At Chelsea and Westminster Hospital NHS Foundation Trust we are committed to improving the care and health of people with learning disabilities. A decision was made by the Chief Nurse at the end of 2014, to appoint a learning disability lead for the Trust to work with our staff to champion the equality of access and care for both adults and children with learning disabilities and to support their carers and families.


Kathryn Mangold started in post as the Lead Nurse for Learning Disabilities & Transition in March 2015 Kathryn is happy to assist in the management of any of your patients with learning disabilities, on a case by case basis, who are booked for treatment or admission to the Trust. Kathryn is also the chair of the Learning Disability Steering Group which meets bi-monthly and includes representatives from all departments within the Trust, carers of people with a learning disability and external partners such as Parents Active, Healthwatch, Full of Life and the local learning disability teams. The agenda includes a regular slot for feedback and learning on in- patient admissions and outpatient visits. We are very keen for GPs to join this group – if you or your colleagues have an interest in learning disabilities we would love to hear from you. We are also working with the Tri-Borough Learning Disability Steering Group to promote the use of an adapted referral letter from you, the GP, to include any ‘reasonable adjustments’ on a case by case basis that we could make to ensure a smooth journey through our services. The Trust is raising awareness of the needs of these patients through a specialist monthly training course to empower staff on how to respond to challenging behaviour by demonstrating how to communicate effectively with people with a learning disability so that your patient and their carers have an improved experience when attending our hospital. Date set for West Middlesex Integration Plans for the integration of Chelsea and Westminster Hospital NHS Foundation Trust and West Middlesex University Hospital NHS Trust continue to make good progress, with the date for the launch of the unified organisation now set for 1 September. Chelsea and Westminster Hospital Chief Executive Libby McManus said: “The energy, enthusiasm and

commitment of staff at both trusts continues to grow and this extension to the ‘go-live’ date gives us a bit more time to cement our joint approach and ensure we are well-placed to provide the best services possible to a potential population of nearly a million people.” Staff at both trusts are getting together, both formally and informally, to talk through opportunities to share best practice and improve services. West Middlesex University Hospital Chief Executive Jacqueline Totterdell said: “The way we provide

health services is rapidly evolving, with rising demand, the changing needs of our patients and a growing need to work more innovatively and efficiently. In this context, it is imperative that we take the time to get the regulatory process right so that we’re in the best shape we can be on Day 1 of the new organisation.” The integration of the two trusts is also due to be ratified by the C&W Board of Directors and Council of Governors, with final approval from the regulators and Secretary of State for Health. Libby added: “It is great to see the work underway already and we will continue this in order to achieve our joint ambition to create an organisation of 5,000 staff, with a turnover of more than £500m and the potential to become a world leader in research and patient care. “The momentum we are seeing from staff who recognise the potential to improve the way we provide care is driving this process and it is vital that we do everything we can to manage it smoothly. “These extra few weeks means we can be sure we have everything in place to ensure a smooth transition to a new way of working in September.”


Birth Centre says hello to baby number 1000

Acute Diagnostic Oncology Clinic patient case studies Following feedback from GPs about the new Acute Diagnostic Oncology Clinic, we have prepared three exemplar patient case studies which we hope help clarify which patients would benefit most from this new service. The popular Birth Centre at Chelsea and Westminster Hospital celebrated its own special new arrival last week when its 1000th baby was born. Baby Bruno (pictured with parents Sam and Paula McMeekin and midwives Magda Da Silva and Louise Johns) became baby number 1,000 when he was born on Wednesday morning. Head of Midwifery Vivien Bell said: “We are delighted to welcome our 1000th baby to the world. The fact we are celebrating this milestone now shows just how popular our Birth Centre is with women, given that the Centre only opened its doors 14 months ago.� The Birth Centre is aimed at women who are assessed as having low risk deliveries and has seven rooms with adjustable lighting and music players to help promote a relaxing birth environment. Four of the rooms have pools for women to use during their labour as well as other birthing aids to assist with delivery. There is also an opportunity to use aromatherapy to enhance the birthing experience. The Centre was officially opened by Call the Midwife actress, Helen George, last month.

75 year old man presented with jaundice to his GP. Bilirubin was >150. Hard craggy liver on examination, consistent with malignancy. Patient was otherwise well. Patient was referred to ED by GP because they were worried he would not last 2 weeks without review. Oncology team saw patient on the ward where a CT guided biopsy confirmed metastatic upper GI cancer. He was referred to the upper GI MDT and went on to the Marsden for further oncology care. Patient’s admission lasted 2 weeks but everything could have been done as an outpatient as he remained well throughout. Patient Case 2 45 year old lady who presented with abdominal bloating. She had a scan done in the private sector which suggested disseminated peritoneal disease consistent with malignancy. She attended her GP to this who referred the patient to ED. She was well apart from the abdominal distention. Patient was assessed in ED and after a few hours there was discharged home with an urgent outpatient appointment for gynae-oncology outpatients. The ED attendance was not necessary. The whole process could have been managed as an outpatient which would have been a better patient experience for this lady.


Patient Case 3 84 year old man with a history of asbestos exposure and asbestosis. Presented to his GP with increasing shortness of breath and was found to have a right pleural effusion on chest xray. Referred to the Acute Assessment Unit for urgent assessment of suspected mesothelioma. Oxygen saturations 96% on admission. Was seen by acute oncology team during admission. Effusion was tapped and patient discharges with surgical followup. He went on to have a surgical biopsy of the lung which fortunately found no evidence of cancer. The admission was not necessary and could have been managed as an urgent outpatient and a day case pleural drain. Dr Tom Newsom-Davis Consultant Medical Oncologist

(The community dermatology contract is being taken over by Imperial Healthcare).
