community consultant. The care plan will be reviewed jointly with yourself and your worker to ensure the help you are receiving is addressing your needs. Focusline provides 24 hour confidential telephone support - ring 08000 212721 for

further details. Contact Nottinghamshire Healthcare NHS Trust for information about the services they provide on 01159 691300. Residential care If you have mental health needs, residential care can help improve the quality of your life ‒ the structured environment and activities provided can help to build up your confidence. The care providers should assess you carefully to make sure the placement is right for you. A qualified member of the residential care staff may visit you at home, to talk to you and look at your assessments of health/social care need. To find out more about mental health care in your region, organise visits to prospective care homes. Ask the residents about the care they receive. Are psychiatric nurses available if needed? Do the activities provided assist rehabilitation? What are the attitudes to medication and other therapies? How does the care home approach different types of challenging behaviour? Does the staff have close links with the Community Mental Health Team? For more information about care home placements and services for people with mental health issues contact the County Council: 0300 500 80 80.

Essential information Safeguarding vulnerable adults Far too many vulnerable adults suffer abuse, neglect and worse, often at the hands of relatives and carers, those who you would least expect to treat them in that way. A vulnerable adult is a person aged 18 years or over who may be unable to take care of themselves, or protect themselves from harm or from being exploited. This may be because they have: • a mental health problem • a disability • a sensory impairment • some form of illness or are old and frail. What is adult abuse? Abuse is mistreatment by any other person or persons that violates a person s human and civil rights. The abuse can vary from treating someone with disrespect in a way which significantly affects the person s quality of life, to causing actual physical harm. It can happen anywhere in someone s own home - in a care home, a hospital, in the workplace, at a day centre or educational establishment, in supported housing or in the street. Forms of abuse could be physical, sexual, psychological, emotional or financial. It could also cover the issues of neglect and institutional abuse ‒ where the abuse affects more than one person within an organisation and is not addressed by the people who manage the service. Who might be causing the abuse? The person who is responsible for the abuse is very

often well known to the person abused and could be: • a paid carer or volunteer • a health worker, social care or other worker • a relative, friend or neighbour • another resident or service user • an occasional visitor or someone who is providing a service • someone who deliberately exploits vulnerable people. We are committed to ensuring that vulnerable adults who use services are protected from abuse and neglect and to responding promptly when abuse is suspected. All agencies that provide services to vulnerable adults have a zero tolerance towards abuse and neglect. Contact us on 0300 500 80 80 to report a safeguarding incident or for further help and advice. Your concerns will be taken seriously and will receive prompt attention. If there is danger to life, a risk of injury or a crime is taking place and immediate action is needed, call the Police using 999. You can also help by keeping an eye out for older or disabled neighbours, friends and family. It might be something as simple as going to the shops for milk, helping to clear a path in bad weather or just popping round for a cup of tea. Please do your bit and be a good neighbour ‒ it could make all the difference. For more information visit www.nottinghamshire. gov.uk/adultsocialcare

Comments, compliments and complaints All organisations need to know how they are performing. They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service. If you do need to make a complaint you should feel able to complain about any aspect of your care that affects your happiness or comfort. This could be about the way you are treated by a staff

member or the quality of the food. You can also make comments and suggestions about possible improvements to your surroundings and the services provided. Making a complaint should not be difficult for you. Providers are required under the essential standards of quality and safety to have a simple and easy to use complaints procedure that they will be happy to give you. Comments, compliments and complaints continued sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any complaints made. Inspection and registration of care services All care providers must be registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time. The Care Quality Commission (CQC) is the independent regulator of health and social care in England. They regulate care provided through a range of methods including: • registering providers to ensure they are meeting the essential standards of quality and safety • monitoring how providers comply with the standards • promoting improvement in services • using enforcement powers where necessary

• publishing up to date information about the compliance of adult social care providers with essential standards of safety and quality. Following an inspection, each service is given a summary of how it rates against national essential standards of quality and safety. Each service s summary can be seen on the CQC website: www.cqc.org.uk For any enquiries or to register a concern or a complaint, contact CQC by telephone: 03000 616161 or by email: enquiries@cqc.org.uk The Care Quality Commission Citygate Gallowgate Newcastle upon Tyne NE1 4PA

Financial arrangements for moving into care Residential or nursing home care If you’re thinking about moving into a care home or a care home with nursing, there are costs involved that you need to consider. One element is free for everyone. If you move into a care home with nursing, the nursing part of your care will be free of charge. Your nursing care requirements will be determined by a nurse, who will visit you to carry out an assessment before you go into the home. More details on this can be found on page 34 (See ‘NHS-funded nursing care contribution’ and ‘NHS continuing healthcare funding’). Accommodation and personal care costs are assessed in a different way. In England, to receive financial support from your local authority, your capital, which includes your savings and your other assets, must be less than £23,250. The value of your home will also be included in the calculations unless your partner; a relative who is over 60 or incapacitated; or a child under 16 who you or a former partner maintain still lives there. If your capital is less than £14,250, the

amount you need to contribute towards your accommodation and personal care costs will be assessed on your income only. If it falls between £14,250 and £23,250 a sliding scale operates, whereby you pay £1 per week for each £250 you have above the lower figure. If your capital is less than £23,250, or likely to fall below that level in the near future, please contact us on 0300 500 80 80 before you arrange a care home or a care home with nursing. We will be able to assess your needs for care and help you arrange the necessary financial support. If your capital is above £23,250, you will have to cover the cost of accommodation and personal care yourself. Please see page 35 (‘Paying for your own care in a nursing or residential home’) for more information. Full details of how your contribution is assessed is available at www.nottinghamshire.gov.uk/ carehomes or by calling the County Council on 0300 500 80 80.

Third party payments The amount that we will pay to a care home or care home with nursing is determined by your needs and an annual quality assessment of every home in the county. This assessment takes into account the quality of care as well as environmental factors and enables every home to be given a rating from band 1, the lowest, through to band 5, the highest. Based on this information, we are able to set Quality Band Rates. In some cases, the home you choose will cost

more than the Quality Band Rate. If this is the case you will need to ask someone, perhaps a family member, a friend or a charity, to make up the difference so you can live in the home. This is known as a ‘third party payment’ or ‘top-up’. With third party payments, it’s important that the person making the payments is confident they’ll be able to keep up their commitment. They also need to be aware that the third party payment may change at any point in the future.

Nottinghamshire County Council – Banded Fee Rates 2013/14 Older people’s care home fee rates are based on a five band quality rating scale. This rating scale is based on 70% quality, which is established via an annual quality audit in each care home, and 30% environmental factors which is supported by a oneoff environmental audit.

The percentage scores from these two audits are combined to give an overall percentage score which then places the home in one of the five fee bands, with band one being the lowest quality and band five the highest. >>

the care. However, please note that some care homes may charge more and the difference will need to be paid via a ‘top-up’ or ‘third party payment’, discussed on page 33.

NHS continuing healthcare NHS continuing healthcare is a package of care arranged and funded by the NHS. It is non-means tested and to be eligible, your main or primary need must relate to your health and meet the criteria set out by the NHS National Framework. Care may be provided in a variety of settings

including a care home or your own home. If it is provided in a care home, it will cover the care home fees, including the cost of accommodation, personal care and healthcare costs. If it is provided in the home, it will cover personal care and healthcare costs

NHS funded nursing care If you live in a care home that provides nursing care, whether as a temporary or a permanent resident, and you do not meet the eligibility criteria for NHS continued healthcare, you may be eligible for NHS funded nursing care if you need care from a registered nurse.

This is non-means-tested and is paid directly to the home by the NHS Primary Care Trust. The current rates are £109.79 a week for the standard rate and £151.10 a week for the higher rate. Contact us on 0300 500 80 80 to find out more about making a claim for financial support.

Paying for your own care in a nursing or residential home If you’re paying for your own care in a nursing or residential home, some forward planning and advice on the options available to you will help you make the most of your money. See page 37 for more information.

Financial support for self-funding your care Even if you are self-funding your care it’s important to have an assessment of your care needs from your local social services department. All people selffunding their care are entitled to an assessment of their social care needs which should always precede any financial assessment. Why is this important?

Because without being assessed as needing a place in a care home you could miss out on any available financial support. Whether you’re receiving care in your home or moving to a care home / care home with nursing, you may be entitled to the following financial support.

Attendance Allowance If you’re aged 65 or over and you need someone to help care for you, you may be eligible for Attendance Allowance, a non-means tested, non-taxable benefit from the Department of Work and Pensions. It is paid at the lower rate of £53.00 a week for

those needing care by day or night and at the higher rate of £79.15 a week to those needing care both during the day and night. Call 0800 88 22 00 to register your claim for Attendance Allowance.

Twelve week property disregard If your former home is included in your financial assessment but your other capital is less than £23,250 and your income doesn’t cover your care home fees, we will help with the cost during the first twelve

weeks of permanent care, providing we agree that care is needed. This is known as ‘twelve week property disregard funding’.

Deferred payment agreements Once the twelve week property disregard period has ended, any financial help from us will be charged against the value of your home and recovered once it is sold. No interest is charged during your lifetime and only starts being added if the property hasn’t

sold when the period of the agreement ends (usually upon death or up to 56 days after). We recommend seeking independent financial advice if you are considering this option as it can have implications for other areas of your finances.

Council tax exemption If your property is left empty when you move into a care home you should receive full exemption from

council tax until such time as your property is sold.

Seeking financial advice on paying for care If you are self-funding your care then meeting the care costs from limited capital and not knowing how long care might be needed can be worrying. Despite this many people do not seek appropriate financial advice to understand how best to meet the costs, as interest rates remain low it would be prudent to seek advice from a specialist financial adviser. Money held on deposit can be depleted very

quickly if being used to pay for care; therefore it would be sensible to consider other forms of investments that could potentially deliver higher income to meet the care home fees. Before deciding on what may be the most appropriate solution for meeting care costs it would be sensible to talk to a specialist Care Fees Adviser who has CF8 , the relevant qualification for advising on funding long-term care.

Funding your own care - specialist care fees information and advice Every year 150 extra people need financial support from Nottinghamshire County Council when their savings dwindle after paying for long-term care. If you are responsible for paying for the full cost of your care we strongly recommend that you seek specialist information and advice and explore all of the funding options available to you. The funding of long-term care is likely to be one of the biggest financial decisions of your life ‒ lack of planning could result in depletion of your assets and inheritance for your loved ones. Specialist care fees advice can help you to understand your entitlement to state and local authority benefits and help you to make informed decisions about your care and your life Visit www.nottinghamshire.gov.uk/ findacarehome for specialist information and advice and details about care homes in Nottinghamshire. PayingForCare is a not-for-profit organisation that provides valuable information on all aspects of long-term care to help you make informed choices about your care and how you pay for it.

Nottinghamshire Care Association (NCA) is a notfor-profit organisation that represents a wide range of independent care homes throughout Nottinghamshire and Nottingham City. members homes come in all shapes and sizes and locations, nearly every town has an independent care home. Although in Nottinghamshire all nursing homes are provided by the independent sector. Uniqueness - Our members homes each provide their own unique service, but all enable older people to enjoy what is best for them. For example: those who felt isolated in their own home gain >>

continued

companionship; people who were struggling to cope alone receive 24-hour help; and others who may have felt like they were a burden can find themselves much more independent. Caring staff However it is our staff who make the difference: that smile on their face that makes those around them smile and feel valued; being dependable and sensitive to the needs of others also brings renewed confidence. Offering help with the things that are hard to do which makes a difference to residents lives and results in an important and highly fulfilling role. Being on hand 24/7. High standards Independent care homes provide care that is over and above the National Standards for Care Homes for Older People - caring about every aspect of residents lives including health, diet, exercise, hobbies and social activities. They aim to ensure that all residents continue to enjoy life whilst receiving the very highest standard of care. All homes provide personal services and registered nursing homes also provide 24/7 qualified nursing care. Some homes also provide dementia care, learning disability support and other more specialist care. Quality Our members are passionate about providing good quality. All independent homes are registered with the Care Quality Commission (CQC) and are subject to stringent inspections by the CQC, the local councils, fire officers, the health and safety executive, environmental health officers and, for nursing homes, by the NHS-PCT. Unsurprisingly our members have a vested interest in providing a high quality service to their residents. Independent government quality reports on every home can be found on the CQC website: www.cqc.org.uk. Good value Accommodation and care charges vary according to the service offered and required - but all homes aim to provide good value for money. You may be surprised how little high standards of care cost. Although higher quality care often does cost more. Details of the financial support available can be found elsewhere in this Directory. However

specialist independent financial advice should always be considered. Look for NCA in the listings If you are looking for a care home in Nottinghamshire why not visit one of our members care homes? Our website provides some useful information to assist you in choosing an independent care home: http://www. nottinghamshirecare.org.uk If you go to the Looking for Care section of the website there is a database of all homes to search. Please look out for our members throughout this Directory - you will find them marked as NCA. Activities of the NCA Nottinghamshire Care Association represents individual companies, groups of homes, individuals and charities in the independent care home sector. The NCA is committed to helping care providers to give the best possible service to the vulnerable people they care for ‒ working in partnership with others to make this a reality. The NCA promotes the care sector by lobbying politicians for the benefit of residents, members and staff. We lobby for equal, fair and consistent support of residents whether they are in the independent or publicly owned care homes. The NCA works with the statutory authorities to negotiate economic levels of fees from the government and local authorities. We also work with the Care Quality Commission, the local councils and NHS PCTs to raise standards and encourage training and development of staff. Choosing the right care home for Mum was made easier. • Nursing Care • Residential Care • Dementia Care • Respite Care

From the moment my mother and I first visited her Barchester home, we knew we had found somewhere pretty special. Elegant surroundings, delicious food and beautiful gardens were made even more appealing by the presence of like-minded residents with whom she can socialise and enjoy activities. For my part, knowing that she’s safe in the care of such a dedicated and compassionate team gives me great peace of mind. And for that I’m extremely grateful.

T hank you, Barchester! Why not call us or drop in for a chat over a cup of tea?

A NEW CHAPTER

CARE HOMES FOR LOVED ONES

ABOUT ASHMERE Our family owned and managed Care Homes for the elderly have a very personal feel to them. We organise regular outings and social activities for residents and relatives and are very much a part of the local community - even hosting lunch clubs. Meals are attractive and well balanced and our catering staff are happy to cook favourite dishes as well as specific diets. The professional care and comfortable facilities are second to none. We are sure you will agree it is the extra things we do that make us special. Our Health and Fitness Centre provides Hydrotherapy, Jacuzzi, Sauna, Aromatherapy, Hair and Beauty treatments. Most of the rooms are single and en-suite. Younger physically disabled beds are available while dementia care is a speciality. All homes are highly rated by CQC and NCC.
