The summary of benefits below is meant to provide a brief overview of the benefits offered to eligible employees, and subject to change at any time, without notice. Where there is a difference or change, the employee handbook, HR Policy & Procedure Manual, and/or Plan document will prevail.

September 2024 - CRNA

I. Health and Welfare Benefits

All regular Full-Time and Part-Time Employees who are scheduled to work 18.75 hours or more per week (FTE 0.5 and above) are eligible for the benefits listed below.

Medical & Prescription Drug Coverage:

• Choice of PPO, EPO, and POS plans, all administered by Anthem Blue Cross Blue Shield.

• By enrolling in a Catholic Health medical plan, the employee (and any enrolled dependents) are also provided with prescription drug coverage, administered by OptumRx.

• Employees enrolled in a Catholic Health medical plan may also fill their prescriptions at MyCHSRx, the Catholic Health employee pharmacy. Prescriptions are confidentially delivered to the hospitals and various locations throughout the system and employees pay a discounted copay.

• All medical plan options include discounts on routine vision care.

• If you enroll a spouse under the medical coverage, and your spouse has access to medical coverage through his/her employer, the $20 per paycheck pre-tax spousal surcharge will apply.

Dental Coverage:

• Choice of core, buy up or DHMO plans, all administered by Cigna.

• Core and buy up plan options provide coverage for both in-and out-of-network providers. The DHMO option provides in-network coverage only.

Vision Coverage:

• Supplemental vision coverage is available by enrolling in the Enhanced Davis Vision Plan by MetLife.

• You may enroll in this plan even if you choose not to enroll in the Catholic Health Medical Plan.

• The Davis Enhanced Vision Plan by MetLife provides substantial savings on eye care and eyewear needs.

Basic Employee Life Insurance and Accidental Death & Dismemberment:

• Employees are automatically enrolled in Basic Employee Life Insurance and Accidental Death & Dismemberment (AD&D) coverage, each of which provide coverage equal to 1X their annual base salary, up to a maximum of $650,000.

• Catholic Health pays the full cost of this benefit.

• The maximum amount of Employee Life Insurance coverage (including Basic and Supplemental Life Insurance coverage) that any employee is eligible for is $1,500,000. Please see below for information on Supplemental Life Insurance.

Employee Assistance Program:

• Provides confidential counseling and/or assistance to employees and their families.

• Assists employees with finding child and elder care services that fulfill their needs.

• This benefit is available at no cost to all Catholic Health employees.

II. Voluntary Benefits

All employee contributions are deducted directly from your paycheck. Eligible employees may enroll in these benefits as new hires, during Open Enrollment or within 31 days of a Qualified Status Change, unless otherwise indicated. Enrollment may be subject to approval by the Plan Administrator.

Supplemental Group Life Insurance:

• Employees may elect to purchase Supplemental Employee Life Insurance up to an additional 6x their annual base salary.

• As a new hire, Evidence of Insurability (EOI) will be needed if the Supplemental Employee Life Insurance election exceeds 3X.

• The maximum amount of coverage (including Basic and Supplemental Group Life Insurance coverage) that any employee is eligible for is $1,500,000.

• Administered by Voya Financial.

Dependent Life Insurance:

• Employees may also purchase life insurance for their spouse in the amount of $5,000, $20,000, $50,000, $100,000 or $150,000* and their child(ren) in the amount of $4,000 or $10,000.

• Dependent children are eligible for coverage until the end of the year in which they turn age 26.

• Administered by Voya Financial.

* Spouse Life insurance cannot exceed 100% of the employee’s combined basic and supplemental life insurance amount.

Supplemental Short-Term Disability:

• Designed to supplement the NY State mandated Short-Term Disability benefit. The NY State benefit pays 50% of your earnings up to a maximum benefit of $170 per week for up to 26 weeks.

• Benefits eligible employees may purchase Supplemental Short Term Disability through the Benefits of Caring program at Catholic Health.

• The Supplemental Short Term Disability benefit is available in 2 increments: $100/week or $200/week. This amount is paid on top of the NY State mandated benefit and will continue for a maximum of 26 weeks.

• Both the NY State plan and the Supplemental plans are administered by Matrix Absence Management.

Long-Term Disability:

• New York State does not offer any Long Term Disability benefits.

• Benefit eligible employees may elect Long Term Disability coverage through the Benefits of Caring program.

• Our Long Term Disability plan replaces 60% of their base salary, up to $10,000 per month.

• Benefits begin after 26 weeks of disability (the first 26 weeks fall under “Short Term Disability”).

• Monthly rates for this plan are based on age and income.

• This plan is administered by Matrix Absence Management.

Flexible Spending Accounts:

Catholic Health employees can enroll in one or all of the following Flexible Spending Accounts, each administered by Baker Tilly:

1. Dependent Care Flexible Spending Account

• Benefits eligible employees may enroll as a new hire, during Open Enrollment, or during the year with a Qualified Status Change.

• Employees may contribute up to a maximum of $5,000 per household to receive reimbursement for eligible dependent care expenses.

• Contributions are made on a pre-tax basis.

2. Health Care Flexible Spending Account

• Benefits eligible employees may enroll as a new hire, during Open Enrollment, or during the year with a Qualified Status Change.

• Employees may contribute up to a maximum of $3,200 per individual to receive reimbursement for eligible healthcareexpenses (medical, prescription, dental and vision).

• Contributions are made on a pre-tax basis.

Transit and Parking Flexible Spending Account

• Benefits eligible employees may enroll at any time during the year.

• Contributions are made on a pre-tax basis.

• The pre-tax limits are $315 per month for Transit and $315 per month for parking

Critical Illness Insurance:

• Helps provide financial protection in the event of a covered serious illness or cancer. The policy pays a lump sum benefit directly to the employee if the employee or covered family member is diagnosed with a covered condition.

• Employees may enroll in this plan as a new hire or during Open Enrollment.

• This plan is administered by MetLife.

Accident Insurance:

• Help cover the out-of-pocket medical expenses and extra bills that can follow an accident. The total benefit you receive is based on the type of injury, its severity and the medical services you received in treatment and recovery.

• Employees may enroll in this plan as a new hire or during Open Enrollment.

• This plan is administered by MetLife.

Hospital Indemnity Insurance:

• Provides payments, in addition to your medical plan, to help cover eligible expenses associated with hospital stay. Benefits are designed to help offset expenses your medical plan doesn’t cover, such as deductibles, coinsurance and everyday bills.

• Employees may enroll in this plan as a new hire or during Open Enrollment.

• This plan is administered by MetLife.

Identity Theft Protection:

• Provides comprehensive, proactive identity theft monitoring and dedicated recovery assistance.

• This service catches fraud early and helps employees to act quickly to limit the damage of stolen information.

• Employees may enroll in this plan as a new hire or during Open Enrollment.

• This plan is administered by Allstate Identity Protection.

Legal Plan:

• Affordable legal assistance through a network of attorneys who can assist with services via office consultation and/or telephone advice.

• Services include: Tax audits, Civil Lawsuit Defense, Real Estate Legal Services, Traffic Ticket Defense (excludes DUI) and documents and Will Preparation.

• This plan is administered by MetLife.

New York State 529 College Savings Plan:

• Catholic Health offers our employees the opportunity to save for college via direct payroll deductions.

• This benefit is available to all regular and per diem employees.

III. Retirement Benefits

Diocesan Pension Plan:

• Funded by Catholic Health and provided at no cost to employees.

• Retirement benefits depend upon earnings, number of years in the plan, and age at retirement.

• Participation begins on the first of the month following the fulfillment of ALL of the following requirements:

◻ Completed one year of continuous service.

◻ Budgeted to work 20 or more hours per week.

◻ Attained age 21.

• Once an employee is a participant, they are vested in the pension plan after completing a total of 5 years of continuous service.

403(b) Retirement Plan:

• Fidelity is the record-keeper for the Catholic Health 403(b) Retirement Savings Plan. To enroll in the 403(b) and choose your investment options, please visit the Fidelity NetBenefits employee portal at www.NetBenefits.com/AtWork

• Catholic Health offers employees a variety of investment options to fit their individual needs.

• All employees may contribute up to $23,000 (2024 maximum) of their annual salary towards a 403(b) tax-deferred annuity.

• Any employee who has attained age 50, or will attain the age of 50 during 2024, may contribute up to $30,500 per year (2024 maximum).

• The 403(b) is funded by employee contributions only.

• This plan is available to all regular and per diem employees.

IV. Education Assistance

Tuition Assistance

• Eligible employees are those budgeted .5 and above (37.5 hours per pay period).

• Eligible after completion of 1 year of employment (prior to beginning of course).

• Reimbursement for job related degree programs up to $7,000 maximum per calendar year, pro-rated by FTE for part-time employees.
