News: Case Management Information GET REPLACEMENT SOCIAL SECURITY TAX FORMS ONLINE WITH EASE

SENIORS AND WINTER HYDRATION

By Rhonda Whitenack & Jim Czechowicz (Social Security in Bloomington/St Paul)

By Gwinnett Medical Center

It’s that time of year again. Preparing for tax season can seem overwhelming. Some forms and paperwork might be difficult to track down. If you misplaced your Benefits Statement or haven’t received it by the end of January, we’ve made it easy for you to go online to request a replacement with my Social Security. SSA-1099, also called a Benefit Statement, is a tax form Social Security mails each year in January to the more than 60 million people who receive Social Security benefits. It shows the total amount of benefits received from Social Security in the previous year so people know how much Social Security income to report to the IRS on their tax return. For noncitizens who live outside of the United States and received or repaid Social Security benefits last year, we’ll send form SSA-1042S instead. The forms SSA-1099 and SSA-1042S are not available for people who receive Supplemental Security Income (SSI). If you currently live in the United States and need a replacement form SSA-1099, we have a way for you to get a replacement quickly and easily. Go online and request a replacement form with a my Social Security account at www.socialsecurity.gov/myaccount. Online replacement forms are available beginning February 1, 2017. Every working person in the U.S. should create a my Social Security account. The secure and personalized features of my Social Security are invaluable in securing a comfortable retirement — for today and tomorrow. 4

We are often told we should drink more water, and guidelines for athletes talk about specific amounts, timing and strategies for hydration, but how much water do we actually need? “When it comes to drinking water,” says Kevin Johnson, MD, medical director of Strickland Family Medicine Center, “I recommend that adults drink at least 64 ounces each day. For children and teens, take their weight in pounds, cut that number in half, and that’s the number of ounces they need for good hydration.” For the elderly, staying hydrated is especially important. Gwinnett Medical Group Geriatrician Stephanie Garrett, MD, urges the elderly to pay special attention to hydration. “According to the American Geriatrics Society, dehydration is the most common fluid or electrolyte disturbance in the elderly,” she says. “This is often due to a decreased thirst or from eating and drinking less at meals.” Dehydration is a major cause for hospitalization in people over age 65, Even if it’s not hot outside, the low humidity of winter’s indoor heating can dehydrate an elderly person faster than expected. As always, consult with your doctor regarding your body and hydration.

Case Managers schedules do vary at the Stoughton Area Senior Center. Please call ahead to schedule appointments, 873-8585.

News: Case Management Information A special THANK YOU to our volunteer Linda Schmidt who helped our Case Managers though the Medicare D annual open enrollment.

2017 SOCIAL SECURITY CHANGES Cost-of-Living Adjustment (COLA) Based on the increase in the Consumer Price Index (CPI-W) from the third quarter of 2014 through the third quarter of 2016, Social Security and Supplemental Security Income (SSI) beneficiaries will receive a 0.3 percent COLA for 2017. This means a retired worker who currently receives the average $1,350 per month benefit will see only $4 more per month from the COLA increase. The slight increase in Social Security income means that the maximum monthly Social Security benefit at full retirement age next year will be $2,687 per month.

Please see our Stoughton Area Guide for Senior Services available at the Senior Center or on our website at www.ci.stoughton.wi.us/senior, for a more complete list of the following services: Food Pantry • Medical • Housing Financial • Mental Health Home Health • Respite Fitness/Wellness • Care Facilities

WHAT IS A SNOW EMERGENCY? What exactly does that all mean? Basically it means that when the city is expecting three (3) or more inches of snow to fall or three (3) or more inches have fallen, the Stoughton Director of Public Works will declare a snow emergency. During a snow emergency you are only able to park on the odd side of the street when the date is an odd number (The date as of Midnight!) and on the even side of the street on an even date. The side of the street is based off of the house numbers. The timing of the parking restriction is from Midnight to 8am for three (3) consecutive sessions unless the emergency is extended or cancelled. How can I find out that a snow emergency has been declared? Here are the ways to find out: 1.Visit the City of Stoughton website at http://www.cityofstoughton.com 2. Sign up to get e-mails from the city website 3. Tune to Stoughton's WSTO TV on Charter Cable Channel 981 4. Check out the WSTO TV Facebook Page 5. Call the Police Department Non Emergency Number at 608-873-3374 6. Listen to the local TV or radio stations

As 2016 comes to a close so does my career as Nutrition Coordinator at the Stoughton Area Senior Center. After 20 years of having the privilege to work with the best co-workers and volunteers anyone could have wished for it is time for me to retire. As I look forward to the next phase of my life I can't help but to reminisce about the wonderful people I have met along the way. Each and every one of you hold a special place in my heart. I have requested that there will not be a retirement party in my honor as this is not a goodbye but rather a so long, see you later. I plan on volunteering and being able to participate in activities that this awesome senior center has to offer. Thanks again for the fond memories that I will cherish forever . Happy 2017 to all.

Bring ¾ cup water to a simmer in a medium saucepan over medium-high heat. Whisk in 3 Tbsp. cocoa powder until no lumps remain, then add 3 Cups whole milk and return to a simmer. Whisk in 6 oz. finely chopped semi sweet chocolate and 3 Tbsp. granulated sugar and cook, whisking frequently, until mixture is smooth and creamy and chocolate is melted, about 5 minutes. Divide hot chocolate among mugs. Top with whipped cream and dust with cocoa powder.
