Routine primary care can add years to your life and help you enjoy more good days than bad. By enrolling with Catalyst Cares you can expect personalized support from a healthcare provider and care team you know and trust.

You may be eligible if you are:

• Ages 16+

• Able to be on site for services when needed

• Not currently insured or do not have benefits covering primary care services

• Not receiving care from a primary care provider or not assigned to one through Medicaid or Medicare Advantage plan

• Interested in partnering to manage one or more of the following chronic conditions:

– Diabetes – Hypertension

– COPD

– Asthma – ADHD

– Depression/Anxiety
