When you align with your colleagues here and utilize services within the network—including Catholic Health hospitals, ambulatory care facilities and home care—you establish a continuum of care designed to promote optimal outcomes for your patients.

• Instructions for gaining access to the Physician Portal, which contains your personalized quality metrics, a list of upcoming CME opportunities, important documents, like managed care forms, and so much more.

• Information about our Practice Unite/Uniphy and Imprivata smart phone apps, which are tools to help you stay connected to your colleagues and up-to-date on important announcements.

In addition, all new physicians participate in our mandatory “New Physician Intro & Welcome Session.” This session is separate from your orientation, providing you operational information and an opportunity to interact with some of your new colleagues.

Benefits of using My CHS Rx

How My CHS Rx works

• Tell your physician to e-prescribe your medication to My CHS Rx.

• Prescriptions will be electronically submitted by your physician and automatically sent to My CHS Rx where they will be verified and filled.

If you are currently admitted to a Catholic Health hospital, your discharging physician will submit the prescription. You’ll be visited as early as possible during your hospital stay by a My CHS Rx staff member who will get the information needed to fill your prescription(s).

• Education will be provided to help you better understand and follow your medication treatment plan. You can involve family and caregivers in the counseling process.

• Doctors, office staff and/or pharmacy staff will discuss and resolve medication issues with you (such as medications not covered by your insurance) before you are discharged from a Catholic Health hospital or leave your Catholic Health Physician Partners office.

FAQs: General

Will you accept my prescription insurance plan?

We accept most prescription insurance plans and can directly bill your prescription to your insurance provider. You are responsible for any copay required by your insurance carrier when your medications are delivered.

What types of payment do you accept for prescriptions?

My CHS Rx accepts credit cards. For patients admitted to a Catholic Health hospital, cash and check are also accepted.

What if I cannot afford my medications?

My CHS Rx representatives can connect you with resources to help lower your prescription drug costs.

FAQs: Using My CHS Rx during your hospital stay

Can my prescription costs be added to my Catholic Health hospital bill? Prescriptions filled by My CHS Rx while in the hospital cannot be added to your Catholic Health hospital bill. However, we are able to collect your copay when we deliver the medication to your bedside.

What if I forget to bring my prescription insurance card to the hospital?

My CHS Rx can contact your current pharmacy or your family members to retrieve your insurance information.

What is the Central Referral Office (CRO)?

Catholic Health is focused on quality and providing patients with efficient, coordinated care. The CRO is the hub for patient care coordination. CRO office staff act as a seamless bridge between a patient’s primary care physician and the local specialist.

Driving in-network utilization

Through the CRO, patients have a single point of contact at Catholic Health—simplifying the process of connecting them to the right specialist or resources appropriate for their health care needs.

• Only one patient per virtual visit is allowed.

• Video Visits should be held in a room with good lighting and no distractions.

• Write down a list of questions and the reason for your visit ahead of time.

• Have your current medication list available.

• If you have home medical equipment:

▸Take your temperature before your Video Visit.

▸Take your blood pressure beforehand.

▸Diabetic patients should take a finger stick for blood sugar.

• Should you need to show the doctor a specific body part, dress in a way that allows you to do so easily.

Download Practice Unite

iPhone users: Download the app from the App Store. Android users: Download the app from the Google Play Store.

Then:

a. Click Install then open to begin.

b. Select Catholic Health as your organization.

c. Click the ‘CHSLI ID Login’ link and enter your network username and password.

d. Accept the User Terms & Conditions.

e. Choose a 4 digit PIN #. Utilizing a PIN is critical in protecting Patient Health Information.

To access Practice Unite on a desktop computer, go to the Catholic Health intranet homepage, click the "My Apps & Tools" button under the "My Apps & Tools" menu. Practice Unite is listed under "P".

Communicating with colleagues

How do I send a secure text message?

Click on the “Secure Text” icon > Click on the “Contacts” tab. Here you can search by name, specialty or unit/department. Click on the name of the individual to whom your message will be sent.

What do the red and green dots next to someone’s name mean?

A solid green dot indicates that the person is available on Practice Unite; a hollow green dot indicates that the person is covered by another user. A red dot indicates that the person is “unavailable.” The absence of a dot indicates that the person has not activated their app, and is not available by secure text messaging.

How will I know if my secure message has been read?

Once the person to whom the message has been sent reads your message, an “R” will appear below the message, along with the date and time the message was read.

How do I take and send secure photos?

If you would like to attach a photo to your secure text message simply click “Secure Text,” enter the person you would like to send a photo to, and then click “Attach Photo.” You can then either (i) attach a photo from your secure photo album or (ii) take a new secure photo.

How do I create a “Group Chat” and send “Group Chat” Messages?

Click “Secure Text,” and select “Groups." To create a group, click the “plus” sign at the top right of the screen and select your group’s members. Once your group is selected, click “Done” and enter the name of your new group.
