Fountain View Fitness features a wide range of strength equipment, free weights and cardio machines equipped with cardio theater, including: Row Machines, Incline Trainers, Spin Bikes, Recumbent Bikes, Olympic Weights, Ellipticals, Stepmills, Arc Trainers, Treadmills. Thirteen Total Access Inclusive Fitness machines are offered that are designed to be accessible to those exercisers who have physical or sensory limitations but are also able to be used by all exercisers.

Two lanes of the 25-yard indoor pool will be available for Fitness Center Members during regular pool hours. Members are notified in advance of any scheduled swim meets.

A free introduction to cardio, strength and selectorized equipment is available to all-inclusive members. Learn how to operate your favorite machines and receive guidance on proper form and technique. Members ages 12-13 must complete an equipment orientation before utilizing the fitness center. Visit the floor specialist at the fitness desk.

Access to gym space during scheduled open gym times, and anytime the gym is not in use for other programming. Scan in at the fitness desk when no attendant is present.

Hit your fitness goals with member-only pricing on personal training and nutritional counseling.

Kids Zone

Ages 6 months-11 years. Supervised care for your children while you work out. Kids Zone features toys, computers and staff trained in CPR/AED and First Aid. Reservations are required and maximum stay is 2 hours. Reservations can be booked at kidszone@csparks.org or call 630-784-6115.

Premium Locker Rooms

Relax in our saunas after a tough workout only accessible to all-inclusive members. Our premium locker rooms also include keyless lockers (no need to bring a padlock) and towel service.

Meet with one of our Personal Trainers and/or Nutritionist for a one-hour free consultation to discuss your health history and personal goals. Get tips based on a consultation and discuss a tailored program to fit your individual needs. Kids have a blast learning new games, sports or creative activities at ActivKids, our Before and After School recreation program for kids in grades K-5. Kids love ActivKids for the fun and friends and parents love knowing their children are safe, supervised and feel at home in the familiar setting of their own school. Good for five Before or After School admissions $65. Flex Passes cannot be utilized for Monday, After School. Flex Passes can be used for either Before and/or After School ActivKids. To use the Flex Pass for After School ActivKids, call your child’s school office and site a minimum of 24 hours in advance to ensure enough staff and supplies. No call is necessary for Before School ActivKids. Same day usage is not allowed for After School.

Your monthly payment will be calculated based on the days of the week your child is registered for and total school days during the year they will attend. Payment amounts already exclude school holidays, breaks and non-attendance days. Total registration is divided into equal monthly payments. EZ pay form required. If your child is registered for less than 5 days per week your monthly payment is less.

One-time nonrefundable registration fee of $25 due at time of registration. Before September 1, 2017

First month’s payment is due at registration. All subsequent payments are automatically withdrawn via EZ Pay on the first of each month beginning September 1, 2017 – May 1, 2018.

How to Register To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available online, at your school’s ActivKids site, or at the Fountain View or Simkus Recreation Centers. Registration must be at least 24 business hours prior to your child’s start date. A minimum of two weeks' advance notice is required if your child needs special assistance from a WDSRA aide (1:1 support staff). More information can be found in the WDSRA section of this guide.

Flex Pass

Days Off Club ActivKids Day Off Club dates follow the District 93 school calendar. Most dates coincide with other school districts such as U-46, D200, and D25. Take part in field trips, special events, and enjoy crafts, games and physical activity. For complete field trip list see csparks.org. Location: Simkus Recreation Center

The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants registering for Forever Young trips, outings and activities must recognize that there is an inherent risk of injury arising out of this activity. You are solely responsible for determining if you are physically fit and/or adequately skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

Understandably, not all hazards and dangers can be foreseen. Participants must understand that depending upon the particular activity, certain risks, dangers and injuries due to acts of God, inclement weather, slips and falls, inadequate or defective equipment, inadequate supervision or instruction, premises defects, carelessness, horseplay, vehicle accidents and all other circumstances inherent to recreational activities/programs exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety.

Become a stronger and more confident gymnast. Develop a foundation for mastering tumbling, vault, beam, and bars. Skills taught are based on USAG levels. Parent Code of Conduct By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at the preseason parent meeting.

Registration - tee ball Registration is by grade level. Players are to be registered for the grade the player will be in for 2017/2018 school year. Preschoolers must be 4 years old by program start date. Participants can participate in Instructional Tee Ball and Instructional Soccer without conflict.

Meet and Greet The meet and greet is a set time where players, parents, and coaches meet each other and hand out any necessary information, and in some cases will receive their uniforms.

Registration - soccer Registration is by age. Players are to be registered based on their age as of September 1, 2017.

Player Evaluation Players get rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend.

Registration - baseball Registration is by age. Players are to be registered based on their age as of April 30, 2018. Registration - softball Registration is by age. Players are to be registered based on their age as of December 31, 2017.

Playing up Playing up is not allowed for instructional soccer or tee ball. For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to: • The player must register at his or her age or grade. • A request to play up must be made to the program supervisor. • The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/ level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation). Placed on a Specific Team No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However, one mutual friendship request will be honored for players in an instructional league, Pinto (8U) baseball, or U8 soccer. Forms are available at csparks. org/youthsports-friendshiprequest/ Request deadlines are the same as registration deadlines (not early bird).

Registration Deadlines Players placed on a wait list are not guaranteed placement on a team. If you register before team formation the player will be moved off the wait list if there are enough to form an additional team. However, if you register after team formation, or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game. New Swimmers Select a swim class level based on your child’s age and previous lesson history or attend a skill test day. Returning swimmers — select a swim class level based on your child’s previous swim lesson report card. Custom and Private Lessons These lessons are specially scheduled for you. Should you fail to show up for a lesson without notifying the instructor at least 24 business hours in advance, you forfeit the lesson. Cancelling a lesson for any reason requires 24 business hours. One make-up class is allowed if above communication is properly followed. Lessons not finished by the end of the session expire. Combined Classes Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Coordinator at 630-784-6187. Parent Attendance Stay for the entire swimmer’s class, in case of emergency where consent is required.

Private Lessons

First Day First day arrive 15 minutes early for check-in. Do not bring — swimming aids; lifejackets, water wings, flippers, or snorkel masks that cover the nose. Snacks and treats are recommended to be eaten after lessons. Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks. Activities with recurring or monthly payments must have an EZ Pay Agreement on file. Your payments are charged to your credit/debit card. For more information, please visit our Registration Services Team at Simkus or Fountain View Recreation Centers to complete an EZ Pay authorization form.

Photography Policy

Carol Stream Park occasionally takes photographs and videos of participants at our parks or facilities, in park district programs, classes or events for marketing purposes.

The Carol Stream Park District staff has made every effort to prepare our marketing material as accurately as possible, however, errors can occur. Circumstances may require that adjustments be made to programs, fees, schedules, etc. The Park District reserves the right to make any such adjustments. The Carol Stream Park District apologizes for any inconvenience these errors may cause.

Lost & Found

Contact us if you lost something at one of our parks (630-784-6107) or facilities (630-784-6100).

Avoid Cancelation

Most classes require a set minimum number of participants. Register at least a week prior to the start date to avoid cancellation.

Program Capacity

All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

FREE ADMISSION

Waiting Lists

Once the maximum enrollment is met for a class, a waiting list will be formed. If space becomes available or additional classes are created, you will be contacted. Payment is due when being waitlisted. Credits will be issued after the first class for those who are not able to be registered.

Inclusion Services

Inclusion Services are designed to provide the least restrictive environment while providing the maximum opportunity to participate in regular park district programs. There is no charge for WDSRA’s support. To register for services, simply indicate that special accommodations are needed on the park district registration form. Two-week advance notice is requested for inclusion support.

Carol Stream Park District will make reasonable accommodations in recreation programs to enable participation by an individual with a disability that meets essential eligibility requirements for that leisure program and facilities. Insurance

The Park District does not carry medical or accident insurance for program participants or park patrons. Review your personal health insurance plan to be certain that you and your family have proper coverage.

Financial Assistance

We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications available at Fountain View Fitness Center and Simkus Recreation Center.

Smoking Policy

The District’s philosophy is to promote health and a clean air environment. The use of any and all tobacco products is prohibited in all park district buildings and facilities and at all parks and trails. Carol Stream Park District joins other park districts in Illinois with a tobacco-free policy. To read the full policy, go to csparks.org.

Refund and Withdrawal Policy

It is the District’s intent to provide quality recreation programs and services. The District recognizes however, that there may be occasions when the event/service doesn’t match a customer’s needs, or when the customer is unable to participate in an activity. For those occasions, the following withdrawal policy will apply. All requests for refunds will be assessed a ‘Service Fee’ of 10% of the original, total registration fee - $5 minimum / $50 maximum. Or 10% of remaining credit balance - $5 minimum / $50 maximum. No refunds under $5; credit will be placed on account. For monthly/quarterly payments, or memberships, there is no refund of the prior or current period; a cancellation of the future months’ charges will be allowed. No refunds after 2nd meeting of class/program. Forever Young Programs – Service Fee will apply after the payment deadline, and refunds may be issued prior to the event as long as Supervisor approval is given. No refunds after event. No refund of annual memberships including but not limited to: Walking Track, Lap Swim, Coral Cove Water Park, or Bark Park. Exceptions in which a Withdrawal Service Fee will not be assessed: Medical note exception is valid any time prior to the start of the last class and will not be subject to a ‘Service Fee’; a prorated refund or credit will be issued. Transferring to another program in lieu of receiving a refund. The credit available to transfer towards another program will be calculated on the prorated balance from the original registration. Leaving on account in lieu of receiving a refund. If funds are not used by the end of any calendar year, the credits will be returned less the applicable ‘Service Fee’. Refunds are not offered: if participant did not attend all or any portion of the class/program (forgot about class, moved and did not withdraw) for Summer Camp, ActivKids, or Preschool because participant’s vacation, illness, or any other absence for returned (new or used) uniforms or costumes for Enrollment, Administration, Registration, or Annual Fees; nor for Tickets for Punch Passes (that have no expiration date) includes Pool, Open Gymnastics, Open Baton for “Private” Recreational Packages: Personal Fitness Packages, Customized/Private Swim Lesson Packages, Private Voice, Private Gymnastics Lessons for trips – no refund unless customer provides a replacement. If replacement is found, Service Fee will still be assessed to the original participant for Adult Athletic Leagues –if a team chooses to drop out Additional Notes: No prorated registration, enrollment, or administration fee for late registrants.

Meet Your Park Board of Commissioners The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait ― a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly.
