The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait - a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings begin at 7:00pm.

SPECIAL EVENTS Doggie Egg Hunt & Spring Bonnet Parade at the Bark Park

Hunt for doggie treat-filled eggs with your four legged friends! Dress your canine in his or her best spring bonnet and join the parade! Take a picture with the Bunny with your own camera! Flashlight Egg Hunt

Hunt for eggs under the moonlight at Coyote Crossing Mini Golf. Bring a flashlight, basket to collect your eggs and camera to snap a photo with the Bunny and Cody Coyote. All Star Sports Instruction’s prep soccer camp is a set of classes over a week to help prepare your young soccer player for the upcoming soccer season. The week long camp is made to go over the basics of the sport to help those new to the sport and hone the skills of those already used to the sport. Along with this we will work on skills to improve their in-game play as in learning the different positions and how to best help their team. Need a more flexible camp schedule? Only pay for what you need when you register for a Flex Pass! Simply choose the exact dates your child will attend and register online or at Simkus or Fountain View Recreation Center. Registration should be completed at least 48 hours in advance. Emergency Flex Passes are available within 48 hours, space permitting, for an additional $5. LEAGUES & TOURNAMENTS YOUTH SPORTS FAQs How do I register? Register online at csparks.org or use the registration form in this guide. By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at their preseason parent meeting.

How do I register for soccer and tee ball leagues? Registration is by grade level. Players are to be registered for the grade the player will be in for 2014/15 school year. Preschoolers must be 4 years old by program start date. Participants can participate in Instructional Tee Ball and Instructional Soccer without conflict.

How do I register for youth baseball? Registration is by age. Players are to be registered based on their age as of April 30, 2015.

How do I register for youth softball? Registration is by age. Players are to be registered based on their age as of December 31, 2014.

Can I request for my child to be placed on a specific team? No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However one mutual friendship request will be honored for players in an instructional league, Pinto (8U) baseball, or grade 1 & 2 soccer. A park district request forms must be used for these requests and the guidelines listed on the form must be followed. These forms are available at Simkus Recreation Center and on our web site. Request deadlines are the same as registration deadlines (not early bird).

How do I volunteer to help? Volunteers are used for the following positions: • • • •

League Coordinators Managers/Head Coaches Coaches/Assistant Coaches Tournament Team Managers (Baseball/Softball/Soccer)

What is the meet and greet?

Can my child play up?

The meet and greet is a set time where players, parents, and coaches meet each other and hand out any necessary information, and in some cases will receive their uniforms.

Playing up is not allowed for instructional soccer or tee ball. For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to:

What is the player evaluation?

1. The player must register at his or her age or grade. 2. A request to play up must be made to the program supervisor. 3. The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/ level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation).

Players are rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend.

Staff Positions - Umpires and Referees Applicants must be at least 16 years of age. Contracts are available at our Simkus or Fountain View Recreation Centers and online at www.csparks.org. Call 630-784-6100 for further information.

What happens if I register after the deadline? Players placed on a wait list are not guaranteed placement on a team. If you register before team formation, the player will be moved off the wait list if there are enough to form an additional team. However, if you register after team formation, or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game.

YOUTH AQUATICS Swim Lesson FAQs

Where are lessons held? Swim lessons are held at Fountain View Recreation Center, 910 N. Gary Ave., Carol Stream, in the new 25-yard indoor pool and warm water pool. How do I register? • New swimmers - select a swim class level based on your child’s age and previous lesson history or attend a skill test day listed below. • Returning swimmers - select a swim class level based on your child’s previous swim lesson report card. Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Swim Lessons Coordinator at 630-784-6100 ext. 195. How does my child progress through levels? Swimmers will receive a report card at the end of the session indicating which level to register for in the next session. What happens on the first day? Arrive 15 minutes early for check-in. Please stay for the entire swimmer’s class, in case of emergency where consent is required. Please bring: Towel, swim suit already on child; non-potty trained participants must wear a swim diaper with tightly fitting plastic pants under swim suit; report card from previous session. Do not bring: Swimming aids; lifejackets, water wings, flippers, or snorkel masks that cover the nose. Snacks and treats are recommended to be eaten after lessons.

The Dolphins Swim Team of Carol Stream Park District is a volunteer based program for youth who want to join the world of competitive swimming. The Dolphins compete in the DuPage Swim and Dive Conference along with 12 other park district swim teams. Team swimsuit fee is not included in registration fee. Team swimsuit is not required to participate in the program. Swim Suit sizing will be held at the team Parent and Swimmer meeting May 5. Swim meets are held Wednesday evenings and Saturday mornings. Open Gymnastics

Spring 2015 classes are 8 weeks long. Parent viewing opportunities are May 26-June 1. No classes Monday, May 25. We use an additional room for tumbling to expand our classes and have more space. Participants may travel back and forth from the Gymnastics Room to Room 5.

The Gymnastics room will be open for extra practice. Participants must only work on skills they can successfully complete without a spotter. An adult must sign a waiver for all children under 18 years before they can participate. A minimum of six participants is required. Open Gymnastics daily visits and Open Gymnastics Passes are available for purchase at the Simkus Recreation Center. For all levels. For cancellations call (630) 784-6100.

Can I watch a gymnastics class?

YOUTH DANCE Seasonal Dance Classes Instructional dance classes are offered on a seasonal basis. These classes do not perform in annual spring dance performance. Preschool classes are held at Elk Trail Recreation Center, our private preschool center which neighbors the open fields, recreational trails and ponds of Mitchell Lakes Park. WAIVER REGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity. WARNING OF RISK Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety. WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK Please read this form carefully and be aware that in signing up and participating in the programs listed, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a Result of participating in any and all activities connected with and associated with these programs (including transportation services and vehicle operations, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ ward may have (or accrue to me or my child/ward) as a result of participating in these programs against the Carol Stream Park District, including its officials, agents, volunteers and employees. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature. Participation will be denied if the signature of adult participant or parent/guardian and date are not on this waiver. REGISTRATION DATES: Residents
