Welcome from Warwickshire County Council This guide aims to explain how you can choose the care and support options you need to enjoy the best quality of life for as long as possible. Options are presented from a wide range of providers, many of which are independent commercial businesses, and some of which work with and alongside public services. We can provide information on a range of services to help you live independently in your own home or in a care home. The Warwickshire Directory Our online Warwickshire Directory currently holds a wide range of information for all ages, over two thousand entries about local organisations, groups and agencies that provide activities, advice, services and support for you – with more being added all the time. The Warwickshire Directory also allows you to search by keyword or location, or to browse through the navigation to find what you are looking for with the ability to leave feedback on providers, compare services, and create and save a shortlist of services for future reference. Young carers A young carer is someone under the age of 18 who helps look after someone. The person cared for may be a parent, grandparent, brother, sister, other relative or a close friend. Country parks There are several country parks in Warwickshire providing activities for all the family. Alongside the activities, there is plenty to do, such as walking, cycling on flat off-road paths, playing football, feeding the ducks, jogging around a way-

Library Service The Warwickshire Library Service provides a wide range of books and materials for all age groups. It also: • provides magazines and newspapers, DVDs, videos, CDs, talking books, local studies material, maps and plays in larger libraries; • adds at least 90,000 new books and other items to library stock every year; • promotes reading and literacy for children and adults through reading groups and a range of activities and events; • presents displays, exhibitions and events

promoting literature, reading, local studies, music and other topics; • participates in national campaigns such as a Black History Month, Adult Learners’ Week, Family Learning Week and the Summer Reading Challenge; • has a wide range of material about Warwickshire and surrounding areas; and • provides a wide range of learning opportunities for all ages. Help to live at home Equipment and adaptations Equipment and adaptations can help people live more productive lives. It enables people to carry out essential activities, with the aim of maintaining or improving independence. In Warwickshire, we currently provide the following equipment and adaptions to eligible people. Assistive technology Assistive technology is a range of equipment that can help people with a physical or sensory impairment, or a mental health condition. It helps people to continue to live at home safely and as independently as possible. You can buy your own assistive technology or you may be eligible for support from the Integrated Community Equipment Service.
Integrated Community Equipment Service The Integrated Community Equipment Service (ICES) helps you to live in your own home more easily by providing a range of equipment. This can range from a simple walking stick, to a more complex item like a mobile hoist. To receive this service, you must be assessed as eligible for social care support. If your needs change you can ask for a re-assessment, which may mean your equipment is exchanged for a different piece of equipment. Meals Service Meals services produce and deliver ready-made meals direct to your home. This type of service is for older and vulnerable people and aims to help

Domiciliary care Domiciliary care is personal and domestic care that helps keep you safe and comfortable in your own home. This could mean helping you to get up in the morning and go to bed in the evening. Organising your own domiciliary care If you are thinking about arranging your own domiciliary care, it is a good idea to ask any agencies you approach some questions. Making life easier at home If you’re having difficulties with everyday tasks at home, these simple solutions could make life easier and keep you independent. These are a starting point; other solutions are available which might better suit your needs.
Finding it difficult to get in and out of chairs? Try putting a piece of hard board under the seat base. Alternatively, buy chair raisers, a higher chair or an electric riser chair. Also try taking regular gentle exercise. If you can’t reach your windows, could you move furniture out of the way? Ask someone to help if you need to move heavy furniture. There are also tools for opening and closing windows. Struggling to keep warm/cool? Consider a fan or heater. Is your house insulated? Are there any draughts? You may also be eligible for the winter fuel payment from the Government. Visit www.gov.uk/winter-fuel-payment If you have trouble using light switches, think about replacing your switches for ones that are easier to use. Consider handi-plugs or light switch toggles, or there’s even technology available so that you can turn your lights on and off using speech. Use subtitles if you can’t hear the TV, or buy wireless headphones. Do you need a hearing aid? Request an assessment from your council.

Do you forget to take your tablets? Try making a note of when you’ve taken them, or buy an automatic pill dispenser or pill box. If you struggle to open your medicine, you can ask your pharmacist for advice on alternative packaging that could make it easier for you. Can you reach everything in your cupboards? If not, try a handi-reacher or rearrange your kitchen so the things you use most are within easy reach. If you are having problems with preparing food, consider buying ready-chopped options or try a chopping board with spikes. There are also longhandled pans, teapot tippers and lid grippers that could help. Palm-held vegetable peelers or a food processor might be a solution and meal delivery services are also available. Is eating and drinking becoming difficult? Large handled cutlery could help, or non-slip mats for the table. Lightweight cups and mugs with two handles could also be a solution. Using taps can be made easier by fitting tap turners. You could also consider changing to leverstyle taps which might be easier for you to use.

If moving whilst in bed is a problem, have you thought about using an over-bed pole? You might also want to buy a pillow raiser or change your bedding so it’s lighter.
If it’s hard to hold your toothbrush, try a toothbrush gripper. You might also benefit from having an electric toothbrush or sitting on a stool while brushing your teeth.
Is it becoming difficult to get dressed? If so, specially adapted clothing is available, or you could buy a long-handled shoe horn, a dressing stick or a button hook. If you are having a lot of difficulty, consider home support, see page 7.
You might like to buy a raised toilet seat, or a seat with a built in support frame if it’s hard to use your toilet. Flush lever extensions are also available.
Clocks are available with large numbers or lights if you can’t read the time in bed. You can also buy clocks that speak the time. If you are finding it harder to read in bed, consider an e-reader that allows you to change the font size. Some also have integrated lights. Look for bedside lamps with a step-on or button switch if yours are difficult to use. Do you struggle to get in and out of bed? You could learn new ways of moving around, purchase a leg lifter or a hoist or install grab rails for support. Seek advice about these options. If the bed is the issue, you could buy an electric adjustable bed or raise the bed to the right height.

Has it become more difficult to wash? Items are available, like long-handled sponges and flannel straps. You could also consider a slip resistant bath mat, grab rails, a half step to help you get in and out of the bath or a bath or shower seat. Tap turners can also be used in the bathroom.

Dementia services To help raise awareness of dementia and provide information on local services for people with dementia, Warwickshire County Council has led the development of the Warwickshire Living Well with Dementia information portal. The portal is a one-stop information website for anyone wanting to know more about dementia services and support available in Warwickshire. It is divided into sections relevant to different audiences. End of life care Although the subject of dying is often painful to contemplate, it is essential that you and your relatives discuss your wishes as you near the end of your life. Good planning for all your needs should mean that you can (if you choose) remain in your home or care home in your last days and not have to be admitted to hospital.

Delivering your care and support – personal budgets We want you to be in control of the care and support you need. If you have been assessed as being eligible for care and support, or you have been a customer in our reablement home care service and have
been assessed as needing continuing support, you will likely be offered a personal budget. A personal budget is an upfront sum of money to meet your care and support needs.
How do I use my personal budget? There are a number of ways you can choose to receive your personal budget:

Direct Payments Direct Payments provide greater choice and flexibility in the way you arrange the support you want, and you can include things we do not organise and directly pay for.
Individual service funds Once you know what your personal budget amount is, you may decide that you want to use an individual service fund instead of managing the money and organising services yourself. An individual service fund is where you choose a provider who will receive the money on your behalf and provide you with a service(s).

We will work with you to choose a provider that will help you meet the needs and outcomes agreed in your care and support plan. If you agree, we will give your personal budget (some or all of it depending on what has been agreed) to the provider on your behalf.
A personal managed budget You may decide that you prefer not to receive your personal budget as money, and instead you want us to make your arrangements for you.
The provider can only use your budget for your support and must account to you. This gives you some flexibility and control as you agree with the provider the exact detail of your support.
If you choose to do this, we will use the money to pay for your support. Your support will be limited to the organisations that we contract with, and the services we provide.

The Warwickshire Association of Care Homes The Warwickshire Association of Care Homes (WACH) represents a majority of independent care homes in the county. The Association is recognised by Warwickshire County Council, Clinical Commissioning Groups and the Care Quality Commission and liaises with them on a range of issues aimed at providing and maintaining the highest standards of residential care to the elderly people of Warwickshire.
› To prevent service users from being referred to facilities which are inadequate for their assessed needs.
Our aims are: › To promote and encourage the highest level of care for service users within its member homes embracing the fundamental principles of good care practice to improve and sustain service users overall quality of life.
› To represent the independent sector in the formulation, development, implementation and interpretation of community care policies for the county with Warwickshire County Council, health authorities, Clinical Commissioning Groups and the Care Quality Commission.
› To provide support and information to potential service users and their families on all matters relating to care assessment and placement. In particular to maintain our web-based vacancy reporting and information service on www.wpic.co.uk
› To represent members honestly and equitably in accordance with the Government directive ‘Building Capacity and Partnerships in Care’, the new Care Act (taking effect in April 2016) and that the guidelines are honestly applied to the people of Warwickshire.
