The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait - a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings begin at 7:00pm.

We’ll begin at Skokie’s Scream Scene, then St. Thecla’s Hair Razor Haunted Scenes and finally to St. Pascal’s Catacombs Haunted House all in one night! We will stop for dinner at a local restaurant so bring a little extra money for dinner or a snack. Zombie Outbreak Dash

Do you have what it takes to survive our zombie infested obstacle course? In this half mile dash, you will encounter many trials to test your abilities. You will navigate through barriers and obstacles while trying to evade the zombies that wait for you! A monster bash will be held to celebrate your victory, enjoy music and dancing with undead friends. Running and hiking gear are strongly encouraged as the terrain will be loose and uneven. Pumpkin Plunge

Everyone needs a pumpkin to decorate for Halloween! And what better way to get one than by plunging into the Fountain View Pool. Pumpkins of various sizes will be floating in our pools and we need you to get them out. Our grand prize will be an extra-large pumpkin sunk to the bottom of our 12ft deep pool. Come see if you can retrieve it! Pumpkin decorating will be available in room 121 following the plunge. Children will be broken up into age group waves as listed below.

Howl’oween Dog Parade and Costume Contest

Dress up your pooch and parade around our Bark Park! Categories include best, funniest, tv/movie character and cutest couple (dog and owner). Judges will be looking for creativity and showmanship and prizes will be awarded. Owners must secure their dog on a six foot leash. Event may be canceled due to inclement weather or low registration. Don’t be scared to attend our family Halloween event! The Simkus Recreation Center will be transformed into a ‘spooktacular’ palace, oozing with Halloween activity. Wear your costume and visit the mad science lab, make spooky crafts, eat ghoulish snacks and hunt for candy in the haystack! The fest also includes games, bounce house, ghost stories and if you dare.....take a stroll in our scary Coral Cove Spook Walk!

Cardboard Boat Regatta

Come out and see if you can build a boat entirely out of cardboard! The first ever Fountain View Indoor Pool Cardboard Boat Regatta will consist of races in the lap pool.The overall race winner will claim their victory prize. Boats and paddles must be made entirely of cardboard and duct tape. Building supplies will not be provided by the park district. Boats are to be constructed before the race begins and brought to the water park. Fee: 43998

Santa Letters

Experience the excitement of opening a personalized letter from Santa Claus! Your child can drop off a letter to Santa that we will deliver to the North Pole. Look for the special mailbox inside Simkus and Fountain View Recreation Centers from Nov 27-Dec 11. Please make sure that your child’s full name and address are included in the letter so that Santa knows where to send his correspondence or you can email him at santa@csparks.org. Don’t miss out on this special holiday tradition that is sure to put a smile on your child’s face! 4

WINTER EVENTS Polar Express

All Aboard! Come along on our annual train ride for a reading of the children’s classic story ‘Polar Express’. Bring a copy of the book and read along with us if you wish. We will ride from the Roselle train station to the ‘North Pole’ (Elgin/Big Timber). Santa will then join us on the train for the ride back to Roselle and sing holiday songs with us. Once we are back at the station, Santa will have plenty of time to visit with each child and hear their holiday wishes. Refreshments will be served and each child will receive a small keepsake. A parent or guardian must accompany children on the train and adults will also need to be preregistered for this event. Please arrive 20 minutes prior to departure. Parking is available. The deadline for registration is November 1. Santa RV Visits

Why wait in line at the shopping mall to sit on Santa’s lap when Santa can come see you? Surprise a child with a visit from Santa right to your house. Santa will arrive in his custom RV and listen to your child’s holiday wish list inside the RV! Due to his heavy schedule, Santa can only stay for five minutes in front of each house and will be able to visit with immediate family members only. You will need to be available 5:30 to 9:00pm. Please register by December 13. The program is open only to residents of the Carol Stream Park District. Get ready for fun on the range! Challenge yourself to learn the sport of golf. Lessons focus on basic swing, chipping, putting and pitching techniques. Participants should bring a five or seven iron to class. Balls are included in registration fee.

Improving your skills with passing, overhand serving and spiking is easy! All students will receive a 6-point evaluation and a youth rating at the end of the course. No class Nov 26.

Volleykidz is an introductory class for children 2nd-4th grade. The lesson plan introduces the pass, set and spike using a very light volleyball. Volleykidz will improve each child’s hand/eye coordination and left/right coordination through the use of fun drills and games.

Boxing

The HPPD Boxing Academy develops self-confidence and selfrespect in a safe and fun environment. Participants will be taught how to perform better and get that competitive edge. Learn how to wrap hands like a boxer, stance, defensive maneuvers, rhythm, coordination, nutrition, punch/counter punch combinations, selfdefensive techniques and footwork techniques. Can I request for my child to be placed on a specific team?

How do I register? Register online at csparks.org or use the registration form in this guide. By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at their preseason parent meeting.

How do I register for soccer and tee ball leagues? Registration is by grade level. Players are to be registered for the grade the player will be in for 2015/16 school year. Preschoolers must be 4 years old by program start date. Participants can participate in Instructional Tee Ball and Instructional Soccer without conflict.

How do I register for youth baseball? Registration is by age. Players are to be registered based on their age as of Apr 30, 2016.

How do I register for youth softball? Registration is by age. Players are to be registered based on their age as of Dec 31, 2015.

No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However one mutual friendship request will be honored for players in an instructional league, Pinto (8U) baseball, or grade 1 & 2 soccer. A park district request forms must be used for these requests and the guidelines listed on the form must be followed. These forms are available at Simkus Recreation Center and on our web site. Request deadlines are the same as registration deadlines (not early bird).

How do I volunteer to help? Volunteers are used for the following positions: • • • •

What is the meet and greet?

Can my child play up?

The meet and greet is a set time where players, parents, and coaches meet each other and hand out any necessary information, and in some cases will receive their uniforms.

Playing up is not allowed for instructional soccer or tee ball. For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to:

What is the player evaluation?

1. The player must register at his or her age or grade. 2. A request to play up must be made to the program supervisor. 3. The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/ level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation).

Players are rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend.

Staff Positions - Umpires and Referees Applicants must be at least 16 years of age. Contracts are available at our Simkus or Fountain View Recreation Centers and online at www.csparks.org. Call 630-784-6100 for further information.

What happens if I register after the deadline? Players placed on a wait list are not guaranteed placement on a team. If you register before team formation, the player will be moved off the wait list if there are enough to form an additional team. However, if you register after team formation, or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game.

Illinois Girls Lacrosse League

This is a league run by women in lacrosse who want to help girls reach their potential in a fun and safe environment. The goals of the program are to teach the rules of lacrosse, promote teamwork and sportsmanship, and foster personal development in a respectful environment led by positive coaching. All players receive equal playing time and play all positions. Practices are 1.5 hours and games will be played on Sunday afternoons. Sunday game locations rotate within your division. Sunday, September 7th will be the first practice of the Fall season for everyone and games will begin the following Sunday, September 13th. YOUTH AQUATICS Swim Lesson FAQs

Where are lessons held? Swim lessons are held at Fountain View Recreation Center, 910 N. Gary Ave., Carol Stream, in the new 25-yard indoor pool and warm water pool. How do I register? • New swimmers - select a swim class level based on your child’s age and previous lesson history or attend a skill test day listed below. • Returning swimmers - select a swim class level based on your child’s previous swim lesson report card. Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Swim Lessons Coordinator at 630-784-6100 ext. 195. How does my child progress through levels? Swimmers will receive a report card at the end of the session indicating which level to register for in the next session. What happens on the first day? Arrive 15 minutes early for check-in. Please stay for the entire swimmer’s class, in case of emergency where consent is required. Please bring: Towel, swimsuit already on child; non-potty trained participants must wear a swim diaper with tightly fitting plastic pants under swimsuit; report card from previous session. Do not bring: Swimming aids; lifejackets, water wings, flippers, or snorkel masks that cover the nose. Snacks and treats are recommended to be eaten after lessons.

Refunds and make-ups will not be given for classes missed or forgotten.

The Dolphins Swim Team is a volunteer based program for youth who want to join the world of competitive swimming. The Dolphins will be competing in the Northern Illinois Swim Conference during the winter season along with 14 other park district teams. Team swimsuit is not required to participate in the program. Swimsuit fee is not included in resgistration fee. This team competes from October through the first week in March. Anyone registering for the Dolphins after the first week of practice will pay a prorated fee. Parents are required to volunteer at swim meets.

Cardboard Boat Regatta

Come out and see if you can build a boat entirely out of cardboard! The Fountain View Indoor Pool Cardboard Boat Regatta will consist of races in the lap pool.The overall race winner will claim their victory prize. Boats and paddles must be made entirely of cardboard and duct tape. Building supplies will not be provided by the park district. Open Gymnastics

Can I watch a gymnastics class? Fall 2015 classes are 13 weeks long. Parent viewing opportunities are Dec 14-19. No classes Nov 23-28. We use an additional room for tumbling to expand our classes and have more space. Participants may travel back and forth from the Gymnastics Room to Room 5.

How do I choose which class level to register for? Assessments are required for Grasshopper, Beginner, Beginner II, Advanced Beginner, and Novice levels. Assessments are also required for any of these levels if more than one session has been skipped. Call (630) 784-6105 to schedule a time.

Can my other children participate? Only the registered child will be allowed to attend and participate. During parent/child classes and parent observation week, please make alternate arrangements for siblings. Siblings under the age of 6 may not attend evening class observation.

What happens if I need to miss a class? Due to our specific student/teacher ratios, make up classes cannot be issued unless a class is canceled by the Park District.

When is the registration deadline? Registration for the spring session classes will be accepted until Oct 13 (except SOCS competitive team).

Springers compete in the Illinois Park District Gymnastic Conference at the tumbling, compulsory, and optional levels. Must have Head Coach permission to register. YOUTH DANCE Recital Dance Classes FAQs

When are dance recital classes? Dance recital classes run from September through May and perform in the annual spring dance performance. When do I register? Recital classes begin in September. Registration is accepted until November. How much does it cost to register? A $47 recital costume fee is due at the time of registration. Costume fee is per registered dancer and is non-refundable beginning December 1. How can I pay for recital classes? Payment for recital classes can be made in-full at the time of registration, or monthly EZ pay payments can be arranged. When is the dance recital held? The 2015 Dance Recital will be held on Friday, May 6 at the Wheaton Academy. What does my child wear to class? Girls may wear leotard, tights, pants or capris depending on the class. Boys may wear athletic pants or shorts, t-shirt and socks. Appropriate shoes are required for each dance discipline. Instructor will discuss dress code at first class. No classes will meet Nov 23-28, Dec 21-Jan 2, and Mar 28-Apr 2.

Seasonal Dance Classes Instructional dance classes are offered on a seasonal basis. These classes do not perform in annual spring dance performance. Parent observation will be held during the last class of the session. Only the registered child will be allowed to attend and participate in the dance class. Combination classes are a great way for students to experience different types of dance in one instructional class. Dancers will enjoy this interactive class that builds on basic movements and progressions.

Introduce girls to poms dance and establish team participation. Practices are held once a week. Performances will be during winter youth basketball games starting in January on Saturdays or Sundays. A schedule will be distributed. Two payments will be made at registration and in January. Participants will own their own uniforms and must be purchased at an additional cost of $35. Balanced Curriculum: Through education, recreation and fun, our preschool program offers a balanced play-based learning approach perfect for first-time students! Structured and unstructured activities provide stimulating handson learning experiences to promote your child’s physical, emotional and intellectual growth. Creative daily lesson plans focus on the development of necessary prekindergarten skills such as letter and name recognition, printing, cooperative play, socialization, listening and more. Community Environment: Since 1977, the Carol Stream Park District Preschool program has been a community tradition. Preschool classes are held at Elk Trail Recreation Center, our private preschool center which neighbors the open fields, recreational trails and ponds of Mitchell Lakes Park. The friendly neighborhood setting of our preschool center creates a safe and welcoming learning environment for all students. Our well-experienced, passionate and enthusiastic teachers are committed to working together to provide your child with the most enjoyable learning experience possible. Registration & Payment: A special registration packet must be completed to register for this program. Packets are available online at www. csparks.org or pick one up at Fountain View or Simkus Recreation Center. Additional Information: Our program follows the District 93 school calendar September-May. Parents/Caregivers must follow the DropOff and Pick-Up procedures at Elk Trail Recreation Center. ActivKids Before & After School 2015-16 Your kids will have a blast learning new games, sports or creative activities at ActivKids, our before and after school recreation program for kids in grades K-5. You will love ActivKids for knowing your children are safe, supervised and feel at home in the familiar setting of their own school. ActivKids is convenient, too. Flexible morning drop-off and evening pick-up times mean no rushing to beat the clock. The ActivKids Before and After School program partners with Community Consolidated School District 93 to offer childcare at the elementary schools that accomodates the new school hours.

To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available at your school’s ActivKids site, at csparks.org/activkids or at our Simkus and Fountain View Recreation Centers. Please register a minimum of 24 business hours in advance. WAIVER REGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity. WARNING OF RISK Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety. All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.
