Welcome from Shropshire Council Welcome to the 2012/13 Shropshire Care Services Directory which provides information for older people and adults with disabilities about the range of social care services available across the county. Shropshire Council’s Adult Social Care teams work with people aged over 18, and their families who may need help with their care. This could be because they have mental health issues, a learning or physical disability, a sensory impairment or because they have become frail and are struggling to continue to live independently and receive support at home or may need to live in a supported environment such as a care home or care home with nursing. Live Life : Your Way – Transforming Adult Social Care The transformation (sometimes called personalisation) of Adult Social Care is a national programme aimed to shift the balance of power from the local authority, over to the person who requires care and support. The programme will give people who use services more choice and control in who, when, and how they can receive services. In Shropshire we are now looking at the way we deliver services and this will mean some changes in how our services work in the future. Our transformation of adult social care programme is called ‘Live Life: Your Way’. We are looking at how best to provide information and guidance to everyone who needs extra support in order to offer ‘Personal Budgets’ to people who meet the eligibility criteria for social care funding. Please see page 6 for more information on funding and eligibility. The offer of Personal Budgets to older people, and people with disabilities, is via a system called ‘Self-Directed Support’. This means exactly what it says – the person who needs a service can make key decisions about their own support. The process involves having an assessment, and then (if you are eligible) you will be told an indicative amount of money (your Personal Budget) that you can spend on your care and support. The next step is to create a Support Plan, which lets us see how you plan to spend your budget. Providing your plan comes within the budget, meets your

needs and identified outcomes and will help you be safe and well, it should be signed off. The transformation of adult social care means that lots of people are working in different ways, and in ways that make the systems fair and equitable. We provide a wide range of preventative services described in this Directory including: home care, day care, equipment that can help with mobility and adaptations to your home. Depending on how you choose to manage your budget, some services can still be provided directly by us, and others by private or voluntary organisations. Where do I start? Assessing your needs Everyone must have an assessment of their care needs to find out whether you are eligible for social care. The assessment will cover all elements of care and support that you may need in your life. We will spend time with you talking about your circumstances and your needs, and work with you to identify things that are important to you in order to live a good life. You may wish to have a relative, carer or friend present at the assessment. Part of the assessment will be about finances. We will ensure that you are receiving all the benefits to which you are entitled. It will also identify if you are required to contribute to the overall cost of your care.
Part of the assessment will be a supported self assessment. This is where you explain what is important to you, and some of the things that you think you need some help with. A social worker will go through this with you when they do their part of the assessment. Some areas that are covered in the assessment are: issues of physical and mental health, personal care, mobility, safety, accommodation, financial affairs and social life. People who help or care for you, a relative, partner or friend may also request an assessment for themselves. This is called a ‘carers assessment’.
Fair Access to Care Services Regardless of your financial situation you are entitled to information and advice about any care needs you may have. We follow Government guidance called ‘Fair Access to Care Services’ (FACS), which has been issued to local authorities across the country. The aim is to make sure all decisions on who receives financial help with care and support are made fairly, openly and consistently. This means each Council has to set criteria to make sure help is allocated fairly and to those in greatest need. FACS eligibility criteria is divided into four bands representing the seriousness of the risk to your
independence: ‘critical’, ‘substantial’, ‘moderate’ and ‘low’. Currently you are only eligible for community care services in Shropshire if your needs are assessed as ‘critical’ or ‘substantial’. This ensures that people in the greatest immediate or longer term need will get most help. Some things are not covered by ‘Fair Access to Care Services’ including Blue Badges and disabled facilities grants. Maximising Independence A new system is currently being developed within Shropshire. It is called ‘Maximising Independence’. This is where you could receive some help on a short term basis to keep you independent, and in your home for longer. This may include supporting you with skills such as cooking, or helping with travel arrangements to avoid the intervention of
social care service too early. Your Maximising Independence Plan will be unique to you, so one size does not fit all! The Maximising Independence service has input from social workers, occupational therapists, and the Short Term Assessment and Reablement Team (START).
START (Short Term Assessment and Reablement Team) The START service provides a short term period of intensive assessment and reablement to people who want to remain living in their own home.

START (Short Term Assessment and Reablement Team) continued ways people can gain confidence and new ways of coping with day to day living tasks (like personal care or food preparation etc). START can also help identify whether support will be needed in the long term, to help people maintain their independence. They do this by mutually agreeing goals with the individual using the service, then recording these in a structured care plan. These goals could be achieved in a variety of ways. They could, for example, involve the implementation of helpful assistive technology in the home, which could help them take control of their medication and health needs or maybe getting in touch with community organisations which can provide physical or emotional support. Whatever the goals agreed, they will be focused on promoting and encouraging people’s independence and helping them lead fulfilling lives in their own homes. Obviously a person’s needs could change over the time the team are involved with them – and these can be discussed at any time.

The team’s aim is to achieve positive outcomes which will reflect the individual’s wishes, maximise their independence and enhance their quality of life. START work with district nurses, social workers, occupational therapists and other professionals, in order to support people to regain independence and stay at home. START will also work with any carers (family members or otherwise) who will be fully involved in developing the care planning process, if they and the person using the service so wish. Carers can be central to the team’s work. START could, for example, support people new to caring to gain confidence and coping skills in their role or provide some much needed respite for established carers, possibly identifying a need for future respite to be addressed. A carer’s assessment will also be offered to them. START believe that by offering a service that is tailored to meet people’s individual needs, people can avoid the traditional route of admission into intensive, long term, possibly residential support.
Community living Support from the NHS NHS Continuing Healthcare is arranged and funded by the NHS, where it has been assessed that an individual’s primary need is a health need. It can be provided in any setting. In a person’s own home, it means that the NHS funds all the care that is required to meet their assessed health needs. In care homes, it means that the NHS also makes a contract with the care home and pays the full fees for the person’s accommodation as well as their care. For further
information contact the Continuing Healthcare Team, Shropshire Primary Care Trust on Tel: 01743 277531. NHS Continuing Healthcare is provided if you have extensive healthcare needs. The cost will be covered by the NHS. Continuing Care is a package of care that involves services and funding from both the NHS and the Department of Community Services. You can ask for an assessment by health and social care professionals if you think you may be eligible for either NHS Continuing Healthcare or Continuing Care. They will visit you to identify and prioritise your needs and the assessment will be used to decide what services will be provided, where and who funds them. Regular reviews will monitor your care needs and if they change, your care and/or funding arrangements will change to meet them. Care in your home If you are finding it difficult managing at home, you could consider using the services of a home care provider. Once you have had an assessment, and received your indicative Personal Budget, you could include this in your Support Plan. Home care providers employ care assistants who can help you with bathing, washing and dressing as well as providing practical support with shopping, laundry and cooking meals. They should be trained in personal care and safety procedures, moving and handling, hygiene and infection control and first aid. If you require nursing care at home, many home care providers employ registered nurses. This is only one option of supporting yourself with care at home, another could be using your Personal Budget to employ a personal assistant to help with home care. Personal Budgets are explained further on page 26. Mentioned briefly on page 7, the Maximising Independence Team also offer a short-term period of support, with the ultimate goal of keeping you
at home for longer. The team will work with you to identify your longer term, ongoing needs. You will have a tailor-made Support Plan, suited to your own requirements, and the plan will be monitored and reviewed to ensure that it continues to meet your needs and desired outcomes. Age UK Shropshire Telford & Wrekin, or other voluntary organisations may also be able to help. Age UK Shropshire Telford & Wrekin’s Help at Home scheme, funded by Shropshire County Council enables older people who live at home to maintain their independence by providing practical help. Daily care/ domestic support Carers can call in on a daily basis to assist with any of the tasks described above. Depending on the level of help required their visits can be just half an hour or up to several hours. Generally, visits are available from 7am until 10pm. Some people will need multiple visits per day. Night services can also be provided. Rural areas may present particular difficulties and a travel charge will probably be made in addition to the normal hourly rate. Live-in care 24 hour live-in can accommodate people with a very high dependency on a permanent basis. It can also provide respite breaks for regular carers and short-term support following hospital discharge. In some cases it’s preferable and more economic to have a carer actually living in the home. This can be for a short period eg a week, or on an ongoing basis. Live-in care is also available to people with permanent physical or mental disabilities who require long-term ongoing care. All home care providers are regulated and inspected by the Care Quality Commission who publish inspection reports on their website www.cqc.org.uk. When considering a service it’s always a good idea to check their report. Before you make any decisions regarding the provision of care, be sure to contact several providers and ask for a ‘Service User’s Guide’.

Agencies • What experience does the agency have in your particular field of need? Can it supply references for you to check? • Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate. • How long has the agency been operating? • How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending? • How can you contact the agency in an emergency or outside office hours? • How hard or easy would it be to make a complaint and how are things then put right? • If this is a private contract ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this.

• You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding? • Carers should be fully trained or be in ongoing training. Ask the agency about their policies on this. • You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency. • C arers must be checked with the Criminal Records Bureau (CRB) and have a criminal records disclosure – make sure this is the case. Paying • If your care needs do not meet our eligibility criteria and have been arranged privately you should check carefully the fee rates charged and exactly what is covered.

Telecare Telecare uses a range of simple sensors matched to your personal needs which can help you to continue to live as independently and safely as possible. These are usually linked to a lifeline (a kind of telephone). Telecare can help you and your family in a number of ways, all of which are designed to support your health and wellbeing. One example is a bed sensor. This can be used in a range of situations, i.e. for people at risk of falling, or to alert a carer to assist. It detects a change in pressure, so if you have got up but are unable to get back to bed, perhaps after a fall, the sensor can raise an alert. However, if you get up at night and usually sit in a chair for a while or have a drink, then the system can delay the alert. If you have not returned after a period of time agreed with you, i.e. half an hour, it can then raise an alert. This avoids the risk of someone falling and being left on the floor all night, cold and in pain. Following an assessment of your needs, telecare could be part of a support plan to assist with things you might find difficult. It can support you or the person you care for by alerting you to certain situations, such as incontinence, or where a vulnerable person leaves the property at an inappropriate time, for example, the early hours of the morning. Or it can react to situations such as epileptic seizures or fire, where you need help from family, carers or the emergency services. Some examples of how telecare can be used are given below: • a short term measure as part of START when you need some support, i.e. after ill health to help you become as independent as you can; • a longer term measure to make you feel safer in your own home. One lady said ‘I don’t feel alone when I am alone. The telecare can let someone know if I have a problem’; • telecare can remind you to take medication; • telecare can raise an alert that the cooker hob has been left on; • telecare can detect gas leaks from the cooker/fire and raise an alert; • telecare can provide early warning of a sink or bath overflowing; • telecare can raise a call to a monitoring centre who can keep a phone line open to you in an emergency; • telecare can give people space and time on their own, instead of being checked up on just in case something has happened. One lady said ‘I love my family but I just want to have some time to myself’.

If a problem is detected then the system sends a signal via the lifeline to a call centre where an alarm can be raised 24 hours a day. The call centres uses trained operators, who can speak to you over a speaker on the lifeline, will have your details and know who to call in an emergency. This could be family, friends, carers or the emergency services. Alternatively, you could choose to have calls made directly to your chosen emergency contact. The Telecare alert process 1. You press the lifeline button or the sensor raises an alert. 2. The lifeline unit is activated and alerts the call centre via the telephone line. 3. The call centre answers your call and talks to you through the lifeline. 4. A response is arranged appropriate to your agreed needs. There is a small weekly charge for using telecare, depending on your circumstances. However, you may be able to apply for a subsidy to help with this. Your circumstances will be established by our adult social care staff through a financial assessment. Day centres We run day centres throughout the county, providing opportunities for older people, and people with disabilities to socialise, meet friends, and take part in activities. Day centres can also provide support for family carers. Your choice to attend a day centre could depend on the type of service and activities that the centre offers. As part of our ‘Transformation’ of Adult Social Care we are currently looking at ways we
can increase the choice of opportunities for people during the daytime, to give people the chance to engage in positive activities, increase skills and reach their full potential. When you are writing your Support Plan, it is important to consider what sort of activities you think would benefit you. There are organisations to advise you on the most appropriate day centre for your needs and wishes.
Support if you look after someone You may not think of yourself as a carer but anybody who helps a friend or relative with care needs, without pay for their services, is a carer. You may be entitled to help with your caring responsibilities, whether or not you live with the person you are caring for. As a carer, you can ask Adult Social Care to look at your situation and what support you might need to continue in your caring role. We call this a ‘carer’s assessment’. Who can receive help from Social Care? If the person you look after is eligible for community care services and the care you offer is ‘substantial and regular’ you are likely to be eligible to receive help from social care. How do you get help? If the person you care for has been seen by Adult Social Care, we should have asked you about the help you need. If this hasn’t happened you can call our First Point of Contact to request an assessment (see number opposite). During the assessment you will be asked about the help you give, whether you are able and willing to continue with this, what might be difficult and what help you need. It might be an idea to keep a diary of what you do for a week or two beforehand.

Respite care and short stays Short breaks can provide respite for the carer as well as a break for the cared for person. This might involve someone coming to your own home, a stay in a residential accommodation or a supported holiday away. The best option for you will depend on your needs, the urgency of the situation and personal
choices. This could be included in the carers support plan or the support plan of the person who requires services. A Carers Emergency Respite Scheme aims to provide peace of mind to carers through home-based care at short notice. Care can be provided for up to 72 hours.
Intermediate care Intermediate care differs from respite care. While respite care provides full-time care on a temporary basis, immediate care is designed to rehabilitate you and enable you to manage by yourself. Intermediate care, including care home fees, provided by the NHS, is free. It can be provided in your own home or in a single care home in Shropshire and is usually limited to a maximum of two weeks.
Based around a programme of therapy and treatment, intermediate care is designed to make you as independent as possible. The structured programme of support may mean that you do not have to go into hospital at all, or may enable you to get home faster after you finish hospital treatment, but cannot manage independently at home. Ask your care manager for further details.
Sheltered housing Sheltered housing is a practical step if you would like more security and companionship but don’t need to move into a care home and is usually provided by housing associations across the region. In a sheltered (or retirement) housing scheme you would live independently in a flat or bungalow built especially for older people within a larger complex, sometimes around a garden or communal facility and linked to a central control centre for security. Some schemes have access to a resident or nonresident manager who provides support, advice and a link to services for older people. After your needs have been assessed, you will still be able to receive care in the home if required. Sheltered housing is available to rent or buy and is usually provided by local councils or non-profitmaking housing associations.
Extra Care Extra Care covers a range of specialist housing models. It incorporates particular design features and has key guiding principles. Extra Care housing can be owned, rented, part owned and part rented and leasehold. Some developments mix types of tenures. Extra Care

includes private accommodation, eg a bungalow or flat, with access to domestic support, meals, recreational facilities and nursing care where necessary. Extra Care housing provides a flexible approach to care with security and 24-hour support from Social and Health Care Teams.

Specialist care Learning and physical disabilities With the introduction of Personal Budgets, everyone will go through the same format of assessment, regardless of whether you have a physical or learning disability, mental health issues, or whether you are an older person who requires additional care and support. Services are always designed around people and reflect the needs of those who use the service, their families and carers We aim to ensure people with disabilities have access to the same community facilities as everyone else. We are working closely with other areas of the community to strengthen community links. We offer residential, supported living and day opportunities as well as work experience, training and employment opportunities. A care manager will carry out the assessment (as described on page 7) with you, and your carer (if appropriate). You will then wait to find out what your indicative Personal Budget will be, and then proceed to develop a Support Plan to meet your identified needs and outcomes. Help will be available from the voluntary sector to complete
your Support Plan. Your Support Plan could include future accommodation needs, employment and meaningful day opportunities, respite and help to support people to remain in their own homes. Carers are offered an assessment in their own right and support services can also be provided to help meet their identified needs as a carer. Dementia care Usually, the initial point of contact to access care services is either via your GP (for health care solutions) or your local Adult Social Care office (for most other services). Your GP, as well as discussing your concerns, will be able to carry out a series of tests which will establish the nature of your condition and advise whether treatment is possible. Following a diagnosis of dementia, your GP may refer you to a specialist like a Consultant in Old Age Psychiatry, a Community Psychiatric Nurse or an occupational therapist with a special interest in dementia care. Support in the community As it can be best for the person with dementia to stay in their own home for as long as possible, a range of community care services is available to facilitate this, including the ‘Maximising Independence’ model, as mentioned on page 7.
Following your assessment, your Support Plan would help to identify the right level of support that you need. This could include help with cooking meals, home care, advice on equipment day centres, and many other options These services can be provided directly by registered providers to those who do not require our financial help, however we will still be willing to offer an assessment of need. You may have a choice of which agency you receive your care from whether or not we assist with the funding of your service. Residential dementia care If the time comes when remaining at home with support is no longer viable, you may wish to consider the option of residential care. Differentiating one home from another can be difficult but the quality and quantity of staff training is especially important, along with the turnover of staff as continuity of relationships in this type of home is critical.

Things to look for in care homes and care homes with nursing specialising in dementia. Residents The best indication of a good home is that the residents appear happy and responsive. • Are residents involved in activities or chatting? • Are residents clean comfortably dressed? • Do they seem alert and interested? • Do they talk to you as you walk round? • Are they encouraged to do as much for themselves as they can? Can you see any examples of this?
Access If the person with dementia needs or is likely to need equipment or adaptations you may want to check whether: • The corridors and toilets are wide enough for a walking frame or wheelchair. • There are suitably adapted toilets and baths. • There are ramps or a lift. • There is adequate signage and cues for different parts of the home such as dining room and bathroom.
Bedrooms You may want to find out whether the person with dementia can have a single room and whether: • Residents are encouraged to bring in some of their own furniture and possessions to increase familiarity.
• The bedrooms are bright and pleasant. • Residents can go to their rooms when they wish to be alone. • Residents are able to keep pets in their room or in other areas of the home. • Staff respect the right to privacy and knock on bedroom doors.
• Is there a TV or radio left on when no one is watching or listening? • Is there more than one room where residents can sit or where they can be quiet or see visitors? • Is there a garden where residents can walk safely?

Activities Residents should be stimulated without feeling stressed. • Are there opportunities for residents to help staff with small tasks if they wish? • Are the activities individualised and peoples’ likes/dislikes recorded in care plans? • Are activities available each day? • Are residents left to sit in front of the TV? • Are reminiscence activities available? • Are residents able to compile memory boxes?
• Do they have any training and experience in dementia care? • Do they make time to sit and chat to residents or talk to them while they are helping them with physical tasks such as washing and dressing? • Do they know about residents’ backgrounds, habits and interests? • Will the person with dementia have a member of staff particularly responsible for their care? • Is there a member of staff you can talk to about your own worries about the person with dementia? A manager who is caring as well as efficient can make all the difference to a home.
Look to see whether chairs are arranged in groups to encourage talking rather than placed in a circle round the outside of the room.

Mental health Services for people with mental health issues are usually accessed via the Community Mental Health Team (CMHTs) which are based on GP practice areas. An assessment of your needs will be done by a member of staff working in the Community Mental Health Team, usually a nurse, social worker or
occupational therapist, this should be similar to the assessment process as described on page 7. You will then develop a Support Plan, as described on page 21. End of life care If you are approaching the end of life, or are the relative or carer of someone who is dying, you may find it useful to have some information about what to expect, some support to help you through this difficult time, and access to some practical tools. The initial point of contact for services and support is through your GP. Hospices are another very useful provider of services and support. They provide in-patient care, day services and Hospice at Home services. The main provider of care in Shropshire is the Severn Hospice, based in Shrewsbury and Telford. If you live in the south-east of the county you may also wish to consider accessing the Compton Hospice in Wolverhampton. Care homes What is a registered home? Fundamentally, there are two types of residential care home: homes offering personal care only, and homes offering ‘care with nursing’. Following your assessment, your care manager can advise you on the different options that may be appropriate for you. Personal care only This type of home (commonly referred to as a ‘residential home’) will suit you if you are reasonably active but require help with tasks such as bathing, eating, dressing and moving about. District nurses visit to handle any nursing needs. It may be worth considering what your future needs may be – will the home be able to meet them in a few years time? Take along the care homes checklist on page 25 to prompt any questions you will have for the manager.

Care home with nursing This kind of home is suitable for those needing nursing care, where it will be delivered by qualified nursing staff alongside care workers. Nurses are on duty around the clock. Fees are generally higher
Inspection and registration of care services Since October 2010, all care providers must be registered under a new system, introduced by the Health and Social Care Act 2008, which will bring adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time. CQC will hold and publish up to date information about the compliance of adult social care providers with essential standards of safety and quality. The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others. Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. Each service’s report can be seen on the CQC

website: www.cqc.org.uk The focus of an inspection is on the standards of care that people who use the service receive and whether they are happy with their care. Virtually all inspections are unannounced. For care homes and care homes with nursing, the inspectors look at seven categories or ‘outcome groups’ based on national essential standards of quality and safety. These are: choice of home; health and personal care; daily life and social activities; complaints and protection; environment; staffing; and management and administration. Paying for care Personal Budgets People who are eligible for social care funding under Fair Access to Care criteria will be offered a Personal Budget which is an indicative amount of funding that could be made available to meet needs and achieve outcomes. This means that the person can exercise choice and control in how that budget could be used
in a way that works for them. They will be asked and supported to develop a Support Plan that sets out how they wish the funding to be used and how they intend to manage the money. This will need to be agreed by the care manager.

Ways in which a Personal Budget can be managed in Shropshire There are a number of ways you can receive your Personal Budget It is important to remember that more than one of
the options illustrated above can be chosen. The Support Plan needs to state clearly how much of the budget will be managed in a certain way.
Direct Payments A Direct Payment is money from a Council’s Social Services department to cover a specific assessed need. People who are eligible are given an agreed amount and can use it to employ a personal assistant themselves or through an agency to help support them. You aren’t allowed to use Direct Payments to pay for permanent residential accommodation - but you may be able to use them to pay for occasional
short periods in residential accommodation, if your Council agrees that is what you need. If you are applying for services for the first time, your social worker should discuss the Direct Payments option with you when they assess your care needs. Direct Payments are made directly into your bank, building society, Post Office or National Savings account.
Individual Service Funds The person can choose a service provider that they wish to provide their care or support. (For existing clients this may be their current provider if they are happy with them.) Some or all of the Personal Budget is then paid by us directly to the service provider as an ‘Individual Service Fund’. In practice, this is not very different to what happens currently but the important

difference is that the person (and/or those closest to them) are making a choice about that organisation. We will enter into a contract with that service provider and the person and the provider agree how the money will be used. The provider is accountable to the person and us to deliver the service they have agreed.

Council-managed account There are three scenarios whereby we may manage all or some of the money on a person’s behalf: 1. The person may wish to receive a service that is directly provided by us. They will have been informed of the cost of this service, and agreed with the service itself the detail (nature, frequency etc.) of what they wish to receive. We will then internally deduct that amount of money from the person’s Personal Budget. The person will have an agreement with that service detailing what will be provided. 2. The care manager makes a decision based on an assessment of risk that the service needs to be directly arranged, purchased or provided by us. There would need to be a good reason for this and the
resulting service still needs to reflect the person’s preferences within the Support Plan as far as is reasonable and it will still need to come within the Personal Budget amount. 3. The person does not wish to exercise choice and control at the current time and requests that decisions about their care and support are made by their care manager. As above, the care manager will need to work within the Personal Budget amount and complete a Support Plan that will need approval. (Note: it may be that at a later stage the person wishes to have more of a say so it should not be assumed that this is a permanent arrangement and should always be questioned at review.)
Third party-managed fund Some people will want greater control over the money than a Council-managed fund gives them, but will not want a Direct Payment and will not want to purchase support from an organisation. In these cases a third party managed fund would be the favoured option whereby all or some of the Personal Budget would be paid directly to a voluntary sector organisation that will manage the money on the
person’s behalf as a ring-fenced budget. Only reputable organisations that have been approved by us to provide this service will be used. Selection is currently underway and a list will be available shortly of those that are providing this service. The organisation may charge the person a small administrative fee that can be met out of the Personal Budget amount.
General advice about Personal Budgets If you are employing someone yourself, we strongly recommend you carry out a Criminal Records Bureau (CRB) check before employing the person, if the person is not known to you. When you complete your Support Plan, risk management and
safeguarding will be covered. A Personal Budget can only be spent on meeting your assessed needs. Your Support Plan will outline the way in which you plan to spend the money.
Self-funding advice If you have capital or savings of more than £23,250 (correct as of October 2012) you will have to pay for any care services you require but you are still entitled to an assessment of your needs by us. If you will be moving into care, you may be entitled to some of the following financial assistance and support, subject to an assessment of need.

Self-funding advice continued lower rate of £51.85 per week (correct as of October 2012) for those needing care by day or night, and at the higher rate of £77.45 per week (correct as of October 2012) for those needing care both during the day and night. Everyone is entitled to claim this benefit. A benefits check is part of your initial assessment for us. NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled to a non-means-tested NHS Nursing Care Contribution (currently £108.70 per week for the standard rate) towards the cost of your nursing care. This is paid directly to the home. NHS Continuing Care Funding You can receive continuing healthcare services in any setting, including your own home or in a care home. The NHS will pay if you need healthcare from a community nurse or a therapist as well as personal care to help you at home like having a bath or dressing. If you live in a care home with nursing (traditionally called a ‘nursing home’), you may qualify for the full
cost of your care home fees, including board and accommodation, under the National Framework for NHS Continuing Healthcare. If you live in a residential home and need help from a registered nurse, this will be free, covered by the NHS again. Running out of money If your capital is likely to reduce to £23,250 (correct as of October 2012), you must let us know well in advance as they may step in to help with your care fees. They must conduct an assessment of your situation and may make a contribution. If the home you have chosen charges more than this, you must find someone to help pay the difference – a ‘top-up’ payment. Whoever does this, whether family or a benevolent charity, they should realise that they may have to pay this for some time. Alternatively, you would have to find a home with lower fees. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself.

What will you have to pay for your care? Do you have capital or savings in excess of £23,250? What is capital? The value of your home is included in the assessment of your capital unless one of the following still live there: • Your partner or • A relative who is over 60 or incapacitated or • A child under 16 who you or a former partner maintain still lives there. Yes.... Ask us for an assessment. Any care homes you consider must be able to meet your assessed needs. If, apart from your property, your savings are less than £23,250, we can help with your care costs for the first twelve weeks. After this time you can apply for a loan to help you whilst your property is being sold. No.... We will contribute financially towards your care costs. How much? You will need to ask us for a Social Care Assessment. This will define your care needs and how they will be best met. Any care homes you consider must be able to meet these needs. We will also make an assessment of your financial situation. This determines how much they will pay and how much you will have to pay. Your choice of care home will be limited to those that accept our funding level. If you want to choose a more expensive home you would have to arrange for
a third party - such as a family member or charity - to ‘top-up’ the difference. You are not allowed to do this yourself if your capital is below £23,250. If you have capital between £14,250 and £23,250 you will pay £1 a week for every £250 you have above £14,250. Whatever your circumstances... Remember: • It is just your own financial circumstances that are assessed, not your partner’s. • Your assessment will be made up of two elements, a care needs assessment and a financial one. • A nursing home will generally be more expensive than a residential home. Consider claiming: • Income support • Pension Credit • Savings Credit (if you are over 65) Definitely claim: • Attendance Allowance, worth £51.85 a week. Comments, compliments and complaints All organisations need to know how they are performing. They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service. If you do need to make a complaint you should feel able to complain about any aspect of your care that affects your happiness or comfort. This could be about the way you are treated by a staff member or the quality of the food you are served. You can also make comments and suggestions about possible improvements to your surroundings and the services provided. Making a complaint should not be difficult for you. Providers are required under the National Essential Standards of Quality and Safety to have a simple and easy to use complaints procedure that they will be happy to give you. If you are concerned
about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any complaints made. If your complaint is about a breach of regulations, contact your local office of the Care Quality Commission, see page 24 for contact details. Out of county care homes Did you know you can choose a care home outside your home county? You may want to be closer to friends, family members or you may want to relocate to another part of the country. If your care home place is funded by the local authority, you can still chose a care home that is in a different county. You can use your Personal Budget to pay for this. The home you choose must be suitable for your assessed needs and comply with the terms and conditions set by the authority. The fees that your local authority will pay may vary, and your Support Plan will need to reflect this. We will only fund those with an assessed eligible need at ‘critical’ or ‘substantial’ levels, as explained

on page 6. They may offer you the fees you would receive if you remained within the county, or they may offer you the fees that the local authority would pay in your chosen region (you will find out about the amount of money that you are entitled to after your assessment – this will be the maximum amount of money that you have available to meet your support needs). If your indicative budget is not enough to cover an out-of county placement, you will have to speak to the care management team to see if there are any other options available. If you have to pay for services yourself (otherwise known as a self-funder), there are no restrictions (apart from your own budget).

Occupying a convenient and picturesque position next to the Uplands at Oxon Care Home, the Day Centre has been designed as a comfortable, safe and stimulating environment which is available for the use of all adults of mature years. It can also provide day respite – often much needed and for less than the cost of four hours domiciliary care – for families and carers of prospective clients. Our Day Centre is open 9am - 6pm Monday to Friday. Full or half day sessions need to be booked in advance. Call 01743 365183.
