Save Our Streams

Online: Browse the full activity guide online, register for activities and pay by credit card at csparks.org In person: Pick up an activity guide; complete a registration form and pay by cash, check or credit card at Simkus Recreation Center. By Mail: Receive a guide by mail, complete a registration form and mail the form and check to Simkus Recreation Center, 849 W Lies Rd, Carol Stream, IL 60188

Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/ AMEX/DISC). Make checks payable to the Carol Stream Park District; There is a $20 fee for returned checks.

Program Capacity All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting Lists Once the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Refunds will be processed after the first class.

Insurance The Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.

Did you know that the storm water drains in our strrets drain directly to local waterways? You can help save our streams from pollution by keeping pollutants, such as used oil from cars, cleaning chemicals and even grass clippings (which often contain fertilizer harmful to marine life), out of the drainage system.

Electronics Recycling Now Law A new electronics recycling law came into effect in 2012. The new law requires that most electronic items be recycled and not included with regular trash. DuPage County has several sites available to drop-off electronics for recycling, including: Glendale Heights Public Services Garage, Hanover Park Public Works Facility and Wheaton Public Works Storage Lot. The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by the community to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait - a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/ or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings open at 7:00 pm.

JustPlay! Family Fun Run Sunday, May 20, 11:30am Don’t miss one of JustPlay!’s featured events, the Family Fun Run! Runners of all ages and abilities are invited to take on our course at Town Center. This is a great activity for the entire family to participate in and everyone comes out a winner! Registration will be taken in advance and on the day of the event. Mini Indy JustPlay! Photo Marathon May 19 & 20 Six topics, six photos, six hours - challenge your creative endurance rather than physical stamina. Bring your digital camera and blank memory card to the PhotoMarathon tent at JustPlay! to receive your topics. During the day take one photo for each topic and return to the tent at the end of the day. The best overall set will win a Fuji camera worth $300. Prizes for runners up. Travel to pitstops including car wash, gas station, noisemakers, and handle bar decorations. Race in our different categories like ride the line, tightrope tollbooth, and wacky obstacle course. Eat a spoke-tacular treat at our drive-up snack stand. Each driver must have their own trike or bike and a helmet. Movie Night in the Park Friday, Aug 3 Join us for our ‘Lady and the Tramp’ Interactive Movie Night! New this year - dinner & a movie....with your dog! This romantic adventure of the pedigreed cocker spaniel, Lady, and a roguish mutt from the wrong side of the tracks, Tramp, is one of Walt Disney’s best-loved animated features and a great love story. Bring your own pup to the park and purchase a fun pack that will allow you to interact with the movie. Games, crafts and a special Lady or the Tramp look-a- like contest will also take place. Before the movie, Hampe Park will be transformed into Tony’s Italian Restaurant. A delicious spaghetti dinner, with pasta, salad and bread, will be served. Make your reservation now for one of three seating’s 5:30, 6:20 & 7:00pm. All ages welcome, under 18 must be accompanied by an adult. Dogs must have updated vaccinations and be secured on a six foot leash. There is no admission fee for the movie but fees will apply for dinner, refreshments and fun packs. Challenger Sports’ British Soccer Camps provide young players of all abilities with the rare opportunity to receive high-level soccer coaching from a team of international experts right in the heart of their own community. Campers will be exposed to individual foot skills, technical drills, tactical practices, small-sided games and scrimmages. Soccer skills are coached within a framework of character development based on respect, responsibility, integrity, leadership and sportsmanship. We offer a cultural education too! Requests

1. Register on line at csparks.org or use the registration form in this guide. 2. Registration is by grade level. Players are to be registered for the grade the player is currently in. 3. Preschoolers must be entering kindergarten for the 2012/2013 school year.

1. One mutual friendship request per player will be honored for players in Preschool thru Grade 2. 2. One practice day conflict form per player will be accepted for players at all levels. 3. Due to the nature of the program, we cannot accept any other coach, player, or carpool requests 4. A park district request forms must be used for these requests and the guidelines must be followed. These forms are available at Simkus Recreation Center and on our web site at www.csparks.org 5. Request deadlines are the same as registration deadlines (not early bird).

Get exclusive use of the water park with a two hour private rental for groups up to 1500, Private rentals are available during the summer on Fridays and Saturdays from 8:15 pm -10:15 pm and on Sundays from 7:15 pm to 9:15 pm. Full payment is required to secure a date. Rentals may be canceled due to inclement weather at the discretion of park district staff and can be rescheduled if dates are open.

Coyote Crossing Mini-Golf Rental Have a Puttin’ Picnic at Coyote Crossing! Rentals are for a minimum of one hour or in full-hour increments. You can reserve your space for as many hours you want to stay and play all day! Mini-golf is additional. Bring your own picnic or make it a party with pizza, pop, and other great options from our add-on menu.

Group Outings

Bring your group to Carol Stream Park District with three fun group outing options: Coral Cove Water Park, Coyote Crossing Mini-Golf and gymnastics. Outings cost $5 per camper and counselors go free. Double up the fun and add a second group outing on the same day and receive the discounted price of $3 per camper for the additional outing (this offer is for organizations only). WAIVER REGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity. WARNING OF RISK Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety. WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK Please read this form carefully and be aware that in signing up and participating in the programs listed, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a Result of participating in any and all activities connected with and associated with these programs (including transportation services and vehicle operations, when provided). Online: Browse the full activity guide online, register for activities and pay by credit card at csparks.org In person: Pick up an activity guide; complete a registration form and pay by cash, check or credit card at Simkus Recreation Center. By Mail: Receive a guide by mail, complete a registration form and mail the form and check to Simkus Recreation Center, 849 W Lies Rd, Carol Stream, IL 60188

Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/ AMEX/DISC). Make checks payable to the Carol Stream Park District; There is a $20 fee for returned checks.

Program Capacity All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting Lists Once the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Refunds will be processed after the first class.

Insurance The Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.

Electronics Recycling Now Law A new electronics recycling law came into effect in 2012. The new law requires that most electronic items be recycled and not included with regular trash. DuPage County has several sites available to drop-off electronics for recycling, including: Glendale Heights Public Services Garage, Hanover Park Public Works Facility and Wheaton Public Works Storage Lot.
