FROM THE CEO Dear Friends, As I look back over the past 18 months, I am deeply inspired by the many people Centerstone has helped and the possibilities that each of those lives holds. At Centerstone, we believe in the possibilities of lives changed forever through prevention, intervention and research-driven treatments. Our ultimate dream at Centerstone is to prevent and cure mental illness and addiction. That is our mission. Last year alone, 74,000 people reached out to Centerstone for help throughout Indiana, Tennessee and beyond – individuals and families so desperately seeking hope and fulfillment in their lives. As the nation’s largest private provider of community-based behavioral healthcare, we are making breakthroughs every day to help those who are impacted by brain disorders, addiction and trauma achieve recovery. With our focus on care, research and education, Centerstone has reset the paradigm in behavioral healthcare. We are making our mission a reality. Our role at the Centerstone Foundation is to connect people, like you, to our mission. And, because of your gifts, last fiscal year was our best ever. Over $4.8 million in donations were made to Centerstone – vital gifts that saved lives, restored hope, rekindled dreams and ensured that countless individuals could be free of illness and live life to its fullest. In addition, four board members, one employee and one retired employee made commitments through their estate plans. Planned gifts are vitally important to Centerstone and the most efficient way to build endowment. Endowments assure that life-saving treatments and cures are available to future generations. ABOUT CENTERSTONE Centerstone is the nation’s largest not-for-profit provider of community-based mental health and addiction services. We provide a full range of mental health, addiction and related educational services to more than 74,000 individuals of all ages each year, operating from nearly 120 facilities and 220 partnership locations throughout Indiana and Tennessee. We also operate the Centerstone Foundation; the Centerstone Research Institute (CRI), which is improving mental healthcare through innovative research and information technology; Not Alone, which provides confidential, no-cost support and mental health services to active military, combat veterans and their loved ones; and Advantage Behavioral Health, a behavioral health administrative management organization. CLIENT ASSISTANCE FUND A vast majority of Centerstone’s clients are uninsured or underinsured and cannot afford the services that they so desperately need – services such as outpatient counseling, psychiatric services, substance abuse treatment, case management and crisis services. While our dedicated Centerstone staff work with clients to help them enroll in appropriate support programs, unfortunately many still do not qualify. Due to the generosity of so many donors, we have been able to establish an emergency Client Assistance Fund, which supports clients and families who otherwise would be unable to access their treatments. These gifts have ensured that needed care is provided.
NOT ALONE
Centerstone’s Not Alone helps military families, warriors, and veterans heal from devastating psychological and emotional traumas including Post-Traumatic Stress Disorder (PTSD), depression, anxiety and addiction. Without immediate access to behavioral health services, these traumas can have disastrous effects on warriors, their families and communities, including divorce, serious addiction, and all too often, suicide. To address this dire need, Not Alone provides free, confidential counseling services both in-person and within professionally facilitated online support groups and forums. We also provide a 24/7 crisis hotline and a web portal, which ensures all military personnel and their families can access immediate, life-saving services. In addition to professional counselors, Not Alone connects warriors
Over the last twelve months, Not Alone provided 771 hours of online group work, 188 hours of in-person support groups, 1,220 in-person counseling appointments and handled 463 cases through our e-clinic crisis center. We also provided three therapeutic retreats, serving over 50 warriors and families from across the country.

and their families with peers who can relate to their hardships and empower them to successfully adjust to life at home after combat. Not Alone offers and ensures these services are provided free of charge. This is made possible through the generosity of our donors.
“RECLAIMING” CAPITAL CAMPAIGN (Nashville, Tennessee) In November 2011, we officially announced the successful culmination of a five-year capital campaign that raised $12,541,143. This inaugural campaign for Centerstone in Tennessee reached 102% of our total goal – an outstanding achievement! The “Reclaiming” Campaign supports three areas of need for Centerstone: Our New Building on the Dede Wallace Campus

Our beautiful new building on the Dede Wallace Campus will be Centerstone’s first major facility built in Nashville, Tennessee. The new building will have two specialty clinics, serving children and families in one and older adults in its other. In total, this historic campus will be home to nearly 40 provider offices and offer space for other specialty programs and group therapy.

ENDOWMENTS Centerstone is grateful when a family or individual wishes to establish a permanent endowment in honor or memory of a family member. Centerstone’s endowment currently has six designated funds that have been established over time. A permanent fund may be established with a gift of $25,000.
The Ella P. Hayes Fund
The Ella P. Hayes Fund, which supports all of Centerstone’s programs, is named in honor of a great humanitarian and friend of Centerstone. The lives touched by Ella Petway Bullard Hayes truly created a chain reaction of kindness. She instilled, in everyone who was blessed to come across her path, a deep respect for people from all walks of life.
The Dede Wallace Fund
The Dede Wallace Fund supports programs in Davidson and contiguous counties. It is named in honor of a woman who fostered hope and whose legacy continues today. In the early 1960s, at a time when the stigma of mental illness was even stronger than it is today, Louise Bullard Wallace, affectionately known as Dede, reached out to those who were suffering in silence.
The Richard D. Sivley Fund
The Richard D. Sivley Fund supports programs benefitting children. As the former Tennessee Commissioner of Mental Health and Mental Retardation, and former President and CEO of the Dede Wallace Center, Rick worked timelessly to improve Tennessee’s, and specifically Nashville’s, mental health programs for children. As commissioner, he established the first state-wide task force on children’s mental health. It was under Rick’s leadership that the state’s therapeutic nurseries/ preschools were developed.
The George Spain Research Fund
The George Spain Research Fund supports behavioral health research. Centerstone’s active involvement in research positions us to collaborate with and assist others in the behavioral healthcare community by finding workable treatments and creating innovative advances in helping the problems faced by so many. Centerstone’s co-founder, George Spain, helped launch these research efforts.
The Deborah Taylor Tate Endowment Fund
The Deborah Taylor Tate Endowment Fund was established June 2007 and is an unrestricted and permanent endowment to advance the mission of Centerstone for generations to come. In 2004, Debi agreed to take on the demanding role of Chairman of the Board of Directors of Centerstone. In early 2005, Debi was contacted by the White House and appointed as a Commissioner on the Federal Communications Commission (FCC). Honoring the request of board and management, Debi agreed to serve as board chair, even though this meant declining a strong request from the White House to dissolve all such commitments. Since 2005, Debi has diligently served both her country and the mission of Centerstone.

ENDOWMENTS (continued) The Richard H. Fitzgerald Endowment Fund
The Richard H. Fitzgerald Endowment Fund was established in May 2008 and is an unrestricted and permanent endowment to advance the mission of Centerstone and for generations to come. After twenty-two exemplary years of service on the boards of directors of Dede Wallace Center and then Centerstone, Dick was elected to serve as Centerstone’s sixth board chair. In taking on this leadership role, Dick led the board through a process that culminated in the first-ever model for a nationally positioned community mental health center. Throughout this process, Centerstone benefited from Dick’s unwavering moral compass, his tireless efforts and his unique gift of rallying others to emerging opportunities. Underlying all of his labor is a deep and abiding commitment to providing hope, healing and indeed, someday a cure for those who suffer from mental illness. This endowment received a supplemental fund in August 2010, in honor of Dick’s continued board leadership at Centerstone.
The Joan C. Sivley Endowment Fund
The Joan C. Sivley Endowment Fund was established in August 2010 and is an unrestricted and permanent endowment established to advance the mission of Centerstone. An amazingly talented and compassionate individual who has served its boards of directors in a variety of capacities since first joining the board in 1993, she has been an extraordinary leader who has left an indelible mark of successful leadership on this organization and, likewise, a lasting impression of tireless dedication on all who have witnessed her commitment and, therefore, know her to be a true servant leader. In May 2008, she became the first chairperson of the Board of the newly formed Centerstone corporation, Centerstone of Tennessee, Inc. Her steadfast dedication, commitment and desire to positively change the lives of those impacted by mental illness and addiction disorders has been a humbling inspiration to this organization’s boards of directors, its staff and the tens of thousands of Tennesseans it is so privileged to serve.
The Stephen J. Pritchard, DDS, Endowment Fund
The Stephen J. Pritchard, DDS, Endowment Fund was established in August 2010 and is an unrestricted and permanent endowment to advance the mission of Centerstone. A gifted and compassionate person, Dr. Pritchard has provided extraordinary and lasting leadership on behalf of the organization and the tens of thousands of southeastern Indiana residents it serves who struggle with mental illness and addiction. As Chair of the Centerstone of Indiana board, he guided the board through the consideration and consummation of a mutually beneficial merger of Centerstone of Indiana and the Dunn Center in Richmond, Indiana, thus creating an expanded organization that now provides services throughout south central Indiana.

The Joseph E. Roche Endowment Fund was established in August 2010 and is an unrestricted and permanent endowment to advance the mission of Centerstone. As Chairman of the Board of Directors for Advantage Behavioral Health, a company of Centerstone, Joe embraced the vision of the young managed behavioral health organization and engaged other Board members in this innovative endeavor. This commitment to a developing Centerstone company yielded the building blocks for the firm foundation of this small thriving company today. With a robust career as a healthcare leader and social worker, by training, Joe has long been an advocate for individuals experiencing behavioral health concerns. He served the mission of Quinco Behavioral Health Systems and now serves Centerstone of America through his insightful mentoring and consultation, compassion and philanthropy.
The David J. Emmert Endowment Fund
The David J. Emmert Endowment Fund was established in August 2010 and is an unrestricted and permanent endowment that honors David’s leadership and advances the mission of Centerstone. As a member of the Centerstone of Indiana Board of Directors and a former board member of Quinco Behavioral Health Systems, David J. Emmert is a gifted and compassionate person who has provided extraordinary and lasting leadership on behalf of the organization and the thousands of Indiana residents who struggle with mental illness and addiction. He has diligently served the fields of both education and mental health. As Chair of the Centerstone of Indiana board, he guided the Quinco board to a mutually beneficial merger of three organizations: Quinco Behavioral Health Systems, Center for Behavioral Health, and Centerstone.
The George Bullard Stadler Endowment Fund
The George Bullard Stadler Endowment Fund was stablished in August 2010 and is an unrestricted and permanent endowment to advance the mission of Centerstone. As the first ever Chairman of the Board for the Centerstone Foundation, George led the development of an inaugural premier national board charged with safeguarding endowment resources from the Centerstone Endowment Trust, a Tennessee trust, and the Centerstone Foundation, an Indiana corporation, to support the mission of this organization for the benefit of all of its communities for future generations. This insightful and compassionate person built a multi-state team of committed board members to this service, and led the board in establishing policy and processes to ensure the critically important stewardship within its charge.
