Health Information Exchange
The Cerner Clinical Exchange Platform (CEP) is an extensible, open, standards-based solution built upon a reliable technology infrastructure for the secure exchange and storage of electronic clinical data and documents. The technology and platform provide a scalable solution for the high transaction volumes, information management and data standards required by contemporary Health Information Exchange (HIE) architectures. It is based on a Service Oriented Architecture (SOA) and incorporates web services standards along with established Integrating the Healthcare Enterprises (IHE) protocols. CEP supports global industry and local national standards for the electronic exchange of healthcare information through HITSP, HL7, ITK2 and IHE; a vendor-agnostic approach to connecting disparate systems across the healthcare community. CEP uses IHEs IT Infrastructure standards and profiles where applicable to support the storage, management, and availability of critical health information across the healthcare economy. These profiles define standard protocols and message formats for cross-enterprise document sharing (XDS profile), reconciling patient identities (PIX and PDQ profiles) and auditing transactions that carry protected health information (ATNA profile). By using industry standards, it allows other vendor systems that also use those same standards to easily work together, avoiding much of the resource investment that is required to connect, upgrade and maintain non standards-based multi-vendor healthcare systems.
CEP uses IHEs Continuity of Care Document (CCD), a XML-based markup standard intended to specify the encoding, structure, and semantics of a patient summary clinical document for exchange. It includes the most commonly needed information about the current and past health status of a patient in a form that can be shared with other software systems and web browsers. The solution presents an aggregated view of patient data taken from multiple sources linked to the CEP including custom queries and data feeds that have been built to link with the solution. A component of CEP is the IHE Gateway. A federated XDS repository that connects a data source to a clinical affinity domain for sharing data. Cerner Millennium users access the longitudinal record through a tab within the patient-centric PowerChart clinical application keeping within patient and user context. This is positive for clinical adoption as users do not have to sign into another system and find the patient. The solution supports web services to allow third party systems to access the data in a similar way or if IHE-enabled to access the data natively and display as required. For users that do not have access to an Electronic Patient Record system, CEP makes provision for a Web Portal, a browser-based interface for querying and displaying data using the latest Web 2.0 techniques, including AJAX.

Cerner Hub is a vendor agnostic ‘Push’ technology for the delivery of electronic messages. A secure bi-directional link that replaces paper or the need for point -to-point networks creating a one-to-many network effect. Working as a single point of connection it will reduce administration overhead associated with sending a variety of clinical data and documents to multiple endpoints and alleviate the need for custom and expensive interfaces. It provides access to time-sensitive health information from one system to another leveraging existing workflows and systems. The solution affords a variety of connectivity protocols and has the potential to connect to any Healthcare software system to exchange data. As a transformation and message broker the Cerner Hub provides a mechanism to leverage industry standards, but is sufficiently flexible to act as a gateway for those systems not yet compliant with interoperable standards. It provides an opportunity to quickly deploy and scale a robust solution without requiring change on the source and destination systems.
